1.The Erythrocyte Complement Receptor 1 Activity and CR1 Genomic Polymorphism in the Patients with Cerebral Infarction

The erythrocyte complement receptor 1 (ECR1) activity and CR1 quantitative genotype distribution were studied and the mechanisms of decreased ECR1 activity in cerebral infarction revealed. By using red blood cell yeast rosette test ECR1 activities were measured and by using PCR-RFLP CR1 Hind Ⅲ genomic polymorphism detected in the patients with cerebral infarction and healthy controls. The results showed that the level of C3bRR was decreased and the level of CICRR increased in the patients with cerebral infarction as compared with healthy controls (both P<0.05). CR1 quantitative genotype distribution in the patients with cerebral infarction was differed significantly from that of healthy controls (P<0.05). It was concluded that the decrease of ECR1 activity in the patients with cerebral infarction was correlated with CR1 Hind Ⅲ genomic polymorphism.

3.EFFECT OF CRYOPRESERVED PLATELETS TRANSFUSION ON 1560 PATIENTS

In this study, cryopreservated platelets were transfused to 1560 patients. of them, 536 were patients with acute leukaemia, 285 with anemia, 253 with thrombocytopenia due to chemical or radicalized treat, 438 in perioperation, and 48 with DIC or hemorrhage of rupture esophagus varix. The storage time of cryopreserved platelets ranged from one week to thirteen months. Most of the cryopreserved platelets had not been washed before application and no obvious adverse-effects were observed except for garlic smell in patients' exhalation after transfusing cryopreserved platelets. The results showed that transfusion of cryopreserved platelets has a remarkable on effect hemostasis and can enhance the platelet count in peripheral blood. Compared with liquid-preserved platelets, cryopreserved platelets for clinic use have the following advantages:1) the potentiality of large stock, 2) much longer shelf life, 3) higher safety, 4) superior hemostasis effect, and 5) capacity to meet emergent massive clinic demand.
4.The efficacy and safety of noninvasive ventilation in chronic obstructive pulmonary disease complicated with chronic respiratory failure

Objective To investigate the treatment of noninvasive ventilator in chronic obstructive pulmonary disease merge chronic type Ⅱ respiratory failure and to analysis its clinical efficacy and safety.Methods According to the random number table,115 cases were randomly divided into two groups,the observation group (n =59)and the control group(n =56).The control group received conventional symptomatic treatment,while the observation group therapy was combined with non -invasive ventilation in the basis of the control group.The changes of HR,RR, PaCO2 ,PaO2 ,Forced expiratory volume in one second 1 (FEV1 ),Forced expiratory volume in one second 1 (Forced expiratory volume in one second 1,FEV1 /FVC)were compared between the two groups before and after treatment 48h.Results Before treatment,there were no significant differences in HR,PaCO2 ,RR,FEV1 ,PaO2 ,FEV1 /FVC (P >0.05).After the treatment,in the control group,HR,RR,PaCO2 ,PaO2 ,FEV1 and FEV1 /FVC were respectively (90.16 ±4.15)beats/min,(23.17 ±1.63)beats/min,(59.68 ±5.85)mmHg,(69.16 ±5.26)mmHg,(2.65 ± 0.25)L and(65.47 ±1.97)%.In the observation group,HR,RR,PaCO2 ,PaO2 ,FEV1 and FEV1 /FVC were respectively (82.93 ±3.76)beats/min,(19.02 ±1.57)beats/min,(50.82 ±6.53)mmHg,(78.06 ±5.48)mmHg,(2.96 ± 0.31)L and (72.38 ±2.15)%.The difference was statistically significant(t =9.800,13.907,7.649,8.877,5.917, 17.942,all P <0.05).No adverse reactions occurred during the treatment.Conclusion The clinical efficacy of non-invasive ventilator in the treatment of COPD associated with chronic type Ⅱ respiratory failure is significant,safe and reliable,which has important research value,and is worthy of further application.
5.Application of multidisciplinary integrated management model in medical management of elderly patients

Objective To investigate the clinical efficacy of multidisciplinary integrated treatment and management model in the medical management of elderly patients.Methods 226 elderly patients treated in our hospital from October 2014 to December 2015 were randomly divided into observation group and control group,113 cases in each group.Patients in the control group were treated with traditional medical management model,while patients in the observation group were treated with multidisciplinary integrated treatment and management model.The therapeutic effect,hospital stay,incidence of complications,treatment satisfaction,MMSE score,FCA score and Tinetti score between the two groups were compared.Results The total effective rate (95.58%) in the observation group was significantly higher than that in the control group (79.65%) (χ2=13.21,P=0.000).The mortality rate in the observation group (1.77%) was significantly lower than 7.96% in the control group (χ2=4.68,P=0.004).The satisfaction rate of the observation group (92.04%) was significantly higher than 77.88% of the control group (χ2=8.86,P=0.003).The MMSE score[(25.17±4.43)points],FCA score[(77.35±8.35)points]and Tinetti score[(26.45±4.25)points]of the observation group were significantly higher than those in the control group[(20.13±3.07) points,(65.32±8.88) points and (21.03±3.30) points,t=-2.93,-3.11,-3.18;P=0.025,0.014,0.006].Conclusion Multidisciplinary integrated treatment management model can significantly increase the therapeutic effect,shorter hospital stay,reduce the incidence of complications,improve patients' satisfaction,and improve the quality of life,so it is a new type of efficient and rational therapeutic management model and worthy of clinical promoting.
6.Solitary Pulmonary Nodule:HRCT Typing and Pathology

Objective To evaluate HRCT typing of solitary pulmonary nodule with resected.Methods 30 cases with solitary plumonary nodules (totally 41) were undergone CT and HRCT scan befor operation.HRCT appearances were retrospectively analyzed and categorized into two types: typeⅠ ground-glass opacity (pure ground-glass opacity, mixed lower opacity, central high opacity with ground opacity ), typeⅡ pure solid opacity.Results TypeⅠ:14 nodules included bronchioloalveolar cell carcinoma (9 nodules) , adenocarcinoma (3 nodules),inflammatory granuloma (1 nodule) and adenoma (1 nodule).TypeⅡ : 27 nodules included adenocarcinoma(13 nodules),bronchioloalveolar cell carcinoma(4 nodules), squamous cell carcinoma(3 nodules),small cell lung cancer(1 nodule),adenoma(2 nodules),tuberculosis(2 nodules), inflammatory granuloma(1 nodule) and hamartoma(1 nodule). Among nodules with resected, ground-glass opacity was found more frequently in bronchioloalveolar cell carcinoma than in non- bronchioloalveolar cell carcinoma (P=0.002).No significant difference in ground-glass opacity existed between tumor lesions and non-tumor lesions (P=0.282) or adenocarcinoma and non-adenocarcinoma (P=0.146).Conclusion In the findings of HRCT in solitary pulmonary nodules,the nodules of ground-glass opacity find more frequently in bronchioloalveolar cell carcinoma than that in other lesions.
7.Effects of Qili Qiangxin capsule on serum concentration of adiponectin and heart function in patients with coronary heart disease combined with congestive heart failure

AIM:To investigate the effects of Qili Qiangxin capsule on serum adiponectin ( APN) , serum N-terminal pro-brain natriuretic peptide ( NT-proBNP) and heart function in the patients of coronary heart disease combined with congestive heart failure .METHODS: One hundred and twenty patients were randomly divided into treatment group and control group , and both groups were given anti-failure routine therapy .The patients in treatment group were treated with Qili Qiangxin capsule and the patients in control group were treated with placebo .The patients in the 2 groups were given a certain dose of the drugs for 6 months.The New York Heart Association (NYHA) heart function classification, left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD),left ventricular ejection frac-tion (LVEF), and 6-min walking test (6-MWT) were observed before and after treatment .The levels of APN, NT-proBNP were measured by ELISA before and after treatment .RESULTS:With the increase in the class of NYHA heart function , the serum concentrations of APN and NT-proBNP in the heart failure cases increased significantly .After 6-month treat-ment, the effective rate in experimental group was 91.7%and that in control group was 75.0%.A significant difference was found between the 2 groups (P<0.01).After treatment, LVEDD and LVESD in both groups were decreased signifi-cantly, and LVEF in both groups was increased significantly .The serum concentrations of APN and NT-proBNP decreased significantly (P<0.05).6-MWT result was improved significantly.Compared with control group, more obvious effect was observed in experimental group ( P<0.05) .CONCLUSION:Treatment with Qili Qiangxin capsule reduces the levels of APN and NT-proBNP in the patients with coronary heart disease combined with congestive heart failure .
8.Effects of the zinc finger protein A20 on LPS-TLR4 signaling pathways in severe acute pancreatitis rats

Objective To investigate the effects of the zinc finger protein A20 on LPS-TLR4 signaling pathways in severe acute pancreatitis(SAP) rats.Methods 24 SD rats were randomly divided into 3 groups:group A(the sham operation group),group B (the SAP group),group C (the SAP group treated with LPS).SAP model was induced by retro-injection of intraductal 5% sodium taurocholate into the biliary-pancreatic duct as previously described.The protein expression of A20,TLR4,NF-κBp65 and p38MAPK in pancreatic tissues was evaluated by immunohistochemistry.Results The positive area of A20 in pancreatic tissues was decreased in group B and group C compared with that in group A (t =17.234,19.698,all P < 0.05).On the contrary,the expression of TLR4,NF-κBp65 and p38MAPK in pancreatic tissues were up-regulated(t =15.909,20.432,16.543,18.629,22.105,19.006,all P < 0.05).A20 was decreased in group C than that in group B (t =14.894,P < 0.05),while TLR4、NF-κBp65 and p38MAPK were increased in group C than those in group B (t =14.047,15.582,17.070,all P <0.05).Conclusion The expression of A20 reduced and TLR4,NF-κBp65 and p38MAPK enhanced in the pancreas of rats with SAP,which indicated that A20 inhibited LPS-TLR4 signaling pathways which play important roles in the pathogenesis of SAP.
9.Differentiation effecs on DNA polymerase beta and X-ray repair cross-complementing protein 1 expression in Eca 109 cells

Objective To study the change of DNA polymerase beta and XRCC1's expression during malignant celI differentiation.Methods The Eca-109 cells were divided inm 2 groups:differentiation group which cultured with 8-Br-cAMP and control group.The 2 groups cells were cultured 48h simultaneously.The immunocytochemistry was performed to detect the expression of DNA polymerase beta and XRCC1.Results Compared with control group,the expression of DNA polymerase beta and XRCC1 was decreased simultaneously(P<0.05).Conclusion The differentiation agent can down-regulate the expression of DNA polymerase beta and XRCC1,suggesting that overexpressed DNA polymerase beta and XRCC1 maybe result in mutator phenotype.
10.Determination of evoked potential and cognitive function in patients with acute leukemia

Objective To explore the value of brainstem auditory evoked potential(BAEP),visual evoked potential(VEP)and Revised Visual Retention Test(VRT)in detecting impairment of central nervous system in patients with acute leukemia(AL).Methods Neuropack-Ml type evoked potential machine(Japan)was used to check brainstem auditory evoked potential(BAEP)and visual evoked potential(VEP).BAEP,VEP and VRT were examined in 30 cases with acute leukemia(group AL)and in 30 age and sex matched healthy subjects as a control(groupC).Indexes of BAEP including peak latency(PL)of waves I ,IB ,V and inter-peak latency(IPL)of waves Ⅰ~Ⅲ,Ⅲ ~Ⅴ,Ⅰ~Ⅴand indexes of VEP(P100PL)were determined.VRT was accomplished meanwhile applying B method with C pattern,C method with D pattern,D method with E pattern.The correct scores and error scores were recorded separately.Results BAEP:the abnormal rate of the group AL was 23.3 %.Compared with control group,we found significant longer latency of wave Ⅲ、Ⅴ(P<0.05)and significant elongation in the mean inter-peak latencies of Ⅰ~Ⅴ(P<0.05).VEP:the abnormal rate of the group AL was 26.7% ,the P100 PL of the patients was longer than that in the control group(P<0.05).VRT:the abnormal rate of the group AL was 33.3%.The results of VRT showed that the score in the group AL in D method with E pattern was lower than that in the control group(P<0.05).Conclusion BEAP,VEP and VRT could detect the impairment of central nervous system in patients with acute leukemia sensitively,and give clues to early diagnosis of central nervous system leukemia(CNSL).
