1.Inhibition of airway inflammation of asthmatic mouse by intravenous injection of T-bet recombinant adenovirus

Objective To investigate the inhibitory effects of recombinant T-bet adenovirus (AdT-bet) on airway inflammation in mice with asthma, and to explore the relevant mechanisms. Methods C57BL/6 mice were randomly allocated into 4 groups with 12 mice each. The asthmatic model was reproduced in group A, B and C by administration of ovalbumin (OVA) in aluminum hydroxide. Mice in group A, B and C received 50?l AdT-bet (108pfu) and AdLacZ and PBS, respectively, 19 days prior to OVA challenge. Bronchoalveolar lavage fluid (BALF) was collected on the 26th day after the induction of asthma for the determination of the cellular composition and concentrations of IL-4, IL-5 and IFN ?. Meanwhile, IgE content in serum was determined. The pathological changes and the expression of GATA-3 in the lungs of these mice were investigated. Results The percentage of eosinophils (EOS) and concentrations of IL-4 and IL-5 in BALF of group A were 0.6%?0.2%, 6.8?3.7pg/ml, 12.5?4.8pg/ml, respectively, which were significantly lower than those in group B (20.9%?6.8%, 92.4?23.0pg/ml, 56.5?11.8pg/ml) and group C (20.8%?6.7%, 90.4?22.8pg/ml, 57.3?12.2pg/ml, P
2.Orthotopic heart transplantation for 5 cases of end-staged coronary heart disease

Objective To summarize the early effect of orthotopic heart transplantation for 5 patients with end-staged coronary heart disease.Methods Orthotopic heart transplantations were performed on 1 patient with left ventricular mechanic circulatory support for 25 months after twice acute myocardial infarction, 3 patients with failing heart after acute myocardial infarction, 1 patient with failing heart after PTCA and CABG.Results Five patients recovered well. No any severe acute rejection and infection have been found. All survivors had good life quality and good heart function (NYHA I).Conclusion Orthotopic heart transplantation is an effective treatment for patients with end-staged coronary heart disease. Proper donor heart, excellent donor myocardial conservation, suitable immurosuppression treatment and appropriate control of hypertension, hyperglycemia, hypercholesterolemia and uricacidemia are key measures of successful orthotopic heart transplantation for patients with end-staged coronary heart disease.
3.Clinical features of hyperuricemia and relationship between serum uric acid and metabolic syndrome in health examination of population

ObjectiveTo evaluate the clinical features of hyperuricemia (HUA) and relationship between serum uric acid (SUA) and metabolic syndrome (MS) in adults.MethodsData were gathered from medical examination among adults age ≥ 20 years,and totally 18 862 subjects were analyzed in this study.ResultsThe prevalence of HUA was 14.8% (2788/18 862).The rate was 18.4%( 1926/10 445) in men,10.2% (862/8417) in women.The prevalence of HUA was increased with age,there was significant difference among different age (P =0.000),the rate in men was higher than that in women (P < 0.01 or <0.05 ).Compared with normal SUA Patients,the levels of body mass index(BMI),systolic blood pressure (SBP),diastolic blood pressure (DBP),total cholesterol (TC),triacylglycerol (TG) and lowdensity lipid cholesterol(LDL-C) were markedly elevated (P <0.01 or <0.05),and the level of highdensity lipid cholesterol (HDL-C) was markedly decreased in HUA patients (P <0.05).Stepwise regression analysis showed that BMI,SBP,DBP,fasting plasma glucose (FPG),TG,HDL-C were the independent correlation factor of SUA.The incidence of hypertension,over weight/obesity,dyslipidosis,hyperglycosemia and MS in HUA patients [24.1% (672/2788),36.2% (1009/2788),31.2% (870/2788),18.0% (502/2788),20.8%(580/2788) ] was significantly higher than that in normal SUA patients [ 18.2% (2925/16 074),26.4%(4244/16 074),22.0%(3536/16 074),15.00(2411/16 074),17.0%(2733/16 074),P< 0.01 ].The level of SUA in patients with MS [ (342.3 ± 64.2 ) μ mol/L ] was higher than that of those without MS[ (319.3 ± 67.1 )μ mol/L,P =0.000].ConclusionsThe prevalence of HUA is increased with age.Multiple factors of dysbolism are clustered in subjects with HUA.There is correlation between SUA and MS.
4.Simvastatin for lung fibroblast function and its inhibitory pathway in rats

BACKGROUND: Statins can block many intracellular signal transductive pathways and suppress the proliferation of various cells by affecting the synthesis of mevalonic acid and the translation following modification of some membrane-connecting proteins.OBJETCIVE: To investigate the influence of simvastatin on the proliferation of lung fibroblasts, the synthesis of collagen and the secretion of matrix metalproteinase-2 (MMP-2).DESIGN:Completely randomized controlled study.SETTING: At Organ Transplanting Research Institute of Fuwai Cardiovasular Diseases Hospital, Peking Union Medical College.MATERIALS: This study was carried out at the Laboratory of Cardiology of Beijing Union Medical College hospital, Peking Union Medical College from June 2004 to October 2004. Lung fibroblasts derived from neonatal SD rats were co-cultured in vitro with different dosage of simvastatin of 0, 1, 5, 10,50 μmol/L, and 50 μmol/L simvastatin + 200 μmol/L mevalonic acid.METHODS: Lung fibroblasts deriving from neonatal SD rat were co-cultured with different dosage of simvastatine in vitro. Methyl thiazolyl tetrazolium colorinetry was used to detect the cell proliferation, and cell immunohistochemical assay was used to determine the collagen synthesis and meanwhile,MMP-2 content in supernatant was examined with enzyme-linked immunosorbent assay.MAIN OUTCOME MEASURES: The proliferation of fibroblasts, the synthesis of collagen and the secretion of MMP-2 due to different dosage of simvastatin intervention and simvastatin combined with mevalonic acid.presenting the expression of type Ⅰ, Ⅲ of collagen in lung fibroblasts and the level of MMP-2 in 5, 10, 50 μmol/L simvastatin group were obviously lower than those of 0 μmol/L simvastatin group (0. 520 ± 0.010, 0. 334 ± 0.011,0.260±0.012, 0.111±0.011; 0.508±0.011, 0.324±0.014, 0.232±0.015, 0. 083 ±0. 015; 0.445 ±0. 017, 0. 305 ±0. 015, 0.216 ±0. 015,0.068±0.012; 0.561±0.013, 0.361 ±0.012, 0.289±0.012, 0.140value, the mean absorbency( A value) in lung fibroblasts and the level of MMP-2 in 50 μmol/L simvastatin + 200 μmol/L mevalonic acid group were obviously higher than that of the 50 μmol/L simvastatin group(0. 567±0.015, 0.354±0.014, 0.283±0.012, 0.138±0.011, t=4.715-10.950, P < 0.01).CONCLUSION: Simvastatin could suppress the fibroblast proliferation and collagen synthesis, attenuate the secretion of MMP-2 and suppress consequently the adhesion and migration of lung fibroblasts; moreover, it has the capability of anti-cell proliferation by affecting the mevalonic acid pathway.
5.Protective effect of inhibiting the activation of nuclear factor-?B by dexamethasone on donor rat lung

Objective To observe the protective effect of dexamethasone, the inhibitor of nuclear factor-Kappa B, on the ischemia-perfusion injury of rat lung during the period of lung preservation. Methods Twenty-four rats were randomly divided into two groups: the control group and the trial group. The harvested lung blocks were flushed with and stored in the low-potassium-dextran (LPD) solution in control group, but in the trail group LPD containing dexamethasone solution was used. The lungs were stored at 4 ℃ for 16 h in both groups. The isolated rat lung reperfusion models were established and the donor lungs were perfused for 1 h. PaO_2 and PawP were measured at every 15 min intervals during reperfusion. After reperfusion, the lung tissue wet-to-dry (W/D) ratio and myeloperoxidase (MPO) activity were obtained. The protein and mRNA expression of intercellular adhesion molecule-1 (ICAM-1), nuclear factor-Kappa B (NF-?B) was also detected by using immunohistochemistry and semi-quantitative RT-PCR at the end of reperfusion. 6.Lower respiratory tract infection caused by Chryseobacterium and its susceptibility to drugs

Objective: To investigate the characteristics of lower respiratory tract infection caused by Chryseobacterium and its antimicrobial susceptibility in vitro . Methods: The clinical data of 52 cases of lower respiratory tract infection caused by Chryseobacterium were analyzed, and the antimicrobial susceptibility of commonly used agents was determined with the method of agar dilution. Also the 50% lethal dosage (LD 50 ) (as the marker of virulence) of 20 randomly selected Chryseobacterium strains for mice were determined. Results: (1) Thirty six was over 60 years old;all of 52 cases had underlying diseases, mainly were chronic obstruction pulmonary disease and malignant tumors. Seventeen cases had the history of incubation or tracheotomy for mechanical ventilation, and 35 had history of broad antibiotics treatment. The mean hospitalization time before infection were 35.6 d, and 38.5% of the cases had mixed infection with other bacteria. No specific clinical manifestations and chest X ray appearance revealed. (2) The in vitro activity of 25 agents showed that these strains were highly resistant. (3) The range of the LD 50 of tested strains was 4.11?10 6 5.68?10 8/mouse, suggesting low virulence of this kind of bacteria. Conclusion: The lower respiratory tract infection caused by Chryseobacterium has no unique features; the incidence of the infection increases in immunosuppressed old patients with various underlying diseases, although the virulence is relatively low. Because the clinical isolates are highly resistant, the antibiotics should be selected according to the results of bacterial sensitivity test.
7.Sacroiliac joint:a study of sectional anatomy and puncture pathway

Objective To investigate the best pathway and safety of sacroiliac joint (SIJ) puncture.Methods The anticorrosive adult specimens were frozen after SIJ CT scan.Horizontal or coronary sectioning was done.The horizontal sectioning was done corresponding to CT scan.The structures of different sections were observed and compared with the corresponding CT scans.Results According to the horizontal sectioning,the SIJ included synovial portion and ligamentous portion.As shown by CT,only part of the space between sacrum and ilium was the synovial portion,which occupied the whole part of the lower 1/3 segment SIJ,and the posterior was not covered with bones.There are no important nerves and blood vessels passing through the route from the buttock to the posterior of SIJ.Thus,the pathway can be regarded as the best and safe one although the lumbosacral trunk and iliac blood vessels go through the anterior of SIJ in the pelvis.Conclusion The best pathway of SIJ puncture is the upper part of the lower 1/3 segment.The safety of puncture depends on not penetrating the pelvic cavity to avoid damaging the structures in pelvis.

9.Interleukin-12 blocking GATA-3 expression in treatment of bronchial asthma:an experimental study

Objective:To observe the expression of GATA-3in asthmatic mice lungs and to explore the feasibility of in-terleukin(IL-12)blockading GATA-3expression in treatment of bronchial asthma.Methods:C57BL/6mice were randomly divided into3groups:control group(group A),asthmatic model group(group B)and IL-12injection group(group C).Asth-matic models were established in group B and C.Normal saline(0.1ml)and IL-12(1?g)were injected in group B and C re-spectively on the1,3,7,9,25,26,27and28d.Six mice from each group were obtained for analyses of bronchoalveolar lavage fluid(BALF)on d31.The airway pathology changes and the expression of GATA-3were observed by hematoxylin/eosin(H-E)and immunohistochemistry respectively.Results:There was no symptom in group A,and the symptoms of group B were more severe than those of group C.Eosinophil(EOS)was not seen in the BALF of group A,while EOS in group B and group C were(20.0?4.0)%and(0.2?0.1)%respectively.In the same microscopic visual field,there was no inflam-mation cell in group A and a large number of inflammation cells in group B,but inflammation cells in group C were signifi-cantly decreased.Immunohistochemistry showed that the expression of GATA-3in group B was strong and no GATA expres-sion was found in group A and group C.Conclusion:The expression of GATA-3in asthmatic mice is high;IL-12can inhibit asthmatic airway and lungs inflammtion,whose mechanism may be the blockade of GATA-3expression in asthmatic models.[
10.Reoperations on Ebstein's anomaly:Report for 17 Cases

Objective To summarize the experiences of reoperations on Ebstein's anomaly. Methods Clinical data of 17 cases of Ebstein's anomaly (6 males and 11 females, aged from 3 to 54 years old, averaged 23) were analyzed retrospectively. All the patients felt short breath and palpitation after exertion, and cyanosis was found in two patients. In the first operation, downwards displaced tricuspid valve leaflet was suspended and atrialized right ventricle was replicated in 14 cases, tricuspid anuloplasty and atrial septal defect were repaired in 2 cases, and tricuspid valve was replaced in 1 case. The interval time between two operations was 1-20 years in an average of 8 years. Before second operation, the cardiac function was NYHA Ⅱ in 6 cases, NYHA Ⅲ in 11 cases. Eight cases received Ebstein's anomaly anatomic correction, including transplanting downward displaced tricuspid valve and excising atrialized right ventricle. Seven cases received tricuspid valve replacement. Two cases received tricuspid valveplasty. All of operations were performed on arrested heart with moderate hypothermic cardiopulmonary bypass. The time of cardiopulmonary bypass was 70-287 (89.3?11.1)min, the time of aorta clamping was 70-287 (64.0?8.6)min. The transesophageal echocardiography was routinely performed before the operations and after the hearts rebeating. Results No case died in hospital, and all the patients were followed up for 1-3 years. Transesophageal echocardiography showed the tricuspid valve was closed well (non-reflow in 9 cases, mild reflow in 1 case) in 10 cases which received Ebstein's anatomic correction and tricuspid valveplasty. The cardiac function states of patients were significantly improved (NYHA Ⅰ-Ⅱ). Conclusion Most of patients with reoperations on Ebstein's anomaly can received anatomic correction. Tricuspid valve replacement is available if the pathology of tricuspid valve is severe.
