Objective To investigate the predictive value of peripheral leukocyte and its subtypes count on cerebral-cardiovascular events in patients with unstable angina pectoris.Methods From December 2000 to January 2003,141 patients(aged 41-84 years) with unstable angina pectoris referred for coronary angiography were enrolled and their peripheral leukocyte and its subtypes were counted.The major adverse cerebral-cardiovascular events rate of these patients during 46.2?6.6 months of follow-up were assessed.Results A total of 55 cerebral-cardiovascular nonfatal events and 8 deaths were observed.After adjusting the risk factors of diabetes mellitus,smoking,hypertension,hyperlipidemia,prior myocardial infarction,family history,and blood fibrinogen and uric acid level,leukocyte count was positively associated with cerebral-cardiovascular events(adjusted relative risk 1.31,95% CI 1.15 to 1.49).The monocyte count in patients with cerebral-cardiovascular events was higher than that in patients without cardiovascular events(P

4.Three-dimensional spiral CT angiography in the diagnosis of Takayasu arteritis

Objective To assess the application of spiral CT angiography in the diagnosis of Takayasu arteritis.Methods Vascular imaging features of three-dimensional CT angiography of 56 patients with Takayasu arteritis were analyzed retrospectively.Results The vascular imaging of all the patients were qualified for diagnosis.A total of 175 arteries were involved,including subclavian arteries (36.00%,63/175) with a high percentage of the left subclavian arteries (24.00%,42/175) ,common carotid arteries (29.71%,52/175) and renal arteries (10.86%,19/175) .The affected artery wall had varying degrees of concentric or eccentric thickening,and stenosis or occlusion of lumen was observed.Conclusion Threedimensional spiral CT angiography can make clear diagnosis of Takayasu arteritis,including the scope,extent and nature,and thus is worthy of clinical application.
5.The Meta-analysis on the effect of different education models on the self care ability of patients with peripherally inserted central catheters

Objective To evaluate the effect of different education models on the self care ability of peripherally inserted central catheters (PICC) placement patients.Methods The randomized controlled trial (RCT),controlled clinical trail (CCT),self-controlled trial and cohort study on self care ability of patients with PICC placement were collected using databases by manual and Internet searching.Data were analyzed with Revman5.0 software,data which could not be gathered were expressed by descriptive analysis.Results Eight articles were included in the study.The results of Meta-analysis revealed that different education models could significantly improve the self care ability of PICC patients in the intervention (experimental) group.Conclusions Different education models had significant meaning on the self care ability of PICC placement patients.

7.The Roleof Using the DBP-CT in the Staging of Advanced Gastric Cancer

Objective:To evaluate the role of using the double body position CT(DBP-CT)in the staging of advanced gastric cancer.Methods:We performed CT examination in 85 patients with advanced gastric cancer which were divided into the double postion group(55 cases)and the single position group(30 cases),after oral intake water agent.The preoperative CT findings of each group was compared with surgical findings respectively and the accuracy in the CT staging of advanced gastric cancer of each group was estimated.Results:The accuracy in the preoperative CT staging of double position group was 89.09%,and in single postition group was 60%.There was a significant difference(P<0.05).Conclusion:Our findings show that the DBP-CT is surperior to the single position group in finding lesion,confineing the range of the tumour,and determing the degree of adjacent invasion.It can improve the diagnostic accuracy in the preoperative staging of advanced gastric cancers significantly.

Objective To evaluate the role of different kinds of body positon in the stomach CT scanning.Methods It included 30 normal adults who underwent CT scanning in four differnent kinds of body positions.The relationship between the body position and the image appearence in the each part of stomach has been analysed.Results The results showed that the adjusting of body position was useul in revealing gastric wall and perigastric organs.The best imaging result of fundus ventriculi part of stomach can be obtained with the head down and the feet high position while patients lie on his back.The best body position to reveal corpus ventriculi part is in prone position and the best body position to reveal pars pylorica part is in right lateral position.There is a very significant difference between supine position and the best revealing position of each part of stomach prespectively(?
9.Influences of differenct pre-hospital care patterns on the effect of acute coronary syndrome

Objective To study the comprehensive quality of pre-hospital care of patients with acute coronary syndrome (ACS) in our country,and the factors affecting the efficiency of medical intervention of pre-hospital care.Methods An epidemiological investigation with target population-based cross-sectional study was carried out,and specifically designed questionnaire for determining the efficiency of pre-hospital care of patients with ACS was used to collect information of 272 patients from 13 emergency pre-hospital care centers.Results After analysis of the data from 13 majormedical centers,it could be classified into 4 patterns of pre-hospital care,namely directive pattern,pre-hospital pattern,dependent pattern and independent pattern.The results of analysis showed that the distance for ambulance driving in directive pattern was shorter than that in dependent pattern,but the call-response in directive pattern was quicker than that in pre-hospital pattern and dependent pattern,while the call-arrival time in directive pattern was longer than that in independent pattern and dependent pattern.The analysis of the on-site preliminary treatment in four patterns of pre-hospital care showed that there were differences in the establishment of intravenous access,aspirin,nitrate and ECG ( P < 0.05 ).The total effective rate of pre-hospital care for ACS was 48%,while effective rate of independent pattern reached 70%.Factors affecting therapeutic effect on ACS resulting in better outcomes included pre-hospital pattern ( OR =4.097 ),directive pattern ( OR =5.158 ),and use of nitrate ( OR =3.045 ) and oral administration of medicine ( OR =8.215 ). Conclusions Independent pre-hospital pattern had the best therapeutic effect in the pre-hospital care of patients with ACS,and pre-hospital pattern had the best on-site preliminary disposal.Use of nitrate and oral administration of medicine showed the best benefit to the therapeutic effect.
10.Catheter-directed thrombolysis through popliteal vein for the treatment of lower extremity deep venous thrombosis

Objective To evaluate catheter-directed thrombolysis through popliteal vein for the treatment of lower extremity deep venous thrombosis (DVT). Method A total of 62 patients with single symptomatic lower limb DVT were analyzed. The popliteal vein of symptomatic limb was punctured, under Doppler guidance a 5-F sheath was advanced into the deep vein. A catheter was advanced beyond the thrombus. After a bolus of 200 000 IU urokinase, 100 000 IU/h urokinase was infused for up to three days, meanwhile 400 IU heparin was administered subcutaneously every 12h. Clinical efficacy grade was evaluated by measuring the perimeter of legs and lysis grade was calculated based on venographic results. Results In addition to total or partial removal of the thrombus collateral circulation increased, the limbs edema reduction rate was 78% and the thrombolysis rate was 67%.
