1.Study of radiation-induced cerebral vascular and carotid atherosclerosis in nasopharyngeal carcinoma patients with radiation-induced temporal lobe necrosis

Objective To investigate radiation-induced carotid and cerebral vascular injury and its relationship with radiation-induced temporal lobe necrosis in nasopharyngeal carcinoma (NPC) patients.Methods Fifty-eight NPC patients with radiation-induced temporal lobe necrosis (TLN) were recruited in the study.Duplex ultrasonography was used to scan bilateral carotid arterials to evaluate the intima-media thickness (IMT) and occurrence of plaque formation.Flow velocities of bilateral middle cerebral arteries (MCAs),internal carotid arteries (ICAs) and basal artery (BA) were estimated through Transcranial Color Doppler (TCD).The result were compared with data from 33 patients who were free from radiation-induced temporal lobe necrosis after radiotherapy and 29 healthy individuals,to study the relationship between IMT,occurrence of plaque and ventricular diastolic dysfunction.Results Significant differences in IMT,occurrence of plaques of ICAs and flow velocities of both MCAs and ICAs were found between patients after and healthy individuals (t =18.624,8.221,P <0.05;x2 =17.886,P <0.05;t =14.367,10.112,P <0.05).IMT had positive correlation with post radiation interval (r =0.368,P =0.049).IMT or incidence of plaques may be related to dose of radiotherapy,course of treatment and T stage (x2 =-17.635,12.006,-3.125,P < 0.05).Compared with result from patients without radiation-induced TLN,the mean IMT was significantly thicker in patients with TLN (t =10.208,P < 0.05).Plaques were more common in patients with TLN than patients without TLN (x2 =13.118,P <0.05).In addition,flow velocities of MCAs and ICAs in patients with TLN were much faster (t =5.011,5.035,P < 0.05).Among patients with unilateral TLN,flow velocity of MCAs was significantly different between ipsilateral and contralateral sides to the lesion (t =18.362,P < 0.05).Conclusions Thickening of IMT,occurrence of plaque formation and hemodynamic abnormality are more common in patients after radiotherapy,especially in those with TLN,compared with healthy individuals.
2.Predicative value of radiomics nomogram based on 18F-FDG PET/CT for the prognosis of patients with postoperative gastric carcinoma

Objective To investigate the clinical value of radiomics nomogram,which is established by 18F-fluorodeoxyglucose (FDG) PET/CT radiomics signature combined with clinical-pathologic risk factors,in predicting the prognosis of patients with postoperative gastric carcinoma.Methods 18F-FDG PET/CT data of 207 patients (143 males,64 females,age range:20-85 years) with postoperative gastric carcinoma from January 2008 to August 2015 was reviewed retrospectively.Patients were divided into training group (n=104) and validation group (n =103),and the clinicopathologic information and disease-free survival (DFS) data were acquired.Significant textural features were selected from PET/CT images,and radiomics score (RS) for individual patient was calculated based on the radiomics signatures.The relationship between RS and DFS was analyzed.Cox regression analysis was performed to determine the risk factors ofDFS.The radiomics nomo-gram,obtained from combination of RS with clinicopathologic risk factors,was established and further evaluated in predictive value for recurrence or metastasis of postoperative gastric carcinoma,and the concordance index (C-index) was calculated.Results Cox regression analysis demonstrated that RS,tumor location,depth of invasion,lymph node metastasis,and distant metastasis were the significant risk factors for DFS (hazard ratios:2.148-2.828,all P<0.05).The radiomics nomogram combined with RS and 4 clinicopathologic risk factors had a better prediction for the estimated DFS,comparing to RS alone.C-index of radiomics nomogram and RS were 0.830 and 0.700 in training group,and 0.776 and 0.681 in validation group,respectively.Conclusion Radiomics nomogram which is established by radiomics signatures and clinicopathologic risk factors may be better for predicting DFS of patients with postoperative gastric carcinoma.
3.Relationship between insomnia and suicidal ideation in adolescents with depressive disorder: the mediating role of rumination

ObjectiveTo explore the mediating effect of rumination on the relationship between insomnia and suicidal ideation in adolescents with depressive disorder, so as to provide guidance for the preventions of suicidal ideation. MethodsA total of 302 adolescents with major depressive disorder in Department of Psychosomatic Medicine of Deyang People's Hospital from January to December 2020 and met the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) clinical significance criterion for depressive disorder were enrolled. All individuals were assessed using Insomnia Severity Index (ISI), Ruminative Response Scale (RRS) and Positive and Negative Suicide Ideation (PANSI). Then the mediating effect of rumination in the relation between insomnia and suicidal ideation in adolescents with major depression disorder was analyzed using Process v3.2 model 4, and testified with the bias-corrected non-parametric percentile Bootstrap method. ResultsISI scores in adolescents with major depression disorder were positively correlated with PANSI score (r=0.400, P<0.01), and were positively correlated with RRS total score, obsessive thinking and introspection factor score (r=0.378, 0.360, 0.333, P<0.01). RRS score was also positively correlated with PANSI score (r=0.292, P<0.01). Rumination exerted a partial mediating effect on the relationship between insomnia and suicidal ideation (β=0.174, 95% CI: 0.098~0.261). ConclusionThe relationship between insomnia and suicidal ideation in adolescents with depressive disorder is partially mediated by rumination, additionally, insomnia can affect suicidal ideation in adolescents with depressive disorder both directly and indirectly through rumination.
4.The diagnostic value of endoscopic ultrasound-guided fine needle aspiration for mediastinal and abdominal lymphadenopathy

Objective To study the diagnostic value and clinical efficacy of endoscopic ultrasound-guided fine needle aspiration ( EUS-FNA ) for mediastinal and abdominal lymphadenopathy. Methods Thirty patients who underwent EUS-FNA for mediastinal or abdominal lymphadenopathy between May 2009 and December 2015 were reviewed. The clinical efficacy of EUS-FNA was evaluated by pathological results and the follow-up. The EUS-FNA effect on clinical decision was also analyzed. Results Lesions were located in the mediastinum in 10 cases and in the abdomen in 20 cases. The total diagnostic accuracy, sensitivity, specificity, positive predictive value ( PPV) and negative predictive value ( NPV) of EUS-FNA were 96. 7%, 94. 7%, 100. 0%, 100. 0% and 91. 7%, respectively. Of all the 30 cases, 20 lymph glands were of unknown origin. The diagnostic accuracy, sensitivity, specificity, PPV and NPV of EUS-FNA in these lesions were 95. 0%, 88. 9%, 100. 0%, 100. 0% and 91. 7%, respectively. The combination of cytological and histological examination had higher accuracy ( 96. 7% VS 73. 3%, P=0. 026) and sensitivity ( 94. 7%VS 57. 8%, P= 0. 019 ) than cytological examination only. Immunohistochemistry stains were performed in 12 neoplastic cases, and 11 ( 91. 7%) were confirmed. The diagnosis by EUS-FNA had positive impact on clinical decisions in 27 patients ( 90. 0%) . Conclusion EUS-FNA is an effective approach for mediastinal and abdominal lymphadenopathy, and the result has a positive impact on clinical decisions. The combination of cytological and histological examination and application of ancillary techniques, such as immunohistochemistry stains, can improve the diagnostic efficacy of EUS-FNA.
5.Comparison of two luminescence detection methods for staphylococcal enterotoxin C content in simulated milk samples.

Objective To compare the sensitivity and accuracy of amplified luminescent proximity homogeneous assay linked immunosorbent assay (AlphaLISA) and magnetic particles-based chemiluminescence immunoassay (MP-CLIA) for detection of staphylococcal enterotoxin C (SEC) in the simulated milk samples. Methods The AlphaLISA was constructed using goat anti-SEC polyclonal antibody-coupled receptor microspheres, biotin-labeled SEC monoclonal antibody and streptavidin-coupled donor microspheres. The MP-CLIA was constructed using goat anti-SEC polyclonal antibody conjugated alkaline phosphatase, biotin-labeled anti-SEC monoclonal antibody and streptavidin conjugated magnetic beads. Results The sensitivity of AlphaLISA to detect SEC content in simulated milk samples was 4.04 ng/L, and the coefficient of variation (CV) was 1.98%~9.82%. The sensitivity of MP-CLIA was 108.19 ng/L and CV was 4.63%~20.40%. Conclusion Compared with MP-CLIA, AlphaLISA is more sensitive and accurate to detecting SEC.
