1.Clinical value of tumor markers SCCA,TPS,NSE and CYFERA21-1 detection in diagnosis of lung cancer

Objective To explore the clinical value of tumor marker squamous cell carcinoma antigen(SCCA) ,tissue polypeptide specific antigen(TPS) ,neuron‐specific enolase(NSE) ,cytokeratin fragment 19(CYFRA21‐1) detections in the patient with lung cancer .Methods 70 patient with newly diagnosed lung cancer(including 26 cases of squamous cell carcinoma ,23 cases of adenocar‐cinoma and 21 cases of small cell carcinoma) ,40 cases of lung benign diseases and contemporaneous 30 individuals undergoing the physical examination were randomly selected as the lung cancer group ,lung benign disease group and healthy control group respec‐tively .Serum tumor markers of SCCA ,TPS ,NSE and CYFERA21‐1 were detected in all cases .Then the detection results were per‐formed the statistical analysis .Results The levels of SCCA ,TPS ,NSE and CYFERA21‐1 in the lung cancer group were significant‐ly higher than those in the lung benign diseases group and healthy control group ,the differences were statistically significant (P<0 .05) .The expression levels of serum SCCA ,TPS ,NSE and CYFERA21‐1 were different in different histological types of lung cancer .SCCA and CYFERA21‐1 were highly expressed in squamous cell carcinoma ,which showed the statistical difference com‐pared with other two pathological types of lung cancer(P<0 .05);the expression level of NSE was highest in small cell lung cancer , which showed the statistical difference compared with other two pathological types of lung cancer(PP<0 .05);the TPS expression level had no statistical difference among different pathological types of lung cancer(P>0 .05) .The combination detection of SCCA , TPS ,NSE and CYFERA21‐1 greatly increased the sensitivity and accuracy for diagnosing lung cancer .Conclusion The combina‐tion detection of SCCA ,TPS ,NSE and CYFERA21‐1 has very high clinical value for the diagnosis ,pathological typing and assisted clinical therapy of lung cancer .
2.Clinical significance of SUMO4 expression in papillary thyroid carcinoma

AIM:Todiscusstherelationshipbetweensmallubiquitin-relatedmodifier4(SUMO4)expression and papillary thyroid carcinoma (PTC).METHODS:The mRNA and protein levels of SUMO4 in PTC and normal thyroid tissues were determined by the methods of real-time PCR, Western blot and immunohistochemistry .The relationship be-tween SUMO4 expression and clinicopathologic parameters in PTC was evaluated .RESULTS:The results showed that both the mRNA and protein levels of SUMO 4 expression in PTC were obviously higher than those in normal thyroid tissues ( P<0.01).CONCLUSION:The high level of SUMO4 expression in PTC suggests that SUMO4 plays an important role in PTC development and it is probably a molecular mechanism of PTC .
3.Inhibitory effects of leuprolide acetate microsphere on experimentally induced endometriosis in rats

AIM: To study the inhibitory effects of domestic leuprolide acetate microsphere (LE-ms) on the growth of explanted endometrium in the rat models of EMT and its possible mechanisms. METHODS: The 60 rats models of EMT were induced surgically by the Jones method. Then the animals were treated with LE (20 ?g? -1?d -1,28 d,sc) , enanton(20 ?g? -1?d -1,sc) and domestic LE-ms (2, 20, and 200 ?g㎏ -1?d -1,sc), respectively; another 30 rats were divided into sham group(N.S, 1 ml?kg -1?d -1,21 d,sc), EMT+ LE group(100 ?g?kg -1?d -1,21 d,sc)and EMT group(N.S, 1 ml?kg -1?d -1,21 d,sc). At the same time, estrous cycle was monitored daily by examination of vaginal cytologic smears. After 3 weeks, blood was drawn and the serum estradiol concentration was assayed. The volume of endometrial implant was assessed. Lateral uterus, bilateral ovary, thymus and spleen were weighed. The NK cell cytotoxicity in the spleen was evaluated with lactate dehydrogenase (LDH) release assay. RESULTS: Implants in control group continued to grow, while those in groups treated with the drugs showed remarkable atrophy. The inhibitory rates were 87.2%, 78.3%, 57.3%, 89.0% and 94.7%, respectively. Aim To study the effect of leuprorelin acetate microspheres (LE ms) on endometriosis in rats, and compare the efficacy of material drug (LE), domestic and imported LE ms (enanton). Methods Endometriosis was induced by endometrial implant in rats. Then the animals were treated with LE (20 ?g?kg -1 ?d -1 ? 28 d ,sc), enanton(20 ?g?kg -1 ?d -1 ,sc)and domestic LE ms ( 2,20,200 ?g?kg -1 ?d -1 ,sc). Results Implants in control group continued to grow, while those in groups treated with drugs shrinked significantly, and domestic LE ms could produce dose dependent inhibitory effect on the growth of endometrial implant in rats. Conclusion The domestic LE ms at the single dose of 20 ?g?kg -1 ?d -1 has the same effectiveness as enanton and routine injection with the same does of LE for 28 days.
5.Dual source CT coronary angiography and selective coronary angiography for evaluation of coronary artery stenosis: a control study

Objective To evaluate the diagnostic accuracy of dual source CT angiography (DSCTA) for coronary artery stenosis.Methods During the period from November 2012 to November 2013, a total of 210 patients with coronary artery disease underwent DSCTA and selective coronary arteriography (CAG). Taking CAG as the gold standard, the diagnostic accuracy of DSCTA for coronary artery stenosis was evaluated. Thirty patients receiving DSCTA and 30 patients receiving CAG were selected, and all of them underwent stent implantation in the anterior descending branch after imaging examination. The angiography positions, the used time of PCI and the used dosage of contrast agent were compared between the two groups. Results DSCTA was performed in 210 patients and a total of 2 630 segments of coronary stenosis or occlusion were detected. Compared with CAG, the diagnostic sensitivity, specificity, positive predictive value and negative predictive value of DSCTA were 95.4%, 96.2%, 91.3%and 100%respectively, which were not significantly different from those obtained by CAG (P=0.066). In performing DSCTA, 2-3 angiography positions were used (2-4 positions less than that of CAG), the used time of PCI was about 15 min (about 10 min less than that of CAG), and the mean used dosage of contrast agent was 48 ml (30-150 ml) (about half less than that of CAG). Conclusion DSCTA has higher accuracy in diagnosing coronary artery stenosis, quite similar to that of CAG. DSCTA is a safe, reliable and noninvasive examination method. Preoperative DSCTA can reduce exposure positions during angiography, can reduce the dosage of contrast agent, and can shorten the time of PCI as well, thus, iatrogenic radioactive radiation dose can be reduced.
6.A quantitative analysis of interactions among ethanol, chloral hydrate and naloxone

AIM: To analyse quantitatively the interactions among ethanol, chloral hydrate and naloxone with isobologram, Q-test and multiple logistic regression methods. METHODS: The hypnotic effects of the three drugs on Kunming mice were observed. In two drugs interaction study, chloral hydrate and ethanol were given at different ratios (25∶75, 50∶50, 78∶22 and 80∶20). In three drugs interactions study, 15 min after treatment of naloxone at fixed dose (0.5 mg*kg-1 and 0.2 mg*kg-1) the mixture of chloral hydrate and ethanol (at 1∶1 and 1∶3 ratio) was given to induce sleep. The ED50 for hypnotic action (righting reflex loss) of chloral hydrate, ethanol, naloxone and their mixtures were calculated by use of isobologram, interaction Q-index test and multivariate logistic regression analysis. RESULTS: The mixtures of ethanol and chloral hydrate in all ratios revealed partial but significant synergism. But in addition with naloxone the three agents showed different natures of interactions according to different naloxone levels. CONCLUSION: There are synergistic interactions in hypnotic ED50s between ethanol and chloral hydrate at different ratios and antagonistic interaction in adding a fixed dose of naloxone. The results coincide with the pharmacologistic mechanism discussed in this paper.
7.MRI and pathological study of targeted mouse lung cancer with USPIO-labeled umbilical cord mesenchymal stem cells

Objective To trace human umbilical cord mesenchymal stem cells (MSCs)labelled by USPIO-PLL which were transplanted into mouse subcutaneous xenotransplanted lung cancer by using 3.0T MR,to investigate the relationship between MSCs and VEGF expression and tumor angiogenesis by using SABC immunohistochemical method and to comprehensively analyze the effect of MSCs transplantation on lung cancer.Methods Cultured MSCs and poly lysine (PLL)was as a transfection agent which was magnetically labeled by USPIO and implanted into mice with subcutaneous xenotransplanted lung cancer through the tail vein.MRI was performed at pre-transplantation, 1 d and 10 d after transplantation and the tissues were performed by immunohistochemistry respectively.Results (1)MSCs could reach the tumor area at the first day after the transplantation and be monitored by MRI.MSCs increased at the 10th day.MRI signal intensity was reducedand the difference was statistically significant (P <0.05).The inhibitory rate of the 1st day and 10th day was positive;(2)At the 10th day after the transplantation,the rate of the VEGF positive expression in MSCs group was 86.67%,the value of MVD was 44 .22 ± 12 .36 ,and the rate of the VEGF positive expression in NS group was 26 .67 % ,the value of MVD was 20 .29 ±8.47 (P <0.05).Conclusion Tracing stem cell transplantation in vivo can be proceeded effectively by using 3.0T MR.Stem cell has bidirectional effect on lung cancer which inhibites the tumor growth by directional chemotaxis and differentiation,and also enhances expression of VEGF and angiogenesis at a certain extent.
8.Value of urinary C-terminal agrin fragment in monitoring the progression of early kidney injury in type 2 diabetic patients

Objective:This study aimed to explore the feasibility and clinical value of monitoring the progression of early kidney injury in type 2 diabetic patients by assessment of the urinary C-terminal agrin fragment (uCAF) with enzymatic chemiluminescence immunoassay.Methods:A total of 251 patients with type 2 diabetes, who attended the Second Affiliated Hospital of Wenzhou Medical University from October 2018 to March 2020, were included in this retrospective analysis. One hundred and fifty-six participants undergoing health check-up at the Second Affiliated Hospital of Zhejiang University School of Medicine in February 2021 served as controls. Basic clinical information, glycosylated hemoglobin type A 1c and serum creatinine values were recorded, and urine specimens were collected for urinary creatinine, urinary α 1 microglobulin(uα 1M), urinary immunoglobulin G (uIgG), urinary albumin, urinary N-Acetyl-B-D-glycosaminidase (uNAG) and uCAF measurements. Based on the estimated glomerular filtration rate (eGFR), 251 patients were classified into G1~G5 stage groups with 116, 22, 28, 55 and 30 patients in each group. One hundred and sixty-six patients with early diabetic kidney disease (stage G1-G3) were divided into subgroups A1 (79), A2 (48) and A3 (39) according to the urinary albumin/creatinine ratio (UACR), the uα1M levels were divided into uα1M subgroup 1 (83 cases), uα1M subgroup 2 (42 cases), and uα1M subgroup 3 (41 cases), and uIgG subgroup 1 (83 cases), uIgG subgroup 2 (42 cases), and uIgG subgroup 3 (41 cases) according to uIgG levels. The Spearman method was used to analyze the correlation between uCAF levels and eGFR, UACR, uα1M and uIgG levels. Results:(1) The linear range of the uCAF detected by enzymatic chemiluminescence immunoassay was 3.97-2 000.00 ng/ml, with a detection limit of 2.28 ng/ml, intra-batch coefficients of variation of 1.15% and 1.57%, inter-batch coefficients of variation of 1.63% and 5.78%, and a biological reference interval of <95.35 μg/g Cr. (2) The uCAF level and positive rate (UACR≥30 mg/g) increased with the decrease of eGFR from G1-G3, uCAF level was negatively correlated with eGFR value ( r=-0.543, P<0.000 1), and the positive rate increased from 24.14% (28/116) to 85.71% (24/28) from G1-G3. The uCAF level and positivity rate decreased with the decrease of eGFR from G4 to G5. uCAF level was positively correlated with eGFR value ( r=0.495, P<0.001), and the positivity rate decreased from 30.91% (17/55) to 23.33% (7/30) from G4 to G5. (3) In patients with early diabetic kidney disease, uCAF levels and positivity rates increased gradually with the increase of UACR. uCAF levels were positively correlated with UACR values ( r=0.602, P<0.001), and the uCAF positivity rate reached 21.52% (17/79) in the A1 subgroup. (4) uCAF level was positively correlated with uα1M and uIgG levels in patients with early diabetic kidney disease ( r=0.757, 0.596, both P<0.001). Conclusion:Analytical performance of enzyme chemiluminescence immunoassay for the detection of CAF is satisfactory and could be used a biomarker for monitoring damage and progression of early diabetic kidney disease in patients with type 2 diabetes.
9.Analysis of Cumulative Live Birth Rate of Selective Single Embryo Transfer by Time-lapse Monitoring System and Conventional Morphological Assessment in IVF/ICSI-ET

Objective:To analyse the clinical significance of selective single embryo transfer by time-lapse mo-nitoring(TLM)or conventional morphology assessment(CMA)in vitro fertilization/intracytoplasmic sperm in-jection and embryo transfer(IVF/ICSI-ET),and to initially explore the predictive value of Raman spectral analy-sis of embryo culture medium for clinical pregnancy rate.Methods:The study is a prospective randomized con-trolled clinical trial.We assigned 139 patients treated with IVF/ICSI-ET in Reproductive and Genetics Center of Suzhou Municipal Hospital from April 2019 to July 2020,which were randomly assigned to either the CMA or the TLM group.We performed selective single-embryo transfer(fresh cycle and FET)after selecting the optimal em-bryos with TLM or CMA respectively.If the patient's first embryo transfer was unsuccessful,a second one would be performed to compare the differences in the cumulative live birth rate of embryo transfer and other pregnancy outcomes between the two groups.Meanwhile,we collected 15 μl of embryo culture medium at day 3 after IVF/ISCI fertilization for Raman spectroscopy analysis.Results:There were no differences in cumulative live birth,cu-mulative clinical pregnancy,cumulative premature birth,cumulative early spontaneous abortion,cumulative ectopic pregnancy and LGA or SGA between TLM and CMA groups(P>0.05).The Neonatal sex ratio in the TLM group was lower than that in the CMA group,but the difference was not significant(P>0.05).Raman spectros-copy analysis of embryo culture medium predicted the clinical pregnancy rate with 67.21%accuracy.Conclu-sions:In young women with a good ovarian reserve,the advantage of using TLM to evaluate embryos is not obvi-ous,so we should remain vigilant that embryo selection based on morphokinetic parameters may affect the sex ratio.Raman spectroscopic analysis of embryo culture medium is not yet able to effectively predict the planting ability of embryos.
