1.Effect on Cytokines by Arthroscopy Combined with Reinforcing Kidney and Activating Blood Method in Treatment of Knee Osteoarthritis

By observing the arthroscopy cleaning technique with postoperative oral administration with reinforcing kidney and activating blood Chinese herbal medicine in the treatment of knee osteoarthritis (KOA), this article was aimed to evaluate the clinical curative effect and its influence on serum C endⅡ collagen type (CTX-Ⅱ) and cartilage oligomeric matrix protein (COMP). Patients who met the diagnostic criteria were randomly divided into the treatment group and control group, with 30 cases in each group. Joint limited debridement was firstly applied. And the degree of articular cartilage degeneration, synovial hyperplasia and inflammatory degree were observed and recorded. Preoperative venous blood was collected. The contents of CTX-Ⅱ and COMP were detected with ELISA method. After surgery, in the treatment group, surgery plus postoperative oral administration with reinforcing kidney and activating blood Chinese herbal medicine was given. In the control group, surgery plus oral administration with celecoxib capsules was given. Four weeks before and after the treatment, the WOMAC knee function score was used in the evaluation of clinical curative effect. The contents of postoperative serum CTX-Ⅱ and COMP were determined. The results showed that the effectiveness of the treatment group was higher than that of the control group (P < 0.05). In the treatment group, the WOMAC score in pain, rigidity, difficulty of activity and total score had significant differences compared with the control group (P < 0.05). There were statistical significances on serum CTX-Ⅱ and COMP in the treatment group before and after the treatment, as well as the comparison between the treatment and control group both before and after the treatment (P < 0.05). Serum CTX-Ⅱ and COMP had positive correlation with the degeneration of cartilage and synovial hyperplasia with statistical significances (P < 0.05). It was concluded that joint limited debridement under arthroscopy combined with reinforcing kidney and activating blood method had synergistic effect in the treatment of KOA. It can effectively relieve the clinical symptoms and improve the knee joint function. It can also obviously reduce the contents of serum CTX-Ⅱ and COMP. There was internal close relation between the articular cartilage degeneration, synovial hyperplasia under the arthroscopy in KOA and cytokines of CTX-Ⅱ and COMP. The cytokines of CTX-Ⅱ and COMP provided important references for the estimation of articular cartilage degeneration and synovial hyperplasia.
2.Significance of erythropoietin for the embryonic development of the lens

BACKGROUND:Studies have found that erythropoietin has a protective effect on embryonic development of the retina, but there are rare studies concerning the embryonic development of the lens. OBJECTIVE: To observe the difference in erythropoietin expression in the lens from Wistar rats at different embryonic periods and to investigate the effect and significance of erythropoietin in the embryonic development of the lens. METHODS:Clean Wistar rats with pregnancy for 10 days (n=5), 12 days (n=5), 14 days (n=5), 16 days (n=5), 18 days (n=5), 20 days (n=5) were randomly colected and divided into six groups. Every two embryonic ratsof the different 30 pregnant rats were obtained randomly by the caesarean operation under ketamine-induced anesthesia. The eye tissues of al the embryonic rats were isolated and cut into sections. The expression of erythropoietin protein and mRNA in rat lens was detected by immunohistochemistry and reverse t ranscription-PCR, respectively. RESULTS AND CONCLUSION:Erythropoietin was distinctly expressed at the six different embryo stages, and the expression of erythropoietin protein and mRNA gradualy increased from embryonic day 10 to embryonic day 16, and decreased from embryonic day 10 to embryonic day 20. There were significant differences between the six groups. These findings indicate that the expression of embryonic appears in a low to high to low fashion during the embryonic development of Wistar rats, which may be closely associated with the developing procedure of lens.
3.The interactive effect of vitamin D and triglyceride/high-density lipoprotein cholesterol on achieving glyce-mic control in hospitalized type 2 diabetes patients

Objective To explore the interactive effect of vitamin D and triglyceride(TG)/high-density lipoprotein cholesterol(HDL-C)on the time to achieve glycemic control in hospitalized patients with type 2 diabetes mellitus(T2DM).Methods A retrospective cohort study was conducted on 82 T2DM patients admitted to Henan Hongli Hospital from March 2021 to December 2023.Patients were categorized into two groups based on the time it took for their blood glucose to reach target levels during their hospitalization:the≤7 d group and the>7 d group.The baseline data,vitamin D levels,and TG/HDL-C ratios of both groups were compared.The relationship between vitamin D,TG/HDL-C levels,and the time it took for blood glucose to reach target levels was analyzed.Further-more,the TG/HDL-C levels of patients with varying vitamin D levels were compared,and the relative risk(RR)and receiver operating characteristic(ROC)curve were used to analyze the interactive effect and predictive value of vitamin D and TG/HDL-C levels on the time it took for hospitalized T2DM patients to achieve target blood glucose levels.Results The hospitalization duration of the group with a duration of>7 days was longer than that of the group with a duration of≤7 days.The fasting blood glucose,glycosylated hemoglobin,and TG/HDL-c levels were higher in the former group,while the vitamin D level was lower.The proportion of patients with vitamin D deficiency and severe deficiency was higher in the former group compared to the latter group(P<0.05).Pearson correlation analysis revealed that vitamin D level was negatively correlated with the time to achieve glycemic control(r=-0.733,P<0.001),while TG/HDL-C level was positively correlated with the time to achieve glycemic control(r=0.830,P<0.001).After adjustment,logistic regression analysis indicated that vitamin D(95%CI:0.482~0.694)and TG/HDL-C level(95%CI:1.053~1.392)remained independent factors influencing the time to achieve glycemic control in hospitalized T2DM patients(P<0.05).Patients with insufficient,deficient,and severely defi-cient vitamin D had higher TG/HDL-C levels compared to those with sufficient vitamin D(P<0.05).The relative risk(RR)of patients with vitamin D deficiency and elevated TG/HDL-C coexisting resulting in a hospitalization duration of>7 days for T2DM patients was 15.867,indicating a synergistic effect.The area under the receiver operating characteristic(AUC)curve for predicting the time to achieve glycemic control in hospitalized T2DM patients using a combination of vitamin D and TG/HDL-C was 0.929,which was greater than that of vitamin D and TG/HDL-C alone(Z=3.849,3.526,P<0.05).Conclusion The vitamin D and TG/HDL-C levels in hospitalized T2DM patients are closely related to the time of reaching glycemic targets.The simultaneous exposure of both factors can affect the time of achieving glycemic targets,and the combined prediction of vitamin D and TG/HDL-C has good reference value for predicting the time of achieving glycemic targets in hospitalized T2DM patients.
4.Influence of shared medical appointments on health-related quality of life and quality of sleep in patients after liver transplantation

Objective:To observe the influence of shared medical appointments on health-related quality of life and quality of sleep in patients after liver transplantation.Methods:By randomized controlled study, a total of 124 patients after liver transplantation were included from our hospital from January 2018 to January 2019, and according to the lottery method, all subjects were divided into the routine management group ( n=64) who received routine outpatient intervention and the shared medical management group ( n=60) who received shared medical appointments management. The health-related quality of life and quality of sleep were investigated and compared by post-liver transplant quality of life questionnaire (pLTQ) and Pittsburgh sleep quality index (PSQI) before intervention (the day of discharge) and after intervention (the end of the last shared outpatient service). Results:After intervention, the dimension scores of worry, economics, body function, emotional function, health service, complication and total score of pLTQ were improved in tow groups than before intervention [the routine management group: (41.90±7.61), (18.13±4.22), (22.22±5.10), (14.92±3.28), (20.39±4.87), (14.63±3.99), and (132.19±37.09) vs (32.25±5.55), (12.77±3.47), (17.58±4.72), (9.23±1.38), (15.17±4.81), (10.89±1.51) and (98.00±29.03) score, t=8.20, 7.85, 3.58, 12.79, 6.10, 7.01, 5.81, all P<0.001; shared medical management group: (46.12±7.92), (24.16±5.34), (25.55±5.42), (17.90±3.60), (24.81±5.12), (17.93±3.60) and (155.47±41.00) vs (32.57±5.69), (12.81±3.82), (17.00±4.70), (9.60±1.39), (15.39±4.84), (11.00±3.52) and (98.37±28.96) score, t=10.76, 13.39, 9.23, 16.66, 10.36, 10.66, 8.81, all P<0.001], and those in the shared medical management group were higher than those in routine management group ( t=3.03, 6.95, 3.53, 4.82, 4.93, 4.83, 3.32, all P<0.05). After intervention, the total score of PSQI scale were lower than before intervention in the routine management group [(10.48±2.14) vs (11.89±2.45) score, t=3.47, P=0.001], and the dimensions score of sleep quality, full-sleep time, sleep time, sleep efficiency, sleep disorders, daytime function, hypnotic and total score of PSQI were lower than before intervention in the shared medical management group [(1.41±0.32), (0.54±0.13), (1.17±0.26), (1.11±0.35), (1.21±0.27), (1.30±0.33), (1.08±0.21) and (8.05±1.75) vs (1.88±0.53), (0.86±0.37), (1.84±0.41), (2.05±0.56), (1.39±0.33), (1.47±0.43), (1.22±0.32) and (11.71±2.43) score, t=-5.88, -6.32, -10.69, -11.03, -3.27, -2.43, -3.65, -9.47, all P<0.05], and those in the shared medical management group were lower than those in routine management group ( t=-6.68, -6.39, -10.43, -10.97, -2.62, -2.12, -3.54, -6.90, all P<0.05). Conclusion:Shared medical appointments model can improve the health-related quality of life and quality of sleep in patients after liver transplantation, and improve the effectiveness of outpatient intervention.
