1.Screening of the serum biomarkers for early pneumoconiosis by SELDI-TOF-MS

Objective To search the specific biomarkers in serum of patients with silicosis for early diagnosis. Methods Surface enhanced laser desorption ionization-time of flight-mass spectrometry (SELDI-TOF-MS) was used to search the specific biomarkers in serum samples from 60 silica-exposed workers (20 with no silicosis served as 0 group, 20 with suspected silicosis as 0+ group and 20 with stage I silicosis as stage I group), and 20 healthy men with no silica exposure as control. Serum samples were analyzed using the new generation of weak cationic exchange chip (CM10, Ciphergen Biosystems, America) and the protein fingerprint automatically collected using ProteinChip Biology System (PBSII-C) instrumentation. OBJECTIVE To study the main bacteria and their drug-resistance of postoperative infections after liver transplantation.METHODS The distribution and drug-resistance profile of 156 strains of bacterial isolates from(various) specimens in 68 liver transplantation inpatients were retrospectively analyzed last year.RESULTS The(incidence) of infections after liver transplantation was 67.6 %.The major bacteria were Klebsilla pneumoniae((17.3%)),Staphylococcus aureus(14.1%),Candida albicans(11.0%),and Pseudomonas aeruginosa((11.0%)).The bacteria had the character of multidrug-resistance and high drug-resistance.The most effective(antibiotics) to Gram-negatives and Gram-positives were still the carbopenems and glycopeptide.CONCLUSIONS(Infection) is a major complication after liver transplantation.Prevention,early diagnosis and treatment of the(infection) are very important.

Marine traditional Chinese medicine (MTCM) is an important part of Chinese medicine (CM),there are some differences in understanding of the current literature and the extension of the connotation of marine medicine,which leads to the definition dispute of MTCM,hindering clinical application and further development of MTCM.In this study,we explored the concept of MTCM in literature,discussed the attributes of ocean marine CM,summed up the differences between the land CM and MTCM over variety characteristics,effect of drug composition characteristics and biological activity characteristics,and discussed the connotation and extension of MTCM from three aspects of theoretical basis and the effect and source of the drug,leading to the formation of the narrow and broad concept of MTCM.The five kinds of disputes in the definition of MTCM were discriminated according to the concept and connotation,which provided a theoretical basis for the definition and the research of MTCM.Moreover,we also defined the English translation and its abbreviation as Marine Traditional Chinese Medicine (MTCM).
4.Application and Research of Clinical Laboratory Information Management System

Objective The application of laboratory information system (LIS) increases extensively. The objective is to expatiate how to widen the application of LIS so that it can be seamlessly combined with HIS, and play a greater role in quality control of clinical laboratory. Methods A consummate and compact working process was designed. A bidirectional communication from equipment to client was established. Module modes and prefect security systems were set up. Results With the existing HIS network environment, corresponding bar codes were generated through the examination applications from the doctor's workstation. At the same time, patients' basic information and test information were generated based on the bar code. In this way, the seamless combination of LIS and HIS were achieved. The switch of the functional module was simple, efficient and easy to maintain. The internet version Kaspersky antivirus software was used in the area of security. Database were backed up automatically and regularly. Authority was strictly defined for operators in different level. Conclusion Through the current application in our section, LIS has already been an essential part in the process to become a modern hospital.

A longitudinal study was carried out on fifty infants selected from both urban and rural areas of Beijing for their body weights, heights, head and mid-upper arm circumferences, tricep and abdomen skinfold thicknesses, and breatmilk and energy and protein intakes at the 1st, 3rd and 6th month of life. Hyperuricemia contributes to renal inflammation. We aimed to investigate the role of Naâºâ€“Kâºâ€“ATPase (NKA) in hyperuricemia-induced renal tubular injury. Human primary proximal tubular epithelial cells (PTECs) were incubated with uric acid (UA) at increasing doses or for increasing lengths of time. PTECs were then stimulated by pre-incubation with an NKA α1 expression vector or small interfering RNA before UA (100 μg mlâ»Â¹, 48 h) stimulation. Hyperuricemic rats were induced by gastric oxonic acid and treated with febuxostat (Feb). ATP levels, the activity of NKA and expression of its α1 subunit, Src, NOD-like receptor pyrin domain-containing protein 3 (NLRP3) and interleukin 1β (IL-1β) were measured both in vitro and in vivo. Beginning at concentrations of 100 μg ml−1, UA started to dose-dependently reduce NKA activity. UA at a concentration of 100 μg mlâ»Â¹ time-dependently affected the NKA activity, with the maximal increased NKA activity at 24 h, but the activity started to decrease after 48 h. This inhibitory effect of UA on NKA activity at 48 h was in addition to a decrease in NKA α1 expression in the cell membrane, but an increase in lysosomes. This process also involved the subsequent activation of Src kinase and NLRP3, promoting IL-1β processing. In hyperuricemic rats, renal cortex NKA activity and its α1 expression were upregulated at the 7th week and both decreased at the 10th week, accompanied with increased renal cortex expression of Src, NLRP3 and IL-1β. The UA levels were reduced and renal tubular injuries in hyperuricemic rats were alleviated in the Feb group. Our data suggested that the impairment of NKA and its consequent regulation of Src, NLRP3 and IL-1β in the renal proximal tubule contributed to hyperuricemia-induced renal tubular injury.
7.Two revascularization strategies in patients with coronary heart disease and left ventricular systolic dysfunction: A systematic review and meta-analysis

Objective To compare the clinical efficacy of coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) in patients with coronary heart disease and left ventricular systolic dysfunction. Methods A computer-based search in PubMed, The Cochrane Library and EMbase up to October 2017, together with reference screening, was performed to identify eligible clinical trials, cohort studies and case-control studies. The outcomes of this meta-analysis included all-cause mortality, myocardial infarction, revascularization and stroke, and the effect sizes for them were presented as relative risk (RR) with its 95% confidence intervals (CI). Results Fifteen cohort studies and 2 randomized controlled trials were finally included with a total of 11 985 patients, of whom 6 322 were in the CABG group and 5 663 in the PCI group. The result of meta-analysis showed that all-cause mortality was significantly lower in the CABG group than that in the PCI group (18.6% vs. 23.0%, RR=0.87, 95% CI 0.81 to 0.94, P<0.001). In addition, CABG was associated with a remarkably reduced risk of revascularization (RR=0.28, 95% CI 0.19 to 0.42, P<0.001) compared with PCI, with no significant difference in incidence of myocardial infarction (RR=0.78, 95% CI 0.47 to 1.32, P=0.36) and stroke (RR=1.28, 95% CI 0.89 to 1.86, P=0.18). Conclusion CABG is superior to PCI in the treatment for patients with coronary heart disease and left ventricular systolic dysfunction. Owing to the limited quality of included studies, additional large, randomized controlled trails are still required to confirm this finding.
8.Value of transvaginal color Doppler ultrasonography for pathologic grading of endometrial cancer

Objective To evaluate the value of transvaginal color Doppler ultrasonography for pathologic grading of endometrial cancer. Methods A total of 68 patients with histopathologic diagnosis of endometrial cancer were recruited as study subjects, and all patients underwent transvaginal color Doppler ultrasonography. The accuracy, sensitivity, specificity, positive predictive value, negative predictive value of transvaginal color Doppler ultrasonography for identification of myometrial invasion by endometrial cancer were evaluated among endometrial cancer patients with postoperative histopathological examinations as a gold standard. Results The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of transvaginal color Doppler ultrasonography were 70.0%, 67.9%, 75.7%, 61.3% and 69.1% for the detection of myometrial invasion by endometrial cancer, respectively. The resistance index values of left and right uterine arteries were both significantly higher in endometrial cancer patients with myometrial invasion ≤ 50% than those with myometrial invasion > 50% (P < 0.01), and the resistance index values of left and right uterine arteries increased with the reduced tumor grade among endometrial cancer patients (P < 0.05). Conclusion Transvaginal color Doppler ultrasonography shows a high accuracy for identification of myometrial invasion among endometrial cancer patients, which may be used as a non-invasive tool for preoperative pathologic grading of endometrial cancer.
