1.The association of epidermal fatty acid binding protein with obesity, metabolic syndrome and cardiovascular dis-eases in children

Objective To study the relationship between the level of serum epithelial fatty acid binding protein (E-FABP) and cardiovascular disease in obese children. Methods Thirty children with metabolic syndrome (MS), 32 obesity children with no MS and 50 healthy children were recruited. Serum E-FABP and glucose/lipid metabolic indices were measured. Results Com-pared with the healthy children, levels of serum E-FABP in MS children were signiifcantly elevated (P=0.001). Compared with obesity children, levels of serum E-FABP of MS children were elevated, but the difference was not signiifcant (P>0.05). The cor-relations of E-FABP with waist to hip ratio, waist-to-height ratio, atherogenic index, fasting insulin, insulin resistance index were positive (P<0.05). According to the multivariate stepwise regression analysis, E-FABP was the independent risk of atherogenic index (P=0.018). Conclusions E-FABP plays a role in the development of atherosclerosis in obesity and MS.
2.Prognostic value of modified Charlson comorbidity index combined with serum albumin for long-term prognosis in peritoneal dialysis patients

Objective:To assess the prognostic value of modified Charlson comorbidity index (mCCI) combined with serum albumin for long-term prognosis in peritoneal dialysis (PD) patients.Methods:From January 1, 2007 to June 30, 2015, patients who started PD in Nanjing Drum Tower Hospital were enrolled in this retrospective cohort study. Clinical data including gender, age, underlying diseases, laboratory examination and prognosis were collected. The mCCI at the beginning of PD was calculated. Whether the duration of PD exceeded 5 years was used as an indicator to evaluate the prognosis. The patients were divided into≥5 years group and<5 years group according to the duration of PD, and the data were compared between the two groups. Cox regression model was constructed to analyze the influencing factors of all-cause death in PD patients. Multivariate logistic regression model and receiver operating characteristic (ROC) curve were used to analyze the predictive value of mCCI and serum albumin levels on whether patients could maintain long-term PD.Results:Of the 183 patients included [males 106(57.9%), females 77(42.1%); (53.35±16.50) years old; 162 cases (88.5%) with hypertension, 55 cases (30.1%) with diabetes], 97 cases had PD duration for ≥5 years and 86 cases less than 5 years. The overall 5-year technical survival rate was 65.1%. At the beginning of PD, compared with the dialysis age≥5 years group, the patients in the dialysis age less than 5 years group had older age, higher mCCI, lower serum albumin level, and higher C-reactive protein (CRP) level (all P<0.05), but there were no significant differences in gender, education level, electrolyte, mean arterial pressure, high densitv lipoprotein (HDL), low-density lipoprotein (LDL) and PD adequacy index between the two groups (all P>0.05). Multivariate logistic regression analysis showed that increased age ( OR=1.022, 95% CI 1.000-1.043, P=0.046), increased mCCI ( OR=1.620, 95% CI 1.300-2.018, P<0.001) and decreased serum albumin ( OR=0.807, 95% CI 0.730-0.893, P<0.001) were independent predictors for the duration of PD<5 years. ROC curve analysis showed that the area under ROC curves ( AUC) of mCCI, serum albumin level and combined prediction probability of the two for the duration of PD<5 years were 0.647(95% CI 0.568-0.727), 0.655(95% CI 0.577-0.734), and 0.767(95% CI 0.700-0.835), respectively, indicating that the accuracy of combined parameters to predict survival outcome was higher than that of any single parameter. Multivariate Cox analysis showed that increased age ( HR=1.073, 95% CI 1.046-1.100, P<0.001), increased mCCI ( HR=1.198, 95% CI 1.044-1.375, P=0.010) and decreased serum albumin ( HR=0.904, 95% CI 0.843-0.969, P=0.004) were independent influencing factors for all-cause death in PD patients. Conclusions:Old age, high mCCI and low serum albumin level are influencing factors for dialysis age<5 years and all-cause death in PD patients. mCCI combined with serum albumin level can improve the accuracy of predicting the long-term dialysis in PD patients.
3.The validity of HbA1C detection in diagnosing and screening dysglycemia in children with obesity

The validity of the recently recommended HbA1C criterion by the American Diabetes Association (ADA) in identification of dysglycemia in children with obesity was evaluated. 293 obese children underwent oral glucose tolerance test. Receiver operating characteristic ( ROC ) curve analysis was used to examine the sensitivity and specificity of fasting plasma glucose (FPG) and HbA1C in identifying dysglycemia. The results showed that the prevalence of type 2 diabetes mellitus (T2DM) was 3.8% and prediabetes 16. 0% based on plasma glucose standard. 4. 1% and 25.6% were categorized as T2DM and “at high risk of diabetes mellitus” based on both HbA1C and plasma glucose criteria. HbA1C was more efficacious than FPG in detecting abnormal glucose tolerance as shown by the areas under the curve in ROC of 0. 875 and 0. 713 respectively (P<0. 01 ). The sensitivity and specificity were 60. 5% and 86. 8% at HbA1C ≥5.7%, and 30. 5% and 94.0% at FPG ≥ 5.6 mmol/L.
4.Diagnostic value of glycosylated hemoglobin combined with homeostasis model assessment of insulin resistance in obese children with prediabetes

Objective To study the diagnostic value of glycosylated hemoglobin(HbA1c)combined with ho-meostasis model assessment of insulin resistance( HOMA-IR)in obese children with prediabetes. Methods Five hundred and ninety-five obese children diagnosed at Outpatient Department of Pediatrics,Tianjin Medical University General Hospital were enrolled in the study from October 2012 to March 2016. Oral glucose tolerance test(OGTT)and HbA1c test were performed after fasting 8 to 10 hours for all the subjects. According to the World Health Organization (WHO)criteria,the obese children were divided into non-prediabetes group(483 cases)and prediabetes group(112 cases). The levels of HOMA-IR and HbA1c were compared between 2 groups. The receiver operating characteristic (ROC)curve was drawn for screening prediabetes,and the diagnosis value of area under curve( AUC)and different cutoff value were calculated. Logistic regression model was used to analyze the combined diagnosis of prediabetic AUC with preserved probability as a single variable and ROC curve. Z test was used to compare the statistical differences of each AUC. Results The levels of HbA1c and HOMA-IR of the prediabetes obese children group[(5. 65 ± 0. 36)%and 11. 60 ± 8. 86]were all higher than those of the non-prediabetes obese children group[(5. 47 ± 0. 32)% and 6. 49 ± 4. 61],and there were significant differences between 2 groups(all P<0. 01). The AUC of HbA1c ROC for prediabetes was 0. 633. When the cutoff was 5. 585%,the sensitivity and specificity were 0. 584 and 0. 601,respective-ly. The AUC of HOMA-IR ROC for prediabetes was 0. 758. When the cutoff was 7. 244,the sensitivity and specificity were 0. 779 and 0. 669,respectively. When combined with HOMA-IR and HbA1c,the AUC was 0. 764. Compared with HbA1c,there were significant differences in AUC of ROC curve between HOMA-IR alone or HOMA-IR com-bined with HbA1c in the diagnosis of prediabetes mellitus(all P<0. 01). There was no significant difference in AUC of ROC curve between HOMA-IR alone and HOMA-IR combined with HbA1c in the diagnosis of prediabetes melli-tus(P>0. 05). Conclusions Only HOMA-IR or HOMA-IR combined with HbA1c may be the optimal indexes for diagnosing obese children with prediabetes.

Berberine was an isoquinoline type alkaloid extracted from Chinese herbs such as Coptis chinensis.Recently, as a great many berberine derivatives had been synthesized with comprehensive physiological functions , the structural modification and ap-plication of berberine showed great promising significance .Recent literatures had been systematically analyzed and summarized in this review.With abundant pharmacological activities of berberine different sites ′derivatives had been presented , systematical information of berberine structural modification as a lead compound and new drug R&D were provided in this article .
6.Tail-suspended model simulating mouse oocytes maturation inhibited with microgravity.

We studied the effects of simulated microgravity on mouse oocytes maturation, and analyzed whether the tail-suspended model can be applied to investigate simulated microgravity effects on reproductive processes in female mice. Mouse oocytes were cultured in vitro with microgravity simulated by a rotating wall vessel bioreactor and by tail-suspended model, and the maturation rate of the mouse oocytes in the two models were examined in vivo. The maturation rate of mouse oocytes cultured in simulated microgravity was 8.93%, and that was 72.33% in 1g gravity. In ratio, oocyte maturation rate had no significant difference between the rotational group and control group. Microgravity simulated by the tail-suspended model inhibited mouse oocytes maturation and increased the rate of oocytes abnormity. The maturation rate of tail-suspended mouse oocytes was 14.54%, which was significantly lower than that of control group. Tail-suspended model should be an ideal model to investigate simulated microgravity effects on reproductive processes of female mice.

7.Correlation between CD8 + T cells' balance and prognosis for patients with diffuse axonal injury

Objective To study the correlation between CD8 + T cells' balance and prognosis for patients with diffuse axonal injury(DAI).Methods To collect the 41 patients with DAI as observation group,with 33 cases light craniocerebral injury (LCI) patients and 35 healthy volunteers as control group.We detect the peripheral blood CD8 + CD28 + T cells and CD8 + CD28-T cell percentage,and calculate the ratio of both.The subjects working curve method (ROC) of.the above CD8 + T cells and its ratio were used to predict the prognosis of patients with DAI efficiency evaluation.Results (1) Compared with control group,CD8 + CD28 + T cells in patients with DAI and LCI increased significantly,and DAI was significantly higher than that of LCI group (P < 0.05).Compared with control group,CD8 + CD28-T cells in patients with DAI were decreased,but no statistical difference was found between LCI and the control group,but DAI group was significantly lower than the LCI group (P < 0.05).(2) CD8 + CD28-T cells and CD8 + CD28 +/CD8 + CD28-ratio were correlated with DAI diagnosis (P =0.003,0.000).When the percentage of CD8 + CD28-T cells =2.95%,the sensitivity of the diagnosis of DAI was 87.49%,86.21%;When the ratio of CD8 + CD28 +/CD8 + CD28-=7.39,the sensitivity was 92.63%,90.71%.(3) a total of 7 cases of patients died within 1 week after injury,CD8 + CD28 + T cells and CD8 + CD28 +/CD8 + CD28-ratio were significantly negative correlated with survival time (r =-0.739,-0.834,P =0.021,0.002),and CD8 + CD28-T cells were significantly positive correlated with survival time (r =0.782,P =0.006).Conclu sions Decreasing CD8 + CD28-T cells and higher CD8 + CD28 +/CD8 + CD28-ratio in patients with DAI immunological characteristics,especially the ratio (balance) is closely corrected with the diagnosis and prognosis of DAI.
8.Analysis on risk factors for dyed cornea of orthokeratology and interventions

Objective To investigate the related risk factors for dyed cornea of orthokeratology, and to analyze the related risk factors, and to determine the independent risk factors, Provide evidence for intervention measures. Methods The clinical date were investigated for 990 patients with fitting orthokeratology lens between may.2014 and may.2016 in our hospital,through access to medical records, follow-up visit,questionnaire investigation to find out the cause of orthokeratology adverse reactions, the related risk factors were analyzed by univariate and multivariate Logistic regression analysis, using SPSS17.0 statistical processing. Results The incidence rate of dyed cornea of orthokeratology lens fitting in was 14.55%(144 in 990 patients); the single factor analysis found gender, age, region, refraction, lens position, family members, no significant difference . Eye disease combined (χ2= 28.73, P<0.01), Schirmer I test (χ2=17.68, P<0.01), lens activity (χ2=67.1, P<0.01), Lens deposit(χ2=64.29, P<0.01), lens wearing time (χ2=43.25, P<0.01), health habits (χ2=38.01, P<0.01) and water resources (χ2=3.81, P<0.05), the difference was statistically significant; Logistic Logistic regression analysis found that Schirmer I test ( OR=4.126, P=0.003), lens activity ( OR=1.733, P=0.104), Lens deposit( OR=3.723, P=0.038), lens wearing time ( OR=5.034, P=0.002), health habits ( OR=6.544, P=0.002) and water resources ( OR=7.501, P=0.002) were independent risk factors for adverse reactions of orthokeratology. Conclusions Intervention measures that improve the fitting technology, complete removal of lens depositthe, Control wearing time, improve the health behaviors of the patients with the habit, use saline and professional cleaning are of great significance to reduce the incidence of adverse reactions of orthokeratology.
9.Risk factors for adverse reaction of Ortho-K lens

Objective To explore the risk factors for adverse reaction of Ortho-K lens so as to provide a reference for formulating targeted intervention measures.Methods From May 2014 to May 2016, retrospective analysis was used to clinical data from 990 patients with Ortho-K lens of Hankou Hospital, Wuhan Aier Eye Hospital. The reasons for adverse reaction of Ortho-K lens were found out through consulting medical record, telephone follow-up or asking and the related risk factors were analyzed.Results Among 990 patients with Ortho-K lens, there were 144 of them with adverse reaction with 14.55% for the incidence. Single factor analysis showed that there were statistical differences in history of ocular disease, schirmer tests, mobility of eyeglass, sediment of eyeglass, time of wearing glasses, health habit, cleaning solution for eyeglass among patients with Ortho-K lens (χ2=28.73,17.68,67.1,64.29,43.25,38.01,3.81;P<0.05). Multivariate Logistic regression analysis revealed that the independent risk factors for adverse reaction of Ortho-K lens included secretion of tears, mobility of eyeglass, sediment of eyeglass, time of wearing glasses, health habit and cleaning solution.Conclusions The major factors for adverse reaction of Ortho-K lens are schirmer tests, mobility of eyeglass, sediment of eyeglass, time of wearing glasses, health habit and cleaning solution. Hospitals should constantly optimize the process, improve optometry technology, promote healthy behaviors of patients to make them form a good living and health habit as well as eye care habit, strengthen education and guidance to ensure patients master correct cleaning and disinfection methods, effectively control time of wearingglasses. We recommend the 0.9% sodium chloride solution and professional cleaning solution to deal with eyeglass and nursing devices.

Ras homolog gene family member A(RhoA)is a small GTPase protein.Rho-associated coiled-coil forming protein kinase(ROCK)can be activated during the occurrence and development of ischemic stroke.RhoA/ROCK signaling pathway is an important regulatory factor in the pathological process of ischemic stroke,and regulation of this signaling pathway has become a research focus in promoting neuronal recovery and improving cerebral ischemia/reperfusion injury after ischemic stroke.However,only Fasudil is currently available as a RhoA/ROCK inhibitor.The rest is still in the stage of research and development or clinical trials,and has broad research prospects.In this paper,the regulatory role and mechanism of RhoA/ROCK signaling pathway in ischemic stroke were analyzed,and the application of inhibitors and regulatory drugs were described,aiming to provide new ideas for the prevention and treatment of ischemic stroke.
