1.Effect of polysaccharide from Porphyra on spleen cell function in immunosuppressive mice

Objective To investigate the effects of polysaccharide from Porphyra on spleen cell function in immunosuppressive mice. Methods The polysaccharide at dosages 0.025, 0.050 and 0.150g?kg~ -1 was injected intraperitoneally for 7 days in the immunosuppressive mice induced by cyclophosphamide. On day 8, the splenic lymphocytic proliferation and activity of TNF were determined. Results The splenic lymphocytic proliferation and cytotoxicity of tumor necrosis factor (TNF) of the group treated with 0.150g?kg~ -1 of porphyra polysaccharide were higher than those of model group. Conclusion Polysaccharide from Porphyra could promote splenic lymphocytic proliferation and raise the level of TNF.
2.Human carcinoembryonic antigen-related cellular adhesion molecules 1 mediated susceptibility of Neisseria gonorrhoeae to mouse

ObjectiveTo develop a transgenic mouse model for N.gonorrhoeae researches.Methods Human carcinoembryonic antigen-related cellular adhesion molecules 1 (hCEACAM1) eukaryotic expression vector,pCDPGICAM1,was used to generate transgenic mice by microinjection.The funder mice were screened by PCR,sequence analysis,Western blot and fluorescence-activated cell sorting analysis,respectively.The transgenic mice expressing hCEACAM1 were inoculated with N.gonorrhoeae intravaginally.Adhesion and infection of gonococci to mice were analyzed by bacteria culture and microscopy.Results Four (lines 50,53,54,and 59) of the 22 F0 generation transgenic mice were found to carry the transgene.The hCEACAM1 protein was expressed on the cell membrane of various tissues in the line 53 transgenic mouse.Compared with normal mice,N.gonorrhoeae can successfully infect and cause inflammation in the transgenic mice.Conclusion The hCEACAM1 transgenic mouse can be used as an animal model for gonococcal infections.
3.Diagnosis and treatment of postoperative gastroduodenal fistulas in patients with severe acute pancreatitis

Objective To investigate the etiologies, mechanisms, diagnosis and management of postoperative gastroduodenal fistulas in patients with severe acute pancreatitis (SAP). Methods The clinical data of 18 cases of postoperative gastroduodenal fistulas (PGF) in patients with SAP admitted in our hospital from Jan, 1996 to Dec, 2007 were analyzed retrospectively. Results Of 18 patients with SAP, 4 patients underwent cholecystectomy and common bile duct exploration, 4 patients underwent pancreatic debridement and open saucer-type drainage, 10 patients underwent pancreatic debridement and pancreas-bed mobilization, abdominal closed drainage. Duodenal fistula occurred in 12 patients (66.7%), gastric fistula occurred in 6 patients (33.3%). 14 patients (77.8%) developed gastroduodenal fistula within 3 weeks of surgery, and 4 patients developed gastroduodenal fistula after 3 weeks of surgery. 16 patients (88.9%) were cured with non-surgical management including drainage, antibiotics and abscess cavity douching. The average length of stay was 65 days. Two patients (11.1%) died of intra-abdominal hemorrhage and intra-abdominal infection complicated with abdominal compartment syndrome and multiple organ dysfunction syndromes, respectively. Conclusions SAP complicated with postoperative gaatroduodenal fistulas could be cured by appropriate non-surgical treatment.
4.Analysis of the clinical features, prognosis and early warning indicators of severe hand, foot and mouth disease

Objective:To explore the type distribution and severity of central nervous system(CNS) complications in severe hand, foot and mouth disease (HFMD) cases prior to the introduction of human enterovirus A group 71 type (EV-A71) vaccine, and provide scientific data for early clinical intervention.Methods:A total of 3 583 laboratory-confirmed severe HFMD cases in Jiangsu province during 2010-2016 were collected and analyzed retrospectively. Related early warning signs of increased HFMD severity were estimated with logistic regression analyses.Results:The severity-fatality rate, severity-pediatric intensive care unit(PICU) admission rate, and sequelae rate were 8.09‰(29/3 583), 11.75% (421/3 583)and 5.30‰(19/3 583). Of them, 39.02% (1 398/3 583) patients suffered from mild CNS involvement, 59.22% (2 122/3 583) patients suffered from severe CNS involvement, 1.76%(63/3 583) suffered from critical CNS involvement. The rates of the cases whose age of onset was 6-11 months, the rates of cases with atypical rash, respiratory-related signs and symptoms (shortness of breath, slowed breathing, dyspnea, etc), neurological-related signs and symptoms [(hand and foot shaking, convulsions, lethargy(sleepiness), etc], circulatory-related system signs and symptoms (faster heart rate, abnormal skin color, arrhythmia, cold limbs), laboratory-related indicators (increased white blood cell count, increased lymphocyte count, increased platelet count, increased C-reactive protein, etc), clinical auxiliary examination [electroencephalogram(EEG), brain CT, chest X-ray)] were highest in the critical CNS involvement group, and the differences were significant ( P<0.05). Multivariate logistic regression analysis showed that with the increase of proportion of convulsions, slowed breathing, vomiting, meningeal irritation and other 7 variables, the severity of CNS complications increased ( P<0.05). Conclusions:The indicators such as easily startled, slow breathing, vomiting, elevated lymphocytes, abnormal EEG and other indicators have important clinical significance for children with severe HFMD to progress to CNS complications of different severity.
5.Meta-analysis of safety and effectiveness of tranexamic acid in orthopedic surgery for patients with adolescent idiopathic scoliosis

Objectives:To systematically analyze the effectiveness and safety of tranexamic acid(TXA)in or-thopedic surgery for patients with adolescent idiopathic scoliosis(AIS).Methods:Literature conceming the use of TXA in the orthopedic surgery of AIS patients of randomized controlled trial(RCT)and retrospective cohort studies(RCSs)were searched in the Cochrane library,Web of Science,Embase,PubMed,China National Knowledge Infrastructure(CNKI),and Wanfang Data Knowledge Service Platform,from the inception of the databases to December 2023.The quality of the included literature was evaluated by researchers according to Cochrane evaluation scale for RCT studies and Newcastle-Ottawa Scale(NOS)for RCS studies.The outcome indicators were extracted,including total blood loss,intraoperative blood loss,postoperative drainage,blood transfusion rate,postoperative hemoglobin(Hb)value,operation time,incidence of deep venous thrombosis and related complications.RevMan 5.4 software was used to analyze statistically the extracted data to evaluate the effect of TXA in orthopedic surgery for AIS patients.Results:A total of 8 RCT and 5 RCS studies were in cluded,and all of which were of high-quality,covering 855 patients,with 439 cases in the TXA group and 416 cases in the control group.Meta-analysis results showed that comparing with the control group,the TXA group was lower in intraoperative blood loss[mean difference(MD)=-310.81,95%confidence interval(CI)(-331.91,-289.72),P<0.01],total blood loss[MD=-431.92,95%CI(-568.72,-295.13),P<0.01],postoperative drainage[MD=-59.87,95%CI(-63.98,-55.75),P<0.01],and the blood transfusion rate[odds ratio(OR)=0.17,95%CI(0.05,0.53),P=0.003];shorter in the operation time[MD=-5.94,95%CI(-10.73,-1.14),P=0.02]and higher in the Hb value after operation[MD=-0.40,95%CI(-0.26,0.54),P<0.01].No deep venous thrombosis or related complications occurred in both groups.Conclusions:TXA can effectively reduce the intraoperative blood loss,total blood loss,postoperative drainage,blood transfusion rate and operation time in the orthopedic surgery of AIS patients,and maintain the postoperative Hb level without raising the likelihood of thrombosis and related complications.
