1.Cross-sectional study of multiple metabolic disorders in patients with essential hypertension and nonalcoholic fatty liver disease

Objective To investigate the incidence of non-alcoholic fatty liver disease (NAFLD) in essential hypertension patients and the relation between NAFLD and multiple metabolic disorders.Methods Cross-sectional study.Between January 2006 and July 2008,1109 inpatients in Beijing Anzhen hospital diagnosed with essential hypertension were enrolled into this study (542 femal,567 male,mean age 61 ± 11).All patients had ultrasound screening for fatty liver.The prevalence of NAFLD,serum alanine aminotransferase abnormality ratio,the relationship between NAFLD and multiple metabolic abnormalitiesincluding glucose,dyslipidemia,hyperuricemia.471 cases of non-EH as a control group in the same period of admission(216 femal,255 male,mean age 62 ± 10).The measurement data of two groups were compared with t test,while the count data were compared with x2 test.Multiple Logistic regression was used to analysis therisk factors of EH with NAFLD.Results Among 1109 recruited patients,37.3% (414/1109) had fatty liver by ultrasonography.EH patients were divided into NAFLD group and non-NAFLD group.In EH coexisted with NAFLD group,the prevalence of diabetes mellitus (21.0% ∶ 10.8%),metabolic syndrome (59.4% ∶ 44.7%),overweight (71.3% ∶ 62.9%) and the levels of BMI (26.90 ± 3.66∶ 26.35 ± 3.56),TC (5.30±1.17∶ 5.12 ± 1.09),TG(2.11 ± 1.43∶1.85 ± 1.33),ALT(45.18 ± 37.62∶39.22 ±32.27) were significantly higher than those in the non-NAFLD group (P <0.05).HDL-C (1.53 ±0.53∶1.62 ±0.54)was significantly lower than that in the non-NAFLD group (P < 0.05).In non-NAFLD group,prevalence of overweight and the levels of BMI,TC,TG,ALT were signific antly higher than those in non-EH group (P < 0.01).By multivariable logistic regression analysis,diabetes mellitus,hypercholesteremia,hyperuricemia and BMI were risk factors in NAFLD patients with essential hypertension.Conclusion NAFLD was more prevalent in patients with EH complicated with multiple metabolic disorders,such as hyperlipemia and hyperuricemia.
2.Comparison of effects of irbesartan and captopril on left ventricular function in elderly patients with cardio-vascular disease and cardiac functional reserve

Objective To compare effects of irbesartan and captopril on left ventricular function in elderly patients with cardiovascular disease and cardiac functional reserve .Methods 100 cases of elderly patients with car-diovascular disease were chosen ,according to a list of numbers randomly divided into the two groups ,50 cases in each group,a group with irbesartan treatment ,as the irbesartan group and the other group received captopril ,as the capto-pril group compared the two groups of patients left ventricular function and cardiac functional reserve .Results (1) Left ventricular function after the irbesartan group therapy LVDd indicators ,IVSd,LVPWd,LVEF,VE/VA,LVM was significantly better than before treatment ,the difference was statistically significant (t=31.23,18.36,28.12,46.36, 51.13,16.03,all P<0.05),the captopril group before and after treatment was significantly better than the treatment of various indicators,the difference was statistically significant (t=24.53,21.36,31.12,14.33,18.19,16.45,all P<0.05);(2) The irbesartan group of patients after treatment resting heart rate ,exercise heart rate,6min walking distance improved significantly over the treatment of former CCRI and CRI ,the difference was statistically significant (t=39.09,23.77,39.01,23.18,22.45,all P<0.05),after treatment with captopril treatment was significantly better than the indexes,the difference was statistically significant (t=91.21,37.16,26.74,46.33,36.74,all P<0.05),6min walking distance from the irbesartan group therapy ,CRI better than the captopril group ,the difference was statistically significant(t=39.02,42.14,all P<0.05).Conclusion Irbesartan and captopril on improving left ventricular function in elderly patients with cardiovascular disease and cardiac reserve function has a significant effect,irbesartan 6min walking distance test and cardiac reserve indices better .
3.Extraction of Alsophila spinulosa(Hook.) Tryon flavonoids by orthogonal experiment and separation by macroporous resin

AIM:To study the optimum extraction process of total flavonoids in Alsophila spinulosa(Hook.) Tryon and technological parameter of macroporous resin. METHODS: The optimum parameters were chosen by ratio of total flavonoids as the assessment index such as static and dynamic adsorption of macroporous resin. RESULTS: The optimum extracting process conditions were as follows: solvent was ethanol,ratio of solid-liquid was 1(∶)12,extraction time was 1 h,temperature was at(80 ?C).The optimum technological conditions of the D104 were: the solution concentract at 1.36 mg/mL,the current velocity of adsorption 2 BV/h,the amount of saturated adsorption at 5 BV,the eluting reagent 70% ethanol,the eluting velocity 3 BV/h,eluent volume at 2 BV. CONCLUSION: The process can separate and concentrate the total flavoionds of Alsophila spinulosa(Hook.) tryon.

BACKGROUND: Previous experiments have shown that transforming growth factor-?1 (TGF-?1),matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-1 (TIMP-1) play an important role in chronic allograft nephropathy,but the mechanisms remains unclear. OBJECTIVE: To investigate the expression and significance of TGF-?1,MMP-2 and TIMP-1 in the renal tissues of chronic allograft nephropathy and normal renal tissues. DESIGN,TIME AND SETTING: A histological and morphological control observational experiment of cytology was performed at Pathological Laboratory of the 175th hospital from June to October 2008. PARTICIPANTS: The transplanted renal tissues extracted from 24 patients with chronic allograft nephropathy were used in the experiment,and the normal renal tissues in 10 patients were assigned as controls. METHODS: Immunohistochemical staining was performed in all the renal tissues. All the renal tissues were fixed with formaldehyde and embedded by paraffin wax. Then the sections were prepared with a thickness of 3 ?m,and were then closed with normal non-immune serum. Subsequently,the sections were incubated as follows: primary antibody,secondary antibody,streptavidin peroxidase complex,and diaminobenzidine (DAB),followed by HE counterstaining. The sections were cleared and coverslipped. MAIN OUTCOME MEASURES: The expressions of TGF-?1,MMP-2 and TIMP-1 in the renal tissues were observed by immunohistochemical staining. The relationships among TGF-?1,MMP-2 and TIMP-1,as well as between them and the grade of chronic allograft nephropathy were analyzed. RESULTS: Qualitative analysis: The TGF-?1,MMP-2 and TIMP-1 showed strongly positive expressions in the cytoplasm and membrane of renal tubular epithelial cells in transplanted renal tissues. ObjectiveTo investigate the clinical efficacy of focused ultrasound for treating non-neoplastic epithelial disorders of vulvar. Methods212 patients with non-neoplastic epithelial disorders of vulvar were treated with focused ultrasound. The patients were followed up after the treatment,and symptom was observed and the efficacy was evaluated. ResultsThe efficacy of non - neoplastic epithelial disorders of vulvar of squamous hyperplasia,lichen sclerosus and mixed dystrophy was 99.2%, 100. 0%, 88.9% respectively, the difference was not significat (P > 0.05 ). The efficacy in different courses of patients was 100.0% ( < 2 years), 100.0% (2 ~10 years) ,91.3%( > 10 years) respectively,the difference was not significat( P > 0.05). ConclusionFocused ultrasound was obviously effective in the treatment of patients with non-neoplastic epithelial disorders of vulvar.
8.An analysis of the expression of P53 protein and the radiosensitlvity in patients with advanced maxmary squamous ceH carcinoma

Objective To explore the relationship between the expression of P53 protein and the radiosensitivity in patients with advanced maxillary squamous cell carcinoma.Methods An immunohistochomical method wag used to detect the expression of P53 protein in patients with advanced maxillary squamous cell carcinoma.The follow up time was 2 years.The local recurrence of the patients having been treated with radical surgery and affiliated radiotheraphy were analyzed.Results The overexpression of P53 protein in 26 cages was 65.4% (17/26).In the P53 overexpression group,the local recurrence after systiem therapy wag occurred in one case within 6 months,6 cages between 7~12 months.3 cages between 13~18 months and 2 cases between 19~24 months.In the P53 low expression group,there were no recurrence within 6 and 12 months and one case ocurred within 18 months.5 cages between 19~24 months.The difference of recurrence within 18 months after system therapy between the expression of P53 Wag statistically significant(P<0.05),but it Wag not significant for those within 24 months(P>0.05).Conclusions The expression of P53 protein Wag correlated with the radiosensitivity in patient with advaneed maxillary squamous cell carciaoma,especially for the resid.rod cells in mitosis phase.The affiliated radiotherapy after radical surgery Wag limited effect.
9.CT Diagnosis of Hepatic Alveolar Echinococcosis

Objective To discuss the CT features and pathologic basis of hepatic alveolar echinococcosis. Methods Total 19 cases were confirmed by ELISA and IHA of antibody of echinococcosis. A part of them were confirmed by pathalogy. Results The main features of the disease were irregular,obscure contour and heterogeneous density with mainly hypodense in the liver. Almost of all accompanied with calcification. The calcification was showed some certainly signification. No contrast enhancement was observed within the lesions after administration of intravenous contrast medium. There were three type of this disease based on CT features including map type, halo type and pseudocystic type. Conclusion CT scanning has value of not only for diagnosis of hepatic alveolar echinococcosis but also useful to display each complications and metastasis.
10.Advances in Management of Gastrointestinal Stromal Tumors with Secondary Resistance to Imatinib

The use of tyrosine kinase inhibitor imatinib in treatment of gastrointestinal stromal tumors(GISTs)has achieved a dramatic therapeutic efficacy. However,secondary imatinib resistance emerged as a clinical problem needs to be solved urgently. The underlying mechanisms of GISTs secondary resistance to imatinib may be related with secondary mutations of KIT/ PDGFRA genes,loss of PTEN gene and induction of cellular quiescence. This resulted in the adoption of new therapeutic strategies such as novel tyrosine kinase inhibitors,combined use of imatinib with downstream signaling inhibitors,KIT/ PDGFRA independent targeted inhibitors such as KIT chaperone inhibitors and aurora kinase inhibitors,as well as inducing apoptosis in quiescent GIST cells. In this article,the above-mentioned issues were summarized.
