1.Investigate of three-dimensional power Doppler ultrasound used in placental perfusion assessment in pre-eclampsia pregnancies

Objective To investigate the placenta perfusion in pre-eclampsia by three-dimensional power Doppler ultrasound and analysing the clinical outcome. Method 80 cases of normal and pre-eclampsia pregnant women from July 2007 to May 2008 in Beijing Obstetrics and Gynecology Hospital were studied. The control group of 36 cases, pre-eclampsia were divided into three groups: a total of 44 cases, mild pre-eclampsia group (9 cases), severe pre-eclampsia group (26 cases) and chronic hypertension with pre-eclampsia group (9 cases). The placental vascular index (VI), flow index (FI), blood vessels and blood flow index (VFI) by three-dimensional Doppler histogram and the umbilical blood flow systolic to diastolic (S/D) by color Doppler flow imaging were calculated and the gestational age after delivery, birth weight, placental weight after birth were recorded respectively. Results (1)VI、 FI、 VFI and umbilical blood flow S/D value :6.3±2.9, 38.6±4.4, 2.7±1.3, 2.5±0.6 in normal group ;5.7±3.8, 36.3±7.2, 2.4±2.0, 2.4±0.3 in mild pre-eclampsia group;3.0±2.4, 31.7±5.0, 1.1±1.0, 2.9±1.3 in severe pre-eclampsia group ;2.2±1.6, 26.1±4.4, 0.8±0.6, 3.1±1.6 in chronic hypertension with pre-eclampsia group. Placenta of normal group and mild preeclampsia group VI, FI, VFI were no significant difference (P>0.05), placental VI, FI, VFI of severe preeclampsia group and chronic hypertension with severe pre-eclampsia group were significantly lower than the normal group (P<0.01) and than mild pre-eclampsia group (P<0.05). The umbilical blood flows were not obvious difference among four groups (P>0.05) ; (2) Gestationul age after birth, birth weight, low newborn weight rate and placental weight:(38.7±1.5 ) weeks, (3280±520) g, 3%, (568±141) g in normal group; (37.9±1.0) weeks, (2971±265) g, 0,(576±98) g in mild pre-eclampsia group; (33.2±2.6) weeks, (1820±737) g,58%, (458±154) g in severe pre-eclampsia group; (32.6±2.6) weeks, (1497±533) g, 7/9, (396±141) g in chronic hypertension with pre-eclampsia group. The normal group and mild pre-eclampsia group in gestationul age after birth, birth weight, low newborn weight rate and placental weight were not significant difference (P>0.05); severe pre-eelampsia and chronic hypertension with severe pre-eclampsia group in them were significantly lower than the normal group (P<0.01) and than mild pre-eclampsia group (P<0.05).Conclusions (1) Placental blood flow perfusion of the severe pre-eclampsia and pre-eclampsia with chronic hypertension of pregnancy decreased resulting in clinically lower placental weight, birth weight and gestational age at delivery, but there were no obvious differences in umbilical blood flow S/D values. (2) The investigation was helpful to clinical diagnosis in the placenta perfusion of pre-eclampsia by three-dimensional power Doppler ultrasound.
2.SURYEY ON THE DISTRIBUTION OF DIETARY GLYCEMIC LOAD IN CHINESE ADULTS

Objectives To investigate the consumption of carbohydrate and distribution of dietary glycemic load (GL) in Chinese adults. Method Data from China National Nutrition and Health Survey in 2002 were used for this analysis. Dietary glycemic load was calculated as a function of glycemic index, carbohydrate content and frequency of intake of individual foods. Results Dietary glycemic load was contributed by 92% from cereals. People with low dietary GL /4184 kJ tended to consume more fat, less cereals, and less carbohydrates. The prevalence of overweight and obesity was higher in those with low GL/4184 kJ, especially in urban residents. Conclusion People with low GL/4184 kJ tend to consume more fat, less cereals, less carbohydrates. The situation in urban residents is more serious. Based on factors including cereals intake, energy contribution from fat and carbohydrates, prevalence of malnutrition, overweight and obesity. A proposed range for dietary GL /4184 kJ in Chinese adults can be 80~120.
3.STUDY ON THE RELATIONSHIP BETWEEN DIETARY CARBOHYDRATE AND GLYCEMIC LOAD AND DYSLIPIDEMIA RISK IN ADULTS

Objectives To analyze the relationship between dietary carbohydrate and glycemic load(GL) and the plasma lipid level and dyslipidemia risk. Method The sample of adults above 18y of age from "National Survey on Nutrition and Health Status of the Chinese People 2002" was used,excluding pregnant women,mothers and people who controled their diets. Data of GI from China Food Composition 2002 and International Glycemic Index Table 2002 were used. The dietary GL as the indicator of dietary carbohydrate was calculated by multiplying the carbohydrate content of each food by its glycemic index,then multiplied by frequency of consumption and summed up from all food. Correlation analysis,multiple regression and Logistic regression were used to analyze the relationship between dietary GL and plasma lipid level and dyslipidemia risk. Results Dietary GL remained inversely associated with plasma total cholesterol(TC) and low density lipoprotein cholesterol(LDL-C) . With the increase of dietary GL,the risk of hypercholesterolemia and high blood LDL-C decreased. In the meantime dietary GL remained negatively associated with plasma high density lipoprotein cholesterol(HDL-C) . With the increase of dietary GL,the risk of low blood HDL-C increased. Dietary GL remained inversely associated with plasma triglyceride(TG) ,but showed no significant influence on risk of hypertriglyceridemia. Conclusion The traditional Chinese dietary pattern which is characterized by cereals as staple food is helpful to decrease dyslipidemia risk.

Objective:
To analyze the status of hypertension and isolated systolic hypertension and to investigate the relationship between the prevalence of hypertension and dietary factors among Chinese elderly aged 60 yea and over during 2010-2012.
Methods:
Using a multi-stage stratified probability proportional to size cluster randomization sampling method during 2010-2012, a total of 14 791 old people aged 60 years old and over were investigated at 150 counties (districts) from 31 provinces (autonomous regions and municipalities) in China. Information on demographic socio-economic status and the physical activities was collected through questionnaires. Blood pressure measurement and physical examination were carried out by standard methods. The 3 days consecutive individual 24 hours dietary investigation was used to calculate food and nutrients intake of the subjects. Multiple logistic regression was used to investigate the possible related dietary factors for the hypertension and isolated systolic hypertension.

6. Analysis of correlation between serum fatty acid profile and cognitive impairment in the elderly

Objective:
To explore the relationship between cognitive impairment and serum fatty acid profile.
Methods:
Participants aged ≥60 years were selected from 3 communities in Shijiazhuang in 2015. The questionnaire was performed to collect basic information. And Montreal Cognitive Assessment was applied for the evaluation of cognitive impairment. Blood lipid parameters and serum fatty acid profiles were analyzed for all the subjects. A total of 529 subjects were finally included in this research. Multivariate ordinal logistic model was used to analyze the relationship between serum fatty acid profile and cognitive impairment.

OBJECTIVETo analyze the serum total cholesterol level and the prevalence of hypercholesterolemia and borderline high cholesterolemia among urban-resident adults in China from 2010 to 2012.
METHODSData were from Chinese Nutrition and Health Surveillance in 2010-2012. Multi-stage stratified proportion to the population cluster random sampling method was conducted to determine 54 042 adult residents in 34 big cities and 41 small and medium-sized cities. Serum total cholesterol was measured by cholesterol oxidase method. Hypercholesterolemia and borderline high cholesterolemia were evaluated by Guidelines on Prevention and Treatment of Blood Lipid Abnormality in Chinese Adults 2007 edition. Standardized cholesterol level (x ± Sx) and prevalence of hypercholesterolemia and borderline high cholesterolemia were calculated through weighted complex sampling processing by population data released by the National Bureau of Statistics in 2009.
RESULTSThe mean level of urban-resident adults's erum total cholesterol was (4.58 ± 0.05)mmol/L, (4.58 ± 0.05)mmol/L for both male and female. The cholesterol concentration level in big cities and small and medium-sized cities were (4.66 ± 0.04)mmol/L and (4.57 ± 0.06) mmol/L, respectively. It increased with age gradually from the minimum level, which was (4.18 ± 0.05)mmol/L in aged 18 to 29 group, till reached the maximum, which was (4.94 ± 0.05)mmol/L in aged 60 to 69 group, and then it declined to (4.92 ± 0.06)mmol/L in aged 70 group. The prevalence of hypercholesterolemia and borderline high cholesterolemia were 5.6%(95%CI:4.5%-6.6%) and 24.7%(95%CI:21.6%-27.9%) totally; 5.1% (95%CI:4.1%-6.1%) and 24.4% (95%CI:21.0%-27.6%) for male, 6.0% (95%CI:4.7%-7.3%) and 25.1%(95%CI:21.8%-28.4%) for female; 6.0%(95%CI: 5.0%-7.0%) and 27.7%(95%CI:24.9%-30.6%) in big cities, while 5.5%(95%CI: 4.2%-6.7%) and 24.2%(95%CI:20.5%-27.9%) in small and medium-sized cities. The minimum prevalence of hypercholesterolemia was in aged 18 to 29 group and maximum in aged 70 group, which were 2.2%(95%CI: 1.4%-3.0%) and 10.3%(95%CI: 7.1%-13.4%), respectively; while the lowest borderline high cholesterolemia prevalence was in aged 18 to 29 group and the highest in aged 60 to 69 year group, which were 12.9%(95%CI: 10.4%-15.4%) and 37.6% (95%CI: 33.6%-41.6%), respectively.
CONCLUSIONThe serum total cholesterol level and the prevalence of hypercholesterolemia and borderline high cholesterolemia were high among urban adults in China from 2010 to 2012, and more attention should be paid for high serum total cholesterol level among older adults.

Objective:
This study was to analyze the association of lipid parameters with insulin resistance of Chinese elderly population in different glycemic status.
Methods:
Data were from China National Chronic Diseases and Nutrition Survey (2015). A total of 15 535 participants aged 60 and above who had completed survey questionnaire, physical examination, fasting blood biochemistry and insulin measurements were included in this study. According to the American Diabetes Association (2010) criteria, the participants were divided into normal glucose regulation, pre-diabetes, newly-diagnosed diabetes and previously-diagnosed diabetes. Multivariable logistic regression was preformed to assess the effects of lipid parameters on insulin resistance in different glycemic among the elderly population.

Objective:To investigate the bacterial composition and antimicrobial resistance profile of clinical isolates from bloodstream infections in China.Methods:The clinical bacterial strains isolated from blood culture were collected during January 2020 to December 2020 in member hospitals of Blood Bacterial Resistant Investigation Collaborative System (BRICS). Antibiotic susceptibility tests were conducted by agar dilution or broth dilution methods recommended by Clinical Laboratory Standards Institute(CLSI, USA). WHONET 5.6 was used to analyze data.Results:During the study period, 10 043 bacterial strains were collected from 54 hospitals, of which 2 664 (26.5%) were Gram-positive bacteria and 7 379 (73.5%) were Gram-negative bacteria. The top 10 bacterial species were Escherichia coli (38.6%), Klebsiella pneumoniae (18.4%), Staphylococcus aureus (9.9%), coagulase-negative Staphylococci (7.5%), Pseudomonas aeruginosa (3.9%), Enterococcus faecium (3.3%), Enterobacter cloacae (2.8%), Enterococcus faecalis (2.6%), Acinetobacter baumannii (2.4%) and Klebsiella spp (1.8%). The prevalence of methicillin-resistant Staphylococcus aureus (MRSA) and methicillin-resistant coagulase-negative Staphylococcus aureus were 27.6% and 74.4%, respectively. No glycopeptide- and daptomycin-resistant Staphylococci were detected. More than 95% of Staphylococcus aureus were sensitive to rifampicin and SMZco. No vancomycin-resistant Enterococci strains were detected. Extended spectrum β-lactamase (ESBL) producing Escherichia coli, Klebsiella pneumoniae and Proteus mirabilis were 48.4%, 23.6% and 36.1%, respectively. The prevalence rates of carbapenem-resistance in Escherichia coli and Klebsiella pneumoniae were 2.3% and 16.1%, respectively; 9.6% of carbapenem-resistant Klebsiella pneumoniae strains were resistant to ceftazidime/avibactam combination. The prevalence rate of carbapenem-resistance in Acinetobacter baumannii was 60.0%, while polymyxin and tigecycline showed good activity against Acinetobacter baumannii. The prevalence rate of carbapenem-resistance of Pseudomonas aeruginosa was 23.2%. Conclusions:The surveillance results in 2020 showed that the main pathogens of bloodstream infection in China were gram-negative bacteria, while Escherichia coli was the most common pathogen, and ESBL-producing strains declined while carbapenem-resistant Klebsiella pneumoniae kept on high level. The proportion and the prevalence of carbapenem-resistant Pseudomonas aeruginosa were on the rise slowly. On the other side, the MRSA incidence got lower in China, while the overall prevalence of vancomycin-resistant Enterococci was low.
