1.Discussion on precision medicine and the scientific research management of cancer hospital

U.S.President Barack Obama proposed a precise medical initiative plan in early 2015 which caused widespread concern.According to the definition given by National Cancer Institute,precision medicine is the use of genetic information for the diagnosis or treatment of individual diseases.In the announcement gave by Barack Obama,one of the specific contents in precise medicine,the short-term goals include strengthen the use of precision medicine in cancer treatment,tumor therapy has thus identified as one of the most important concerns in precise medicine,which was given a great hope.As a result,research management personnel in cancer hospital should aware of the great importance of precise medical research,empower themselves by learning related ideas and models of precision medicine,explore the precise medical development related to the management of scientific research in the new theories,new methodologies,new mechanism and new models,in order to play an important role in scientific research management department in the hospital precise medical research to fulfill the coordination and management functions.
2.Wound healing effects of silk fibroin-bone morphogenetic protein-2 scaffolds on inflammatory pulp in rats

Objective:To identify the healing effect of electrospun silk fibroin-BMP-2 as a biologic pulp capping agent to inflammatory pulp in rat caused by lipopolysaccharide ( LPS) .Methods:A total of 30 healthy adult male Wistar rats were randomly divided into five groups:(1) normal control group without operation;(2) blank control group without capping agents;(3) calcium hydroxide capping group;(4) electrospun silk fibroin capping group;(5) electrospun silk fibroin-BMP-2 capping group .Bilateral up-per first molars of each rat in group 2-5 were drilled to expose the pulp to LPS which was used to estab-lish a model of inflammatory pulp .The exposed pulp was capped with different capping agents or without capping agents.Then the hole was sealed.The animals were sacrificed on days 3, 7, and 14 post-opera-tion and histological analysis was carried out , including HE stain and CD 14 immunohistochemical stain . Results:On day 7 and 14 , the lowest inflammatory reaction score in HE stain among pulp capping groups was that of silk fibroin-BMP-2 group .The next were calcium hydroxide group and silk fibroin group .That of blank control group was the highest .The ranking of reparative dentine scores of those groups was just reversed.The D values of immunohistochemical stain of CD 14 were not significantly different in groups applied pulp capping agents but significantly lower than blank control group on days 3 and 7.However, the D value of silk fibroin-BMP-2 group ( 0 .145 ±0 .011 ) was significantly lower than blank control group (0.287 ±0.019), calcium hydroxide group (0.170 ±0.017) and silk fibroin group (0.175 ± 0 .018 ) on day 14 .Conclusion:Electrospun silk fibroin compounded with BMP-2 promoted wound hea-ling of exposed pulp and had better potential to stimulate formation of reparative dentine to establish a suitable environment for pulp recovery .
3.Preliminarily Proposed Diagnostic Criteria of Pathological Internet Use

Objective:To establish a preliminary diagnostic criteria of pathological internet use for field test in China.Methods:Through searching common "pathological internet use" diagnostic criteria or screen criteria in literature,a diagnostic criteria items pool and a preliminary diagnostic criteria of "pathological internet use" were established .According to the diagnostic criteria,each 2 senior professional psychiatrists in turn interviewed 79 patients and their parents,and then made diagnosis respectively. In addiction,44 high school students were each evaluated by one psychiatrist.The definite diagnosis was made when 2 evaluators make the same diagnosis for one patient.Result:In 123 patients,54 were pathological internet use.For diagnosis,the value of kappa for inter-rater reliability was 0.812( P<0.001).In the 12 criteria items,2 for poor inter-rater reliability and 4 for little contribution to diagnosis were eliminated.In the reserved 6 items,if 4 items(or more)were positive,the diagnostic sensitivity was 78.9% and specificity was 95.3%.Functional impairment criteria were made strictly.In the patients who were made the definite diagnosis,90.7% had duration of illness above 3 months,and 77.7% above 6 months.Conclusion:The preliminary diagnostic criteria of pathological internet use after revised includes 6 symptom criteria,3 functional impairment criteria,duration of illness and exclusive criteria.The criteria proposed is with high consistency on evaluations made by psychiatric raters,and with operational convenience.The criteria,after further revised,may fit the clinical application.
4.Clinical effect of cognitive behavioral Intervention on the cognitive level and quality of life in patients with venous thromboembolism

Objective To explore the effectof cognitive behavioral Intervention on the quality of life and cognitive lever of patients with venous thrombus embolism. Methods 164 patients with Venous Thrombus Embolism were randomly divided into control group and research group, the control group received routine care, The study group received cognitive behavior intervention. The cognitive lever and quality of life score was measured between before and after the intervention. Results Before the intervention, the difference score of living quality and cognitive lever in the two groups has no statistical significance (P > 0.05), and after intervention, the cognition lever and the three aspects of risk factors, anticoagulant knowledge and treatment methods of research group scored (28.47 ± 7.63), (7.38 ± 2.48), (6.82±2.73), (3.64±1.59) points which were significantly higher than the control group scored (20.54±5.49), (4.31±2.14), (3.72±1.62), (2.18±1.36) points (t=7.635, 8.469, 8.822, 6.299, P<0.01),the quality of life and four dimensions of daily life ability, pain, social activities, mental scored ( 37.04 ± 7.08), [9.00 (5.00, 11.00)],(9.04 ± 1.55),(5.52 ± 1.59),(14.00 ± 3.06) points which showed great improvement than the control group scored [49.50(39.75-59.25)], [11.50 (6.75, 16.00)], (10.05 ± 1.65), (7.95 ± 2.19), (19.18 ± 3.88) points. There were significant difference in the quality of life and four dimensions between the two groups. (Z=-3.558,-2.143, t=2.102, 4.274, 4.991, P<0.05 or 0.01). Conclusions The quality of life for the venous thrombus embolism patients were in lower level. The cognitive behavioral intervention could correcting their bad behavior and negative emotions, eventually improve the quality of life of venous thrombus embolism patients.
5.Reflection on the development of Artificial Intelligence in the field of medical scientific research management

Objective:To put forward suggestions for the problems identified in the application of Artificial Intelligence in the field of medical research management.Methods:Literature review were conducted to analyze the concept of Artificial Intelligence and its application in the field of medical scientific research management. The existing problems of medical Artificial Intelligence projects were identified and feasible countermeasures were put forward.Results:A total number of 16 Artificial Intelligence projects were going in the hospital. Some problems were identified during the management process, which including lack of regulatory regulations and professional standards, the communication barriers between the two partners are also prominent, the collection, sharing, and quality control of medical big data need to be standardized and managed ethically.Conclusions:Taking into account of the problems identified in the management of Artificial Intelligence projects in hospitals, scientific research management should put forward some countermeasures, such as improving management system and supervision mechanism, strengthening top-level guidance and improving employment capacity, paying attention to process management and fine business process, focusing on data supervision to promote business format promotion, cultivating talents to help comprehensive development, strengthening ethical awareness and risk management.
6.Evaluation of scientific research development trend of young talents based on Bibliometrics: The case of Peking University Cancer Hospital

Objective:To evaluate the scientific research development trend of young talents in Cancer Hospital of Peking University.Methods:The papers published by the young talents in the hospital as first or corresponding author from 2010 to 2019 were retrieved and analyzed, bibliometric method was used to evaluate the scientific and technological output of young talents in the hospital.Results:In the past 10 years, the number of papers increased from 149 to 402, and the quality of papers also improved significantly. SCI papers contributed a lot to the growth of the number, and its impact factors increased year by year.Conclusions:In recent years, the scientific research ability of young talents in hospital has been significantly improved, and some advantageous disciplines have been formed. At the hospital level, we should strengthen the combination of basic science and clinical medicine, strengthen the discipline construction, and attach importance to the strategy of young talents training. The management department should pay more attention to the significance of scientific and technological evaluation, making appropriate considerations of paper publication, and promote the development of young talents.
7.Clinical efficacy of percutaneous transluminal angioplasty in treatment of renal artery stenosis resulting from fibromuscular dysplasia

Objective To investigate the efficacy and safty of percutaneous transluminal angioplasty (PTA) of renal artery stenosis resulting from fibromuscular dysplasia.Methods A total of 32 patients with fibromuscular dysplasia complicated with hypertension in level 2 to 3 or refractory hypertension were enrolled,who underwent PTA.The blood pressure,types of medication,restenosis,survival rates and complications were observed after treatment.Results Technical success rate of PTA was 93.94% (31/33).The blood pressure decreased from (189.6±26.0)mmHg/(121.6± 21.7)mmHg before operation to (136.6±8.0)mmHg/(81.1±11.5)mmHg after operation (t=9.117,7.745,both P< 0.001).No renal artery rupture,interlayer,branch blockage,thrombosis and other related complications and postoperative adverse events occurred.All patients were followed up for 5 months to 100 months,with an average of (40.4±26.1) months.Survival rate was 100% (32/32).Conclusion PTA is safe and effective for treating renal artery stenosis from fibromuscular dysplasia.
8.Correlation of substantia nigra hyperecho with essential tremor and Parkinson disease

Objective To investigate the correlation of substantia nigra hyperecho with essential tremor (ET) and Parkinson disease (PD).Methods The clinical data of 158 patients with ET or PD who underwent transcranial ultrasonography in Tongji Hospital from March 2016 to March 2018 were retrospectively analyzed.There were 35 patients with ET (ET group),113 patients with PD who had no previous history of ET (PD group),and 10 PD patients with previous history of ET (ET-PD group).And 58 healthy subjects served as controls (control group).The hyperechoic area of substantia nigra in different groups was compared.Results The hyperechoic areas of the substantia nigra were [0 (0,0)]cm2 (control group),[0.27(0,0.41)]cm2 (ET group),[0.33(0.21,0.40)]cm2 (ET-PD group) and [0.35(0.29,0.45)]cm2 (PD group);the differences between control group and ET group,between the ET group and PD group were statistically significant (Z=-5.24,P=0.01;Z=-3.09,P=0.02),and there were no significant difference between the ET group and ET-PD group,between ET-PD group and PD group (Z=-0.98,P=0.32;t=-0.98,P=0.33).The ratio of substantia nigra hyperechoic positive to negative in ET-PD group was 9.00 (9/1),while that in ET group was 0.94 (17/18) (OR=9.53,95% CI:1.09-83.43,x2=3.91,P=0.04).Conclusion Substantia nigra hyperecho is an objective imaging indicator for patients with ET and PD,and has a certain differential value for their diagnosis.
9.Immunogenicity of MDCK cell-based quadrivalent influenza split vaccine with different adjuvants in mice

Objective:To evaluate the immunogenicity of Madin-Darby canine kidney (MDCK) cell-based quadrivalent influenza split vaccine (MDCK-Va) combined with different adjuvants.Methods:Different doses of MDCK-Va and chicken embryo-based quadrivalent influenza split vaccine (egg-Va) were intramuscularly immunized BALB/c mice twice with an interval of three weeks. Serum samples were collected to detect antibody titers using hemagglutination inhibition (HI) assay. BALB/c mice were immunized with different doses of MDCK-Va combined with QS21, AddVax, PolyI∶C, CpG ODN 1826 and AddVax/PolyI∶C (Add/Poly), respectively. HI and microneutralization assays were used to detect antibody titers 21 d after the first and booster immunization. Spleen tissues were collected from the mice immunized with 10 μg MDCK-Va combined with the above adjuvants 5 d after the booster immunization to analyze spleen index and the types of spleen cells.Results:The immunoprotective effect of MDCK-Va was not inferior to that of egg-Va. MDCK-Va combined with each of the above adjuvants could induce higher HI antibody titer than MDCK-Va alone, especially the QS21/Va and Add/Poly/Va groups, and the differences were statistically significant. For H1N1 vaccine, the Pearson′s correlation coefficient ( r) between HI antibody and neutralizing antibody was 0.737-0.910, and for H3N2 subtype vaccine, the value of r was 0.839-0.947. Compared with the MDCK-Va group, the QS21/Va group showed significantly increased spleen index and decreased proportion of single lymphocytes. QS21 and Add/Poly were much better than other adjuvants in stimulating mouse splenic neutrophils and CD4/CD8 cells. Conclusions:Add/Poly had a stronger immune enhancement effect on MDCK-Va, suggesting that it was a potential adjuvant for MDCK-Va. The antibody titer detected by HI and MN assays had a strong positive correlation.
10.Revaluation of clopidogrel: let the data speak for themselves.

Clopidogrel was believed to be superior to aspirin by the well-known CAPRIE trial. However, no other large clinical trials demonstrated the same results, but all focused on the combination use of clopidogrel with aspirin, and combination therapy in CREDO was called the "Emperor's New Clothes". However, no one overturned the results of these clinical trials by quantitatively analyzing them. We reviewed ten large-scale clinical trials about clopidogrel. On the basis of results of CAPRIE, CREDO and CHARISMA trials, we re-estimated their minimal sample sizes and their powers by three well-established statistical methodologies. From the results of CAPRIE, we inferred that the minimal sample size should be 85 086 or 84 968 but its power was only 30.70%. A huge gap existed. The same was also true of CREDO and CHARISMA trials. Moreover, in CAPRIE trial, 0 was included in the 95% confidence interval and 1 was included in the 95% confidence interval for the relative risk. There were some paradoxical data in CAPRIE trial. We are led to conclude that the results in CAPRIE, CREDO, and from the subgroup analysis in CHARISMA trials were questionable. These results failed to demonstrate that clopidogrel was superior to aspirin or that clopidogrel used in combination with aspirin was better than aspirin alone. The cost-effectiveness analyses by some previous studies were not reliable.
