3.Theoretical Analysis of Prevention and Treatment of Around-adolescence Women with IGT Type PCOS by Wenshen Huatan Prescription Combined with Food Exchange

[Objective]The purpose of this study is to provide a new idea for the prevention and treatment of PCOS in around-adolescence period through the cooperation of Wenshen Huatan prescription and food exchange. [Methods]By referring to and sorting out the related literatures, combined with clinical practice, this paper discusses how to form a specific plan of prevention and treatment of around -adolescence PCOS complicated with IGT by Wenshen Huatan prescription and food exchange. [Results]Food exchange can scientifically control diet, combined with kidney-warming and phlegm-Tiaojing, spleen phlegm, which has a remarkable effect on the treatment of around-adolescence PCOS complicated with IGT, such as improving clinical symptoms, shortening the menstrual cycle, improving endocrine disorders and insulin resistance.[Conclusion]It is significant for patients with IGT-type PCOS around-adolescence by the exchange of food with Wenshen Huatan prescription combined with diet intervention, which provides a new train of thought for clinical research.
4.A case of Mullerian duct cyst torsion combined with bladder cancer.

The diagnosis and treatment of a patient with Mullerian duct cyst torsion combined with bladder cancer were retrospectively analyzed. The patient received an open abdominal operation with right accessory resection plus pelvic adhesion release, and conducted microscopic examination for the bladder under general anesthesia. The results of histopathological examination confirmed the diagnosis of Mullerian duct cyst and papillary urothelial carcinoma at low level. After surgery, the patient received chemotherapy and follow-up observation, and all the indices for the patient were normal and no obvious discomfort. Although the final diagnosis of Mullerian duct cysts is based on histopathological examination, ultrasonography, CT scan and MRI can also detect it. Most of the Mullerian duct cysts are benign, and surgical excision is safe and effective.

5.Serum GP73 for diagnosing decompensated cirrhosis in population with chronic HBV infections

Objective To evaluate the role of serum GP73 for diagnosing decompensated cirrhosis in population with chronic HBV infections.Methods The present study included three populations:200 patients with chronic HBV infections and receiving liver biopsy; 200 patients with decompensated cirrhosis;and 200 patients with hepatocellular carcinoma.All patients were HBsAg-positive over six months.Results Comparing with those in the patients with chronic hepatitis B (67.38 ±57.45 ng/ml),the serum GP73 levels in cirrhosis patients (221.9 ± 108.5 ng/ml) and HCC patients (152.4 ± 102.7 ng/ml) were significantly increased.Taken the population with chronic hepatitis B as "control group",the patients with decompensated cirrhosis as "patients group",the area of ROC analysis was 0.91 (95% CI:0.88-0.94) (P <0.0001).Set the cut-off value at 150 ng/ml,the diagnosing sensitivity and specificity were 72.5% and 93.5%,respectively.Conclusion For patients with chronic HBV infections and a higher GP73 levels,decompensated cirrhosis should be considered,except diagnosis of hepatocellular carcinoma.
6.Clinical Efficacy of Wandai Decoction in Preventing and Treating VVC with Spleen Deficiency-Dampness Abundance Syn-drome and Its Impact on DNA Damage

OBJECTIVE To explore the clinical efficacy of Wandai Decoction in the treatment of vulvovaginal candidiasis(VVC)with spleen deficiency-dampness abundance and its effect on DNA damage.METHODS 70 VVC patients with spleen deficiency-dampness abundance who met the inclusion criteria were randomly divided into Wandai Decoction and fluconazole groups,35 cases each.The fluconazole group took 150 mg fluconazole orally once;the Wandai Decoction group took Wandai Decoction orally for 14 d.After treatment,changes in TCM syndrome scores of the two groups of patients were compared;the clinical cure(test of cure,TOC)rate and clinical improvement(CI)rate of the two groups of patients were evaluated;8-hydroxydeoxyguanosine(8-OHDG)in the vaginal lavage fluid was detected by colorimetry to assess DNA damage;clinical complete remission(follow up,FU),mycological negative conversion and recurrence rate were evaluated 3 months after treatment.RESULTS After treatment,the TCM syndrome scores of the two groups of patients were improved to varying degrees(P<0.05,P<0.01);the Wandai Decoction group was superior to the fluconazole group(P<0.05,P<0.01);there was no significant difference in TOC,CI,mycology negative conversion between the Wandai Decoction group and the fluconazole group(P>0.05);however,the FU and recurrence rate of the Wandai Decoction group were significantly better than those of the fluconazole group(P<0.05,P<0.01).After treatment,the expression of 8-OHDG in the vaginal lavage fluid of the fluconazole group increased significantly(P<0.001),while there was no significant change in the Wandai Decoction group,which was significantly lower than that of the fluconazole group(P<0.001).CONCLUSION The overall clinical efficacy of Wandai Decoction is equivalent to that of fluconazole,but it is better than fluconazole in improving TCM syndromes and pre-venting recurrence,with the advantage of not aggravating DNA damage in vaginal cells.
7.Application of health education based on feedback theory in perioperative nursing of patients undergoing endoscopic retrograde cholangiopancreatography

Objective:To explore the application effect of health education based on feedback theory in perioperative nursing of patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) .Methods:A total of 105 patients undergoing ERCP in the Third Hospital of Shanxi Medical University from February 2019 to June 2020 were selected as the research objects by the convenient sampling method. Patients were divided into the observation group ( n=53) and the control group ( n=52) according to random number table method. The control group was given routine health education, while the observation group was given health education based on feedback theory. The awareness of health education, self-management ability, bad mood and quality of life were compared between the two groups before and after intervention. Results:After intervention, scores of all dimensions of health education awareness questionnaire and the subscale scores of Adult Health Self-Management Ability Scale (AHSMSRS) in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . The scores of Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) in the observation group were lower than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Health education based on feedback theory can improve the health education awareness, self-management ability of patients undergoing ERCP and improve their negative emotions.
8.Molecular detection and genetic diversity of bovine papillomavirus in dairy cows in Xinjiang, China

Background:
Bovine papillomatosis is a type of proliferative tumor disease of skin and mucosae caused by bovine papillomavirus (BPV). As a transboundary and emerging disease in cattle, it poses a potential threat to the dairy industry.
Objectives:
The aim of this study is to detect and clarify the genetic diversity of BPV circulating in dairy cows in Xinjiang, China.
Methods:
122 papilloma skin lesions from 8 intensive dairy farms located in different regions of Xinjiang, China were detected by polymerase chain reaction. The genetic evolution relationships of various types of BPVs were analyzed by examining this phylogenetic tree.
Results:
Ten genotypes of BPV (BPV1, BPV2, BPV3, BPV6, BPV7, BPV8, BPV10, BPV11, BPV13, and BPV14) were detected and identified in dairy cows. These were the first reported detections of BPV13 and BPV14 in Xinjiang, Mixed infections were detected, and there were geographical differences in the distribution of the BPV genotypes. Notably, the BPV infection rate among young cattle (< 1-year-old) developed from the same supply of frozen sperm was higher than that of the other young cows naturally raised under the same environmental conditions.
Conclusions
Genotyping based on the L1 gene of BPV showed that BPVs circulating in Xinjiang China displayed substantial genetic diversity. This study provided valuable data at the molecular epidemiology level, which is conducive to developing deep insights into the genetic diversity and pathogenic characteristics of BPVs in dairy cows.
9.Effect and Mechanisms of Kaiyu Zhongyutang on Insulin Resistance, Glucose and Lipid Metabolism, Psychological State, and Embryo Outcome in Infertile Patients with PCOS Due to Liver Depression and Kidney Deficiency

ObjectiveTo explore the effects and mechanisms of Kaiyu Zhongyutang on insulin resistance, glucose and lipid metabolism, psychological state, and embryo outcome in the infertile patients with polycystic ovary syndrome (PCOS) due to liver depression and kidney deficiency. MethodThe 126 infertile patients with PCOS due to liver depression and kidney deficiency who underwent in vitro fertilization-embryo transfer (IVF-ET) in the Department of Reproduction of the affiliated hospital of Nanjing university of Chinese medicine were randomly assigned into the observation (Kaiyu Zhongyutang + metformin) and control (metformin) groups. The two groups were compared in terms of traditional Chinese medicine (TCM) syndrome scores, body mass index (BMI), glucose and lipid metabolism, homeostasis model assessment-insulin resistance (HOMA-IR), interleukin (IL)-6, IL-8, C-reactive protein (CRP), self-rating depression scale (SDS) score, self-rating anxiety scale (SAS) score, and clinical and laboratory scores of IVF after treatment. Result① After treatment, the observation group showed decreased scores of primary and secondary TCM syndromes and total TCM syndrome score (P < 0.05), and the control group presented decreased scores of irritability and depression and total TCM syndrome score (P<0.05). Compared with the control group, the observation group demonstrated reduced primary and secondary syndrome scores and total score after treatment (P<0.05). ② After treatment, both groups showed decreased BMI, lowered levels of fasting insulin (FINS), fasting plasma glucose (FPG), cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL), and HOMA-IR (P<0.05), and elevated high-density lipoprotein (HDL) level (P<0.05). Moreover, the observation group had lower BMI, FINS, FPG, TC, TG, LDL, and HOMA-IR and higher HDL than the control group (P<0.05). ③ The treatment in both groups lowered the levels of IL-6, IL-8, and CRP and decreased SDS and SAS scores (P<0.05). Moreover, the declines in the observation group were more obvious than those in the control group (P<0.05). ④ Correlation analysis before treatment, IL-6, IL-8, and CRP had positive correlations with BMI, HOMA-IR, TC, TG, LDL, SDS, and SAS (P<0.05) and a negative correlation with HDL (P<0.05). ⑤ The observation group showed reduced gonadotropin (Gn) using days and total Gn dose and higher two-pronuclear (2PN) fertilized oocytes, 2PN cleavage rate, normal fertilization rate, D3 transferable embryo number, D3 high-quality embryo number, high-quality embryo rate, and blastocyst formation rate than the control group (P<0.05). ConclusionKaiyu Zhongyutang can treat PCOS patients by improving the emotional and reproductive functions and alleviating insulin resistance and glucose and lipid metabolism disorders. Moreover, it can reduce the Gn dose and Gn using days in the IVF process, improve the quality and maturity of eggs, increase the egg fertilization rate, enhance the potential of embryo development, and increase the rate of blastocyst formation by inhibiting inflammation.
