1.Application of multi-disciplinary team in the diagnosis and treatment of metastatic colorectal cancer

The traditional single-disciplinary treatment has been transformed into a multi-disciplinary comprehensive treatment,because of the further understanding of oncology.Multi-disciplinary team (MDT) is the representative of this change.MDT has a unique model of organization and conduction.It improves the prognosis of patients through multi-disciplinary conference and standardized individualized treatment.MDT has been applied in metastatic colorectal cancer for a long time,and remarkable achievements have been made.With the development of molecular biology and the application of new targeted agents,MDT has also been progressing to adapt to the trend of modern medicine.

3.Tremella Polysaccharides attenuated sepsis through inhibiting abnormal CD4+CD25 high regulatory T cells in mice

Objective:To determine the effects of TPS on peripheral blood Tregs in sepsis mouse induced by burn plus P.aeruginosa infection.Methods: The experimental mice were separated into five groups randomly ,including sham burn group ,burn plus P.aeruginosa infection group ,burn plus P.aeruginosa infection with TPS (50,100,200 mg/kg) treatment group.Peripheral blood Tregs were isolated with Magnetic Microbeads and cultured in vitro from the day after burn (PBD0) to 4 days after burn(PBD4).IL-10, IFN-γ,IL-4 levels in Tregs culture supernatants were determined by sandwich enzyme-linked immunosorbent assays ( ELISA ) . Purification of CD4+CD25high Tregs and CD4+T cells in C57BL/6 mice were administrated by magnetic beads sorting .Tregs and CD4+T cells were cultured in vitro after joining TPS to without TPS cells as a control .The phenotypes of Tregs were analyzed by flow cytometry , and cytokines were measured by ELISA .Results:Vis-a-vis the results of the untreated group ,TPS could markedly decrease IL-4 and IL-10 secretion level and significantly increase the secretion of IFN-γ,and the secretion of IL-10 level and concentration of TPS dose effect.Vis-a-vis the results of the untreated group ,in vitro experiment ,without stimulation of TPS ,CD4+T cell proliferation and IFN-γwere significantly reduced ( P<0.05 ) and IL-4 levels increased significantly;CD4+T cell proliferation and IFN-γwere significantly increased and IL-4 levels were significantly reduced in the group of TPS with antibody-1;there was no significant difference in CD 4+T cell proliferation and the levels of IFN-γand IL-4 in the group of TPS with antibody-2.Conclusion:TPS could inhibit the abnormal ac-tivities of CD4+CD25highTregs in burn with P.aeruginosa infection mice,at least in part via inhibiting IL-10 secretion,and trigger a shift of Th2 to Th1 with activation of CD4+T cells in burn with P.aeruginosa infection mice.
4.Multidisciplinary team improves the comprehensive quality of colorectal surgeons.

Colorectal cancer is a systemic disease that requires multidisciplinary treatment. The comprehensive quality of colorectal surgeon directly impacts on the efficacy of diagnosis and treatment of colorectal cancer. Multidisciplinary teams help surgeons enhance their ability of evidence-based medicine, improve the quality of main specialty, expand the knowledge of other specialty, enhance the doctor-patient communication, and increase the research level. Thus, multidisciplinary teams can improve the comprehensive quality of colorectal surgeons.

5.Biomechanical experimental study on brace with stiletto needle therapy for scoliosis.

OBJECTIVE:
To investigate clinical biomechanical principle of brace with stiletto needle therapy for scoliosis.
METHODS:
Based on design ideas of teasing needle therapy, building an experimental mechanical model was built, seven specimens with scoliosis were chosen, and treated by brace therapy and then added to stiletto needle therapy.
RESULTS:
The two experimental mechanical model methods could predict load of scoliosis by stiletto needle therapy, and was verified accuracy and effectiveness of model. The degree of initial scoliosis of 7 patients was (59.7±3.37)°, improved to (49.57±2.79)° by correction of brace, and (39.43±1.94)° by correction of brace with stiletto needle therapy, had significant differences(<0.05). Lateral distraction force of thoracolumbar fossa from scoliosis as V, compressive force of scoliosis as T, brace with stiletto needle therapy could save effort for 45% to 46% than that of brace, while running torque Mw and compressive torque Mv could save effort about 45% to 47%, save effort of tension torque MT of muscle and ligament for 52%, and had statistical difference(<0.05).
CONCLUSIONS
Experimental biomechanical model of teasing needle therapy confirmed that the therapy could significantly reduce Cobb angle, improve correction efficiency of brace and beneficial for correction effect. It is an effective treatment for scoliosis.

6.Studies on phenolic compounds from Polygonum aviculane.

OBJECTIVETo study the chemical constituents from the aerial parts of Polygomun aviculane.
METHODThe chemical constituents were isolated by silica gel column chromatography and preparative silica thin layer chromatography, and their structures were elucidated on the basis of physico-chemical evidences and spectroscopic analysis (IR, MS, 1H and 13C-NMR).
RESULTSeven phenolic compounds were identified as rosmarinic acid (1), gallic acid (2), gentisic acid 5-O-(6'-O-galloyl)-beta-D-glucopyranoside (3), caffeic acid (4), p-coumaric acid (5), ethyl caffeate (6) and acteoside (7), respectively.
CONCLUSIONCompounds 1, 3, 6 and 7 were isolated from this plant for the first time. These results provided theoretical evidences for the further bioactive investigation on this plant.

7.Effect of trauma control concept on severe multiple injuries

Objective To explore the effect of trauma control concept on severe multiple injuries.Methods From March 2016 to March 2018,74 emergency patients with severe multiple injuries were selected in Yiwu Central Hospital.The patients were divided into the observation group and the control group according to the digital table method,with 37 cases in each group.The control group was treated with one-time surgical treatment,and the observation group was given treatment measures for severe multiple trauma in emergency department under the concept of trauma control.The recovery time of body temperature,the recovery time of prothrombin (PT),the time of shock correction,the occurrence of complications,and the therapeutic effect were compared between the two groups.Results The body temperature recovery time,PT recovery time and shock correction time in the observation group were (6.73 ± 3.29) h,(5.18 ± 1.89) h and (5.84 ± 3.14) h,respectively,which were shorter than those in the control group [(9.85 ± 1.62) h,(9.86 ± 2.41) h,(8.45 ± 0.87) h],and the differences were statistically significant (t =5.175,9.295,4.873,all P <0.05).The incidence rate of complications in the observation group was 8.11% (3/37),which was lower than that in the control group [32.43% (12/37)],the difference was statistically significant(x2 =6.773,P < 0.05).The cure rate of the observation group was 97.30% (36/37),which was higher than that of the control group [91.89% (34/37)],but there was no statistically significant difference (P > 0.05).Conclusion The treatment effect of severe multiple injuries under the concept of trauma control is obvious.It is an effective and feasible method,which can effectively reduce the complications and fatality rate.
8.Robotic surgery in colorectal cancer: present and future.

After more than a decade of development, the role of robotic rectal cancer surgery has been established. However robotic right hemicolectomy has just commenced. For rectal cancer surgery, the Da Vinci system is more flexible, accurate, and stable than traditional laparoscopy. The robotic surgery is safe and efficient, and protects the pelvic autonomic nerve function better. At the same time, robotic surgery is easy to learn, promoting its popularization. In robotic right hemicolectomy, the surgery suitable for Da Vinci system is also improved. Da Vinci system is not perfect. High cost is a major obstacle to the wide applications. Lack of tactile feedback and limited mobility of robotic arms also should not be ignored. The next generation of robotic system may make up for these deficiencies through a number of new technologies. The combination of robotics with single port laparoscopic techniques and multi-organ resections is expected to expand the advantages of robotic surgery in colorectal cancer and promote its development.

9.Overall management strategies for colorectal cancer patients during the COVID-19 outbreak

The Corona Virus Disease 2019 (COVID-19) since December, 2019 has a wide range of infection due to the strong infectious characteristics. Both medical staff and patients are at increased risk of infection. It is an urgent clinical problem for specialist doctors to work with diagnosis and treatment of cancer patients during the epidemic situation. Based on the colorectal cancer diagnosis and treatment guidelines (2019 CSCO guideline), combined with their own experience, the authors propose the overall management strategies for colorectal cancer patients. This strategies cover the key diagnosis and treatment of colorectal cancer, and provide targeted clinical practice. These work will be helpful for colorectal cancer specialists to carry out the diagnosis and treatment of colorectal cancer effectively under the epidemic of COVID-19.
10.Inhibition of subicular seizure-labeled c-fos+ neurons alleviates cognitive deficit in epilepsy

OBJECTIVE Cognitive deficit is a com-mon comorbidity in temporal lobe epilepsy(TLE)and that is not well controlled by current therapeutics.Currently,how epileptic seizure affects cognitive performance remains largely unclear.The subiculum is the major out-put of the hippocampus,which projects to entorhinal cor-tex and other more distinct brain regions.Physiologically,the subiculum codes spatial working memory and naviga-tion information including place,speed,and trajectory.Importantly,prior studies have noted the importance of the subiculum in the beginning,spreading,and generaliz-ing process of hippocampal seizure.How seizure-activated neurons in subiculum participate in cognitive impairment remains largely elusive.METHODS In this study,we sought to label the subicular seizure-activated c-fos+ neu-rons with a special promoter with enhanced synaptic activity-responsive element E-SARE in the subiculum,combined with chemogenetics and designer receptors exclusively activated by designer drugs(DREADDs),Ca2+ fiber photometry approaches,and behavioral tasks,to reveal the role of these neurons in cognitive impairment in epilepsy.RESULTS We found that chemogenetic inhibi-tion of subicular seizure-tagged c-fos+ neurons(mainly CaMK Ⅱ α+ glutamatergic neurons)alleviates seizure generalization and improves cognitive performance in the hippocampal CA3 kindling TLE model.While inhibition of seizure-labeled c-fos+ GABAergic interneuron shows no effect on seizure and cognition.As a comparison,che-mogenetic inhibition of the whole subicular CaMK Ⅱ α+ neuron impairs cognitive function in na?ve mice in basal condition.Notably,inhibition of subicular seizure-tagged c-fos+ neurons enhances the recruitment of cognition-responsive c-fos+ neurons via increasing neural excitability during cognition tasks.CONCLUSION Our results dem-onstrate that subicular seizure-activated c-fos+ neurons contribute to cognitive impairment in TLE,suggesting sei-zure-tagged c-fos+ neurons as the potential therapeutic target to alleviate cognitive impairment in TLE.
