1.A case report of the vasculitis misdiagnosed as pneumonia with stubborn heart failure

Wegener's granulomatosis (WG) is a kind of systemic necrotizing granulomatous vasculitis characterized by capillary and small arteriovenous involvement.The typical WG triad is lesions of the upper and lower respiratory tract and kidneys.Only changes in the respiratory tract without renal lesions are localized WG.Localized WG is very easy to be misdiagnosed because of its nonspecific symptoms and radiological manifestations.The author found one case of localized WG,which showed pulmonary symptoms and refractory heart failure.The chest CT showed that multiple irregular nidus,blurred and density enhanced lesions in both lungs.The patient was misdiagnosed as pulmonary infection for a long time.The patient was eventually diagnosed as asantineutrophil cytoplasmic antibody (ANCA) associated vasculitis and WG after two times of examination of protease-3(PR3) ANCA were positive.This article reviewed the diagnosis and treatment of the patient,and reviewed the relevant literatures.
2.Clinical Observation of Bleeding in Zhongfu on Pulmonary Arterial Hypertension Induced by Chronic Cor Pulmonale

[Objective]It is to observe the clinical effect of bleeding in Zhongfu on pulmonary arterial hypertension induced by chronic cor pulmonale. [Methods]60 cases of patients with chronic cor pulmonale were randomly divided into two groups. Control group was given standardizing normal drugs , and the treatment group was added with bleeding in Zhongfu on the basis treatment above. Pulmonary artery pressure and index of blood rheology were compared between the two groups after 14 days. [Results]Compared with each other,the difference was significant( P<0.05). Indexes of pulmonary artery and blood rheology were al improved obviously after treatment in both groups, especial y in treatment group( P<0.05). [Conclusion]Combination with bleeding in Zhongfu can more effectively increase the clinical effect, low down pulmonary artery pressure and improve blood viscosity.
3.A comparative study of biodegradable and metallic fixation for the treatment of dislocation of acromioclavicular joint

Objective To compare the results of treatment with biodegradable ligaments and metallic fixation for total acromioclavicular joint dislocation. Methods Twenty eight patients with total acromioclavicular joint dislocation were treated with two different materials, 1) by Kirschner pin and steel wire or screw and steel wire tension band (MF group,16 cases), and 2) reconstructed by artificial coracoclavicular ligament and coracoacromial ligament (BF group,12 cases). There were 17 males and 11 females. All patients had acute dislocation. The time from injury to hospital admission varied from 0-8 days (average, 1.5 days). Results All patients were availble for an average duration of follow up of 39.5 months (range,8-70 months). The results were evaluated by radiographic representations and joint function according to Lazcano's standards. In BF group, 10 patients were assesed as good and 2 as fair. In MF group, 13 patients were assesed as good, 2 as fair and 1 as poor. There were no statistic differences between these two methods. Conclusion Biodegradable ligament fixation is believed to be a good simple method for the treatment of acromioclavicular joint dislocation without the necessity of a second operation for removal of implants.
4.Genitourinary phenotype of androgen receptor knockout mice

Objective To investigate the genitourinar y phenotype in androgen receptor knockout (ARKO) mice. Methods Using Cre-lox technique, the female Flox-AR mice were crossed with male ACTB-Cre mice,and the genotype of their filial generations was confirmed by PCR .By screening,5 ARKO mice were generated as study subjects and 5 wild type mice served as controls.The genitourinary phenotypes of the 2 groups were compared.Th e anal genital distance and testis weight were measured;serum testosterone and e stradiol levels were assayed by ELISA. Results The anal genital distance in ARKO group was (0.5?0.1)cm,while it was (1.1?0.1)cm in wil d type group.The prostate,seminal vesicle, epididymis and spherical cavernous bo dy muscle were absent in ARKO group with the reduced testis weight of (0.006 ? 0.001)g;while those were normal with the testis weight of (0.086?0.002)g in w ild type group.The serum testosterone level in ARKO group was (0.056?0.045)nmol /L,and it was (0.843?0.736)nmol/L in wild type group.The serum estradiol level in ARKO group was (1390.1?294.3)pmol/L,and it was (786.2?150.8)pmol/L in wild type group.The differences were statistically significant between the 2 groups ( P

6.Effect of Shenfuhuang Injection on Function of Organs and Mortality of Septic Rats

Objective To investigate the effect of Shenfuhuang injection (SFH Injection) on function of organs and mortality of septic rats, using a sepsis mode by cecal ligation puncture(CLP). Methods 140 male Wistar rats were randomly divided into four groups: normal control group, sham operation group, CLP group (being further divided into 2, 8, 24, 48 h subgroups), SFH injection treatment group (being further divided into 2, 8, 24, 48 h subgroups, drug were inraveous injected just after operation and per 12 h). The mortality of animals and altering organ’s function were observed within 7 days. Results Compared to CLP model, mortality of Septic rats in SFH injection group was significant lower, and the treatment prolonged survival duration. SFH Injection group owned lower numerical values of ALT, AST, BUN and Cr in serum. Conclusion SFH Injection can obviously decline the mortality of septic rats and play an important role in protecting organ function.
7.Effect of Shenfuhuang Injection on Tissue and Plasma Tumor Necrosis Factor ? Level in Septic Rats

Objective To evaluate the effect of Shenfuhuang injection (SFH) on tissue and plasma TNF-? level and multiple organ dysfunction in septic rats. Method Wistar rats were subjected to sepsis by ceral ligation and puncture, then they were randomly divided into four groups, normal control, sham operation grop, septic group, SFH group. Animals were sacrificed at 2, 8, 24, 48 h. TNF-? level of lung, liver and plasma were measured. DAO activity was determined. Result TNF-? level of liver, lung and plasma were markedly increased at 2~8 h post-CLP compared with sham operation group. In SFH group, TNF-? level decreased in different degrees. DAO activity was much lower in SFH group than in CLP group at 2, 8 h. Conclusion Early applying of SFH could markedly inhibit TNF-? level of vital organs and injury of intestine, thereby preventing the development of excessive inflammation response and subsequent multiple organ dysfunction syndrome associated with CLP-induced sepsis.

9.Kaino-sinus syndrome: a case report.

Kaino-sinus syndrome is one new complication of endoscopic sinus surgery. The patient had an operation of endoscopic surgery, because he was suffered from an illness of frontal nasal inverted papilloma again. After operation he was suffering pain in left epicanthal folds and frontal part, and had dry scab in nasal cavity when he breathed cold air. The symptoms could relieve by blocking prenaris. We found he had a big aperture of frontal sinus and dry scab in nasal endoscopic examination. His symptoms disappeared after cleaning and washing accessory nasal cavity by salt water.

Objective To evaluate the efficacy of combination of traditional educational methodology with problem-based learning pedagogy in urology.Method The traditional educational pedagogy of urological tuberculosis was modified by combining with PBL methodology.Result More students were interested and active in this pedagogy.Conclusion The combination of traditional educational methodology with PBL is a helpful pedagogy.
