AIM: To observe the clinical effect of phacoemulsification and posterior chamber intraocular lens ( IOL) implantation in the treatment of unilateral high myopia combined with age-related cataract.
METHODS: A retrospective clinical study was carried out in a single ophthalmology center. All patients diagnosed of unilateral high myopia with age- related cataract were performed phacoemulsification and posterior chamber IOL implantation. The axial length of operation eye was ≥27mm, and the axial length of unoperated fellow eye was≤25mm. Visual acuity and complications were followed up for 1, 7d, 1, 3mo postoperative.
RESULTS:Thirty-three patients with average age 68. 8±9. 26 (41-87) y were enrolled in this study, 7 cases were men, 26 cases were women. The mean axial length of operation eyes was 29. 44±1. 80(27. 1-32. 71) mm and the mean axial length of unoperated fellow eyes was 23. 39±0.92 ( 21. 72 - 24. 90 ) mm. The average axial length difference of binocular eyes was 6. 06±1. 97(2. 78-9. 92) mm. The best corrected visual acuity ( BCVA ) increased from 0. 02±0. 02(light perception~0. 12) preoperatively to 0. 20±0. 20 ( 0. 02 - 0. 8 ) at 3mo postoperatively. There was statistically significant difference (t= -5. 278, P<0. 05). Among the 10 patients whose axial length difference of binocular eyes was 2. 01 - 5. 00mm, 60% of their postoperative BCVA were >0. 3. Among the 13 patients with binocular axial length difference of 5. 01-7. 00mm, 46.2% of BCVA were >0. 3,46.2% achieved 0. 05-0. 25. Another 10 patients of binocular axial length difference between 7. 01-9. 99mm, 90% of BCVA were <0. 3, of which 40% were<0. 05. No complications occurred during operative and postoperative .
CONCLUSION: Phacoemulsification and posterior chamber IOL implantation can effectively improve the postoperative visual acuity and it is safe for unilateral high myopic patients combined with age-related cataract. The recovery of visual acuity is related with binocular axial length difference.

Objective To investigate the clinical application and diagnostic value of impulse oscillometry lung function in children with bronchial asthma.Methods Thirty-two children (19 children in acute attack group,13 children in clinical remission group) with bronchial asthma were selected,and 20 healthy children as control group.The lung function simation by Master Screen impulse oscillometry device was detected.Results The outcome of the R5,R20,R5-R20,the negative of X5 and Fres in acute attack group (21.90±2.70) Hz]were higher than those in clinical remission group [(7.75±0.97),(3.82±0.50),outcome of the R5,R5-R20 and Fres in clinical remission group were higher than those in control group (P＜0.05),but there were no statistical significance in R20 and X5 in the two groups (P＞0.05).Conclusion Impulse oscillometry can provide various objective indicators to bronchial asthma in acute attack and clinical remission stage.which can be used as a method in detecting pulmonary function and monitoring therapy of bronchial asthmatic children.

Objective To test the effect and safety of′cocktail′wet compression on alleviate the swelling arm after the inter‐vention through the radial artery .Methods Patients whose arm were swollen after the intervention through radial artery were di‐vided into two groups (experiment group ,47 patients vs .control group ,46 patients) .The experiment group adopt the′cocktail′wet compress plus traditional therapy (including elevate and immobile the swelling arm);the control group only use the traditional ther‐apy .The levels of pain ,swelling index and the number of the blister were compared between two groups .Results None of the limbs necrosis was observed in two groups .The level of pain(2 .62 ± 0 .86 vs .5 .46 ± 1 .02) ,swelling index (3 .3% ± 2% vs .5 .8% ± 3% ) of the arm ,and the number of blister (0 vs .4) in experiment group were lower than in the control group (P<0 .05) .Conclusion The bundle of therapy strategy which named'cocktail'wet compress can lessen the swelling of the arm induced by intervention through radial artery ;it is both safe and effective thus deserves widely application .

ObjectiveTo compare the effects of two methods for the treatment of delayed defecation of neonatal meconium. Methods Sixty-seven neonates with delayed defecation of neonatal meconium were divided into experiment group (n=37) and control group (n=30). The former group was managed with abdominal massage followed by glycerol enema and the latter with abdominal massage followed by anus stimulation.Then the groups were compared in terms of the time for initial defecation of meconium,the volume of defecation,exhanstion time for defecation,abdominal distention and vomiting and serum bilirubin within 7 days.Result The treatment group was superior to the control one in terms of time for initial defecation of meconium,the volume of defecation, exhanstion time for defecation (P<0.05) and the incidences of abdominal distention,vomiting and serum bilirubin within 7 days were significantly lower than the control group (P<0.05).Conclusions The method of glycerol enema combined with abdominal massage is more effective for promoting defecation of neonatal meconium than the method of anus stimulation.It can reduce the incidence of abdominal distension,vomiting and pathological jaundice.

Objective To investigate the value of serum IgG4 in diagnosis of IgG4-RD and in differentiation from rheumatic diseases.Methods Total of 23 patients with IgG4-RD and 502 patients with rheumatic diseases were enrolled,who presented at Changhai Hospital in 2010 to 2011.In the study,rheumatic diseases were categorized into groups of Sj(o)gren syndrome (n =26),ankylosing spondylitis (n-50),systemic sclerosis (n =3),rhcumatoid arthritis (RA,n =125),mixed connective tissue disease (n =15),systemic lupus erythematosus (SLE,n =212),adult onset still disease (n =20),Behcet syndrome (n =17),polymyositis (n =12),dermatomyositis (n =12),polymyalgiarheumatica (n =10).Serum IgG and IgG4 levels were measured by a rate nephelometer assay.The ROC curves were constructed to identify the optimal serum IgG4 cutoff value for diagnosing IgG4-RD and evaluate its sensitivity and specificity.Results The mean levels of serum lgG4 in the group with IgG4-RD were 11.4(5.0-14.8) g/L.In about 95.6％ IgG4-RD patients,the serum IgG4 level was higher than ＞ 1.4 g/L and other rheumatic diseases (U values were 6.0,21.0,0,58.5,0,9.0,3.0,4.0,0,3.0,3.5,P ＜0.01).The levels of serum IgG4 with RA was 0.6(0.3-1.2) g/L,the levels of serum IgG4 with SLE was 0.2 (0.1-0.4) g/L.There were statistical differences between RA and SLE (U value was 5847,P ＜ 0.01).At the same time,some patients with other rheumatic diseases were found serum IgG4 level higher than ＞ 1.4 g/L,which was about 10％ in the patients whith RA,ankylosing spondylitis,adult onset still disease and polymyalgiarheumatica.According to the ROC constructed the cut off value in present study was 2.2 g/L,and sensitivity and specificity were 95.7％ and 97.4％,respectively.Area under the cerve (AUC) was 0.995.There were no significant differences between the sensitivity and specificity values obtained with a cutoff value of 2.2 g/L.In patients with other rheumatic diseases,the ratio of high serum IgG4 level (＞ 2.2 g/L) were declined obviously,except polymyalgiarheumatica,it was less than 10％.For differentiation from rheumatic diseases specificity values were higher.Conclusions The cut off value of 2.2 g/L is useful for diagnosing IgG4-RD,and in differentiation from rheumatic diseases.The high serum IgG4 concentrations are not specific to IgG4-RD.The cut off value of 2.2 g/L is better to diagnose IgG4-RD,and contributes to the differential diagnosis of IgG4-RD and other rheumatic diseases,but it needs to be further confirmed in clinical practice.

Objective To investigate the efficacy of neoadjuvant chemotherapy combined with radical resection of cervical cancer in the treatment of stage IB2-ⅡB cervical cancer.Methods According to the treatment plan from October 2012 to October 2016 in the People′s Hospital of Luohu District of Shenzhen in 80 cases of stage IB2-II B cervical cancer patients were divided into observation group(n=43) and control group(n=37),the observation group was treated with neoadjuvant chemotherapy combined with radical surgery for cervical cancer,patients in the control group directly treated for radical resection of cervical cancer.Chemotherapy effect,operation time,intraoperative blood loss,postoperative pathological risk factor differences of two groups of patients were compared.Results (1)Evaluated the curative effect of neoadjuvant chemotherapy,squamous cell carcinoma group complete remission(CR) 4 cases,partial remission(PR) 22 cases,stable disease(SD),6 cases of disease progression(PD) in 0 cases,in adenocarcinoma group CR 1 cases,PR 3 cases,SD 5 cases,PD 2 cases of squamous cell carcinoma group adjuvant chemotherapy was significantly better than that of adenocarcinoma group,the difference was statistically significant(z=2.4968,P=0.0063).(2)The operation time((215±57) min) and intraoperative blood loss((682±145) ml) in the observation group were significantly lower than those in the control group(((259±62) min,(758±193) ml)),the difference was statistically significant(t=3.8780,2.2528,P=0.0002,0.0263).(3)The two groups of patients with ureteral fistula(P=0.5039),vesicovaginal fistula(P=0.3639),wound healing(P=0.5182),lower extremity deep venous thrombosis(P=0.4818) complications had no significant difference.(4)The positive rate of the observation group of lymph nodes(χ2=8.2005,P=0.0000),parametrial infiltration rate(χ2=8.1553,P=0.0000) was significantly lower than the control group,the difference was statistically significant.Two groups of patients with deep myometrial invasion rate(χ2=0.0991,P=0.7516),the incidence of cancer embolus(χ2=0.0130,P=0.9176) compared to no statistical significance.Conclusion The effect of neoadjuvant chemotherapy on cervical squamous cell carcinoma is better than that of adenocarcinoma.Compared with the direct for radical resection of cervical cancer,neoadjuvant chemotherapy combined with radical resection of cervical cancer patients for the treatment of stage IB2-ⅡB cervical cancer,operation time and blood loss are lower,node positive and parametrial invasion and postoperative pathologic risk factors in lymph node also can get better effect.

Objective To explore the effect of transitional intervention on compliance and life quality of patients after percutane-ous coronary intervention(PCI) in cardiovascular internal department ,and provide the basis for the compliance in patients after PCI operation .Methods From January 2010 to September 2012 ,80 patients after PCI infection who had coronary heart disease were di-vided into observation group and control group ,and 40 cases in each group .The control group used conventional methods to rehabil-itation guidance for patients after PCI while the observation group used continued intervention system .According to the exercise of selfcare agency scale(ESCA) and health promoting lifestyle profile(HPLP) to improve the evaluation ,the compliance and life quali-ty of the two groups patients who discharged from hospital after 6 months and 12 months were evaluated .Results In the 6 months and 12 months after discharge ,compliance and life quality of the observation group was better than the control group ,and the differ-ence was statistically significant (P<0 .05) .And the difference of ESCA and HPLP scores at different time points between groups were statistically significant (P<0 .05) .Conclusion Continued intervention system can significantly improve the compliance and life quality in infection patients after PCI .

To assess the postoperative outcomes of piggyback implantation using Acrysof Toric intraocular lens ( lOL ) in high myopia combined with corneal astigmatism.
●METHODS: Sixty patients who had phacoemulsification with lOL implantation due to high myopia, cataract and corneal astigmatism from January 2010 to June 2013 were randomly divided into observation group (piggyback Toric lOL implantation, both an Acrysoft lQ Toric lOL and a minus foldable acrylic three piece lOL were implanted in the capsular bag, n= 30) and control group (foldable lOL implantation, n = 30). Postoperative follow - up went on 6mo. lnformation collected included uncorrected visual acuity ( UCVA), lOL position, residual astigmatism and complications.
● RESULTS: The UCVA increased from 3. 52 ± 0. 03 preoperatively to 4. 78±0. 01 at 6mo postoperatively in the observation group, from 3. 51±0. 03 preoperatively to 4. 30± 0. 13 at 6mo postoperatively in the control group. The observation group's postoperative UCVA was better than that of the control group. There was statistically significant difference ( t = 3. 612, P < 0. 05 ). The preoperative corneal astigmatism was 2. 97 ± 0. 87 (1. 70 -4. 27) D in the observation group and 2. 92 ± 0. 97 (1. 50 -4. 90)D in the control group. The postoperative residual astigmatism was 0. 48 ± 0. 23 ( 0. 25 - 1. 00 ) D in the observation group and 2. 87 ± 1. 11 (1. 00 - 5. 20) D in the control group. There was statistically significant difference postoperatively (t = - 11. 995, P < 0. 05) between the two groups. No complications occurred.
●CONCLUSlON: Piggyback implantation using Toric lOL can help to solve the problem of no matching Toric lOL power for the high myopia combined with corneal astigmatism at the current stage. lt improves the UCVA and reduces the astigmatism after the cataract surgery.

Objective To find out households consumption of iodized salt in Xinjian county, and to provide scientific basis for prevention and treatment of iodine deficiency disorders(IDD). Methods From 2006 - 2010 in Jiangxi province, according to the direction of east, west, south, and north, nine townships(streets) were selected,in each township (street), 4 administrative villages (committees) were selected, in each administrative village(committee) 8 households were selected to collect their edible salt each year, direct titration method was adopted to detect salt iodine. Results From 2006 - 2010 a total of 1440 salt samples were collected, of which 1379 were qualified iodized salt, 34 unqualified, 27 non-iodized salt; iodized salt coverage rate, qualified iodized salt and iodized salt consumption rates were 98.13% (1413/1440), 97.59% (1379/1413) and 95.76% (1379/1440),respectively, and the rate of non-iodized salt was 1.88% (27/1440). Conclusions The intake rate of qualified iodized salt in Xinjian county have reached the standards of eliminating IDD. The quality of iodized salt should be improved in few counties. In the future, we should also increase the use of various forms advocacy of IDD prevention and treatment, and educate the masses to enhance self-protection awareness, so that they can consciously resist the salt smuggling, and refuse to buy non-iodized salt.

Objective To investigate the occurrence of postoperative healthcare-associated infection(HAI)and its risk factors in neurosurgical patients undergoing removal of intracranial tumor,so as to provide theoretical basis for formulating intervention measures.Methods Prospective survey was adopted to monitor the occurrence of postoperative HAI in patients who admitted to the department of neurosurgery of a hospital and underwent selective removal of intracranial tumor between April 2013 and December 2014 ,risk factors for HAI were analyzed with univariate and multivariate logistic regression analysis.Results A total of 1 218 patients were surveyed,163 patents developed 193 times of postoperative HAI,inci-dence of postoperative HAI was 13.38%,case incidence of HAI was 15.85%.The main HAI site was intracranial site(n=125,64.77%),the next was lower respiratory tract (n=55,28.49%).Multivariate logistic regression analysis showed that operation grade and subtentorial operation were independent risk factors for postoperative HAI in neurosurgical patients undergoing removal of intracranial tumor,OR and 95%CI were 4.352(1.878-10.080)and 1.812(1.280-2.564)respec-tively.Conclusion Risk of postoperative HAI in neurosurgical patients undergoing high grade operation and subtentorial removal of intracranial tumor is high,effective prevention and control measures should be taken to prevent the occurrence of HAI.
