1.Prevention of perioperative pressure ulcer for hypoalbuminemia patients with renal transplantation

Objective To investigate the effect of intervention nursing on the formation of pressure ulcer in hypoalbuminemia patients with renal transplantation.Methods From February 2011 to June 2015,the 134 cases of hypoalbuminemia patients with renal transplantation in our hospital were divided into the observation group (73 cases)and the control group (61 cases).The control group were given conven-tional nursing methods for prevention of pressure ulcers,and the observation group were given corresponding intervention nursing measures on the basis of pressure ulcer risk factors.Then assessed the risk factors of pressure ulcers,the occurrence of pressure ulcers,and the rate of nursing satisfaction.Results The incidence of pressure ulcer in the observation group was significantly lower than that in the control group (P <0.05).The rate of nursing satisfaction in the observation group were significantly higher than that in the control group (P <0.05).The risk factors for hypoalbuminemia patients with renal transplantation were decrease of sensory response,increase of moisture in the compressed part,and increase of friction and shear forces in the compressed part.The occurrence of these risks could be significantly decreased after inter-vention nursing.Conclusion Effective nursing care can significantly reduce the incidence of pressure sores in hypoalbuminemia patients with renal transplantation and improve the nursing satisfaction.This method can be recommended for clinical nursing care for prevention of periop-erative pressure ulcer.
2.Reparation of large coloboma raw surface after facial tumour resection in elderly patients

Objective To investigate the surgical reparation of large coloboma raw surface after facial tumour resection in elderly patients.Methods According to the position and characteristics of tumor as well as the age and tolerance of the patients, full thick skin graft, the skin flaps with subcutaneous pedicle and free skin flap were designed and used in the reparation.Results 24 cases with large coloboma raw surface (5 cm×7 cm-12 cm× 16 cm)were treated by the utilization of three approaches after tumor resection.The large coloboma raw surface in all patients achieved the healing with satisfactory appearance.Conclusions After facial tumour resection, the large coloboma raw surface can be repaired by using the skin graft, skin flaps after tumor resection or free skin flap if designed reasonably.The procedure of operation is simple and the therapeutic effect is satisfactory.

6.Application of QCC in Improving the Compliance of Asthma/COPD Outpatients Handling Inhaler Devices

OBJECTIVE:To evaluate the effects of quality control circle(QCC)in improving the compliance of asthma/COPD outpatients handling inhaler devices. METHODS:By simple random sampling method,90 asthma/COPD outpatients receiving Tiotropium bromide powder for inhalantion,Salmeterol xinafoate and fluticasone propionate powder for inhalantion and Budesonide and formoterol fumarate powder for inhalantion were selected from our hospital during Apr. to Nov. 2015. The reasons for poor com-pliance of handling inhaler devices were analyzed by QCC. The countermeasures were formulated to improve the compliance,in-cluding clinical pharmacist education,follow-up,issuing education manuals,developing WeChat consulting. Operation step score of inhaler devices,compliance score and activity ability score of QCC members were analyzed statistically before and after QCC. RESULTS:After the development of QCC,operation step score of inhaler devices and compliance score in 90 asthma/COPD outpa-tients increased from 7.7 and 7.7 before activity to 8.8 and 9.3 after activity,the achievable rate of compliance was 114.3%,and the improvement rate was 20.8%. The sense of honor,responsibility,self-confidence,QCC techniques,communication and coordi-nation,team cohesion of all members were all improved obviously. CONCLUSIONS:QCC activity improves the compliance of asthma/COPD outpatients handling inhaler devices and shows the professional service level of clinical pharmacists,which is of sig-nificance to the improvement of pharmaceutical care.
7.Chemokine score is a novel biomarker for lupus patients with active diseases and organ damages

Objective To correlate the chemokine score with disease activity,organ damages and clinical features in systemic lupus erythematosus (SLE) patients.Methods Peripheral blood cells obtained from 60 SLE patients,20 rheumatoid arthritis (RA) patients and 23 healthy donors (HDs) were subjected to real-time PCR to measure the transcriptional levels of seven chemokines (RANTES,MCP-1,CCL19,MIG,IP-10,CXCL11,and IL-8).Chemokine scores were calculated and were compared between various groups of SLE patients as well as between patients and controls.Results Chemokine scores were significantly elevated in SLE patients compared with RA patients and HDs (P=0.0112 and P=0.0019,respectively).Chemokine scores were correlated positively with SLEDAI (P=0.0061) and negatively with C3 levels (P=0.003).Compared to patients without lupus nephritis (LN),chemokine scores were elevated in SLE patients with active LN,especially when their daily prednisone dosage was less than 30 mg (P=0.0418 and P=0.002,respectively).Chemokine scores were also associated with cumulative organ damage (SLICC damage index [SDI]>0) and positive anti-Sm and anti-RNP autoantibodies.Conclusion The chemokine score may serve as a new biomarker for disease activity and organ damage in SLE patients.
8.Preparation and evaluation of Shedan in situ forming gel based on ocular characteristics.

To develop an ophthalmic preparation of Shedan, an in situ forming gel was prepared with the formulation containing 18% of poloxamer 407 and 5% of poloxamer 188 by response surface designs plus central composite designs. The rheology results showed that LVE range gamma should limited within 0.5%, Shedan high-frequency region, and the thixotropy recovery time is less than 5 seconds. The phase transition temperature was 33.25 °C according to curve of storage modulus and loss modulus determined by temperature scanning. Surface tension and osmometer of it determined by surface tension meter and dew point osmometer were 36.43 mN · m(-1), and 320.6 mOsm · kg(-1), respectively. Fluorescein sodium was selected as the marker to monitor the corneal residence time, and the results showed that Shedan gel could prolong drug residence for 180 min. In line with zero-order kinetics, releases of muscone and salvianolic acid B in vitro depends on gels erosion. The results of rabbit ocular irritation experiments suggested that Shedan in situ forming gel was biocompatible and nonirritant. In conclusion, a novel Shedan in situ forming gel was developed and characterized for potential drug treatment of retinal vein occlusion.

9.High-frequency electromagnetic field exposure on reproductive and endocrine functions of female workers.

OBJECTIVETo analyze the toxicity of high-frequency electromagnetic field (HF-EMF) on the reproductive and endocrine functions of female workers.
METHODS180 female workers exposed to HF-EMF for more than one year were selected as the exposure group, and 349 female workers in the marketplace were selected as the control group. Their menstrual status and pregnancy outcomes were evaluated with historical cohort study. Meanwhile, the serum levels of reproductive hormones including follicle stimulation hormone (FSH), luteinizing hormone (LH), estradiol (E(2)) and progesterone (P(4)) from 30 workers respectively in the exposure group and the control group selected randomly (in periovulatory period) were detected with the radio immunoassays.
RESULTSWith the increase of residue of HF-EMF, the incidence of menstrual disorder was increased (33.8% and 26.8% in the exposure group compared with 12.0% in the control group) (P < 0.01) and the incidence of menorrhagia in the exposure I group (16.9%) was significantly higher than that in the control group (8.0%) (P < 0.01). However, serum P(4) was significantly lower in the exposure group [(2.10 +/- 1.57) and (2.39 +/- 1.35) mg/L] than in the control group [(3.80 +/- 2.67) mg/L] (P < 0.01). There was no significant difference in serum E(2), LH and FSH between the exposure and the control groups.
CONCLUSIONIncrease of the incidence of menstrual disorder as well as that of menorrhagia, and decrease of levels of some reproductive hormones are closely associated with HF-EMF (25 approximately 30 MHz).

10.LSD1 acetylation mediates TSA-induced apoptosis of ovarian cancer cells

Objective Lysine-specific demethylase 1 (LSD1) is a flavin adenine dinucleotide-dependent oxidase, which participates in many biological processes , such as cell proliferation and differentiation and gene activation and repression .The aim of this study was to investigate LSD1 acetylation by histone deacetylase inhib -itor trichostatin A ( TSA) and its effect on TSA-induced apoptosis of ovarian cancer cells . Methods LSD1 shRNA was synthesized and implanted into the pLKO-Tet-On lentiviral vector , which was transfected into HO8910 and SKOV3 ovarian cancer cell lines , and then the transfected cells were screened with 1.5μg/mL puromycin for one week until stable clones were established .The cells were treated for 48 hours with methanol (2 mg/mL, control), TSA (200 nmol/L), TCP (100μmol/L), or TSA+TCP.And in the experiment of RNA interfering the LSD1 expression, the cells were also treated for 48 hours with methanol (2 mg/mL, control), TSA (200 nmol/L), Dox (100 ng/mL), or TSA +Dox.The levels of LSD1 acetylation and its substrate histone H3 lysine 4 dimethylation (H3K4me2) were de-tected by immunoprecipitation (IP) and Western blot.The apoptosis of the cells was determined by Annexin Ⅴ/PI staining and flow cytometry, the transcription levels of the Bax and p21 genes detected by real-time quantitative PCR, and the H3K4me2levels in the promoter regions of Bax and p21 measured by chromatin immunoprecipitation ( ChIP ) .Results In comparison with the methanol control, the TSA group showed significantly increased levels of LSD 1 acetylation in the HO8910(1.00±0.29 vs 5.83±0.46, P<0.01) and SKOV3 cells ( 1.00±0.24 vs 5.07±0.35, P<0.01) as well as that of H3K4me2 ( P<0.01);the total apoptosis rates of HO 8910 and SKOV3 cells were remarkably increased in the TSA, TCP, and TSA+TCP groups (P<0.05), even more significantly in the TSA+TCP than in the TSA and TCP groups ( P<0.05) .The mRNA expressions of Bax and p21 in the HO8910 cells were markedly upregulated in the TSA, Dox, and TSA+Dox groups (P<0.05), even more significantly in the latter than in the former two groups (P<0.05).The TSA group exhibited a higher level of H 3K4me2 than the methanol control in the promoters of Bax(2 .92±0.26 vs 0.68±0.19, P<0.01) and p21 (3.07±0.29 vs 0.93±0.17, P<0.01). Conclusion TSA induces the LSD1 acetylation, while suppression of LSD1 expres-sion and activity may enhance the antitumor activity of TSA .
