1.Clinical study on abdominal lymph node metastasis from thoracic esophagus carcinoma

Objective:To analyze the metastasis rule of abdominal lymph node from thoracic esophagus carcinoma. Methods:The abdominal lymph node data on 164 patients who had undergone resection of thoracic esophageal carcinoma were analyzed retro-spectively. Grouping was based on the upper, middle, and lower thoracic esophagus. Differences in tumor infiltration depth, differentiat-ed degree, pathological type, pathological stage, and metastasis rate of the abdominal lymph node among the three groups were com-pared. The metastasis rates of the abdominal lymph nodes among the different tumor infiltration depths, differentiated degrees, and path-ological types were also compared. Results:The base condition of tumor infiltration depth, differentiated degree, pathological type, and pathological stage has no statistical significance among the upper, middle, and lower thoracic esophagus. The metastasis rate of the ab-dominal lymph node also has no statistical significance among the three groups (upper, 6.9%;middle, 27.4%;and lower, 39.6%). More-over, the metastasis rate of the abdominal lymph node has no statistical significance among the different tumor infiltration depths, differ-entiated degrees, and pathological types. Conclusion:A special bound lymph node metastasis was present in the esophageal carcinoma. If the tumor in the upper thoracic esophagus infiltrated the submucosa, then it could bound metastasize down to the abdominal lymph node by the lymphatic capillary net. The majority of the esophageal carcinoma was more than T1b period when diagnosed. The tumor has infiltrated the submucosa. Thus, early stage, well-differentiated, and upper thoracic esophageal carcinoma does not indicate mini-mal metastasis of the abdominal lymph node. Routine abdominal lymph node dissection through radical surgery for esophageal carcino-ma was necessary.
2.Comparison of Steam Distillation and Supercritical CO2 Method in the Extraction of Volatile Constituents from Compound Kuhuang Prescription by GC-MS

Objective:To compare the steam distillation( SD) and supercritical CO2 ( SFE-CO2 ) method in the extraction of vola-tile constituents from such 3 traditional Chinese drugs as cnidium monnieri, radix sileris and atractylodes lancea in compound Kuhuang prescription. Methods:Essential oil was extracted from the 3 traditional Chinese drugs by SD and SFE-CO2 method, respectively. The components were identified by GC-MS, and their relative contents were calculated with peak area normalization method. Results:To-tally 36 components were isolated and identified using SD extraction and 31 components were identified using SFE-CO2 extraction, a-mong them, 22 components were the same and their relative molecular weight mainly concentrated within 200-230. Conclusion: SFE-CO2 method can extract effective components from the 3 traditional Chinese drugs in compound Kuhuang prescription with higher selec-tivity, which is the more suitable extraction method for essential oil from the prescription.
3.Research progress of anti-influenza virus agents.

Influenza is a major threat to millions of people worldwide. Vaccines and antiviral agents are two main options available to reduce the impact of the influenza virus, while anti-influenza agents are the most effective means to prevent the transmission of the highly contagious virus and to treat the epidemics of disease. At present, four anti-influenza agents have been approved by the FDA for the treatment of influenza, including two M2 protein ion channel inhibitors-amantadine and rimantadine and two neuraminidase inhibitors-zanamivir and oseltamivir. Arbidol hydrochloride, launched in Russia, is a potent inhibitor of influenza virus, too. Neuraminidase inhibitors could be classified generally by structure into six different kinds: sialic acid derivatives, benzoic acid derivatives, cyclohexene derivatives, cyclopentane derivatives, pyrrolidine derivatives and natural products. In this paper, recent progress in the research of the action mechanisms and structure-activity relationships of these anti-influenza virus agents were reviewed.
4.Inhibitory Effects of Typhonium Giganteum Soft Capsules on Transplantation Tumor and Expression of p53 Gene in Human Liver Cancer Hep-2 Tumor-bearing Nude Mice

Objective To observe the inhibitory effects of Typhonium Giganteum soft capsules on tumor growth in Hep-2 tumor-bearing nude mice and its effects on the expression of tumor suppressor p53 gene; To discuss its mechanism of action.Methods Human liver cancer Hep-2 cell suspension back subcutaneous vaccination was conducted to prepare tumor models. BALB/c nude mice were randomly divided into model group, cisplatinum group, and TCM high-, medium-, and low-dose groups. Each group was given relevant medicine for gavage, once a day for 21 days. Growth changes of tumor volume were monitored; HE staining was used to observe pathological changes of tumor tissues; RT-PCR was used to detect the expression of p53 gene in tumor tissue of Hep-2 nude mice.Results Compared with the model group, all medication groups could inhibit the tumor growth, and the anti-tumor rates were 55.1%, 42.8%, 30.1%, and 79.5%, respectively; the expression of p53 gene increased; pathological observation results showed that the number and the volume of tumor cells increased, with cytoplasm rarefaction and nucleus anachromasis. All medication groups had karyopyknosis, slight staining, and decreasing blood capillary and focal necrosis in varying degrees.ConclusionTyphonium Giganteum soft capsules can inhibit the growth of human liver cancer, and its mechanism may be associated with the up-regulating expression of p53 gene leading to apoptosis.
5.Analysis of the in vitro quinolones resistance in Helicobacter pylori strains isolated in Zhejiang prov-ince

Objective To investigate the in vitro quinolones resistance in Helicobacter pylori ( HP) strains isolated in Zhejiang province and to provide references for the clinical application of quinolones in the eradication of HP infection.Methods A total of 713 HP strains were isolated from several hospitals located in different regions of Zhejiang province from April to June 2013.Drug sensitivity test was performed to measure the resistance of those HP strains to levofloxacin and moxifloxacin by using the agar dilution method recommended by the Clinical and Laboratory Standards Institute ( CLSI) .The antibiotic solutions were added to the agar and diluted into corresponding critical concentrations.The mixed liquors were placed in dilution plates and added with the bacteria suspension.If there were bacterial expansion on the plates after incuba-tion, the strains were considered to be drug-resistant strains.Results The resistance rates of 713 HP strains to levofloxacin and moxifloxacin were 19.50%.In addition to the Jinhua area, drug resistance rates in the other 7 areas including Hangzhou, Wenzhou, Shaoxing, Jiaxing, Taizhou, Lishui and Zhoushan were above 15%.The highest drug resistance rate of HP was found in Wenzhou, which was significantly higher than that in Jinhua area (26.40%vs 9.52%, P<0.05).No significant differences in the resistance rate of HP to quinolone were found between Wenzhou area and the other regions in Zhejiang.Conclusion Com-pared with amoxicillin and furazolidone, higher resistance rates of HP to levofloxacin and moxifloxacin were observed in Zhejiang province.Because the mechanisms of drug resistance to levofloxacin and moxifloxacin were same and the drug targets within HP to which the two antibiotics binding were similar, levofloxacin and moxifloxacin shared common characteristics of drug resistance among HP strains, indicating that prescription of the two antibiotics in replacement should be avoided in clinical treatment of HP drug-resistant strains.
6.Protective effects of standard dose prednisone combined with glutamine on Graves ophthalmopathy after 131I treatment

Objective To investigate the protective effect of standard dose prednisone(starting dose about 0.4 mg/kg bw) combined with glutamine(2 g/d) for Graves' ophthalmopathy(GO) after 131I treatment.Methods The prospective randomized controlled trial was performed and included 116 consecutive patients with mild-to-moderate GO or no active GO after 131I treatment,but with high risk factors.The experimental group(59 cases) started to take oral prednisone(0.44 ± 0.13)mg/kg at 2 d atter theatment,meanwhile replenishing glutamine,and prednisone was gradually decreased by 5mg per 2 weeks and stopped until 2 months;the control group(57 cases) received the initial dose prednisone(0.43± 0.14)mg/kg without replenishing glutamine,and the rest was same as the experimental group.The GO change and prednisone adverse reactions in 2,4,6 months after treatment were evalua ted in the two groups.Results The baseline characteristics had no significant difference between the two groups.In comparison of the experimental group and control group after 6-month treatment,the CAS score was (2.8 ± 0.8 vs.3.5 ± 0.9),exophthalmos degree was (19.6±8.1)mm vs.(21.7±3.0)mm,eyelid width was (11.3±2.9)mm vs.(13.8±3.1)mm,the improvement degree in the experimental group except for degree was superior to the control group (P<0.01).No new onset GO or deterioation oc curred in the two groups;the experimental group had 42 cases(71.2 %) of GO improvement and 17 cases(28.8 %) of stability,while the control group had 39 cases(68.4%) of GO improvement and 18 cases(31.6%) of stability,and the curative effects had no sttis-tical difference between the two groups(P>0.05).The side effects in the control group were more than those in the experimental group(63.1% vs.30.5%,P<0.05).the body mass increase was more obvious [(3.8±1.8) kg vs.(1.4±1.2)kg,P<0.01],and SBP and DBP in the two groups were slightly increased[(10±4)mm Hg vs.(9±5)mm Hg P>0.05].Conclusion Using the initial dose of oral prednisone(about 0.4 mg/kg) for 2 months and simultaneously replenishing glutamine (2 g/d)can effectively improve mild-moderate GO,and effectively reduce the adverse reactions of GC.However it is needed to extend the follow-up time to assess whether it can truly prevent the deterioration of GO,and to conduct a further study for the role of glutamine.
7.Effects of combined human thymus and cord blood hematopoietic stem/progenitor cell transplantation on cellular immunity of nude mice

BACKGROUND: T cells are believed to play an important role in anti-infection, anti-tumor and immune function. However, the mechanism underlying the differentiation and development remains poorly understood. OBJECTIVE: To investigate the distribution of T cells in nude mice that are jointly transplanted human thymus and cord blood and the reconstruction of the immune function. METHODS: Thirty Balb/c nu/nu nude mice were randomly divided into two groups: an experimental group and a control group. In the experimental group, human thymus tissue was transplanted into the renal capsule of nude mice. Two weeks later, freshly isolated human cord blood CD34+ cells suspension was back perfused into the nude mice via the vein. In the control group, CD34+ cells transplantation was performed directly without thymus transplantation. After 60 days of breeding, the immune function of nude mice was detected in two groups. RESULTS AND CONCLUSION: Human thymus tissue in the renal capsule of nude mice survived and expressed CD3 and HLA-DR molecule. In the experimental group, CD3+ cells which distributed in the form of dots were observed in the mouse spleen. The proportion of CD3+, CD4+, CD8+, and CD4+CD25+ cells were significantly higher in the experimental group than in the control group. The nude mice from the experimental group rejected human gastric cancer BGC823 cells, while those from the control group did not. These findings demonstrated that combined human thymus and CD34+ cell transplantation allow nude mice to acquire T cell-mediated cellular immune function and possess the ability of anti-tumor.
8.Preparation of Tilmicosin Imprinted Polymers and Its Application to Solid Phase Extraction

Molecularly imprinted polymers (MIPs) with high selectivity to tilmicosin (TIM) were prepared using tylosin(TYL) as dummy template, methacrylic acid(MAA) as monomer, ethylene glycol dimethacrylate (EGDMA) as cross-linker.The effects of 4 porogens including dimethyl formamide, methanol, acetone, and chloroform on the recognition capability of MIPs were investigated.Orthogonal test was used to optimize the preparation of MIPs, and the optimal composition was as follows; 1.0 mmol TYL, 8.0 mmol MAA, 20.0 mmol EGDMA, 6.0 mL chloroform, 20.0 mg azobisisobutyronitrile.The solid phase extraction condi tions and characteristics of MIPs as adsorptive material for the selective extraction and enrichment of TIM were also studied.The recovery of TIM was above 90% when the following procedure was applied to MIPs cartridge: conditioning with methanol and water(pH 9.0), loading with acetonitrile, cleaning with methanol and chloroform respectively, and eluting with 3 mL methanol-ammonia(95:5, V/V).The recovery of TIM on non-imprinted polymers cartridge was only 32%.
9.Radiation-induced adaptive response of nucleoplasmic bridges in human peripheral blood lymphocytes

Objective To explore whether a low dose of 60Co γ-rays could induce the adaptive response in the formation of nucleoplasmic bridges (NPB) in human peripheral blood lymphocytes,and if so,the range of the priming dose.Methods Human peripheral blood samples from healthy males were collected and irradiated with 0,20,50,75,100,150 and 200 mGy (dose-rate was 25 mGy/min) of 60Co γ-rays.After 6 h,the samples were irradiated with a challenge dose of 2 Gy (dose-rate was 1 Gy/min).The cytokinesis-block micronucleus (CBMN) assay was carried out to analyze the NPB and micronuclei (MN) formation in binucleated cells.Results Within the dose range of 0-200 mGy,the yields of NPB and MN increased with irradiation dose of γ-rays and the dose response of NPB followed with a linearquadratic equation of y =(1.5 × 10-4) x2-(5.67 × 10-3)x + 0.598 (R2 =0.893 8).Compared with the samples irradiated with 2 Gy alone,the yields of NPB and MN were significantly reduced when the samples were irradiated with a priming dose of 75-100 mGy before 2 Gy irradiation (U =2.66,2.97,3.96,5.89,P <0.05).The biggest decrease ratio of NPB yields approached to 43.2% at the priming dose of 100 mGy.Conclusions Low doses in the range of 75-100 mGy of 60Co γ-rays could induce the adaptive response of NPB formation in human peripheral blood lymphocytes.
10.Tibial locking multidirectional interlocking intramedullary nail for fractures of tibial plateau

Objective To observe the clinical effects of tibial locking multidirectional interlocking intramedullary nail (TLMIIN) for tibial plateau fractures. Methods 38 cases with closed tibial plateau fractures treated by TLMIIN from October 2008 to May 2012 were retrospectively analyzed. There were 22 males and 16 females, with an average age of 48.7 years (range, 28-67 years). There were 24 fractures on the left side while the other 14 factures were on the right side, which were all fresh frac?tures. According to AO/OTA classification of tibial plateau fractures, there were 4 cases of B1, 1 case of B2, 14 cases of B3, 8 cas?es of C1, 5 cases of C2 and 6 cases of C3. Close reduction were performed on 4 cases (10.5%of all cases). Open reduction were performed on the other 34 cases. The limited incision was decided by the distribution of the fragments and fracture line. Hohl?Luck evaluation system was applied for the follow?up. Results The mean follow?up period was 18.1 months (range, 11-23 months). All fractures were healed at an average period of 87.4 days (range, 48-131 days). The average time from operation to full weight?bearing was 108.9 days (range, 80-128 days). Hohl?Luck evaluation system was used in the final follow?up. The excellent and good rate of functional score was 94.7%(36/38), including 28 cases excellent, 8 cases good and 2 cases fair from functional as?pect. The excellent and good rate of radiological score was 84.2%(32/38), including 23 cases excellent, 9 cases good and 6 cases fair from radiological aspect. No complications such as infection, breakage and loosening of the screw, malunion, nonunion oc?curred at the time of the latest follow?up. 6 cases with serious swelling of the knee joint and the soft tissue of crus were cured by an?ticoagulation, dehydration and physiotherapy treatments after operation. The 2 cases with a little exudates and incrustation was bacterial cultured negative and healed after 16 days and 18 days on the incision. 1 case had lost of reduction due to weight?bearing 1 week after operation, who had 25 degree of varus deformity, which was left dispose, and the bone healing and joint function were unaffected. Conclusion The intramedullary support, restrictive and non?restrictive multidirectional tridimensional fixation of TLMIIN technology had satisfactory effects in treating plateau fractures. It can supply a new therapeutic method, with little dam?age of soft tissues when taking out the internal fixation and reduce some postoperative complications.
