1.Expressions of miRNA-21 and PTEN and the clinical significance in colorectal carcinoma

Objective To explore the correlations between expression of miRNA-21 and PTEN and the invasion, metastasis of colorectal carcinoma.Methods The mRNA level of miRNA-21 and mRNA and protein levels of PTEN were assayed by real-time quantitative PCR and immunohistochemical methods respectively in 65 colorectal carcinoma specimen.Results The expression of miRNA-21 mRNA in cancer tissues was significantly higher than that in paracancerous tissues (t =3.50, P < 0.05).Both mRNA and protein expressions of PTEN in tumor tissue were significantly lower than that in paracancerous tissues (t =7.35,t =12.23;P < 0.05).Expression of miRNA-21 mRNA, PTEN mRNA and protein were obviously related with depth of invasion(F =18.36 ,F =17.26 ,F =12.83;P < 0.05), Dukes stage (F =31.25, F =24.43, F =57.12;P < 0.05) and lymph node metastasis (t =5.85, t =2.18, t =4.05;P < 0.05), while it was not related to tumor differenciation and tumor location (F =7.39, t =4.62;F =7.78, t =1.29;F =5.14,t =1.37;P > 0.05).Positive relation between miRNA-21 and PTEN in colorectal carcinoma was identified (r =-0.994, r =-0.927;P < 0.05).Conclusion High expression of miRNA-21 and low expression of PTEN are both closely associated with invasion and metastasis of colorectal carcinoma.

Hepatoid adenocarcinoma of the stomach ( HAS) belongs to one of the rare cases in gastric cancer types ,which has extremely high malignant degree and poor prognosis .Lymph node metastasis and liver me-tastasis are common in HAS.In this article,we reported alpha-fetoprotein-high-producing hepatoid adenocar-cinoma of the stomach(HAS),and reviewed the related literature at home and abroad in order to improve clinical physicians understanding of these diseases and treatment experience .
3.The time-dependent effect of insulin on the expression of SREBP-1,FAS and lipid droplet formation in HKC cells

Aim To investigate the time-dependent effect of insulin on the expression of SREBP-1(sterol regulatory element binding protein-1),FAS(fat acid synthase)and lipid droplet formation in HKC cells(human renal proximal tubular epithelial cells line).MethodsHKC cells were respectively treated with 100 nmol·L~(-1) insulin for 0,2,4,6,12 h and 24 h.The analysis of SREBP-1 and FAS mRNA was performed by RT-PCR and the expression of SREBP-1 protein was detected by Western blot and immunocytochemistry.Furthermore,Oil Red O staining was used to determine cellular lipid droplet formation.ResultsCompared with HKC cells of 0 h group,there was no difference of SREBP-1 and FAS mRNA in HKC cells of 2 h group.However,the expression of SREBP-1 and FAS mRNA was significantly increased in HKC cells of 4,6 h and 12 h group.Further,the most expression of SREBP-1 and FAS mRNA was at 6 h group and was respectively increased by 3.578 and 4.272 times compared with 0 h group.The results of Western blot showed that the precursor and mature segments of SREBP-1 protein in 4,6 h and 12 h group HKC cells were increased and those of 6 h group HKC cells were the highest and about 4.106 and 5.167 times than those of 0 h group HKC cells.Immunocytochemistry presented the result that SREBP-1 protein was located in the plasma and the expression of 4,6 h and 12 h group HKC cells was significantly higher than that of 0,2 h and 24 h group HKC cells.The result of Oil Red O staining showed that lipid droplet markedly deposited in 6 h group HKC cells,contrarily,no lipid droplet was found in HKC cells of other groups.ConclusionAbove results suggested that insulin up-regulated SREBP-1 and FAS in time-dependent manner that led to cellular lipid droplet deposit,which may play an important role in the pathogenesis of renal lipid accumulation in metabolism syndrome.
4.Applied value of muitislice CT in selecting living donor kidneys and excision methods

Objective To evaluate the applied value of multislice CT (MSCT) in the selection of living donor kidneys and excision methods.Methods Ninety living renal donors underwent MSCT assessment.The nonenhanced,arterial,venous and excretory phase examinations were performed.Using maximum intensity projection and volume rendering techniques for vascular imaging,two blinded radiologists independently analyzed and evaluated all MSCT images.According to the CT reconstructive images,radiologists and physicians selected the left renal or the right renal donors,and chose laparoscopic or open live donor nephrectomy.Results On the 90 cases of donors,78 donors underwent nephrectomy in the left kidney.Seventy-one left kidney donors having no significant variation received the routine laparoscopic live donor nephrectomy.Seven left kidney donors on both sides had relatively obvious anatomical variations such as accessory renal artery,multi-branch renal vein and renal vein in the back of the abdominal aorta,and they were subjected to the left kidney open donor nephre.ctomy.Other 12 donors having significant variation in the left kidney were given nephrectomy in the right kidney,and all of them received hand-assisted laparoscopic live donor nephrectomy.All intraoperative records of urine collection system and renal vascular anatomy were consistent with the preoperative evaluation of MSCT,and the accuracy was 100 %.Two imaging experts in the evaluation of renal artery,renal vein and urine collection system showed good consistency.Nephrectomy was successfully performed on 90 cases of donors,and.postoperative recipients had no renal vein thrombosis and other vascular complications.Conclusion MSCT can provide accurate and valuable information for the selection of living donor kidneys and excision methods as a “one-stop” technique for the preoperative evaluation of living renal donors.
5.Role of S3I-201 in tubular interstitial lesion in lupus nephritis

Purpose To investigate the role of S3I-201 on tubular interstitial lesion in lupus nephritis.Methods MRt/MpJ mice were designated as the control group.MRL/lpr nice were randomly divided into LN group,S3I-201 group and DMSO group.The serum and 24 h-urine were collected to detect the serum creatinine,blood urea nitrogen and urine protein.Immunohistochemistry was used to detect the expression of FN.Western blotting analysis was used to determine the expression of E-cadherin,α-SMA,MCP-1,ICAM1,STAT3 and p-STAT3.Results Compared with the expression level in control group,the protein level of α-SMA,MCP-1,ICAM1 and FN were increased in renal tissue of MRL/lpr mice,while the expression of E-cadherin was markedly decreased.And the STAT3 was activated in renal tissue of MRL/lpr mice.The administration of S3I-201 could inhibite the activation of STAT3 and ameliorate the expression of E-cadherin,α-SMA,MCP-1,ICAM-1 and FN.Conclusion S3I-201 can relieve the tubular interstitial leison,which maybe concerned with the phosphorylation of STAT3.
6.Clinical value of 18F-FDG PET-CT to patients with follicular lymphoma staging Ⅲ-Ⅳ: monitoring response to therapy

Objective To evaluate the clinical value of 18F-FDG PET-CT in monitoring response to therapy in patients with follicular lymphoma (FL) staging Ⅲ-Ⅳ.Methods The clinical data of 20 patients with FL were analyzed retrospectively.All patients underwent PET-CT at three times:baseline,after 3-4 cycles of chemotherapy,and end of treatment.Kaplan-Meier method was used to analyze the survival of patients.Results 14 patients achieved CR at the interim PET-CT,all patients achieved CR at post therapy PET-CT:all patients were alive with no evidence of disease in the follow-up.2 patiens achieved PR at the interim PET-CT,and 1 patient achieved CR,and 1 patient was PD,at post therapy PET-CT,all patients were relapsed in the follow-up.4 patients achieved PD,and all patients were changed the chemotherapy plan.At post therapy PET-CT,2 patients achieved CR,1 patient achieved PR,and 1 patient was PD,1 patient was no evidence of disease,2 patients were relapsed,and another was died in the follow-up.The negative predictive values (NPV) of the interim and the post therapy PET-CT were 100 % (14/14),88.2 % (15/17) respectively.The interim PET-CT was no significantly correlated with PFS (P =0.135),while the post therapy PET-CT was significantly correlated with PFS (P =0.0006).Conclusion 18F-FDG PET-CT is useful to monitor response to chemotherapy in patients with FL staging Ⅲ-Ⅳ,and set personalized treatment plan.
7.Clinical value of 16-row multislice CT in the preoperative assessment of live kidney donor

Objective To analyze the clinical value of 16-row multislice CT in the live kidney donor preoperative assessment. Methods Thirty-six kidney donors underwent 16-slice CT preopera-tive assessment. The non-enhanced, arterial phase, venous phase and excretory phase examinations were performed. Two radiologists independently studied renal vessels and urinary system of each case by maximum intensity projections and volume rendering techniques. The specificity and accuracy of 16-row muhislice CT in the evaluation o{ renal vessels and urinary system were calculated after comparing with surgical findings as reference. Results 16-row muhislice CT found 7 variant renal arteries. One variant artery confirmed in surgery was missed in the CT study. 16-row muhislice CT made cor-rect diagnosis of all variant anatomy of renal veins and ureters which were confirmed in surgery. For i-dentification of variant anatomy of renal arteries, veins and ureters, the specificity of 16-slice CT was 100% (29/29), 100% (32/32), and 100% (35/35), the overall accuracy was 97%(36/37), 100% (36/ 36), and 100%(36/36), respectively. Conclusions 16-row muhislice CT shows excellent value in the evaluation of renal vessels and urinary system in live kidney donors. 16-row multislice CT is suit-able for the noninvasive examination in live kidney donor preoperative assessment and provides all sup-portive and relevant information required by the surgeons.
8. Complete genome characteristics of coxsackievirus A4 isolated from Taian, 2017-2018

Objective:
To analyze the genetic characteristics of Coxsackievirus A4 isolated from Taian, 2017-2018.
Methods:
Sixty throat swab samples of the children who visited Taian Maternal and Child Health Hospital during the year 2017-2018 and were diagnosed as hand, foot and mouth disease, were collected and aseptically inoculated. Fluorescent quantitative PCR analysis was performed using the universal primer for enteroviruses. The high-throughput sequencing was applied to the enterovirus-positive samples, and the full-length genome sequences of the viruses were obtained. Phylogenetic analysis was performed using Mega5.05 and RaxML respectively, and sequence homology and amino acid mutation sites were also analyzed using Mega5.05.
Results:
Four whole genome sequences of CV-A4 isolated from infants aged 17-19 months old were obtained. Phylogenetic analysis of the full length CV-A4 genomes showed that apart from MG550920/AA/Henan/2016, the remaining CV-A4 strains from China (97.2%), including the four strains from Taian, fell within Group 3. The VP1 genes could be classified into four genotypes and 98.5% of the Chinese strains belonged to genotype D, and the four strains from Taian belonged to D2. It was notable that the Taian isolate A1/Taian is closely related to two strains C179 and C062 from Australia both in the complete genome and the VP1 gene, as well as one strain YT184R isolated from Yantai in 2016 by us. Compared with the prototype CV-A4 strain High Point, 18 amino acid mutations were found in the P1 region.
Conclusions
Both phylogenetic trees estimated using the complete genome and the VP1 gene sequences revealed that the four CV-A4 isolates from Taian fell within the same clade with the majority of CV-A4 strains circulating in China. Compared with the prototype CV-A4 strain, several amino acid variations have occurred in the P1 region, which warrants further investigation.
9.Impact of uremic toxins on cognitive function in end-stage renal disease patients: the mediating effect of depression

【Objective】 To explore the mediating role of depression between uremic toxins and cognitive function in end-stage renal disease (ESRD) patients, so as to provide a basis for early clinical intervention. 【Methods】 A retrospective study involved 49 predialysis ESRD patients diagnosed in the Nephrology Department of The First Affiliated Hospital of Xi’an Jiaotong University between August 2018 and October 2021, along with 50 healthy controls (HC). General information of the two groups was collected. Montreal Cognitive Assessment (MoCA), Auditory Verbal Learning Test-Huashan Version (AVLT-H), Trail Making Test A (TMT-A), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI) were used to collect data on cognitive function, anxiety, and depression in both groups. Serological indicators in the ESRD group were used to clarify the impact of uremic toxins on cognitive function. PROCESS v3.4.1 was applied to explore the relationship between uremic toxins, depression, and cognitive function, as well as the mediating effect of depression. 【Results】 Significant differences were found between the ESRD group and the HC group in MoCA total score (P<0.001), AVLT-H (word learning; short-term delay; long-term delay,P<0.001; word recognition, P=0.001), TMT-A (P<0.001), BDI (P<0.001), and BAI (P=0.009). Cystatin C was a negative influencing factor for short-term delay in AVLT-H (B=-0.834, P=0.019), while BDI was a negative influencing factor for long-term delay in AVLT-H (B=-0.102, P=0.002), word recognition in AVLT-H (B=-0.071, P<0.001), and MoCA total score (B=-0.135, P=0.002). BDI partially mediated the effect of cystatin C on short-term delay in AVLT-H (total effect, c=-0.3346; direct effect, c′=-0.223 5; mediating effect, a×b=-0.111 0; and mediating effect proportion, 33.2%) and long-term delay in AVLT-H (total effect, c=-0.318 7; direct effect, c′=-0.218 8; mediating effect, a×b=-0.099 9; and mediating effect proportion, 31.3%). 【Conclusion】 ESRD patients experience cognitive decline as well as anxiety and depression. Cystatin C and depression are both negative influencing factors for cognitive decline in ESRD patients. Cystatin C indirectly affects cognitive function in ESRD patients through depression.
