1.Cochlear Implants in Patients with Large Vestibular Aqueduct Syndrome

Objective To study the feasibility of the cochlear implantation and effects in the treatment of large vestibular aqueduct syndrome.Methods A total of 37 patients(74 ears)diagnosed with large vestibular aque-duct syndrome received cochlear implants since 2002 at the Renmin Hospital of Wuhan University.We made a retro-spectively analysis of these 37 patients(37 ears).While 37 patients(37 ears)with normal cochlear structures were considered as the control group.After activation,their speech perception at pre-operation and at 3,6,9,12 months affer the operation were evaluated.Their CAP and SIR scores were collected through questionnaires at each assess-ment interva1 .Results In LVAS group,5 patients(5 ears)had serious gusher,while no gusher was abserved in the control group.The two groups of the electrodes were all inserted in the cochlea.No serious complications occurred after implantation in the two groups.The impedance of the electrodes,the T level,C level and the hearing thresholds were similar with the normal group with cochlear implantations.The results had no significant differences when compared with the normal cochlear group (P>0.05).The speech perception ability,CAP and SIR scores were im-proved along with the increase time of implant use.At the same recovery time,the results had no significant differ-ence in the two groups(P>0.05).At the same group,the results between pre-operation and 3,6,9,12 months post-operation after device activation had significant differences (P<0.05).Conclusion Cochlear implants could be performed safely in patients with large vestibular aqueduct syndrome.The primary outcomes for patients with large vestibular aqueduct syndrome were similar to those with normal cochlear structure following the cochlear im-plantation.
2.CD4~+ CD25~+ regulatory T cells in infection immunity

Immune suppression is a very important mechanism in alleviating immunologic injury,and recent study has showed that CD4~(+)CD25~(+) regulatory T cell is an important component of the immune suppression system.This T cell subset has an obvious immune-suppressing effect and produces its effect not by secreting cytokine but contacting cell directly.In this review,we describe the biological characteristics of CD4~(+)CD25~(+) regulatory T cell and its functions in infection immunity.
3.Research on the function of network information in the medical research teaching

Objective: To explore the function of network information in the medical research teaching. Methods: To analyze the function of network information in the medical research, combined with the current actual medical research teaching, to understand correctly the related problems, and find the corresponding appropriate solutions. Results: In medical research teaching, the students should be led to use correctly the network tool for keeping track the research forefront, solving scientific problems, broadening the research mindset; avoiding addiction, split personality and internet plagiarism. Conclusion:In the medical research teaching, the students should be guided to use the network tools correctly, which maximize its effect.
4.Clinical analysis of ultrasound-guided catheter drainage and vacuum-assisted Mammotome system in treatment of lactational breast abscess

Objective To investigate the clinical application value of ultrasound-guided catheter drainage and vacuum-assisted Mammotome system in the treatment of lactational breast abscess. Methods The clinical data of 46 patients with lactational breast abscesses were analyzed retrospectively. All patients were divided into control group (28 patients) and treatment group (18 patients). Homemade irrigation and drainage tube was used in control group,and vacuum-assisted Mammotome system was used in treatment group. The curative effect of 2 groups was compared. Results All patients were cured. The mean healing time in treatment group was (9.5 ± 1.3) d,in control group was (12.5 ± 2.2) d, and there was significant difference (P < 0.01). The rate of breast-feeding in control group was 57.1% (16/28), in treatment group was 13/18,and there was no significant difference (χ2=1.069, P>0.05). All patients recovered well without recurrence. Conclusions Ultrasound-guided catheter drainage should be employed as first line treatment of breast abscess with diameter greater than the 3 cm. If patient′s economic conditions are good and have high requirement on the beauty outcome, vacuum-assisted Mammotome system can be applied as treatment option.
5.Effect of Shenfuhuang Injection on Tissue and Plasma Tumor Necrosis Factor ? Level in Septic Rats

Objective To evaluate the effect of Shenfuhuang injection (SFH) on tissue and plasma TNF-? level and multiple organ dysfunction in septic rats. Method Wistar rats were subjected to sepsis by ceral ligation and puncture, then they were randomly divided into four groups, normal control, sham operation grop, septic group, SFH group. Animals were sacrificed at 2, 8, 24, 48 h. TNF-? level of lung, liver and plasma were measured. DAO activity was determined. Result TNF-? level of liver, lung and plasma were markedly increased at 2~8 h post-CLP compared with sham operation group. In SFH group, TNF-? level decreased in different degrees. DAO activity was much lower in SFH group than in CLP group at 2, 8 h. Conclusion Early applying of SFH could markedly inhibit TNF-? level of vital organs and injury of intestine, thereby preventing the development of excessive inflammation response and subsequent multiple organ dysfunction syndrome associated with CLP-induced sepsis.
6.Construction of Tre enzyme eukaryotic expression vector and identification of its function in loxLTR sequence-specific recognition

Objective Clearing HIV provirus is the key to cure AIDS .The study was to construct the Tre enzyme eukaryotic expression vector and identify its function in specific recognition of loxLTR sequence in HIV provirus . Methods Tre gene was in-serted into eukaryotic expression vector pcDNA 3.1 gene recombination manipulation by genetic recombination techniques including gene synthesis , PCR, restriction enzyme digestion and ligation .EGFPpA-LoxLTR sequence was inserted into pmCherry-N1 vector and was tested by restriction enzyme digestion , PCR and sequencing .Constructed vectors were electroporated into HeLa cells , then using fluorescence microscopy to observe fluorescence intensity changes . Results PCR, restriction enzyme digestion , electrophoresis and sequencing confirmed that Tre enzyme eukaryotic expression vector had been constructed successfully , and it could specifically recog-nize and cut loxLTR sequence after being transfected into Hela cells . Conclusion Constructed Tre enzyme eukaryotic expression vector can be expressed in Hela cells and specifically recognize loxLTR sequence , which has prepared the experimental ground for fur-ther studies of clearing HIV provirus .
7.Effect of Shenfuhuang Injection on Function of Organs and Mortality of Septic Rats

Objective To investigate the effect of Shenfuhuang injection (SFH Injection) on function of organs and mortality of septic rats, using a sepsis mode by cecal ligation puncture(CLP). Methods 140 male Wistar rats were randomly divided into four groups: normal control group, sham operation group, CLP group (being further divided into 2, 8, 24, 48 h subgroups), SFH injection treatment group (being further divided into 2, 8, 24, 48 h subgroups, drug were inraveous injected just after operation and per 12 h). The mortality of animals and altering organ’s function were observed within 7 days. Results Compared to CLP model, mortality of Septic rats in SFH injection group was significant lower, and the treatment prolonged survival duration. SFH Injection group owned lower numerical values of ALT, AST, BUN and Cr in serum. Conclusion SFH Injection can obviously decline the mortality of septic rats and play an important role in protecting organ function.
8.Evaluation of the accuracy of administration of propofol by a TCI system in children

Objective To develop a target-controlled infusion (TCI) system incorporating population pharmacokinetics of propofol for children and evaluate its accuracy. Methods The TCI system was composed of a Samsung Q20 laptop computer, Graseby 3500 infusion pump, Stanpump software (Version 1.0 designed by Shafer et al. Stanford) and pharmacokinetic parameter set for propofol in children reported by Lian et al. Twenty-four ASA Ⅰ children undergoing elective orthopedic or urological surgery under general anesthesia were divided into 2 age groups: group A 3-5 yrs ( n = 12) and group B 5-10 yrs (n = 12). Radial artery and internal jugular vein were cannulated. The pediatric patients were sedated with ketamine 4 mg?kg-1 IM (uncooperative patients) or 2 mg? kg-1 Ⅳ( cooperative patients) . Anesthesia was induced with fentanyl 3 ?g?kg-1, propofol by TCI and vecuronium 0.1 mg?kg-1. Target plasma concentration of propofol was set at 3 ?g?ml-1. TCI of propofol was maintained for 60 min. Arterial blood samples were taken at 1, 3, 5, 10, 20, 30, 40, 50 and 60 min after TCI was started and at 2.5, 5, 10, 20 and 30 min after termination of TCI for determination of blood propofol concentration by HPLC. The median performance error ( MDPE) , MDPE without first five minutes ( MDPE1) , median absolute performance error ( MDAPE), wobble and divergence were calculated. Results During the first 40 minutes of TCI there was a remarkable difference between the measured plasma propofol concentration ( Cm) and the target plasma concentration (Cp). The difference was narrowing gradually until the 50 min of TCI. After the termination of TCI the Cm was significantly lower than Cp. The MDPE was 27% in group A and 26% in group B; MDPE1 was 7% (A) and 12% (B) and MDAPE 27% (A) and 26% (B) during TCI. The wobble was 23 % (A) and 24% (B) and the divergence - 0.75%?h-1 (A) and -0.80%?h-1 respectively. Conclusion The bias and divergence of our TCI system for propofol are small and the accuracy is high and a stable plasma concentration of propofol can be maintained in children.

The paper elucidated the necessity of programmed operation in competence education of medical postgraduate students.This paper introduced the specific methods of programmed operation mode,exactly as selection of the best,scientific combination,coordination and arrangement,allocation of responsibility to individual,exact procedures,succession and development.The seven years practice experience in their colleges testified that these operational methods aroused the enthusiasm of postgraduate students' participation in campus activities and trained postgraduate student cadres.Meanwhile,these methods activated the campus culture and atmosphere and enhanced the comprehensive competence of postgraduates wholly.
10.A Case Study Of Auditory Brainstem Implant in a Patient with Neurofibromatosis Type Ⅱ

Objective To study the clinical characteristics and treatment of neurofibromatosis type Ⅱ (neuro-fibromatosis type 2 ,NF2 ,bilateral acoustic neuroma) ,and the effects of auditory brainstem implant for treating to-tal deafness after bilateral acoustic neuroma resection .Methods One case of bilateral acoustic neuroma and all clini-cal data in terms of diagnosis ,treatment and hearing -speech rehabilitation after surgery were retrospectively stud-ied .Results The patient was a thirteen years old boy .His clinical symptoms were hearing loss on the right ear ,tin-nitus ,hoarseness and gait instability three years .MRI showed space occupying lesion in the cerebellopontine angle . The postoperative pathological diagnosis was bilateral acoustic neuroma .The initial switch -on was peformed six weeks after the surgery ,and confirmed that all electrodes generated listening responses .As the extension of recov-ery time ,the correct recognition rate of patients on the natural environment sound ,vowel ,monosyllabic were on the rise and the pure tone hearing threshold gradually decreased .The vowel correct recognition rate of postoperative 6 , 9 ,12 ,24 ,and 36 months were 14% ,18% ,20% ,24% ,and 20% ,respectively .The recognition rate of monosyl-labic and open words at each postoperative rehabilitation stage were 0 .Conclusion The clinical characteristics and treatment of bilateral acoustic neuroma were different from the unilateral acoustic neuroma .The individualized treat-ment should be followed .Auditory brainstem implant could be performed in patients with post - bilateral acoustic neuroma resection .The accurate location of the cochlear complex during the surgery was the crucial point for the success of ABI .
