1.Fifteen Years' Blood Pressure Change in 1079 Workers

Aim To study the change of blood pressure in 1079 workers for fifteen years and the effect of anti-hypertension drugs therapy. Methods We investigated the blood pressure of 1079 workers who were enrolled in six organizations in the year 1983 and 1998 respectively. Results During the 15 years: (1)Mean value of blood pressure: systolic blood pressure increased 22 mmHg in man and 16.9 mmHg in women; diastolic blood pressure increased 9 mmHg in man and 12.7 mmHg in women (P<0.05);(2)The incidence of hypertension increased by 25.03% in man and 28.28% in women;(3)The prevalence rate of hypertension is 27.9 percent in people with initially normal blood pressure (1.86%/y) and 72.6 percent in initially broder line hypertension (4.84%/y);(4)The control rate of hypertension is 2.9 percent;(5)The incidence of stroke is highly related to hypertension (P<0.001);(6)46.6 percent patients had a regular drug therapy, mainly reserpini complex(25.2%);(7)Drug therapy has no obviously effect of the control of hypertension and the incidence of stroke. Conclusion Both the mean value of BP and the incidence of hypertension were increased with age. The control rate of hypertension was low and the drug therapy shows little advantage. We should do mach more works to popularize the knowledge of the prevention of hypertension. Improve people's self-prevention. Regular cheek should be given to the hypertension patients.
2.A qualitative study of self-management dilemmas in adolescents with type 1 diabetes mellitus transitioning to adult medical care

Objective:Exploring the dilemmas faced by adolescents with type 1 diabetes mellitus in self-management during the transition to adult healthcare, to inform healthcare professionals in the delivery of transitional care interventions.Methods:Using descriptive qualitative research methods, in-depth interviews were conducted from August to December 2023 to adolescents with type 1 diabetes mellitus (selected by objective sampling) who met the inclusion and exclusion criteria in the Department of Endocrinology of the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine and the Shenzhen Children′s Hospital, and the data were analysed inductively using directed content analysis.Results:A total of 12 adolescents with type 1 diabetes mellitus were included for interview, 6 males and 6 females, aged 12-18 years. The self-management dilemmas for adolescents with type 1 diabetes in the transition to adult healthcare can be summarised into 4 themes and 10 sub-themes: disease management dilemmas (weak awareness of disease management, insufficient disease-related knowledge and competence, low adherence to disease management, and restriction of daily activities), role adaptation difficulties (lack of perceived transition to the healthcare role, difficulty in transitioning to the family role, and maladaptation to the social role), negative emotions Inadequate social support system (lack of school support, limited accessibility of social organisations, insufficient socio-economic support).Conclusions:Adolescents with type 1 diabetes mellitus face multiple dilemmas in disease self-management during the transition to adult healthcare, and healthcare professionals should pay attention to transition self-management education, improve patients′ awareness of transition self-management and their ability to participate in it, formulate a transition preparation plan, build an emotional support network, and improve the social security system, so as to help juvenile patients with type 1 diabetes master the method of self-management and make a successful transition to adulthood.
3.Chinese version of Decisional Balance Scale for Diabetes Diet and its reliability and validity test

Objective:To translate the Decisional Balance Scale for Diabetes Diet (DB-DD) into Chinese, and test its reliability and validity.Methods:After obtaining the authorization of the author of the source scale, the scale was sinicized by translation and back translation method. After expert consultation, cognitive interview and pre-survey for cultural adjustment, the Chinese version of DB-DD scale was formed. From September 2020 to January 2021, a total of 496 patients with type 2 diabetes mellitus in endocrinology department of 4 Class Ⅲ Grade A hospitals in Guangdong province were surveyed by the convenient sampling method to evaluate the reliability and validity of the scale. A total of 496 questionnaires were sent out and 440 were effectively received, with the effective recovery of 88.7%.Results:There were 12 items in the Chinese DB-DD, including 2 dimensions of benefit (6 items) and damage (6 items) . The content validity index of the total scale was 0.98, and the content validity index of the item level was 0.86 to 1.00. The correlation coefficients of the Chinese version of DB-DD total scale, benefit subscale and impairment subscale and diabetes self-management behavior scale were 0.413, 0.222 and -0.221, respectively ( P<0.001) . The Cronbach's α coefficients of the total scale and each subscale were 0.730-0.899 and the split-half reliability coefficients were 0.725-0.856. The test-retest reliability coefficient of the total scale was 0.916 and the test-retest reliability coefficients of the benefit and damage subscales were respectively 0.933 and 0.902. Conclusions:The Chinese version of Decisional Balance Scale for Diabetes Diet has good reliability and validity, which can be used to evaluate the dietary decision balance of diabetic patients in the Chinese cultural context.
4.Intelligent management of hospital drugs and consumables through Internet of things

Under the policy background of zero addition of drugs and consumables in public hospitals, with clinical practice as the guide and information platform as the means, the hospital constructed an intelligent management system for hospitals′ drugs and consumables through the Internet of things. The hospital adopted the intelligent medicine cabinet extended ward management, consumables " one material, one code" two-level warehouse management mode, reengineering the material supply process.Finally, it can realize the whole process traceability of information, unify the flow of goods and data, save human resources, improve the efficiency of operation and management, and achieve the purpose of ensuring safety, reducing costs and increasing benefits.
5.Status quo of ICU nursing and standardized training for ICU nurses in China

Objective?To understand the status quo of the construction of ICU, ICU nursing staff and standardized training, so as to provide evidence for improving the construction of critical care medicine and promoting the development of ICU nursing in China. Methods?In the form of network questionnaire, the director of nursing department or head nurse of each province and city filled in the questionnaire on the current situation of ICU nurses in China in 2016. Results?A total of 1 956 hospitals and 2 747 ICU in 30 provinces and cities of China participated this survey. The number of ICU in He'nan Province was the largest, accounting for 9.87%, and the number of ICU in Class Ⅲ Grade A hospitals accounted for 58.50%. ICU in comprehensive hospitals accounted for 91.99% of the total hospitals. The comprehensive ICU in China accounted for 68.37%. The total number of ICU qualified nurses in different provinces and cities was 70 517. The number of nurses in Guangdong Province was the largest, accounting for 15.58% of the total. The nurses in the Class Ⅲ Grade A hospital accounted for 70.83% of the total. Among them, the proportion of male nurses was 9.05%, and the number of ICU nurses' turnover was 8.61%. 3 504 ICU head nurses were surveyed, the age range was 19.0 to 56.0 years old, the mean age was (38.73±5.38), the professional title was mainly nurse-in-charge, accounting for 62.39%, and the proportion of bachelor's degree head nurses was 82.48%; 4 163 people attended the Chinese Nursing Association ICU Specialty Nursing Certification and 12 022 people attended the provinces/autonomous regions/municipalities directly under the central government specialty nursing certification; after participating the ICU specialty nurse training, 36.21% nurses were going to be the group leader, 27.05% be the teaching managers; the teacher of specialist nurse training were mainly nurse-in-charge, and the proportion of nurse-in-charge was 55.10%.Conclusions?With the continuous development of ICU in China, the construction of ICU nursing staff and the standardized training of ICU specialist still need to be improved.
6.Expert consensus on postoperative care of patients with a left ventricular assistant device

Objective To standardize nursing management on postoperative patients with a left ventricular assist-ant device(LVAD).Methods The first draft of the Consensus was formed on the basis of literature review.2 rounds of expert consultations and a round of online meeting discussion were held for adjustments and modifications the draft of the Consensus.Results The recovery rate of the inquiry questionnaire was 93.75%.The authority coefficients of the 2 rounds of inquiry experts were 0.927 and 0.920.The concentration degree of expert opinions for each indicator was greater than 3.5 score,and the coefficient of variation was less than 0.25.The Kendall harmony coefficients for 2 rounds of correspondence were 0.402 and 0.407(P<0.01).The final Consensus formed through expert consultations and meetings includes 7 themes:hemodynamic monitoring,LVAD function monitoring,coagulation function monitoring,percutaneous cable and wound care,exercise rehabilitation care,health education and guidance,and pre-discharge assessment.Conclusion The Consensus is scientific,rigorous,and authoritative.The Consensus covers all aspects of postoperative care for patients with LVAD,and it will benefit to clinical practice.
