1.Functional training of feces and urine incontinence after operation

Objective To investigate the training methods of urine and feces incontinence. Method 77 children aged 4 to 14 years with urine and feces incontinence induced by operation received induced training for pelvic floor muscle and related mescles. Abodominal muscles were trained. Regular defecation training, dilation of anus and urine tract, crede methods, Vslsava and Kegel training were conducted. Result The rehabilitation of defecation was found in 67.53% children, the urination was found in 83.12 children. Conclusion The above methods are effective in restoring defecation and urination function.
2.The application of diversified training mode in the training of intensive care unit nurse specialists

Objective To discuss the training mode of intensive care unit (ICU) nurse specialists. Methods The trainees of class of 2014-2015 ICU nurse specialists were chosen as the object of the study, diversified training mode was utilized, namely, an optimized combination of multiple different training modes, theoretical courses+clinical practice+assessments (scenario simulation in small groups and individual practical capability)+review (a survey for the training contents, methods and effectiveness of the courses and instructors/teaching assistants, as well as position competency of the trainees)+case study in nursing+comprehensive review. Results After the training of ICU nurse specialists with diversified training mode, the scores had increased from 54.88 ± 12.45 to 79.63 ± 6.15 in theoretical courses, and from 64.69 ± 11.61 to 92.38 ± 3.10 in clinical operating skills, the differences between the above indexes were statistically significant (t=101.60, 169.96, both P<0.01);the numbers of required 140 skills in clinical practical programs mastered by trainees had increased from 66.53 ± 16.17 to 84.78 ± 21.34, the differences between the above indexes were statistically significant (t=14.866, P<0.01). After the ordered chi-square test the value of position competency before and after the training were statistically significant (χ2=24.62, P < 0.05). The trainees also expressed 100% satisfaction rate of the training courses, instructors/teaching assistants and the training effectiveness. Conclusions The diversified training mode benefits to the improvements of professional knowledge and skills, position competency and team-working, as well as the cultivation and comprehensive quality of ICU nursing specialists.
3.The nutritional status of radical gastrectomy patients during the late course of chemotherapy and its main influencing factors

Objective To explore the nutritional status of radical gastrectomy patients during the late course of chemotherapy and its main influencing factors, providing reference for the development of targeted nursing intervention. Methods The Scored Patient-Generated Subjective Global Assessment, The Chemotherapy Side Effects of Patients with Gastric Cancer Questionnaire were used to collect clinical data that received complete chemotherapy after radical surgery from September 2014 to March 2016. Results A total of 103 cases finally involved in this study. The incidence of malnutrition in patients during pre-chemotherapy and late course of chemotherapy was 45.63%(47/103) and 73.79%(76/103) respectively, and there was significant difference (χ2=16.97, P<0.05). The nutritional assessment score, the hemoglobin levels and body mass index of patients pre-chemotherapy was (3.64±0.78) points, (124.11± 21.78) g/L, (21.29±2.77) kg/m2 , (8.07±3.75) points, (108.54±11.68) g/L, (20.29±2.68) kg/m2 late course of chemotherapy and there were significant differences(t =-11.74, 6.39, 2.63, all P<0.05). The symptom of depression was at a higher level 65.05%(67/103), patients with anorexia in late course of chemotherapy accounted as much as 75.73%(78/103). Logistic regression analysis showed that educational level, loss of appetite and parenteral nutrition were the main factors which influenced the nutritional status of patients during the late course of chemotherapy. Conclusions With the increase in the number of chemotherapy, the nutritional status of the patient is getting worse. The study suggests medical personnel should pay more attention to the patients with low educational level, give patients and their families targeted diet counseling, correct the mistaken conceptions thus enhance their understanding of dietary knowledge. Strengthening the management of complications during chemotherapy, reducing the adverse factors that affect the patient′s nutritional status to improve the nutritional status of patients.
4.Nutritional risk assessment and inspiration for patients in chemotherapy after radical gastrectomy

Objective To investigate the nutritional status of patients with chemotherapy after radical gastrectomy and to provide dietary guidance and nutritional support for patients in time. Methods Patients who received chamotherary after radical gastrectomy inthe Gastrointestinal Surgery of the First Affiliated Hospital of Zhengzhou University from September 2014 to March 2016 were included in the study. The nutritional status was assessed by unified training investigators within 48 h after hospitalization. The questionnaires of Scored Patient-Generated Subjective Global Assessment (PG-SGA) and the Chemotherapy Side Effects of Patients with Gastric Cancer Questionnaire were used to investigatethe patients by trained investigators. Results The scores of the nutritional status of patients with different stages of chemotherapy were 6.107 ± 2.947, 6.505 ± 3.134, 8.068 ± 3.748, which was statistically significant (F=10.202, P<0.01). The incidence of anorexia was the highest rate of diet-related symptoms, the rate of early, mid-term and later chemotherapy rates were 34.95%(36/103), 56.31%(58/103), and 75.73%(78/103) respectively. Patients acquired the diet knowledge mainly from medical staff. The proportions of early, middle and later stages of chemotherapy were 92.23% (95/103), 79.61% (82/103), 75.73%(78/103). Conclusions With the increase of chemotherapy times, the nutritional status of patients with radical gastrectomy is getting worse. The results suggested that medical staff should provide an effective basis dietary guidance in time, improving the patients′nutritional status and clinical outcomes.
5.Effect of hospital-family cooperative nursing model on postoperative rehabilitation and nursing ability of children with posterior urethral rupture

Objective:To explore the effect of hospital-family CCM on postoperative rehabilitation of children with posterior urethral rupture and its effect on the nursing ability of children′s family members.Methods:Forward-looking selection between January 2015 and January 2019 in our hospital 72 cases of traumatic posterior urethral disruption of children as the research object, the random number table method were randomly divided into 2 groups. The control group ( n=36) was implemented routine nursing, observation group ( n=36) was given hospital-family CCM. The prognosis of postoperative pain and complications of two groups, children′s family psychological status before and after the intervention and nursing ability were compared. Results:Compared with the control group, the scores of Numerical Rating Scale (NRS) of 12h after the operation, 1d after the operation and 2d after the operation respectively 5.89±0.71, 4.50±0.51, 2.42±0.50, all of which were significantly lower than those of the control group 6.92±0.87, 5.89±0.85, 4.31±0.47, with statistically significant differences ( t values were 5.23, 8.904, 16.525, P<0.05). The total incidence of complications in the observation group was 5.56% (2/36), significantly less than 22.22% (8/36) of the control group with statistical significance difference ( χ2 value was 4.181, P<0.05). The scores of Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SDS), Family Caregiver Task Inventory (FCTI) in the families of children of the observtion group were 34.6±5.36, 31.67±6.11, 8.65±2.40 after the intervention, significantly lower than the control group (41.09±6.44, 39.64±7.31, 11.64±3.10) with statistical difference ( t values were 4.647, 5.019, 4.576, P<0.05). Conclusions:The hospital-family CCM can effectively promote the rehabilitation of children undergoing posterior urethral rupture and improve the nursing ability of their families.
6.Effects of family caregivers'cognitive behavioral intervention on nutritional status of postoperative gastric cancer patients during chemotherapy

Objective To observe the influence of nutrition cognition and behavior intervention of family caregivers on nutritional status of gastric cancer patients during chemotherapy. Methods We selected 100 patients who received complete chemotherapy at the Department of gastrointestinal surgery from September 2015 to February 2017.Each patient selected 1 family caregiver.It was divided into the control group and the intervention group by the draw method.The control group carried out routine nursing measures.The intervention group carried out nutritional cognitive behavior intervention.The effects of cognitive behavior intervention were investigated by observing the scores of PG-SGA,albumin, prealbumin in gastric cancer patients and the scores of nutrition knowledge,attitude and practice in family caregivers. Results The scores of nutrition knowledge, attitude and practice of the control group and the intervention group were respectively 44.17 ± 9.35 and 46.50 ± 9.32, and the difference was statistically significant(t=15.02, P<0.05).There was significant difference in PG-SGA score, hemoglobin and BMI in patients with gastric cancer patients (P<0.05). There was no significant difference in the incidence of malnutrition and prealbumin in patients with gastric cancer patients (P>0.05). Conclusion Nutrition cognition and behavior intervention can effectively improve the nutritional cognition of family caregivers, improve the nutritional status of gastric cancer patients during chemotherapy, and promote the rehabilitation of patients with gastric cancer and improve their quality of life.
7.Nutritional knowledge and needs of caregivers of patients with gastric cancer

Objective To investigate and analyze the nutritional knowledge needs and cognitive level of the main family caregivers of patients with gastric cancer. Methods A total of 208 patients who underwent radical resection of gastric cancer and received complete chemotherapy at the Department of gastrointestinal surgery from September 2015 to February 2017 were selected.Each patient selected 1 family caregivers.A questionnaire survey was conducted among 208 family caregivers, including general information, nutritional knowledge needs questionnaire, nutrition knowledge, attitude and practice and so on. Results The total score of nutrition knowledge, attitude and practice among the family caregivers of patients with gastric cancer was (41.11 ± 9.33) points. The score of nutrition knowledge was (6.26 ± 1.87) points. The score of nutrition attitude was (15.45±4.75) points.The score of nutrition practice was (17.47± 4.45) points .There was a significant positive correlation between nutrition knowledge and attitude(r=0.88, P<0.05), knowledge and practice (r=0.766, P<0.01), attitude and practice (r=0.186, P<0.01) by using Pearson correlation.Family caregivers had a strong demand for nutrition knowledge. 37.5%(78/208) of family caregivers chose to cooperate with doctors and nurses to carry out nutrition knowledge. 51.4%(107/208) of family caregivers preferred bedside individualized guidance. Conclusions Family caregivers have a strong demand for nutrition knowledge,but the nutrition cognition level is poor. Health care providers should strengthen the education on patients and family caregivers' knowledge of nutrition, improve the nutrition knowledge level of caregivers and care ability.
8.Construction and application of nurse′s training and assessment information system in level three general hospitals

Objective To construct nurse′s training and assessment information system in level three general hospitals and apply it in clinical. Methods According to our hospital designed nursing training and assessment plan, medical nursing management information system divided itself into Nursing Department, Education Department, and Ward three grade nursing training management model, and the function maintenance and application of system were took by Nursing Department, Education Department, and Ward. All nursing staff members could choose the type of training according to the study plan and work requirement, and were assessed theory and skills once per half year. Before (December 2013) and after (December 2014) training management system application, Nursing Department investigated 468 nurses for training effects by the awareness of training information and satisfaction of training management. Results After the application of system, it improved the awareness of training information and training management satisfaction (χ2 = 31. 26,28. 64;P < 0. 01). Conclusions Nursing training and assessment system effectively achieves annual training plan made by Nursing Department, and further strengthen nursing training management and nursing quality management.
9.Application effects of peer assisted learning method in the skill training for new nurses

Objective To explore the application effects of peer assisted learning method on the skill training on clinical nursing for new arrival nurses, so as to provide evidence for clinical skill training. Methods Cluster sampling method was adopted and randomly grouped it as the experimental group (n=185) and the control group (n=186) involved 376 new recruited nurses in the First Affiliated Hospital of Zhengzhou University in 2015. The nurses in experimental group used peer assisted learning method and conventional nursing, and the only conventional nursing was adopted in the control group. The effects in two groups were compared.Results A total of 185 nurses acquired (93.58±4.69) average score in skill examination of experimental group while it was (88.73±4.39) in the control group, and there were statistical significance between two groups (t=10.144,P<0.01). Nurses in experimental group believed they developed greater skills in mastering the theoretical and operation knowledge, as well as the critical thinking ability, communication ability, cooperation ability and so on, and so does the teachers and head nurses in the ward(P<0.05). Conclusions The peer assisted learning method is a good way for nursing skills training and improving the nurse's overall service quality and clinical nursing quality.
10.Qualitative study on working experience of nurses with master degree in rotation period

Objective? To explore the working experience of nurses with master degree in different departments duringrotation period. Methods? Using the phenomenological method of qualitative research, 15 nurses with regularly trained masters' degree from December 2018 to March 2019 in the First Affiliated Hospital of Zhengzhou University were selected for in-depth interviews. Theme analysis was used to analyze data and refine themes. Results? A total of 5 themes were extracted as follows: diversification of work motivation, strengths and weaknesses of "contradictory" , strong thirst for knowledge, poor sense of belonging and desire for career development. Conclusions? The nurses with regularly trained master's degree showed strong work enthusiasm and curiosity during their rotation period, but they expressed work pressure and lack of career planning. Nursing managers are advised to pay attention to listening to the voice of the nurses with Master's degree and provide them with opportunities and platforms to help them make career planning and meet the needs of career development.
