Conclusions Transactivation competence of genotype B HBx was higher than that of genotype C HBx, while the antiproliferative and apoptosis effects of genotype B HBx was lower than of genotype C HBx. B and C genotype-specific functional differences of HBx may closely co-related with the pathogenicity of HBV.

Objective To investigate the efficacy and safety of intraarterial or intravenous recombinant tissue plasminogen activator(rtPA) for patients with acute cerebral infarction.Methods 60 patients consistent with the inclusion criteria were divided into intra-arterial thrombolysis group, intravenous thrombolysis group and control group. rtPA of 10～20 mg was infused over the course of 30 minutes in intra-arterial thrombolysis group. rtPA was administered in a dose of 0.7～0.8 mg/kg(maximum dose 50 mg),with 100% as a constant infusion over a period of 90 minutes in intravenous thrombolysis group.24 hours late, both groups were followed by subcutaneous injection of low molecular weight heparin(0.4 ml,twice daily) for 7 days. The control group received only routine treatment. European Stroke Scale(ESS) and Barthel Index(BI) were used to evaluate the recovery of neurological functions.Results The efficacy in both thrombolysis groups was significantly higher than that in the control group(all P 0.05).But in intraarterial group, the stenosis of intracranial arteries or extracranial arteries could be found, and new thrombosis could be prevented by transluminal stent-assistant angioplasty. One case of intracerebral hemorrhage and one case of asymptomatic hemorrhage occurred in intraarterial thrombolysis group. One case of asymptomatic hemorrhage was found in intravenous thrombolysis group and control group respectively. No dead case was found in the three groups.Conclusion Thrombolytic therapy of intra-arterial and intravenous rtPA can be performed safely and efficaciously in patients with acute cerebral infarction within 6 hours from the onset.

Objective To investigate the diagnostic value of the craniocerebral magnetic resonance angiography(MRA) in adult moyamoya disease.Methods 20 adult patients with Moyamoya disease were examined by DSA,MRI and MRA.Results DSA showed that the ending of internal carotid artery system were stenosis or occlution,and the fog-like blood vessel networks at base of skull were observed in the 20 patients.By MRI,the cerebral ischemical changes were found in 11 cases,intracerebral hamrrahge was found in 9 cases.MRA showed that cerebral artery were stenosis or obstruction in all of the 20 patients,16 cases(80%) with the fog-like blood vessel net in the thalamus-basal ganglia region.The detectable rate of abnormal cerebral basal vascular network was no statistical difference between DSA and MRA.Conclusions MRA can provide more information of cerebral artery straitness or obstruction and fog-like blood vessel net in the adult patients with Moyamoya disease,and without hammerless.MRA has a very importent value of diagnosis in the adult moyamoya disease.

To investigate the inhibitory effects of silymarin on diabetic blood vessels chroniccomplication. Methods:Silymarin was given to streptozocin-induced diabetic rats. Rats were killed 9 weeksafter treatment. Plasma LPO, fructosamine and RBC-SOD were measured. LPO, fruct0samine, fluores-cence intensities of AGEs, pentosidine and liperperoxide adducts in aorta were also measured. Results:The early nonenzymatic glycation products-fructosamine were not inhibited, however, LPO and f1uores-cence intensities of AGEs, pentosidine,MDA and HNE adducts in aorta were more significantly reducedthan DM group. Conclusion: The results suggest that silymarin may inhibit nonenzymatic glycation andoxidation in aorta of streptozocin-induced diabetic rats and control the diabetic chronic complication.

Silymarin 100mg?kg~(-1)/d was given to streptozotocin induced diabetic rats for 9 weeks. The results showed that LPO,fructosamine and the fluorescence intensities of AGEs,pentosidine,MDA adducts,HNE adducts in renal cortex of diabetic rats were significantly higher than in normal control rats. After being treated with Silymarin,LPO and the fluorescence intensities of AGEs,pentosidine,MDA and HNE adducts in renal cortex were significantly reduced than in untreated DM group.The albumin excretion in Silymarin group was significantly decreased than in untreated DM group.Silymarin may inhibit nonenzy- matic glycation and oxidation in kidneys of streptozotocin-induced diabetic rats and control the diabetic chronic complication.

The extraction of the fatty acids from viscera gland of Chlamys farreri and Pecten yesoensis cultured off shore Dalian was studied and the fatty acids in fat were analysis.The results show that extraction rate,3.01%,is the highest when gas(CO 2) is filled up to expel oxygen,the ratio of material to water is 1∶2,the time is 60 minuites and temperature is 100℃.The contents of the unsaturated fatty acids of viscera gland fatty acids in Chlamys farreri and Pecten yesoensis are 32.0% and 32.8% respectively,in which the contents of EPT+DHA are 13.6% and 13.8% respectively.

Recent studies show that resveratrol is characterized with its antioxidative and anti-inflammatory effects, and plays an important role in the prevention and treatment of colorectal cancer through modulating intracellular carbohydrate and ceramide metabolism, activating tumor suppressors such as activator of transcription 3 and peroxisome proliferators-activated receptor γ, down-regulating the expression of kirsten rat sarcoma viral oncogene and inhibiting the epithelial-to-mesenchymal transition process of cancer cells.In addition,resveratrol exhibits its broad application prospects in combination therapy.

Objective:To investigate the efficacy of an additional augmentative locking compression plate combined with bone graft for aseptic subtrochanteric fracture nonunion after intramedullary nailing.Methods:From October 2016 to October 2019, 32 patients (25 males and 7 females) were treated at Department of Orthopedics, The Sixth People's Hospital Affiliated to Shanghai Jiaotong University for aseptic subtrochanteric fracture nonunion after intramedullary nailing. Their ages ranged from 27 to 68 years (average, 50.5 years) and their nonunion time from 9 to 24 months (average, 12.2 months). According to the Weber-Cech classification for nonunions, 9 cases belonged to the highly vascular type and 23 to the ischemic type. With the original intramedullary nails retained, all patients were fixated with an additional augmentative locking compression plate before simultaneous autologous iliac bone grafting. All patients were followed up regularly for fracture union, function of the affected hip and complications.Results:The 32 patients were followed up for 6 to 24 months (average, 12.9 months). All nonunions obtained clinical and imaging union. The clinical union time ranged from 3 to 8 months (average, 5.2 months), and the imaging union time from 6 to 10 months (average, 7.4 months). At the last follow-up, the therapeutic efficacy was evaluated as excellent in 28 cases and as good in 4, giving a good to excellent rate of 100%(32/32), according to the Sanders scoring for post-traumatic hip joint; the efficacy was evaluated as excellent in 21 cases, as good in 10 cases and as fair in one, giving a good to excellent rate of 96.9%(31/32), according to the Harris hip scoring. No such complications occurred as infection or loosening or breakage of internal fixation.Conclusion:An additional augmentative locking compression plate combined with bone graft is a reliable and effective treatment for aseptic subtrochanteric fracture nonunion after intramedullary nailing.

Objective To investigate the effect of sufentanil and midazolam assisted epidural anesthesia for cholecystectomy on sedative effects,visceral pulling reaction and the functions of respiratory and circulatory system.MethodsFourty patients underwent cholecystectomy received informed consent, were randomly allocated to two groups:group A and group B( n =20 each). When the level of epidural block is appropriate,auxiliary medicine infusion was given 5min before the skin incision. In group A,first injection was sufentanil 0. lμg · kg-1 and midazolam 0. 02mg · kg-1 in 1-2 min, then continuous infusion of sufentanil and midazolam was given at the rate of 0. 2μg ·kg-1 · h-1 and 0. 04μg · kg-1 · h-1 respectively,and the infusion was stopped at the time of wound closure. Group B received slow intravenous injection of pethidine 1mg · kg-1 and droperidol 0. 5mg · kg-1. We recorded the differences of SBP、DBP、SpO2 、HR、Ransay score and visceral pulling reaction classification. Results SBP、DBP、HR after anesthesis,were decreased in different degree in the above groups,but there was no statistically significant difference( P ＞0. 05). According to Ramsay score,in group A there were 3 cases in grade Ⅱ、8 cases in Ⅲ and 9 cases in Ⅳ ,while in group B there were respectively 4 cases in Ⅱ、13 cases in Ⅲ and 3 cases inⅣ. By comparision,the differences had statistical significance ( u = 3.75, P ＜ 0. 05 ); Group A according to visceral pulling reaction classification (0- Ⅲ ), were 9 cases in grade 0,9 cases in Ⅰ , 1 cases in Ⅱ and 1 cases in Ⅲ, while in group B there were respectively 4,5,7 and 4 cases. By comparision, the differences had statistical significance(u = 4. 01,P ＜ 0. 01 ). Conclusion In the cholecystectomy,sufentanil and midazolam assisted epidural anesthesia,could improve the level of sedation ,prevent visceral pulling reaction ,and had a minor interference to respiratory and circulatory function.

Objective Through comparing ultrasound-guided microwave ablation (MWA) with surgical resection for the treatment of nodular goiter to evaluate the effect of RFA on the thyroid function.Methods A total of 50 patients with nodular goiter,who were admitted to authors' hospital during the period from January 2010 to January 2013 to receive ultrasound-guided MWA,were selected and used as the study group;and other 96 patients with nodular goiter,who were encountered at authors' hospital during the same period to receive partial thyroidectomy,were collected and used as the control group.Of the 50 patients in the study group,RFA of unilateral lobe nodules was performed in 8 and RFA of bilateral lobe nodules in 42.Among the 96 patients in the control group,unilateral lobe nodules were seen in 52 and bilateral lobe nodules in 44.Postoperative complications were recorded,the serum FT3,FT4,TSH levels were determined at one week,as well as one,3,6 and 12 months after the treatment.The results were statistically analyzed.Results All patients were followed up for 12 months.In the study group,no complications occurred;one week after RFA the serum FT3 and FT4 levels increased while the serum TSH level decreased when compared with preoperative ones,and the differences were statistically significant (P＜0.05).In the control group,bleeding asphyxia occurred in one patient,hoarseness in 5 patients and hypocalcemia convulsion in 3 patients.One week after partial thyroidectomy,the serum FT3 and FT4 levels increased while the serum TSH level decreased when compared with preoperative data,and the differences were statistically significant (P＜0.05).One week after surgery thyroxine replacement therapy was routinely given to 44 patients who had received bilateral subtotal thyroidectomy.Three patients who had received unilateral subtotal thyroidectomy developed hypothyroidism at 3,6 and 8 months after the surgery respectively.In other 49 patients receiving unilateral subtotal thyroidectomy the serum FT3,FT4,TSH levels determined at one,3,6 and 12 months after surgery were significantly different from those in the patients of the study group (P＜0.05).Conclusion For the treatment of nodular goiter,ultrasound-guided MWA is quite safe.MWA has very slight effect on thyroid function and is definitely superior to subtotal thyroidectomy.Therefore,MWA is a minimally invasive technique which is worthy of clinical promotion.
