OBJECTIVETo explore the effect of the expanded capsule on biomechanics of the transplanted autologous costal cartilage.
METHODSTen New Zealand white rabbits aged 3 months were involved and four 15 ml tissue expanders were implanted symmetrically on the back of each rabbit. After 1 month, expanded capsules were removed (experimental group) on the left side and were reserved (control group) on the right side. Meanwhile, the 6th and 7th costal cartilage without the perichondrium were taken out and each cartilage was divided into two pieces and put under the expanded capsule symmetrically. The cartilage at the upper sides and lower sides were taken out at 4 weeks and 8 weeks later, respectively. All the cartilages were sculptured for the demand of the machine text. The tension and compression property of the cartilages were assessed by the material testing machine (Instron 5967) and the results were analyzed statistically.
RESULTS8 weeks lateral, the average ultimate tension strength was (4. 954 + 2. 8) MPa in experimental group and (2.939 + 0.842) MPa in control group. The average compressive strength was (58.74 ± 13.77) MPa in experimental group, and (47.61 ± 11.41) MPa in control group. The biomechanics property of the cartilages in the experimental group is better than the control group, and the differences were statistically significant.
CONCLUSIONSRemoving the expanded capsule is benefical to maintain the biomechanics of autologous costal cartilage.

3.Clinical significance of monitoring of anti-HLA antibodies in kidney recipients after renal transplantation

Objective To detect de novo development of anti-HLA antibodies after renal transplantation, and to investigate their influence on graft function. Methods 384 kidney recipients,who were negative for anti-HLA antibody before transplantation, were monitored for anti-HLA antibodies over a period of 3-96 months, and a sensitive enzyme-linked immunosorbent assay (ELISA) was used to detect anti-HLA antibodies. HLA antibody >10 % was defined as positive levels. Results Among 384 recipients tested, 318 recipients (82. 8 %) were negative for anti-HLA antibody after transplantation; 66 recipients (17. 2 %) developed de novo HLA antibodies, 3 recipients with HLA class Ⅰ, 61 with HLA class Ⅱ, 2 with both HLA class Ⅰ and Ⅱ. According to amino acid residue matching, 7 cases developed de novo antibodies among 92 recipients with 0 HLA-DR mismatches,compared with 59 cases among 292 recipients with 1-2 mismatches, which showed significant difference between two groups (P<0. 01 ). 87. 4 % (278/318) recipients negative for HLA antibodies after transplantation achieved good graft function, in comparison with 65. 2 % (43/66) recipients positive for HLA antibodies (P<0. 05). Conclusion De novo production of HLA antibodies posttransplantation may be closely associated with HLA-DR mismatch. De novo HLA antibodies posttransplantation might damage graft function and reduce graft survival rate. The detection of de novo development of anti-HLA antibodies after renal transplantation has clinical significance for assessing renal allograft function.
4.Primary non-Hodgkin′s Lymphoma of the right humerus bone:A case report

Objective To investigate the clinical characteristics and surgical treatment of primary bone non-Hodgkin′s lymphoma.Methods Recalling a case of primary bone non -Hodgkin′s lymphoma patient data in Affiliated Maojian Hospital of Hubei University of Medicine .This patient was hospitalized because of pain in the right upper arm.By improving the relevant examination and diagnosis of right humeral bone tumors ,We had successfully completed a bone tumor resection and graft as well as dual plate fixation .Results Pathological diag-nosis after surgery showed the primary non -Hodgkin′s lymphoma of bone ,9 months follow-up after surgery ,the treatment effecincy was good .There were no tumor recurrence and distant metastasis .Conclusion Primary non-Hodgkin′s lymphoma of bone is uncommon in clinical practice .If there is primary bone tumor with no obvious sys-temic symptoms in elderly patients , we should considered it as a possible diagnosis of primary bone non -Hodgkin′s lymphoma,in order to reduce the possibility of misdiagnosis and mistreatment .Short-term effects of surgical treatment of this disease is excellent .
5.Analysis of epidemiological survey results of hydatid disease in Gannan Tibetan Autonomous Prefecture Gansu Province

Objective To understand the epidemic situation of hydatid disease in Gansu Tibetan Autonomous Prefecture(Gannan state) Gannan Province,and prevalence of the disease in population,livestock and final host dogs,and to evaluate the effectiveness of prevention and control measures.Methods In 2012:①An investigation was carried out according to the requirements of Technical Solutions of Hydatid Disease Prevalence,Gansu Province; in every county(city) of Gannan Prefecture,administrative villages were selected as survey units,by stratified cluster sampling based on the semi agricultural semi pastoral areas,pastoral areas,agricultural areas and towns.According to the proportion of the population of each layer to the population of each county,the number of people and the number of villages to be surveyed were determined.Sixteen villages were selected,and 200 people were selected in each village(from a neighboring village to make up the insufficient number in case of need),and B ultrasound method was used to carry out the census.②Children serum survey:stratification was done according to pastoral areas,semi agricultural semi pastoral areas,agricultural areas and towns population in each county(city),1 primary school was selected,children less than 12 years old were examined by B Ultrasound,and serum antibodies of Echinococcus granulosus were tested by enzyme-linked immunosorbent assay(ELISA).③ Surveillance of source of infection:in the selected villages,20 kennel households were selected in each village,dog feces was collected,and canine Echinococcus antigen was detected by double antibody sandwich ELISA method.④)Monitoring of intermediate host:1 000 sheep (or 500 cattle) were selected in each county(city),and hydatid disease was examined by anatomical method.⑤Investigation of health education was done in the form of a questionnaire survey of hydatid disease prevention knowledge and behavior survey on the awareness rate.Results The prevalence rate of population was 0.10% (29/28 960); prevalence rate of herdsmen was 0.13% (17/13 015); farmers prevalence rate was 0.06%(8/12 780); in children under 12 years of age,the infection rate was 2.64% (336/12 728) ; dog infection rate were 3.90% (117/3 001) ; livestock infection rate was 1.72% (121/7 027); and the infection rates of cattle and sheep were 2.41%(38/1 574) and 1.52%(83/5 453),respectively.The rate increased from 40%(720/1 800) to 70%(1 260/1 800) of hydatid disease prevention knowledge awareness in farmers and herdsmen after health education.Conclusions The scope of hydatid disease in Gannan Prefecture is broad; the patient population mainly is herdsmen,and farmer is second.Epidemic factors include contact canine and vices; the farmers and herdsmen have low awareness of hydatid disease prevention knowledge.Poor production and lifestyle are difficult to change in the short term,we need to further strengthen the prevention measures.
6.Comparison of Luminex vs.ELISA method to detect HLA antibodies in renal transplantation

Objective To compare Luminex vs.ELISA methods in detecting HLA antibodies in kidney transplant recipients and their relation to acute rejection.Method Blood samples from 34 kidney transplant recipients were collected and the HLA antibodies were detected by both Luminex and ELISA methods.The sensitivity and specificity of both methods for predicting the development of acute rejection were analyzed.Results Fourteen recipients (14/34,41.17%) positive for HLA class Ⅰ antibodies were detected by using Luminex method,whereas only 1 case (1/34,2.9%) was detected with positive HLA class Ⅰ antibodies by ELISA method (P<0.05).Similarly,13 recipients (13/34,38.24%) positive for HLA class Ⅱ antibodies were detected by using Luminex method,whereas the positive rate of HLA class Ⅱ antibodies by using ELISA method was 8.8% (3/34,P<0.05).The sensitivity and specificity of Luminex method for predicting the acute rejection were 80% and 92.3% respectively,in comparison to 30% and 77.4% respectively by ELISA method.Conclusion Compared to the traditional ELISA-based method,Luminex method has a better sensitivity and specificity for predicting the development of acute rejection.
7.Application of single antigen antibody test in living-relative renal transplantation

Objective To analyze the clinical application of donor specific antibodies (DSAs) detected by a single antigen Luminex virtual crossmatch,and to discuss the treatment of DSA and the impact of DSA on renal function.Methods Serum from living-relative renal recipients before and after transplantation was investigated using a Luminex single antigen assay.The relation between DSA and renal acute rejection as well as renal function was analyzed.Results A total of 30 patients and 173 serum samples were tested,including 47 serum samples before transplantation,and 126 after transplantation.DSA was positive in one patient before transplantation,and 8 patients after transplantation.Three of the patients positive for DSA were treated by Bortezomib,3 by addition of MMF,2 by addition of CNI,1 by addition of Sirolimus.The MFI of DSA in one of the patients treated by Bortezomib was decreased to below 1000,while that in the other two decreased by more than 50 %.The renal eGFR at the time with and without DSA was (1.50 ± 0.59) and (1.23 ± 0.38)ml/s respectively (P<0.05).Conclusion Dynamic monitoring of single bead antigen antibody DSA conduces to direct the adjustment of immunosuppressant.The appearance of DSA contributes to the declination of renal function.Application of Bortezomib decreased the MFI of DSA.
8.Relationship between amount of HBV DNA in serum/liver tissue and hepatitis G virus (HGV) infection in patients with chronic hepatitis B.

OBJECTIVETo observe the relationship between the amount of HBV DNA in serum/liver tissue and HGV infection in patients with chronic hepatitis B (CH-B) for exploring the effect of HGV infection on hepatitis B virus (HBV) replication of CH-B.
METHODSHGV RNA in serum, HGV nonstructural region 5 (NS5) antigen (HGV Ag) in liver tissue and the amount of HBV DNA in serum, liver tissue were detected for 56 patients with CH-B by reverse transcription-polymerase chain reaction (RT-PCR) assay, peroxidase antiperoxidase (PAP) immunohistochemical method and fluorescence quantitative PCR assay, respectively. Then the relationship between HGV Ag expression in liver tissue and HGV RNA expression in serum was analysed and the amount of HBV DNA in serum and liver tissues from the serum HGV RNA or liver tissue HGV Ag positive patients were compared with those of the serum HGV-RNA or liver tissue HGV Ag negative patients, respectively.
RESULTSTen (17.9%) and eight (14.3%) patients were positive for serum and liver tissues,respectively.HGV RNA expression in serum was closely related to HGV Ag expression in liver tissues, but there was HGV RNA in serum from some of the liver tissues HGV Ag negative patients ?cases of HGV RNA and HGV Ag positive or negative,HGV RNA positive but HGV Ag negative, HGV RNA negative but HGV Ag positive, respectively: 5,43,5,3,(P<0.01). There was no significant difference in the amount of HBV DNA in serum and liver tissues between HGV RNA or HGV Ag positive and negative patients (P>0.05).
CONCLUSIONSHGV infection may not affect HBV replication. Liver is the site of HGV replication, but HGV probably also replicates in extrahepatic tissues. HGV hepatic pathogenicity is probably mild and further studies are still needed.

9.Risk Factors Analysis for Prevalence of Acute Myocardial Infarction in Young and Middle-aged Population

Objective: To study the risk factors for prevalence of acute myocardial infarction (AMI) in young and middle-aged population. Methods: A prospective cohort study was conducted in 110100 subjects at the age of (18-98) years who received physical examination in Kailuan Group from 2012-06 to 2014-10. Based on the limitations of male≤53 years and female≤63 years, a total of 62367 subjects were enrolled in our study. The subjects were followed-up for 2 years by the end point event of AMI to analyze the risk factors ofAMI occurrence. Results: According to AMI occurrence at the follow-up period, the subjects were divided into 2 groups: AMI group, n=56 and Control group, n=62152. Compared with Control group, AMI group had increased BMI, SBP, DBP and elevated blood levels of LDL-C, TG; AMI group also showed the higher ratios of subjects with the history of diabetes and taking anti-hypertension medication. Cox proportional hazard regression analysis indicated that age (RR=1.37), male (RR=60.54), LDL-C (RR=1.12), and TG (RR=5.93) were the risk factors forAMI occurrence in young and middle-aged population, allP<0.05. Conclusion: Age, male gender, blood levels of LDL-C, and TG were the risk factors for AMI occurrence in young and middle-aged population.
10.A Nomogram for Predicting the Infectious Disease-specific Health Literacy of Older Adults in China

Purpose:
To identify the predictors of infectious disease-specific health literacy (IDSHL), and establish an easy-to-apply nomogram to predict the IDSHL of older adults.
Methods:
This cross-sectional study included 380 older adults who completed the IDSHL, self-rated health, socio-demographic and other questionnaires. Logistic regression was used to identify the IDSHL predictors. Nomogram was used to construct a predictive model.
Results:
Up to 70.1% of older adults had limited IDSHL. Age, education, place of residence, self-rated health, and Internet access were the important influencing factors of IDSHL. The established nomogram model showed high accuracy (receiver operating characteristic curve: 0.848).
Conclusions
The IDSHL of Chinese older adults was significantly deficient. The constructed nomogram is an intuitive tool for IDSHL prediction that can not only contribute toward rapid screening of high-risk older adults with limited IDSHL but also provide guidance for healthcare providers to develop prevention strategies for infectious diseases.
