1.Rational Drug Use Based on Drug Withdrawal

[Objective]To analyze the characteristics of outpatient prescription drug withdrawal,reduce out-patient drug withdrawal to ensure the safety of patients with medication.[Method]Make statistics on outpatients from January to September of 2007,1292 prescription drug withdrawals,drug withdrawal rate accordingly,drug dosage forms,drug types,the cause of drug withdrawal classification.[Result]There're many pediatric drug withdrawal,injection drug withdrawal,retreat antibiotic drugs,drug withdrawal and the main reason is unreasonable adverse drug reaction.[Conclusion] Analyzing the characteristics of drug withdrawal to strengthen rational drug use can reduce the greatest degree of adverse drug reaction,reducing out-patient drug withdrawal to ensure the drug safe,effective,rational and economic.
2.METADATA EXPANDED SEMANTICALLY BASED RESOURCE SEARCH IN EDUCATION GRID

With the rapid increase of educational resources, how to search for necessary educational resource quickly is one of most important issues. Educational resources have the characters of distribution and heterogeneity, which are the same as the characters of Grid resources. Therefore, the technology of Grid resources search was adopted to implement the educational resources search. Motivated by the insufficiency of currently resources search methods based on metadata, a method of extracting semantic relations between words constituting metadata is proposed. We mainly focus on acquiring synonymy, hyponymy, hypernymy and parataxis relations. In our schema, we extract texts related to metadata that will be expanded from text spatial through text extraction templates. Next, metadata will be obtained through metadata extraction templates. Finally, we compute semantic similarity to eliminate false relations and construct a semantic expansion knowledge base. The proposed method in this paper has been applied on the education grid.

4.Epidemic characteristics of falls and its related injury among elderly female in Changing district of Shanghai

Objective To study the epidemic characteristics of falls among older female adults.Methods Stratified random sampling was applied in this study.Totally 969 elderly female adults aged 60 years and over from communities of Changning district were recruited and self-designed questionnaire was used in survey.Results Among 969 interviewees,238 (24.6%) had an injury in the past years,and 4.7% had more than one fall.Among those who fell in the past year,84.5% had suffered from different kinds of fall-related injury with injury incidence of 20.7 %,and 32.4 % suffered from fracture with the fracture rate of 7.9%.There was a significant difference in fall incidence between different age groups,and the incidence increased with aging (x2 =30.26,P<0.01).After a fall,71.6% cases needed certain medical service,and 11.8% were hospitalized for treatment.Among those suffered from fall-related injury,the activities of 29.3% patients were decreased,and 3.0% lost the ability to live independently.Most falls (31.2%) occurred at home.Among those,61.8% of patients considered the reason of their falls was the interaction between intrinsic hazards and environmental hazards.Conclusions High incidence and severe consequence of fall appear among elderly female adults.More attention should be paid to this population.
5.Value of spatio-temporal image correlation in prenatal diagnosis of complex heart malformations

Objective To evaluate the value of spatio-temporal, image correlation (STIC) in prenatal diagnosis of complex heart malformations.Methods STIC was used to examine 25 fetuses with 20-30 weeks of gestation who were doubted with complex heart malformations prospectively by two-dimensional (2D) echocardiography.Dynamic three-dimensional imaging was achieved by postprocessing technique with STIC across the entire fetal heart and thorax.An offline analysis of the acquired volume was carried out to examine the fetal cardiac anatomy with multiplanar mode, surface rendering and minimum mode.The results of prenatal diagnosiswere compared with postnatal color Doppler and surgical findings.Results Successful STIC volume datasets were obtained in all 25 fetuses.The findings of all 25 fetus with complex heart malformations included double outlet of right ventricle in 1 case, truncus arteriosus in 1 case, complete transposition of the great arteries in 3 cases,corrected transposition of the great arteries in 1 case, complete endocardial cushion defect in 4 cases, hypoplastic right ventricle syndrome in 1 case, hypoplastic left heart syndrome in 2 cases, tricuspid atresia in 1 case,pulmonary atresia in 1 case, tetralogy of Fallot in 3 cases, single atrium in 1 case, single atrium and single ventrium in 4 cases,in 1 case, atrial septal defect and ventricular septal defect with coronary sinus dilatation in 1 case.The major diagnoses derived from STIC in 23 of 25 cases were consistent with the results of postnatal color Doppler and surgical findings.Conclusions STIC can provide more accurate information of structures of complex fetal heart malformations and has great value in pregnant prognosis.
6.The effect of Fas gene transfection on bladder cancer cells

Objective To study the effect of Fas gene transfection on bladder cancer cells. Methods Fas cDNA were transduced into bladder cancer cells EJ by DOTAP liposomal transfection regeant. Northern blot,in situ hybrization and flow cytometric evaluation were used to confirm the Fas mRNA and protein expression. The apoptosis and proliferation of EJ cells pre and posttransfection induced by cisplatin were analysed by flow cytometry、DNA ladder and MTT methods. Results Transfection of Fas gene can significantly upregulate the expression of Fas in human bladder cancer EJ cells. Apoptosis and decrease of proliferation were easily induced by cisplatin in the transfected EJ cells. Conclusions Fas system might involve in the development and progression of urogenital malignant tumors. Transfection of Fas gene by lipofectin can significantly upregulate the Fas expression in target cell. The synergistic cytotoxic effect obtainted in EJ cells suggested that combined use of Fas gene transfection and cisplatin may help in the treatment of cisplatin resistant bladder cancer.
7.Evaluation of improvement of subhypothermia in cerebral vasospasm after severe craniocerebral injury

BACKGROUND: It is indicated in latest research that in cerebral protective measures of cerebral vasospasm induced by subarachnoid hemorrhage due to various factors in acute stage, mild hypothermia has been drawn the attention specially and it has been recommended in clinical practice. But such therapy is generally limited in experimental research and aneurysm hemorrhage, the clinical research on subhypothermia probably provides important influence on cerebral vasospasm in severe craniocerebral injury.OBJECTIVE: Based on cerebral vascular hemodynamical indexes (CVDI),the cerebral protection of subhypothermia was observed on cerebral vasospasm in severe craniocerebral injury.DESIGN: Randomized controlled experiment.SETTING: Department of Neurological Surgery of No.16 Hospital of Chinese PLA and General Military Institute of Neurological Medical Science in Zhujiang Hospital Affiliated to First Military Medical University of Chinese PLA.PARTICIPANTS: Totally 36 cases of severe craniocerebral injury were selected in General Military Institute of Neurological Medical Science in Zhujiang Hospital Affiliated to First Military Medical University of Chinese PLA from July 1997 to August 1999, which were randomized into the control and treatment group, 18 cases in each one. At same period, 24 cases with normal CVDI were screened and taken as normal group. All of receptors participated in the experiment in volunteer.METHODS: In both the control and treatment group, the treatment was applied with anti-inflammation, stopping bleeding, fluid limitation, dehydration, supporting, hyperbaric oxygen, etc. In the control, the normal body temperature was maintained and in treatment group, anus temperature was dropped to about 33 ℃ in 4 hours to 8 hours, which was maintained for 3-test was given on the day of injury (0), on the 1st, 3rd, 7th, 14th and 21st days assay was done on the day of injury (0), on the 1st, 3rd, 7th, 14th and 21st days successively, in which, minimum blood velocity (Vmin) and minimum blood flow (Qmin) reflect blood supply of distal cerebral vessel and blood flow.Cerebral vessel resistance (CVR) reflects smooth degree of cerebral microcirculation. Dynamical resistance (DR) reflects auto-regulation of cerebral vessel. Criteria of evaluation: Recovery state of consciousness was justified according to Glasgow Coma Scale (GCS) in 1 week after injury. The outcomes were evaluated according to Glasgow outcome scale (GOS) in 3 months (5 score: good, 4 score: moderate handicapped, 3 score: severely handicapped, 2 score: vegetative state and 1 score: death). The case over 4 score indicated good outcome.ery of consciousness and outcomes in 1 week after injury in the control and treatment group.RESULTS: Totally 36 cases of severe craniocerebral injury entered result phases after injury in the control, named hypoperfusion phase (0 day), hyperperfusion phase (1-3 days), cerebral vasospasm phase (4-14 days) and improving phase (>15 days). In treatment group, 3 phases were manifested,named hypoperfusion phase (0 day), improving phase (1-3 days) and recovery phase (> 4 days), without hyperperfusion phase. Eight cases and 2 cases of cerebral vasospasmodic changes in CVDI presented in the control in focus: The maximum volume (140.9±22.95) cm3 was on the 14th day after injury in the control and that (95.83-±14.97) cm3 was on the 3rd day in treatment group. On the 14th day after injury, the volume in treatment group in 1 week after injury: It was 22.2% (4/18) in the control and 55.6%(10/18) (P < 0.05) in treatment group. Improving outcome rate: It was 38.9% (7/18) and 66.7% (12/18) in treatment group.CONCLUSION: Subhypothermia reduces incidence of cerebral vasospasm by stabilizing cerebral circulation after severe craniocerebral injury, especially by inhibiting acute hyperperfusion after the injury so that the volume of cerebral edema in focus is lessened remarkably and the prognosis is improved.
8.Application of Vacuum sealing drainage in full-thickness skin grafts

BACKGROUND: Full-thickness skin graft was a base for bum and plastic surgery, while uniform pressure and regional brake were key factors to ensure skin graft survival and avoid from necrosis. Traditionally, package and pressurized fixation were performed after skin transplantation; however, it induced residual dead space and unclear skin graft fixation, as well as suturing scar.OBJECTIVE: To study the effect of vacuum sealing drainage applied to full-thickness skin graft.METHODS: A total of 8 New Zealand rabbits were used to establish full-thickness skin graft models in three regions of bilateral spine. Vacuum sealing drainage, traditional pressurized suture and common wrapping were performed in the three regions, respectively. The skin graft survival was observed, and survival rate was calculated at 14 and 21 days. On the 3rd, 7th, and 14th days, samples were selected from skin graft and stained. The morphology was observed under light microscope and transmission electron microscope. The time to remove the drain vessel was that when the fluid was not increased or the fluid was clear.RESULTS AND CONCLUSION: The survival rate of vacuum sealing drainage group was significantly higher than that in other two group (P < 0.05), while the survival rate of traditional pressurized suture group was significantly higher than that in the common wrapping group (P< 0.05). Morphology examination demonstrated that regional cuticular layer was necrotic in the vacuum sealing drainage group after early skin transplantation, while inflammatory cell infiltration, fibroblast degeneration, and mitochondrial swelling were also observed. At later skin transplantation, fibroblast and basal cell were proliferated, and function of mitochondria and plasmid was active. This suggested that vacuum sealing drainage promoted survival rate of skin graft.
9.Taurine protects neuronal cells by suppressing Caspase 9 activation

Objective Neurological diseases are closely associated with the apoptosis of neuronal cells .This article aims to study the inhibitory effect of taurine on the apoptosis of hippocampal neurons by activating Caspase 9 as well as its protective effect on the nervous system and its mechanisms . Methods Mouse hippocampal neuronal cells were randomly divided into four groups:control, injury and apoptosis, low-dose taurine protection, and high-dose taurine protection.The proliferation of the neuronalcells was observed, their apoptosis examined by MTT colorimetric assay, and the expression of Caspase 9 in different groups detected by immunofluorescence and Western blot. Results The injury and apoptosis group showed a poor proliferation of the hippocampal neuronal cells and decreased cell viability (A=0.102 ±0.025), significantly lower than the control group (relative A=0.643 ± 0.013), the low-dose taurine group (relative A=0.504 ±0.072), and the high-dose taurine group (relative A=0.452 ±0.029) ( all P<0 .05 ) .Immunofluorescence assay revealed significantly increased Caspase 9 activation in the injury and apoptosis group (A=61386.8 ±10083.6) compared with the control (A=4502.2 ±2518.1), the low-dose taurine (A=20077.4 ±4187.5), and the high-dose taurine group (A=13976.2 ±7044.1) (all P<0.05).Western blot showed a remarkably higher expression of Caspase 9 in in the injury and apoptosis group (A=1.23) than in the control (relative A=0.17), the low-dose taurine (A=0.21), and the high-dose taurine group (A=0.19) (all P<0.05). Conclusion Taurine can protect neuronal cells by inhibi-ting Caspase 9 activation.
10.Distributive Properties and Susceptibility of Clinical Candida spp Isolates to Antifungal Agents

OBJECTIVE To study the distributive properties and susceptibility of yeasts to six antifungal agents. METHODS To analyze the distributive properties of 264 clinical Candida spp isolates and study the susceptibility to amphotericin B,nystatin,fluconazole,ketoconazole,miconazole and clotrimazole.The susceptibility of yeasts was tested according to the National Committee for Clinical Laboratory Standards guideline(NCCLS M27-A2). RESULTS Strains of Candida albicans were the most frequent organism isolated accounted for 62.5% of all the isolates.C.tropicalis,C.glabrata,and C.parapsilosis accounted for 20.8%,12.5%,and 1.9%,the others accounted for only 2.3%.The main infected organs were lungs,urinary tract,and digestive tract;the susceptibility tests showed strains of Candida spp to nystatin,amphotericin B,and fluconazole were more active than to the other antifungal agents.The resistance to triazole antifugal agents could be shown. CONCLUSIONS We should strengthen the diagnosis of Candida spp and strengthen the surveillance on susceptibility of clinical isolates Candida spp so as to help the doctors choose the antifungal agents reasonably.
