4.Relationship between serum albumin level and severity of coronary heart disease

Objective: To investigate the relationship between serum albumin level and severity of coronary heart disease (CHD). Methods: A total of 237 patients admitted in our hospital from 2008 to 2009 and undergoing coronary angiography were enrolled. CHD severity was evaluated according to clinical types, number of diseased coronary vessels and coronary artery score respectively, and patients were divided into non-CHD group (n=84), angina pectoris group (n=92) and myocardial infarction group (n=61). Serum total protein, albumin and globulin were measured in all patients on second day after admission. Relationship among indexes of CHD severity and serum proteins were analyzed. Results: Compared with non-CHD group, there was significant decrease in albumin level [(40.22±3.56) g/L vs. (39.81±3.56) g/L vs. (38.68±3.43) g/L, P<0.05] in angina pectoris group and myocardial infarction group. Spearman correlation analysis indicated that serum albumin level was significant negatively correlated with clinical types (r=-0.18, P=0.005), number of diseased coronary vessels (r=-0.195, P=0.003) and coronary artery score (r=-0.188, P=0.004). Serum total protein, globulin and ratio of albumin/globulin were not significantly correlated with clinical types, number of diseased coronary vessels and coronary artery score (P>0.05). Conclusion: Serum albumin level is negatively correlated with severity of coronary heart disease. Low serum albumin may be a risk factor for coronary heart disease.
5.Association of cell-free plasma DNA with mortality and severity of disease in ICU patients

The cell-free plasma DNA of the 132 critically ill patients admitted to the emergency intensive care unit ( EICU) was measured by real-time quantitative polymerase chain reaction assay for the humanβ-globin gene. Ac-cording to the prognosis, the patients were divided into survival group (95 cases) and death group (37 cases). Blood samples were collected after EICU admission, 24 hours after admission, 48 hours after admission, and 1 week after admission. 30 age-mached controls were collected from healthy volunteers healthy care centre. Com-pared with control group, at each time point, cell-free plasma DNA concentration was significantly higher in EICU patients (P<0. 05). Cell-free plasma DNA concentrations were higher in hospital non-survivors than in survivors, of which plasma DNA concentration of 24 hours after admission and 48 hours after admission was statistically signifi-cant ( P<0 . 05 ) . The area under the ROC curves ( AUC ) for plasma DNA was bigger than the lactate and A-PACHE IIscore, the maximum of AUC was 0. 847(95% CI 0.648~1.000) at the point of 48 hours after admis-sion.
6.Influence on negative emotion of ICU patients relative by comprehensive needs intervention

ObjectiveTo investigate the influence on negative emotion of ICU patient's relatives by comprehensive needs intervention.Methods 100 ICU patient's relatives were randomly divided into intervention group and control group,50 cases per group.Patients in intervention group were given comprehensive needs interventions for two weeks,then were tested by self-rating anxiety scale(SAS),self-rating depression scale(SDS) and symptom checklist 90 (SCL-90),and their negative emotion were compared.ResultsExcept for interpersonal relationship factor (P> 0.05 ),the other SCL-90 factorial scores of ICU patient's relative were all significantly higher than the national norm before intervention(P < 0.01 ~0.05 ).Anxiety and depression rates in two groups were 64% and 53% respectively,SDS standard scores were 49.62 ± 9.18 and 49.70 ± 9.73,SAS scores were 40.08± 11.93 and 40.04 ± 11.76,and they were all significantly higher than normal norm.After intervention,SAS,SDS standard scores and SCL-90 factors of intervention group were significantly decreased,and were superior than control group,and there were statistical difference (P< 0.01 ~ 0.05 ).ConclusionThere are so much psychological diseases with the relatives of ICU patients,especially anxiety and depression.
8.EFFECT OF 1,25-DIHYDROXYVITAMIN D_3 ON THE PROCESS OF PHAGOCYTIZING STAPHYLOCOCCUS AUREUS IN MACROPHAGES

Objective To investigate the effect of vitamin D (VD) on macrophage to phagocytize Staphylococcus aureus (SA). Method Macrophoge cell lines RAW264.7 were allocated into 3 groups:control group(C),bacterium group(B),active vitamin D+ bacterium group (VD+B). Cells in the VD+B group were incubated with 10-8mol/L active vitamin D for 24h,then cells in the VD+B group and the B group were cultured with SA for 1h,and phagocytosis rate,mitochondrial membrane potential,[Ca~(2+)]i,reactive oxygen species were determined by flow cytometry (FCM). Results The phagocytizing activity of macrophage in VD+B group was significantly higher than that in B group 1h after infection,At the same time,the mitochondrial member potential and [Ca~(2+)]i of macrophage in VD+B group were distinctly lower than that in B group; but reactive oxygen species of macrophage in the VD+B group was insignificantly different from B group. Conclusion Vitamin D can reinforce the phagocytizing activity of macrophage and inhibit the apoptosis of macrophage after phagocytize Staphylococrcus aureus.
9.Application of 18F-FDG PET/CT in epileptogenic zone localization

Objective To discuss the application of 18F-FDG PET/CT in epileptogenic zone localization.Methods Tirty-two patients with epilepsies were studied with interictal 18F-FDG PET/CT cerebral imaging,and all of them had scalp EEG,CT or MRI results for comparision.Results The sensitivity of 18F-FDG PET/CT was 90.6%,27 patients(84.5%) showed focal hypometabolism,2 patients(6.3%) showed focal hypermetabolism and rest 3 patients(9.4%) had negative result.The sensitivity of EEG was 65.6%(21/32),the sensitivity of CT and MRI was 31.0%(10/32).Conclusion 18F-FDG PET/CT was a sensitive and accurate image modality in the localization of epileptic foci.
10.The Application of Intramedullary Decompression and Pressor Infusion for ischemic Necrosis of Adult Femoral Head

Objective To observe the curative effect of adult ischemic necrosis of femoral head by intramedullary decompression and arterial pulse pressor infusion.Methods 82 patients with ischemic necrosis of femoral head were treated in the combined treatment of percutaneous intramedully decompression and pulse pressor five medicine combined infusion in target vessels using ZC-Ⅰ-discome.Results By follow-up for 12~48 months,the painful in hip joint was alleviated and joint movement was also improved.The regenerative vessels and bone in femoral head and neek were obvious.Conclusion This therapcutic methods is safe and superior to single interventional medicine treatment for ischemic necrosis of femoral head.
