1.Stabilization of unstable distal clavicle fractures (Neer ⅡB) with lateral clavicle anatomic locking compression plate

Objective To explore clinical and radiographic outcomes of unstable distal clavicle fractures (Neer ⅡB) fixed with lateral clavicle anatomic locking compression plate (LCP).Methods Between January 2009 and October 2010,eleven consecutive patients with unstable fractures of the distal clavicle (Neer ⅡB) were treated using lateral clavicle anatomic LCP.There were 9 men and 2 women,with the mean age of 37.2 years (range,23-43 years).The right shoulder was involved in 6 patients and the left in 5 patients.The interval between injuries to operation was 24-72 h (mean,48 h).After fracture reduction,the plate was place on superior of the distal clavicle.According to the distal fragment length,3 to 6 locking screws were carefully inserted,3 locking screws were used to fix proximal fractures.Coracoclavicular ligament was not repaired.Functional recovery of the shoulder joint was assessed using the American Shoulder and Elbow Surgeons (ASES) rating scale score.Plain radiographs of clavicles were used to assess bony union.Results All the patients were followed up for 9 to 12 months (mean,10.3 months).Solid bony union was eventually achieved in all patients.The mean ASES scores were 89.1 (range,84-91) on the injured side versus 96.2 (range,94-100) on the contralateral side.No implant-related fracture,fixation failure and rotator cuff injury occurred.Conclusion Lateral clavicle anatomic LCP fixation in the treatment of distal clavicular fractures is a reliable and simple technique.
2.The clinical application and results of tracer methodology in the maintenance of peripherally inserted central catheters for chemoradiotherapy

Objective To investigate the quality of nursing care after the application of tracer methodology in patients undergoing chemoradiotherapy with peripherally inserted central catheters (PICC),evaluate the nurses' adherence to the procedural protocol of PICC,and calculate the number of infectious complications related with the placement.Methods We compared the nurses' adherence to the procedural protocol and the number of infectious complications before (April 2014 to June 2015) and after (July 2015 to October 2016) the tracer methodology was applied at the Chemoradiotherapy Center of Ningbo First Hospital in July 2015.Results After the tracer methodology was introduced to the care of patients with PICC,the nurses had better adherence to the procedural protocol,as shown in the rise of quality control score from (94.00± 3.35) to (97.87± 1.42).The incidence of infectious complications dropped significantly (all P<0.001).Conclusion The clinical application of tracer methodology can improve the quality of nursing for patients with PICC,reduce the incidence of PICC-related infectious complications,and promote patient safety.
3.Construction of fusion expression vector of human-derived neurotrophin-6 gene encoding mature peptide and purification of its expressed product.

To get the mature peptide of human-derived neurotrophin-6 (NT-6), NT-6 gene encoding mature peptide was amplified by PCR, using the NT-6 cDNA that had been cloned as templet. The gene encoding mature peptide of NT-6 gene was cloned into pGEX1-lambdaT plasmid to construct the fusion expression vector. Expression of fusion protein in Escherichia coli was defected after induction by isopropyl beta-D-thiogalactoside(IPTG). The mature peptide of NT-6 was collected with GST fusion protein purifying kit. It was shown that a fragment of 460bp was gained by PCR. With the techniques of double-cleave and electrophoresis, the recombinant vector was identified as pGEX1-NT-6. The recombinant vector pGEX1-NT-6 transformed Escherichia coli expressed fusion protein of 41KD after induction by IPTG. Cleaved by thrombin, the mature peptide of NT-6 was obtained; its molecular weight was about 15KD. The cloning and expression of human-derived NT-6 gene encoding mature protein has provided a basis for further studies on the function and clinical application of NT-6.

4.Cloning of human brain-derived neurotrophin-6 gene and its expression in procaryotic cell.

OBJECTIVETo clone human brain-derived neurotrophin-6(NT-6) gene and to observe its expression in the procaryotic cell.
METHODSTotal RNA was extracted from aborted antenatal cerebral cortex, and cDNA fragment of NT-6 was amplified through reverse transcript-polymerase chain reaction. After being incised and recovered, the NT-6 gene was cloned into pBK-CMV plasmid to construct a NT-6 gene expression vector. Expression of NT-6 gene in Escherichia coli was studied after being induced by isopropyl beta-D-thiogalactoside(IPTG).
RESULTSThe NT-6 gene expression vector was constructed and Escherichia coli with recombinant vector expressed specific protein after induction by IPTG.
CONCLUSIONThe cloning of human brain-derived NT-6 gene provides a basis for further studying the structure, function and clinical application of NT-6.

5.Relationship between PPARG rs2290449 and the efficacy of combination therapy in sepsis with MODS in Yanbian area

Objective To investigate the relationship between the single nucleotide polymorphism of PPARG rs2290449 and the efficacy of combination therapy in sepsis patients with MODS.Methods 382 cases of sepsis with MODS were selected and divided into the effective group and the ineffective group,and a case-control study was conducted.PCR-RFLP and Sequenom MassARRAY were used to detect the genotype and allele frequencies of the loci.Unconditional Logistic regression was used to calculate odds ratio(OR)and 95%confidence interval(CI)to evaluate the efficacy of combination therapy for sepsis with MODS in patients with different genotypes.Re-sults There were G and A alleles in PPARG rs2290449 locus,GG、GA and AA genotype.Non-conditional Logis-tic regression analysis showed that compared with the GG genotype group,the GA genotype group had a significant correlation with the efficacy of combination therapy for sepsis with MODS(OR=0.449,95%CI=0.280-0.722,P=0.001).In the dominant model,there was a significant correlation between the GA+AA genotype and the effi-cacy of combination therapy compared with the GG genotype(OR=2.104,95%CI=1.332-3.321,P=0.001).After adjusting for confounding factors,unconditional Logistic regression analysis showed that compared with the GG genotype,the GA+AA genotype was significantly correlated with the efficacy of combination therapy in sepsis pa-tients with MODS(OR=2.307,95%CI=1.438-3.701,P=0.001).Stratified analysis showed that compared with the population carrying GG genotype,the population carrying GA+AA genotype was significantly correlated with the efficacy of combination therapy in sepsis with MODS in the stratified analysis of age,gender and ethnicity.Conclusion PPARG rs2290449 single nucleotide polymorphism is significantly correlated with the efficacy of com-bination therapy in sepsis patients with MODS in Yanbian area.
6.Primary hepatic adenosquamous carcinoma: report of 5 cases

Objective:To explore the clinicopathological characteristics of the primary hepatic adenosquamous carcinoma(ASC).Methods:A retrospective analysis was performed on the clinical data of 5 ASC patients admitted to the First Affiliated Hospital of Anhui Medical University from 2006 to 2019 who underwent surgical resection and were pathologically confirmed.Results:Among the 5 ASC cases, there were 4 males and 1 female. The age ranged from 48 to 73 years. As for the initial symptoms, there were 5 cases complaining upper abdominal pain, 2 cases presenting fever, 1 case presenting weight loss and 1 case presenting jaundice. CA19-9 was significantly higher than normal in 4 cases, while AFP was normal in all. None had definite preoperative diagnosis.All the 5 patients underwent surgical resection with pathology proved primary hepatic ASC. Lymph node metastasis was found in 4 cases and nerve invasion in 2 cases. There were 4 cases at TNM stage ⅣA, one at stage ⅠB. The median disease-free survival (DFS) was 5 months and the overall survival (OS) was 9 months.Conclusions:Primary hepatic adenosquamous carcinoma is a rare type of liver malignant tumor with an extremely poor prognosis. Surgical resection helps little in improving the prognosis.
7.Numerical Simulation on Support Performance of NiTi Alloy Thoracic Aortic Stent

Objective To study the influence of different support heights, support numbers and cross-sectional dimensions on support performance of NiTi thoracic aortic stents. Methods Twenty-seven scaffold models with different parameters were established by using AutoCAD 2016 and SoildWorks 2014 software. HyperMesh 14.0 was used for tetrahedral mesh division, and ABAQUS 2017 was used for support performance simulation analysis. Results With the decrease of support height, the support stiffness would increase; a larger cross-section size would lead to a larger support stiffness; with the increase of support numbers, the support stiffness would increase. Among the influencing factors of support performance, the order of influence degree was support height>section size>support numbers. Conclusions The research findings have certain guiding significance for the development and research of thoracic aortic stents, and provide theoretical basis for the selection and optimization of clinical stents.
8. Effects of hand-transmitted vibration on subjective symptoms in mine drilling workers

OBJECTIVE: To investigate the effect of hand-transmitted vibration on the subjective symptoms of mine drilling workers. METHODS: A total of 117 mine drilling workers exposed to hand-transmitted vibration were selected as the exposure group, and 46 workers without hand-transmitted vibration exposure were selected as control group by judgment sampling method. The Occupational Epidemiology Questionnaire of Mine Drilling Workers was used to investigate their subjective symptoms. RESULTS: The prevalence of tinnitus, hearing loss, cough and sputum in the exposure group was higher than that in the control group(P<0.05). The occurrence rate of numbness, tingling, self-reported white fingers and cold hands in the exposure group was higher than that in the control group(P<0.05). The total rate of symptoms of hand in the exposure group was higher than that in the control group(38.5% vs 21.7%, P<0.05). The multivariate logistic regression analysis results indicated that hand-transmitted vibration exposure and service length were risk factors of abnormal hand symptoms(P<0.05). CONCLUSION: Occupational hand-transmitted vibration exposure can affect the hand subjective symptoms of mine drilling workers, and the length of service is a synergistic influencing factor.
9.Molecular genetic diagnosis of a carrier with rare α-thalassemia mutations.

OBJECTIVE:
To explore the cause of inconsistent genotypes for an α-thalassemia carrier by using two commercial genotyping kits.
METHODS:
GAP-PCR and PCR-reverse dot blotting (PCR-RDB) were employed to determine the genotype of the carrier, while Sanger sequencing was used to verify the results.
RESULTS:
Sequencing analysis demonstrated that the subject has carried a α1 globin gene with a 3.7 kb heterozygous deletion. In addition, two novel mutations, IVS-II-55(T>G) and IVS-II-119(G>TCGGCCC), were found in intron 2 of α2 globin gene.
CONCLUSION
The two mutations located in the binding regions of PCR primers have caused failure of PCR amplification and misreading of the genotype. Combination of clinical and hematological phenotypes is indispensible to infer the genotype of carriers for accurate diagnosis.
