1.ANTI-INFLAMMATORY EFFECT OF TOTAL ALKALOID OF LEONTICE KIAGNANENSIS AND ITS MECHANISM OF ACTION

The anti-inflammatory of waterand fat-soluble alkoloids of Leon-tice Kiagnanensis (WSALK, FSALK ) were described after sc of WSALK (150, 300 mg/kg ) and FSALK (13, 26mg/kg ) . The increased capillary permeability in abdominal cavity and skin of mice induced respectively by 0.7% HAc and histamin were inhibited abviously. WSALK (75, 150, 300mg/kg, ip ) and FSALK (6.5, 13, 26mg/Kg, ip ) markedly inhibited the ear inflammation induced by xylene in mice, carrageenin-iaduces swelling of the ankle in normal on adrenalecto-mized rats, and carrageenin-induced pleurisy in rats. Both compound also inhibited the granuloma induced by cotton pellet after sc of WSALK 300, 600 mg/kg and FSALK 13, 26 mg/kg qd ? 7d.Both compound decreased the content of PGE in exudate formed after injecting carrageenin into the hind paw of rats. These results suggest that both compounds passesses the anti-inflammatory activity and their actions might be relatied to their inhibiting the synthesis or relase of pro-staglandin E. WSALK TI 9.35, FSALK TI 4.50.

According to our exploration on it on 7-year-systemmedical students in our hospital for nine years, we think that if you want high quality 7-year-system medical students,five concepts should be emphasized besides traditional basic knowledge, basic theory and basic skill, which are familyplanning and eugenics, loving infants, whole concept, new medical model and renewing knowledge.
3.Detection of survivin in esophageal cancer patients' serum with colloidal gold immunochromatography

Objective To investigate the feasibility of screening for esophageal cancer with survivin colloidal gold immunochromatographic test strip. Methods The serum samples from 158 esophageal cancer patients and 146 healthy individuals were tested with survivin colloidal gold immunoohromatographic test strips. Results 20 nm-diameter colloidal gold solution was prepared and survivin polyclonal antibodies with different concentrations were labeled to the gold particles. It showed that the optimal concentration for labeling was 12 μg/ml. The prepared survivin colloidal gold immunochromatographic strip teat showed positive rates of survivin in esophageal cancer group and control group were 51.9 % (82/158) and 15.1% (22/146) respectively (χ~2 = 45.7, P < 0.05). No statistical differences were observed in age groups (between 42-54 years: 47.1%; between 56-68 years: 40.9%; between 69-81years: 63.9%, χ~2= 1.11, P > 0.05), gender groups (male: 50.4%, female: 58.1%, χ~2= 0.59, P > 0.05) , primary site groups (upper segment: 56.3%, middle segment: 46. 1%, lower segment: 32. 1%, χ~2 = 2.64, P > 0.05), differentiation groups (well-differentiation: 56.3%, moderate differentiation: 43.2%, poor-differentiation: 32.7%,χ2= 1.63, P >0.05)and lymph node metastasis groups (with metastasis:43.3%, without metastasis:43.4%, χ~2 = 0.00, P > 0.05). The sensitivity, specificity and accuracy of survivin colloidal gold immunochromatographic test strip was 51.9% (82/158), 84.9% (124/146) and 67.8% (206/304) respectively. Conclusions Survivin colloidal gold immunochromatographic test strip is fast, simple, easy-to-read. It could be used as a valuable tool for screening of high-risk patients with esophageal cancer.
4.Effect of oral alloy post and core on magnetic resonance imaging.

OBJECTIVEThis study aims to observe and compare the influences of the casting alloy post and cores on magnetic resonance imaging (MRI).
METHODSSix casting alloy post and sores were fabricated for the experimental groups, and zirconia post and core served as the control. The seven types of post and cores were of the same size and shape. Each post and core was orally fastened to a volunteer and then imaged with sequences of T1-weighted spin echo imaging (SE-T1WI), T2-weighted turbo spin-echo imaging (TSE-T2WI), and diffusion weighted imaging (DWI).
RESULTSCobalt chromium alloy post and core generated severe artifacts, whereas gold palladium alloy post and core generated no obvious artifacts, and no difference was observed between the zirconia and the gold palladium alloy post and cores (P > 0.05). The pure titanium alloy post and core produced mild artifacts. The post and cores of the titanium, nickel-cadmium, and gold platinum alloys generated moderate artifacts with no significant differences (P > 0.05).
CONCLUSIONDifferent metal post and cores have different influences on MRI. With the exception of that generated by cobalt chromium alloy, the artifact generated by the single-alloy post and core does not influence the image of the normal head and neck.

5.The effects of commonly used dental cast metal posts and cores on MRI

Objective:To observe the influence of commonly used dental cast metal posts and cores on magnetic resonance imaging (MRI)for head and neck.Methods:Six kinds of posts and cores made of metal alloy (Ni-Cr alloy,Co-Cr alloy,titanium alloy,pure titanium,Pd-Au alloy and Au-Pt alloy)were respectively fixed on the mucosa of left palate of a volunteer.The maxillofacial region of the subjects was scanned by routine head sequences SE-T1WI and TSE-T2WI respectively.Results:ANOVA of factorial design analy-sis indicated significant differences of artifacts among metal materials(F=153.63,P<0.05),scanning sequences(F=70.94,P<0.05)and the number of cast posts and cores(F=102.73,P<0.05).In both sequences the size of metal artifacts were as following:Pd -Au alloy>Co-Cr alloy>Ni-Cr alloy>Au-Pt alloy>titanium alloy>pure titanium;two posts and cores>one post and core;SE-T1WI>TSE-T2WI.Conclusion:Different cast metal posts and cores have different influences on MRI artifact.Reasonably choosing the metal materials and scanning sequences can minimize artifacts on MRI.
6.Clinical value of non-invasive positive pressure ventilation in the therapy of acute pulmonary edema

Objective To investigate the change of HR,RR and arterial blood gas in the treatment of BiPAP ventilation in patients with acute pulmonary.Methods Fifty eight patients with acute pulmonary edema were randomized into two groups.The control group(n =29)were given conventional general treatment only,but treatment group(n =29)were given BiPAP ventilation besides conventional treatment.4 h later,heart rate (HR),respiratory rate(RR),SaO2,pH,PaO2 and PaCO2 were compared between the two groups.Hospitalization duration and incidence of invasive mechanical ventilation were recorded after discharge.Results Compared with pre-treatment,HR,RR,SaO2 and PaO2 in treatment group were improved significantly(HR 124 ± 12 beat/min vs 83 ±6 beat/min,t =5.372,P <0.01)(RR 37 ±5 beat/min vs 19 ± 8 beat/min,t =4.285,P <0.01)(SaO2 81.4% ±5.4% vs94.1% ±4.2%,t=2.731,P<0.05)(PaO2 53.2±5.4 mm Hg vs 89.1 ±8.5 mm Hg,t=5.763,P <0.O1).And these four indicators were also improved in control group after treatment,(HR 123 ± 10 beat/min vs 95 ± 8 beat/min,t =t =3.459,P < 0.01)(RR 36 ± 7 beat/min vs 24 ± 6 beat/min,t =3.127,P <0.01)(SaO2 81.8% ±5.7% vs 88.3 ±4.5%%,t =2.314,P <0.05)(PaO2 53.5 ±4.6 mm Hg vs 72.8 ±9.5 mm Hg,t =3.756,P <0.01).HR,RR,SaO2 and PaO2 in treatment group were more significantly improved than that of control group(P < 0.01 or P < 0.05).Hospitalization duration in treatment group was significantly shorter than that of control group(9 d vs 15 d,t =3.763,P < 0.01).The incidence of invasive ventilation were lower than that of control group too(but P > 0.05.Conclusion These results suggested that BiPAP ventilation can regulate HR RR and blood gas value to accetable levels,shorten hoptipitalization duration and reduce the incidence of invasive ventilation.It is proved to be an effective therapeutic technique in the treatment of acute pulmonary edema patients.
7.Randomized controlled trial ofYinggencaoformula on psoriasis vulgaris with blood-heat TCM syndrome

ObjectiveTo evaluate the clinical curative effect and security by performing the clinical study ofYinggencao formula in treatment of psoriasis vulgaris with blood-heat TCM syndrome.MethodsA total of 75 patients, diagnosed with Psoriasis Vulgaris Blood-heat RCM syndrome, were randomizedly divided into the treatmeat group with 39 patients and the control group with 36. The treatment group tookYinggencao formula twice daily, while the control group tookQingdai capsules three times daily. All the patients were treated 12 weeks. PASI scores were used as the main outcome and to estimate the curative effect rates.ResultsThe PASI scores of patients in the treatment group (6.97 ± 2.02vs. 16.88 ± 2.91;t=14.380,P=0.009) and the control group (13.14 ± 3.18vs. 17.49 ± 2.32;t=7.780,P=0.013) after treatment showed significantly lower than the scores before. The PASI scores showed significant difference between the two groups after the treatment (P=0.027). The total effective rate of treatment group was significantly higher than the control group (76.9%vs. 61.1%;χ2=5.120, P<0.05).Conclusions TheYinggencao formula therapy showed better effect thanQingdai capsules therapy in treatment of psoriasis vulgaris with blood-heat TCM syndrome.

Objective: To investigate the effects of adrenaline on pharmacodynamics and pharmacokinetics of bupivacaine for interscalene plexus block. Method:Sixteen ASA gradeⅠ-Ⅱadult patients undergoing shoulder or upper limbs operation were randomly divided into two groups (n=8)). 9.Determination of baicalin in Shuanghuanglian oral liquid on composite mercury film electrode based on chemical modified.

OBJECTIVEA novel Eu-PB composite mercury film electrode (GC/Eu-PB/MFE) based on chemical modified was developed for the determination of baicalin and the investigation of its the electrochemical behavior.
METHODCyclic voltammetry and differential pulse voltammetry were used.
RESULTThe adsorption characteristic of baicalin was observed in Britton-Robinson buffer solution (pH 6.0) at GC/Eu-PB/MFE. The reduction peak of baicalin was observed at -1.22 V (vs SCE) and peak current was linear to the baicalin from 8.0 x 10(-9) to 1.0 x 10(-6) mol x L(-1) (r = 0.9992, n=8). The limit of detection was 1.2 x 10(-9) mol x L(-1) (S/N = 3). The average recovery was 99.83%, with RSD of 2.3% (n=8). The baicalin in Shuanghuanglian oral can be directly determined without pretreatment.
CONCLUSIONThe GC/Eu-PB/MFE was used to determine baicalin in Shuanghuanglian oral with good repeatability and high sensitivity. The method can be used for the quality control of Chinese traditional medicines containing Scutellaria baicalensis.

10.Predicting clinical chemo-sensitivity of primary ovarian cancer using adenosine triphosphate-tumor chemosensitivity assay combined with detection of drug resistance genes

Objective To predict clinical chemotherapy sensitivity of primary ovarian cancer by jointing adenosine triphosphate(ATP) - tumor chemo-sensitivity assay(TCA) method in vitro and detection of drug resistance genes, provide reference for clinical treatment. Methods Forty-seven primary epithelial ovarian tumor samples were collected from the patients who received cytoreductive surgery. Viable ovarian cancer cells obtained from malignant tissue were tested for their sensitivity to carboplatin (CBP), cisplatin (DDP), paclitaxel(PTX) and CBP + PTX using ATP-TCA method in vitro; at same time, real-time quantitative PCR was used to analysis BRCA1 and ERCC1 mRNA relative expression in forty-six specimens (1 frozen tumor samples mRNA were not detected due to serious degradation). The relationship between ATP-TCA test results, clinical indicators, and the effectiveness of the joint prediction on clinical chemosensitivity by combining these two methods were statistically analyzed using chi-square test. Results (1)The results showns that three programs of DDP,CBP and PTX + CBP were significantly related with clinical results(P<0.05) in vitro, in which the compliance rate in PTX + CBP program was the highest 83%(39/47) ,and the predictive sensitivity, predictive specificity, positive predictive value, negative predictive value and predictive accurate rate were 90%,71%,84% and 80% ,respectively.PTX + CBP combined in vitro test results was also related with residual tumor size and neoadjuvant chemotherapy, which was more prone to drug resistance with residual tumor larger than 2 cm (P = 0. 023) and with neoadjuvant chemotherapy (P = 0.011). (2) BRCA1 mRNA expression levels in the clinical-resistant group and the clinical-sensitive group was 0.673 ± 2.143 and - 1.436 ± 2.594 (P=0.008), ERCC1 mRNA expression levels in the clinical-resistant group and the clinical-sensitive group was -0.529 ± 1.982 and - 3.188 ±2.601 (P =0.001). There were also significant correlation among the expression levels of BRCA1 ,ERCC1 mRNA and clinical efficacy (P<0.01). (3)ATP-TCA and detection of drug resistance genes combined to predict the clinical application of PTX + CBP resistance may occur in 8/9 cases. Conclusions ATP-TCA may be an ideal method of in vitro drug sensitivity testing method, which could effectively predict clinical chemotherapy sensitivity. Combination of the drug-resistant associated genes detection method and the ATP-TCA method can increase the predictive effectiveness of ovarian cancer chemosensitivity and guide individual chemotherapy of ovarian cancer.
