2.Protective effects of total glucosides of paeony on the injury of neonatal rat cadiocytes ;induced by H2O2

Objective To investigate the protective effects and mechanism of total glucosides of paeony (TGP) on the injury of neonatal rat cadiocytes induced by H2O2. Methods Cadiocytes of 3 days neonate rat was cultivated for 72 hours and divided into the model group, the TGP low (30μg/ml), medium (60 μg/ml), high (120 μg/ml) dose groups, Shuxuening injection (100 μg/ml) group and normal control group (n=8);and except the normal control group, the cells in other groups were induced by H2O2 (100μg/ml). After 6 hours, the morphology changes were observed and the survival rates were detected;the content of AST, CPK, LDH in culture medium were detected; the activity of SOD, CAT, GSH-Px and the content of MDA in cardiomyocytes were determinted;the apoptosis rates were detected. Results Compared with the model group, the survival rates of cardiomyocytes in TGP medium and high dose groups significantly improved (64.1%± 7.3%, 81.3%± 7.8%vs. 42.3%± 6.2%;P<0.05 or P<0.01);the content of AST (26.72 ± 6.03 U/ml, 23.86 ± 5.38 U/ml vs. 34.75 ± 6.91 U/ml), CPK (1.96 ± 0.35 U/ml, 1.59 ± 0.32 U/ml vs. 2.54 ± 0.40 U/ml), LDH (811.55 ± 162.32 U/L, 683.48 ± 134.14 U/L vs. 936.07 ± 140.94 U/L) in culture medium were significantly decreased (P<0.05 or P<0.01);the activity of SOD (85.34 ± 16.87 U/mg , 94.62 ± 17.75 U/mg vs. 56.45 ± 13.76 U/mg), CAT (22.76 ± 3.38 U/mg, 26.49 ± 3.72 U/mg vs. 16.13 ± 3.18 U/mg) in cardiomyocytes were significantly increased and the content of MDA (8.74 ± 2.05 nmol/mg, 6.91 ± 1.80 nmol/mg vs. 11.56 ± 2.19 nmol/mg) were significantly decreased (P<0.05 or P<0.01), the apoptosis rate (24.2% ± 5.5%, 13.4% ± 3.9% vs. 51.2% ± 9.1%) were significantly decreased (P<0.05 or P<0.01);and the activity of GSH-Px (3.54 ± 0.83 U/mg vs. 2.50 ± 0.67 U/mg) in cardiomyocytes of TGP high dose treatment group was significantly increased (P<0.05 or P<0.01). Conclusion TGP had dose-dependent protective effects on the injury of neonatal rat cadiocytes induced by H2O2, which perhaps related to its effects of improving the activity of antioxidase, inhibiting the oxidative stress, and decreasing the apoptosis rates.
3.Application of nerve root retractor in lumbar disc surgery: 116 cases report

ObjectiveTo investigate the applying value of nerve root retractor in lumbar intervertebral disc operation.Methods116 consecutive patients underwent lumbar discectomy performed with a new approach using a series of semi-tubular retractor with consecutively increasing diameters, and their clinical data was analyzed retrospectively.ResultsThe mean operative time was 30~90 min per level, average 50 minutes and estimated blood loss was 50~200 ml per level, average 120 ml, no complications occurred. All patients were followed up 3 to 18 months after operation. All patients had substantial relief of their lumbago and radiculopathy, in which 96 without any symptoms, 3 cases with gently lumbago and limb numbness, 8 cases with gently lumbago, and 9 cases with lightly limb numbness.ConclusionThe nerve root retractor for lumbar discectomy made specially by ourselves can facilitate a good exposure to lumbar disc and intervertebral space, prevent injury of nerve roots or cauda equina and decrease volume of bleeding during operation, so as to make the procedure safe and effective.

OBJECTIVE:To probe into a feasible work mode for special clinical pharmacist of hypertension.METHODS:Clinical pharmacists' works were summarized by considering the experience of clinical pharmacists and examples of pharmaceutical care in hypertension wards.RESULTS&CONCLUSIONS:Special pharmacists of hypertension should participate in clinical treatment team,check prescription every day and acquaintance with status quo of patients' medication so as to carry out pharmaceutical care suitability.It also should find out and resolve situation of ADR in time and master the characteristics of hypertension.
5.Effect of Tilting Table Exercise and Shengmaiyin on Alpha 1 Adrenogenic Receptor after Spinal Cord Injury in Rabbits

Objective To observe the effect of tilting table exercise and shengmaiyin on alpha 1 adrenogenic receptor (α1AR) in the superior mesenteric artery after spinal cord injury (SCI) in rabbits.Methods 46 rabbits were randomly divided into the sham group (n=6), SCI group (n=10), shenmaiyin group (n=10), exercise group (n=10) and combined treatment (shenmaiyin plus exercise) group (n=10). The rabbit model of SCI was established by complete transection of spinal cord at the T5 spinal segment. Tilting table exercise and shengmaiyin were applied to different groups for 28 days from 4 days after operation. The expression of α1aAR,α1bAR,α1dAR mRNA in the superior mesenteric artery was detected by RT-PCR at 31d after SCI.Results The mRNA expressions of α1aAR, α1bAR, α1dAR subtypes increased to different extent in rabbits of the shengmaiyin group, exercise group and combined treatment group compared with those of SCI group.Conclusion The role of tilting table exercise and shengmaiyin improving orthostatic hypotension after SCI in rabbits may be related to up-regulation of α1aAR, α1bAR, α1dAR mRNA in the superior mesenteric artery.
6.Relationship of the change of calcitonin gene-related peptide and degeneration of the motor endplate after spinal cord injury in the rat

ObjectiveTo observe changes of calcitonin gene related peptide (CGRP) and aetylcholinesterase (AChE) in motor endplate (MEP) after complete spinal cord injury (SCI) in the rat.MethodsSCI model of Wistar rats were established by transecting T10 spinal cord completely. The venters of the tibial anterior muscle were taken at 1, 2, 4 and 8 weeks postoperatively to carry out the experiment.The changes of CGRP and AChE in the MEP were investigated with immunohistochemical test.ResultsAfter complete SCI, the quantity and distribution of CGRP decreased in MEP of 1-week sample, and the changes of AChE were observed until 4-week sample, and although the staining was light and sparse, CGRP and AChE existed in MEP during 2 months postoperatively.ConclusionThere is degeneration of MEP in skeletal muscle after upper motor neuron injury; CGRP and AChE are related to the degeneration of MEP; CGRP test can show the changes of MEP earlier.
7.Combined use of modified chitosan and methylprednisolone promotes repair of sciatic nerve injuries in rats

BACKGROUND:Previous studies have shown that chitosan can promote the repair of peripheral nerve injury and methylprednisolone can improve the microenvironment around nerve injuries, which are commonly used in clinical treatment of acute central nervous system injury.
OBJECTIVE:To observe the effects of modified chitosan and methylprednisolone combination on repair of sciatic nerve injury in rats.
METHODS:The rat sciatic nerve was transected and microscopical y anastomosed immediately. Then, modified chitosan, methylprednisolone, methylprednisolone+modified chitosan, and saline were injected respectively around the anastomosis site, compared with the sham surgery group.
RESULTS AND CONCLUSION:Compared with other groups, the combination group showed shorter claw extending reflex recovery time (P<0.05). Significant differences were found between the combination group and other groups in nerve conduction velocity, the remnant rate of gastrocnemius wet weight, the diameter and section area of gastrocnemius cells (P<0.05). The nerve fibers through the anastomotic site were significantly increased, with consistent thickness and arrangement and less neurodegeneration when observed 12 weeks after operation. In conclusion, the modified chitosan combined with methylprednisolone could significantly promote the sciatic nerve repair.
8.Curative effect analysis of memantine and donepezil in the treatment of moderate to severe Alzheimer's disease

Objective To investigate clinical efficacy and safety of the memantine and donepezil in the treatment of moderate to severe Alzheimer's disease,in order to provide the basis for clinical treatment.Methods One hundred and twelve cases of moderate to severe Alzheimer's disease patients were given memantine plus donepezil(observation group) ,and single use of memantine treatment(control group), and the treatment for 24 weeks.Respectively before and after treatment, Mini Mental State Scale (MMSE), Alzheimer' s disease assessment scale (ADAS-cog), Alzheimer' s disease collaborative learning, daily life ability questionnaire (ADCS-ADL) ,and neuropsychological questionnaire (NPI) score, and adverse reactions were observed in the process of two kinds of therapy, the efficacy and safety of two kinds of treatment methods were evaluated.Results After treatment, the patients in observation group with MMSE, ADAS-cog, ADL and NPI evaluation results were (12.1 ± 2.1), (32.9 ± 8.3), (33.4 ± 5.0), (6.1 ± 3.1) scores, significantly improved compared with scoring (9.9±2.8), (46.2±7.6), (42.1±6.0), (10.5±2.9) scores before treatment,and the differences were statistically significant (t =2.138,-2.411,2.398, 2.107 respectively, P < 0.05).The control group after treatment in patients with MMSE, ADAS-cog, ADL and NPI evaluation results were (12.3±2.6), (33.1 ±7.2), (35.1 ±6.6), (6.7 ± 2.9) scores, significantly improved compared with scoring 11.0 ± 2.5,44.9 ± 6.9,42.2 ±6.6,10.9 ± 3.5 before treatment, and the difference had statistical significanc (t =2.101,-2.033,2.105, 2.400 respectively, P<0.05).After treatment, the difference of patients in the two groups of MMSE, NPI score had statistical significance(t =2.553,2.176, P<0.05).The adverse reactions in two group respectively were 32.14% (18/56) and 26.79% (15/56), less difference was no statistical significance (P>0.05).Vital signs checks,regular laboratory examination and ECG examination showed no obvious abnormalities.Conclusion Memantine combined with donepezil in the treatment of moderate and severe AD patients is superior to the singleeffect of memantine, and long-term use will not increase the risk of adverse reactions, which is safe and effective in the combined application.
9.Study of morphological and immunohistochemical characteristics of phyllodes tumors

Objective To investigate the morphological and immunophenotypic characteristics of phyllodes tumors (PT) and explore their significance in differential diagnosis and evaluation of prognosis.Methods 48 specimens with PT were observed under light microscope with HE and immunohistochemical staining (EnVision method).The antibodies including CD34,CD10,CD117,p53,and Ki-67 were used.10 cases of fibroadenoma were compared and the clinical data and follow-up results were analyzed retrospectively.Results All of 48 cases with PT presented as well-circumscribed masses with typically leaflike structures composed of double-layered epithelial component arranged in clefts and overgrowing hypercellular mesenchymal component.31 benign,12 borderline,and 5 malignant PT were diagnosed based on stromal overgrowth,cellular pleomorphism,mitosis,margins,and others.Focal necrosis was detected in 3 malignant cases and liposarcoma component existed in 1 case.Immunohistochemical staining showed that the positive rates of CD34 were 93.5 %,58.3 %,0 in benign,bordline,and malignant PT respectively,there were significant differences between each groups (P < 0.05).The expression of CD10 were 25.8 %,83.3 %,80.0 %,its expression in benign group was significantly different compared with bordline and malignant PT (P < 0.05).p53 was expressed in three groups with no significant difference (P > 0.05).CD117 and Ki-67 had high expression comparatively in malignant PT.All cases were treated surgically with a local recurrence rate of 22.9 %.Conclusions Reasonable surgery pattern based on accurate histopathological diagnosis is crucial to reduce the local recurrence rate of PT.Immunohistochemistry examination in PT is helpful to the differential diagnosis and evaluation of prognosis.
10.Determination of Stachydrine Hydrochloride in Different Preparations of Herba Leonuri

Objective:To investigate the quality of Herba Leonuri Preparations sold in market. Methods: The alkaloid content in preparations was determined according to the determination method in China Pharmacopoeia (1995). Results: There were significant differences among Herba Leonuri Preparations from different pharmaceutical factories, place of origin and batches. Conclusions: It is suggested that the determination item of stachydrine hydrochloride be added.
