1.Development of HIV prevention strategy targeted to low price sex industry in the context of organizational culture:a case from BANBANCHA in LZ City
Chinese Journal of AIDS & STD 2006;0(01):-
Objective Health promotion aims to empower people to enjoy good health by providing conditions at the micro-,meso-and macro-levels.So the organizational culture on which the target population rely in their daily lives should be considered as specific context for promoting their healthy behavior at the micro-level.The study of BANBANCHA,a specific form of sex industry in LZ city of southwest China, has revealed that it is characterized with culture of quasi-family and commercial sense.Accordingly,a relevant HIV prevention strategy has been developed, such as motivating the boss to participate in peer education, involving marketing discourse in health promotion, encouraging the target population to care for their own health, and integrating public and private topics into the health education activities.
2.Study on Combined Prescriptions
World Science and Technology-Modernization of Traditional Chinese Medicine 2015;17(3):633-637
Even though a single prescription may be applied in different treatments, it may not be effective in treating patients with complex conditions. In cases like this, the problem can only be solved through the expanding of medicines in a prescription. Because a new prescription is not easy to create, the usage of adding another or more appropriate prescriptions into the prescription can be considered to expand the curative effects. The existing prescriptions were left by the confirmation of their curative effects from clinical tests which made by doctors of several generations. Therefore, the combination of prescriptions is a method which is convenient and easy to control the curative effects. By analogical reasoning, prescription user often uses a single prescription as a basic unit to generate the combined prescriptions from the existing corresponding prescription. It follows the rule of treatment based on syndrome differentiation to take a dynamic adjustment for complex symptoms. Disease with multiple major symptoms is the main basis for a doctor to use combined prescriptions. The combination of different prescriptions may produce the curative effects of addition, reinforcement, antagonism, or even new effects. Since traditional prescriptions have experienced many years of clinical tests, doctors who utilize traditional prescriptions actively will have more methods and ways to treat diseases.

Objective To evaluate the effects of different dosage of aspirin and clopidogrel on the platelet inhibition rate by thromboelastography (TEG)before stent-assisted embolization of aneurysms. Methods The clinical data of 57 patients with unruptured aneurysms treated by stent-assisted coil embolization in General Hospital of Nanjing Military Command were analyzed retrospectively. The patients were divided into a low dose group (aspirin 100 mg + clopidogrel 75 mg,26 cases)and a high dose group (aspirin 300 mg +clopidogrel 75 mg,31 cases). All the patients were treated with aspirin and clopidogrel before surgery. The blood samples were collected on day 3 after oral administration with anti-platelet aggregation drugs. TEG was used to detect the arachidonic acid (AA )-induced inhibition rate of platelet aggregation and adenosine diphosphate (ADP)-induced inhibition rate of platelet aggregation. The platelet inhibition and drug resistance,as well as ischemic complications in the perioperative period between the two groups were compared. Results (1 )The platelet inhibition rates:there was no significant difference in the inhibition rates of platelet aggregation of AA and ADP between low dose group and high dose group at day 3 after oral antiplatelet drugs [AA inhibition rate:(76 ± 21)% vs. (80 ± 21)%;ADP inhibition rate:(72 ± 26)% vs. (73 ± 29 )%;all P >0. 05 ]. (2 )Drug resistance:in the low dose group, the patients of aspirin and clopidogrel resistant were 2 cases(7. 7%)and 1 case(3. 8%),and the patients in high dose group were 3 cases (9. 7%)and 4 cases(12. 9%). No statistical significant difference in the aspirin and clopidogrel resistant was detected in the two groups (all P>0. 05). (3)The ischemic complications in the perioperative period:there was 1 case (3. 8%)with ischemic complications in the low dose group, and 2 cases (6. 5%)in the high dose group. There was no significance difference in the incidence of ischemic complications between the two groups(P >0. 05). Conclusion For patients treated with stent-assisted embolization of aneurysms,no difference could be detected in the inhibitory effect of platelet aggregation between low and high dose groups of aspirin.

OBJECTIVE:To investigate the significance of tacrolimus blood concentration monitoring to the therapy of mem-branous nephropathy. METHODS:41 patients with membranous nephropathy received tacrolimus, and the blood concentration of ta-crolimus reached to steady state. The trough concentration of tacrolimus was determined by EMIT. The patients were followed up, and clinical therapeutic efficacies were recorded. The relationship of blood concentration of tacrolimus with clinical efficacy was evaluated by SPSS 16.0 software. RESULTS:The blood concentration of tacrolimus was(7.47±2.74)ng/ml in complete remission (CR)group,(5.72±1.19)ng/ml in partial response(PR)group,and(3.30±1.08)ng/ml in no response(NR)group, with total remission rate of 75.61%. The blood concentration of CR group was the highest, followed by PR group and NR group, there was statistical significance among 3 groups(P<0.05). CONCLUSIONS:The clinical efficacy of tacrolimus in the treatment of nephrot-ic syndrome is correlate to the blood concentration intimately. Trough concentration monitoring of tacrolimus has important signifi-cance to the treatment of membranous nephropathy.

Objective: Due to the high cost and complicated imaging interpretation of intravascular ultrasound (IVUS), we conducted a comparative study between AngioLight system and IVUS measurement of coronary diameter to explore the safety and efifcacy of AngioLight performance in experiment pigs.
Methods: There were 3 reference target points established in left anterior descending (LAD), left circumlfex (LCX) and right coronary arteries (RCA) in 3 Chinese mini-pigs. Each coronary branch had①target point, ②proximal target point, which was (10-15) mm up to target point,③distaltarget point, which was (10-15) mm down to target point. The coronary diameters were measured and compared by both IVUS and AngioLight system. Upon stent implantation, the diameters were measured again by both IVUS and AngioLight system at distaltarget point, stent edge of distal target point and proximal target point, stent edge of proximal target point respectively. And results comparison between AngioLight and IVUS were also compared.
Results: In 25 measurements, neither adverse complication nor death were occurred during the study. There were no significant statistical differences between IVUS real-time max diameter and AngioLight catheter max diameter measurements (2.57 ± 0.54) mm vs (2.70 ± 0.49) mm,P=0.09, as well as between IVUS analyzed max diameter and AngioLight catheter max diameter (2.68 ± 0.6) mm vs (2.59 ± 0.42) mm,P=0.37. In addition, good correlations were observed (r= 0.73,P<0.001) and (r= 0.67,P=0.001) respectively.
Conclusion: Angio Light system has good safety and correlation with IVUS for quantitative measurement of coronary diameter in experimental pigs.

Objective Toinvestigatetheapplicativevalueof3D-DSAandheadMRIorCTfusion technology for guiding the individualized treatment of intracranial arteriovenous malformation (AVM ). Methods Twenty-onepatientswithAVMdiagnosedwithDSAattheDepartmentofNeurosurgery,Nanjing General Hospital of Nanjing Military Command from January 2015 to May 2015 were analyzed retrospectively. All patients performed DSA,MRI,and CT scan respectively before procedure, and they also performed 3 D-DSA and MRI or CT fusion. Of the 21 patients,15 performed MRI and 3D-DSA fusion,6 performed CT and 3D-DSA fusion. According to the image fusion results of the patients, the individualized treatment regimens were further developed, including microsurgical resection, endovascular embolization, and stereotactic radiotherapy (alone or combined treatment). The patients were followed up and observed for 2 to 6 months after procedure. Results Fromthe3D-DSAwithheadMRIorCTfusionimagesofthepatientsbeforetheprocedure not only could observe the vascular architecture of AVM,the relationship between the niduses and the surrounding nerve structures, but also could precisely locate the positions of AVM with small aneurysms or tiny AVMs. According to the results of image fusion,17 patients with AVM were treated with microsurgical resection,2 were treated with interventional embolization and stereotactic radiotherapy, and 2 were treated with stereotactic radiotherapy only. Of the 17 patients with AVM underwent microsurgical resection, none experienced intracranial rebleeding during the follow-up period. The last Glasgow outcome scale (GOS)score was 5 in 13 cases, and 4 in 4 cases. One patient with AVM underwent combined stereotactic radiotherapy had intracranial rebleeding during the follow-up period, and their last GOS score was 4. The other remaining 3 patients did not have new neurological deficits or rebleeding during the follow-up period, and theirGOSscorewas5.Conclusions 3D-DSA,headMRI,andCTfusiontechnologyarenovel, the operative method is simple, and the fusion image is accurate. They can effectively develop the individualized treatment regimens for patients with AVM.
