1.Application of root cause analysis on the Swiss cheese model in the psychiatric nursing adverse events

Objective To study the clinical methods and effects of root cause analysis on the application of the Swiss cheese model in the psychiatric nursing adverse events. Methods Safety management groups were established to train nurses, use Swiss cheese model to analysis the root cause,formulate the corrective measures. The annual rates of incidence and reporting rate of adverse events were compared. Results One year after the root cause analysis on the application of the Swiss cheese model,the annual number of patients increased from 3 420 to 3 730,the cases of nursing adverse events reduced from 141 to 120,and the rate of nursing adverse events dropped from 4.12 percents to 3.21 percents (X2=4.16,P<0.05). The reporting cases of nursing adverse events increased from 65 to 102,and the reporting rate of nursing adverse events enhanced from 46.10 percents to 85.00 percents ( X2=42.57,P<0.01). Conclusions The use of root cause analysis on the application of the Swiss cheese model is the effective management model to analyze the nursing adverse events and to improve the nursing safety.
2.Impact of C-reactive protein, albumin and bilirubin on elderly patients with acute suppurative cholecystitis with somatostatin

Objective To observe the efficacy of somatostatin on elderly patients with acute suppurative cholecystitis and its impact on the C-reactive protein (CRP), prealbumin (PA) and total bilirubin (TBIL) effects before or after operation. Method 110 elderly patients with acute suppurative cholecystitis, which from January 2011 to June 2013, were randomly divided into observation group and control group, each group had 55 cases, the control group were gave operation, the observation group were treated with somatostatin on basis of control group before and after operation. The operative time, blood loss, postoperative visual analog scale (VAS) score and morbidity were observed. At the same time, the levels of CRP, PA and TBIL were detected before and after operation. Results After treatment, the blood loss, postoperative hospital stay and postoperative complication rate in observation group were significantly lower than those in control group (P<0.01), while the differences in operative time and postoperative VAS score between two groups were not significant . After treatment, the levels of CRP, and TBIL appear significantly lower than those before treatment (P<0.01), while the PA levels appear significantly higher compared with those before treatment (P<0.01), the decreasing or increasing levels in observation group were more obvious compared with control group (P<0.01). Conclusion The adjuvant therapy efficacy of somatostatin were obvious before and after operation, they can significantly reduce the body's inflammatory response, and promote the postoperative recovery.
3.Apoptosis of human leukemia K562 cells induced by snake venom of Agkistrodon Halys Pallas in Zhejiang Province

Objective To study the apoptosis of human leukemia K562 cells induced by snake venom of Agkistrodon Halys Pallas in Zhejiang Province.Methods IC_ 50 value and cytotoxity of K562 cell were detected by MTT method.Apoptotic cells were dyed by Hoechest 33258.Sub-G1 peak and cell cycle were detected by FCM.Protein expression of Bcl-2 gene was detected by FCM and western-blot method.Results The snake venom of Agkistrodon Halys Pallas in Zhejiang Province inhibited the growth of K562 cells,which appeared dose-dependent.The snake venom induced apoptosis of K562 cells.Meanwhile,protein expression of Bcl-2 was down-regulated.Conclusion Snake venom of Agkistrodon Halys Pallas in Zhejiang could induce apoptosis of human leukemia K562 cells.The mechanism may be related to down-regulation of Bcl-2 gene expression.

Objective To study the mRNA expression of Toll-like receptor (TLR) 2 and 4 in human urinary tract epithelium. Methods The epithelial cells of nephric tubule, pelvis, ureter and bladder were cultured in vitro, and their mRNA expressions of TLR2 and TLR4 were measured by RT-PCR. Results The mRNA expression of TLR2 in nephric tubule, pelvis, ureter and bladder was 0.326, 0.589, 0.602 and 0.735, and that of TLR4 was 0.223, 0.327, 0.379, 0.365, respectively. Objective:To understand the relationship between psychiatric nurses' communication ability, personality characteristics and conflict management patterns, and to provide a basis for training psychiatric nurses' conflict management ability courses.Methods:A convenient sampling method was used to investigate and investigate 208 nurses in the first-line clinical hospitals of a third-class psychiatric hospital, used the nurses′ clinical communication ability scale, the nurse′s interpersonal conflict management morphology scale, and the simplified version of the Chinese Big Five Personality Questionnaire.Results:Integration dimension (18.67±2.58) points, concession dimension (12.93±2.43) points, escape dimension (14.05±2.02) points, dominance dimension (14.49±3.06) points, and integration dimension and gender ( t value was -2.57, P=0.01)), education ( F value was 3.76, P=0.02), technical title ( F value was 3.03, P=0.02), position ( t value was 2.08, P=0.03), whether to participate in nurse-patient communication training ( t value was 2.54, P=0.02). Nurses with pleasant personality could better deal with interpersonal conflict ( r=0.361, P<0.01), and the nurse′s personality had a direct effect on the form of interpersonal conflict ( P=0.036, 95% CI 0.107-0.422), the nurse′s communication ability plays a part of indirect effect ( P=0.031, 95% CI 0.001-0.063). Conclusion:The form of interpersonal conflict management of psychiatric nurses was closely related to communication ability and personality characteristics. Nursing managers could carry out targeted training to improve the conflict management ability of nursing staff.
8.The relationship between coagulation-coagulation suppression system disorders and portal vein thrombosis in portal hypertensive patients

Objective To explore the correlation between coagulation and coagulation suppresion system disorders of portal vein thrombosis in patients of portal hypertension undergoing splenectomy.Methods Clinical data of 33 patients with postoperative portal vein thrombosis were enrolled.The clotting and coagulation inhibitor in portal vein blood and peripheral blood was detected and analyzed.Results The Hb,APTT,FIB,factor Ⅶ,protein C,AT-Ⅲ,CD62P of portal vein blood and peripheral blood before the surgery and on postoperative day 1,day 7,day 14 were no significant difference (P > 0.05).The WBC,PLT,PT,D-Dimer of in portal vein blood before surgery were (2.9 ± 1.4) × 109/L,(37.5 ± 20.7) × 109/L,(16.1 ± 2.9) seconds,(0.7 ± 0.3) μg/ml,which were significantly different from those on postop day 1 (13.7 ±4.4) × 109/L,(86.3 ±34.6) × 109/L,(6.9 ±5.7) seconds,(16.1 ±2.9) μg/ml; day 7 (10.7 ±4.3) × 109/L,(312.4 ±137.2) × 109/L,(14.4 ±2.9) seconds,(7.6 ±4.4) μg/ml and day 14 (7.7 ± 3.3) × 109/L,(486.3 ± 216.7) × 109/L,(14.4 ± 2.9) seconds,(5.5 ± 4.4) μg/ml (P < 0.05).WBC,PLT,PT,D-Dimer in preop peripheral blood were (2.4 ±0.8) × 109/L,(44.4 ± 25.8) × 109/L,(16.3 ± 3.0) seconds,(0.6 ± 0.4) μg/ml,which were significantly different from those on postop day 1 (13.7 ± 5.7) × 109/L,(75.1 ± 29.3) × 109/L,(13.7 ± 2.6) seconds,(6.8 ± 5.3) μg/ml; day 7 (10.6 ± 4.8) × 109/L,(337.9 ± 141.3) × 109/L,(14.0 ± 2.1) seconds,(7.6 ± 5.5) μg/ml and day 14 (7.8 ±3.9) × 109/L,(504.9 ±237.4) × 109/L,(14.0 ±2.1) seconds,(5.4 ±4.9) μg/ml postoperative (P < 0.05).Conclusions The cause of postsplenectomy portal vein thrombosis is multifactorial.The dysfunction of coagulation-coagulation suppression system was just one of the conditions conducive to portal vein thrombosis after splenectomy.
9.INHIBITION OF Na+/Ca2+ EXCHANGE BY TETRAPEPTIDE FMRFa IN INTACT RAT VENTRICULAR MYOCYTES

AIM To study the effects of Phe-Met-Arg-Phe-NH2 (FMRFa) on Na+/Ca2+ exchange and its specificity for Na+/Ca2+ exchange in rat ventricular myocytes. METHODS Na+/Ca2+ exchange current and other currents of ion channels were measured using whole cell voltage clamp techniques. RESULTS A dose-related inhibition of tetrapeptide FMRFa on Na+/Ca2+ exchange was observed in rat ventricular myocytes. Inward and outward INa+/Ca2+ were inhibited by 60.1% and 56.5%, respectively, at highest concentration (100 μmol*L-1) and its IC50 were 20 μmol*L-1 and 34 μmol*L-1 in inward and outward INa+/Ca2+, respectively. Inward and outward INa+/Ca2+ were inhibited 38.7% and 34.9%, respectively, at FMRFa 5 μmol*L-1. FMRFa 5 μmol*L-1 and 20 μmol*L-1 did not affect L-type calcium current, sodium current, transient outward current and inward rectifier potassium current. CONCLUSION These data indicate that FMRFa is a specific inhibitor of Na+/Ca2+ exchange in intact rat ventricular myocytes.
10.Holistic nursing research in China: a systematic review

Objective To systematically review the domestic studies on holistic nursing,so as to provide suggestions for evidence-based strategic decision about how to set upa long-term mechanism of holistic nursing.Methods The databases of China Biology Medicine (CMB),VIP,China National Knowledge Infrastructure (CNKI) and Wan Fang Data were searched to identify the relevant studies.Literatures were screened with data extraction in accordance with the predefined inclusive and exclusive criteria.The quality of literatures was assessed with the standard of Cochrane Handbook 5.0 and Crombie criteria,and the data were comprehensively analyzed with qualitative research methods.Results The total 33 cross-sectional studies were included.The results showed that the cognition of holistic nursing was good;10 literatures from the scope of business,organizational forms,strengthening the management and establish a system expounded how to implement the holistic nursing;Since our country carry out holistic nursing,effectwas significant,but there were still some problems.Discussing and solvingproblemsby using the method of evidence-basedwasthe future development direction.Conclusions The current researches about holistic nursing are lack of randomized controlled trials,most of which are the cross-sectional survey research.But the research content is wide.analyzing the present status and effect of holistic nursing,summarizing the experience and problems,promoting our country to set up a long-term mechanism of holistic nursing have the certain enlightenment function.
