Objective To discuss the economic development and social inclusion of persons with disabilities. Methods The international conception of rehabilitation was applied as the tools. Results Many persons with disabilities are in marginal status, and their social development are limited. Conclusion Several recommendations had been made for empowerment of persons with disabilities through social protection and empowerment through poverty reduction, as well as an enabling legal and policy framework.
2.Management of the sentinel lymph node of papillary thyroid carcinoma in surgery.

OBJECTIVE:
To investigate the value of sentinel lymph node of papillary thyroid carcinoma exploring and measuring in the metastasis of the cervical lymph node of thyroid carcinoma.
METHOD:
The sentinel lymph node were investigated by injecting the methylene blue around 25 cases of thyroid tumors. The lymph nodes dyed in blue and obtained from selective lymph node dissection were examined by pathology, respectively. The value of the sentinel lymph nod were evaluated.
RESULT:
The sentinel lymph nodes dyed in blue were identified in 22 cases which account for 88.0% of all the 25 cases. Nineteen cases of the sentinel lymph node were positive by pathology, of whom 13 cases were verified positive in other area. Six cases were negative.
CONCLUSION
It is of great value that sentinel lymph node of thyroid carcinoma can indicate the metastasis of the cervical lymph node of thyroid carcinoma.

3.Pedicled bridge transplantation for soft tissue defects at the contrallateral leg with medial leg skin flap and medial hemi-soleus muscle flap

Objective:To evaluate the pedicled bridge transplantation with medial leg skin flap and medial hemi-soleus muscle flap for the treatment of soft tissue defects at the contrallateral leg.Methods:Between January of 2012 and January of 2016, 8 patients with soft tissue defects at the leg were treated at Department of Orthopedic Surgery, Hand and Foot Surgery Hospital of Lanzhou. They were 5 men and 3 women, aged from 19 to 50 years (mean, 35 years). All of them were treated by bridge transplantation with medial leg skin flap and medial hemi-soleus muscle flap pedicled with posterior tibial artery. The size of the defects ranged from 10 cm×9 cm to 13 cm×8 cm. The immediate coverage of the muscle flaps and vessel pedicle was repaired by a meshed split-thickness skin graft. The donor site was closed directly. The therapeutic efficacy was assessed at the final follow-up according to the criteria by Iowa for tibial fractures.Results:All the skin flaps and muscle flaps survived without any vascular crisis. One case developed necrosis of small skin graft at the distal muscle flap which spontaneously healed after dressing change for 2 weeks. Their follow-up ranged from 2.5 to 4.5 years (mean, 3.8 years). A good contour was confirmed at the recipient area. By the Iowa criteria at the final follow-up, 3 cases were excellent, 4 good and one fair.Conclusion:Pedicled bridge transplantation with medial leg skin flap and medial hemi-soleus muscle flap is a good treatment for soft tissue defects at the contrallateral leg which has only one major blood vessel, reducing damage to the donor site.
4.The characteristic of metabolics of female papillary thyroid carcinoma and its relationship with primary tumor size and cervical lymph node metastases

Objective To analyze the metabolic profiles of the female papillary thyroid carcinoma (PTC) and the relationship between the metabolic profiles and primary tumor size and cervical lymph node metastasis using a metabolomics approach.Methods Forty-three cases of female PTC were enrolled in this study.Gas chromatography-time-of-flight mass spectrometry and ultra performance liquid chromatography-time of flight-mass spectrometry were employed for analyzing metabolic profiles of tumor tissues and adjacent normal tissues in the female PTC.Cases were divided into Group T1 (tumor size ≤2.0 cm) and Group T2 (tumor size > 2.0 cm) according to the tumor size,and divided into Group N-(with negative cervical lymph node) and Group N + (with positive cervical lymph node) according to the cervical lymph node conditions.We compared the metabolomic profiles between these groups.Results A panel of 46 differentially expressed metabolites was identified in the PTC specimens,compared with normal tissues.Increased metabolism of amino acid,purine and pyrimidine,tryptophan acid,one carbon,glycolysis,taurine and hypotaurine,and fatty acid were found in PTC tumors tissues.Amino acids,purine and pyridine,tryptophan,and carbon metabolism increased significantly in the tumor tissues of Group T2 compared with Group T1,while glycolysis,amino acid,purine and pyridine,tryptophan,and carbon metabolism increased significantly in the specimens of Group N +.Conclusion Distinct metabolic profiles were identified in the female PTC tissues,which were related to the primary tumor size and cervical lymph node metastases.
5.Therapeutic evaluation of surgical procedure in treatment of secondary hyperparathyroidism

Objective To analyze the short-term and long-term outcomes of the three operation methods in treatment of secondary hyperparathyroidism.Methods Clinical data of 88 patients who underwent parathyroidectomy for secondary hyperparathyroidism from October 2004 to October 2014 were reviewed retrospectively.Patients were divided into three subgroups,including subtotal parathyroidectomy (group Ⅰ),total parathyroidectomy with autotransplantation (group Ⅱ) and intraoperative 99Tcm-MIBI radioguided total parathyroidectomy with autotransplantation (group Ⅲ).Their serum calcium,phosphorus,serum iPTH results in the pre-operation,short-term (≤ 6 months) and long term (> 6 months) of postoperation were collected and compared.The improvements of clinical symptoms together with the postoperative recurrence rate and the complication data were observed and analyzed.Results The symptoms of all patients were improved after the operation.The short-term postoperative serum calcium,serum iPTH and recurrence rate of Group Ⅰ and group Ⅱ were all not statistically significant.The short-term postoperative serum calcium,iPTH and recurrence rate of group Ⅲ were significantly lower than those of group Ⅰ and group Ⅱ (P < 0.05).Postoperative serum phosphorus values of the three groups had no statistical difference.The operation time of group Ⅲ was significantly shorter than that of group Ⅱ ((77 ± 13)vs (108 ± 17) min,P < 0.05).The positive rates were more accurate in group Ⅲ than in group Ⅱ (98.5% vs 88.7%,P < 0.05).For the long term follow-up,the postoperative serum calcium,phosphorus,serum iPTH and recurrence rate of group Ⅰ and group Ⅱ were not statistically different.The postoperative recurrence rate of group Ⅲwas lower than that of group Ⅰ and group Ⅱ (3.6% vs 31.6% and 21.4% respectively,P < 0.05).Conclusions Total parathyroidectomy with autotransplantation has a good efficacy to treat secondary hyperparathyroidism.The intraoperative 99Tcm-MIBI radioguided total parathyroidectomy with autotransplantation can find the ectopic and supernumerary glands to improve the success rate of operation.And it also can permit omission of frozen section to reduce the operative time.In the postoperative follow-up,the recurrence rate is low,so it is a safe and effective treatment.
6.Impact of cecal ligation and puncture-induced sepsis on the proliferation and differentiation of intestinal stem cells

Objective:To analyze the impact of cecal ligation and puncture (CLP)-induced sepsis on the proliferation and differentiation of intestinal epithelial cells.Methods:① Animal experiment: sixteen male C57BL/6 mice were divided into sham operation group (Sham group) and CLP-induced sepsis model group (CLP group) by random number table method, with 8 mice in each group. After 5 days of operation, the jejunal tissues were taken for determination of leucine-rich-repeat-containing G-protein-coupled receptor 5 (LGR5) and intestinal alkaline phosphatase (IAP) by polymerase chain reaction (PCR). The translation of LGR5 was detected by Western blotting. The expression of proliferating cell nuclear antigen (Ki67) was analyzed by immunohistochemistry. IAP level was detected by modified calcium cobalt staining and colorimetry. Immunofluorescence staining was used to detect the expression of Paneth cell marker molecule lysozyme 1 (LYZ1) and goblet cell marker molecule mucin 2 (MUC2). ② Cell experiment: IEC6 cells in logarithmic growth stage were divided into blank control group and lipopolysaccharide (LPS) group (LPS 5 μg/mL). Twenty-four hours after treatment, PCR and Western blotting were used to analyze the transcription and translation of LGR5. The proliferation of IEC6 cells were detected by 5-ethynyl-2'-deoxyuridine (EdU) staining. The transcription and translation of IAP were detected by PCR and colorimetric method respectively.Results:① Animal experiment: the immunohistochemical results showed that the positive rate of Ki67 staining in the jejunal tissue of CLP group was lower than that of Sham group [(41.7±2.5)% vs. (48.7±1.4)%, P = 0.01]. PCR and Western blotting results showed that there were no statistical differences in the mRNA and protein expressions of LGR5 in the jejunal tissue between the CLP group and Sham group (Lgr5 mRNA: 0.7±0.1 vs. 1.0±0.2, P = 0.11; LGR5/β-actin: 0.83±0.17 vs. 0.68±0.19, P = 0.24). The mRNA (0.4±0.1 vs. 1.0±0.1, P < 0.01) and protein (U/g: 47.3±6.0 vs. 73.1±15.3, P < 0.01) levels of IAP in the jejunal tissue were lower in CLP group. Immunofluorescence saining analysis showed that the expressions of LYZ1 and MUC2 in the CLP group were lower than those in the Sham group. ②Cell experiment: PCR and Western blotting results showed that there was no significant difference in the expression of LGR5 between the LPS group and the blank control group (Lgr5 mRNA: 0.9±0.1 vs. 1.0±0.2, P = 0.33; LGR5/β-actin: 0.71±0.18 vs. 0.69±0.04, P = 0.81). The proliferation rate of IEC6 cells in the LPS group was lower than that in the blank control group, but there was no significant difference [positivity rate of EdU: (40.5±3.8)% vs. (46.5±3.6)%, P = 0.11]. The mRNA (0.5±0.1 vs. 1.0±0.2, P < 0.01) and protein (U/g: 15.0±4.0 vs. 41.2±10.4, P < 0.01) of IAP in the LPS group were lower than those in the blank control group. Conclusion:CLP-induced sepsis inhibits the proliferation and differentiation of intestinal epithelial cells, impairing the self-renewal ability of intestinal epithelium.

Bone formation and deposition are initiated by sensory nerve infiltration in adaptive bone remodeling.Here,we focused on the role of Semaphorin 3A(Sema3A),expressed by sensory nerves,in mechanical loads-induced bone formation and nerve withdrawal using orthodontic tooth movement(OTM)model.Firstly,bone formation was activated after the 3rd day of OTM,coinciding with a decrease in sensory nerves and an increase in pain threshold.Sema3A,rather than nerve growth factor(NGF),highly expressed in both trigeminal ganglion and the axons of periodontal ligament following the 3rd day of OTM.Moreover,in vitro mechanical loads upregulated Sema3A in neurons instead of in human periodontal ligament cells(hPDLCs)within 24 hours.Furthermore,exogenous Sema3A restored the suppressed alveolar bone formation and the osteogenic differentiation of hPDLCs induced by mechanical overload.Mechanistically,Sema3A prevented overstretching of F-actin induced by mechanical overload through ROCK2 pathway,maintaining mitochondrial dynamics as mitochondrial fusion.Therefore,Sema3A exhibits dual therapeutic effects in mechanical loads-induced bone formation,both as a pain-sensitive analgesic and a positive regulator for bone formation.
