Objective:To elucidate the signal transduction mechanism of the modulation of immune cells by estrogen,the effect of 17?-estradiol(E2)on the production of nitric oxide (NO)from spleen mononuclear cells(MNC)of rats and the expression of inductible NO synthase(iNOS) in estrogen receptor(ER) knockout mice were examined.Methods:Culture of the spleen MNC from normal,Freund's incomplete adjuvant(FIA)or myelin basic protein(MBP) immunized Lewis rats.Detection NO production in the supernatant of culture was determined by Griess reagent.The expression of iNOS in spleen sections were observed by immunofluorescence staining.Results:①E2 dose-dependently induce NO production in spleen MNC from normal rats,MNC from FIA or MBP immunized rats manifest in the same manner.iNOS INHIBITOR l-NAME or MAPK blocker could suppress the induction.②In vivo E2 enhanced iNOS expression in wild type mice(WT),but had no effect in ER ? knockout mice(EPKO).ER? knockout mice(BERKO)appear the same with wt.Conclusion:E2 could modulate the immune system via induction of NO,which involves both genomic and nongenomic mechanisms.

3.The effect of short-term intensive insulin therapy on insulin secretion of different phases in newly diagnosed type 2 diabetics

Objective Hyperglycemic clamp technique (HGCT) was performed to evaluate the effect of short-term intensive insulin therapy on the first and second-phase (1PH and 2PH) insulin secretion and maximum insulin secretion (MIS) in newly diagnosed type 2 diabetics. Methods Twelve volunteers with normal glucose tolerance (NC group) and six newly diagnosed type 2 diabetics (DM group) were included and HGCT was performed to assess the function of pancreatic islet beta cell. Then HGCT was repeated in the 6 patients following two week intensive insulin therapy. Results The levels of secreted insulin in 1PH, 2PH and MIS were 257?36 mU/L, 63?5 mU/L and 80?5 mU/L in NC group respectively, and 95?19 mU/L, 34?9 mU/L and 39?12 mU/L in DM group respectively. 1PH insulin secretion was significantly improved in the diabetics following 2 week treatment compared with that before the treatment (135?27 mU/L vs 95?19 mU/L, P=0.01). The insulin secretions in 2PH and MIS were slightly increased (40?9 mU/L vs 34?9 mU/L, P=0.09, 46?11 mU/L vs 39?12 mU/L,P=0.08, respectively). Conclusions Short-term intensive insulin therapy can improve the insulin secretions significantly in 1PH and slightly in 2PH and MIS in newly diagnosed type 2 diabetics.
4.Effect of insulin therapy on serum level of insulin-like growth factor-I

To explore the effect of insulin therapy on serum level of insulin-like growth factor-I(IGF-I)in patients with type 2 diabetes mellitus.The results showed that serum IGF-I level increased[(126.70±51.91 vs 90.04±43.68)μg/L,P<0.01]and was positively correlated with insulin level in patients with type 2 diabetes mellitus after exogenous insulin therapy(r=0.298,P<0.05).
5.Association between ankylosing spondylitis and HLA-B27 gene in Jiangxi province

OBJECTIVE:To study the relevance between HLA-B27 gene and ankylosing spondylitis in Jiangxi province,and to discuss the diagnostic significance to ankylosing spondylitis.METHODS:A total of 1246 patients with suspected ankylosing spondylitis and related diseases were collected from the First Affiliated Hospital of Nanchang University from October 2007 to December 2008.Among all cases,323 were diagnosed as ankylosing spondylitis,and other 923 were rheumatoid arthdtis or related diseases.A total of 135 healthy subjects were collected as control group.Sequence-specific primer-polymerase chain method(PCR-SSP)was used to detect HLA-B27 gene in 1246patients and 135 healthy controls.The distribution of HLA-B27 gene,correlation between HLA-B27 gene and age,and relation between ankylosing spondylitis and HLA-B27 gene were studied.RESULTS:Total positive rate of HLA-B27 gene was 32.5%in 1246 patients with suspected ankylosing spondylitis.3.7%in 135healthy controls.and 91.6%in 323 patients with ankylosing spondylitis,respectively.The positive rate and incidence rates of male were significantly higher than female(P<0.01)and mainly concentrated in the age of adolescence.The sensitivity of HLA-B27 genewas 91.64%and the specificity was 88.19%in patients with ankylosing spondylitis.CONCLUSION:HLA-B27 gene was highly correlated with ankylosing spondylitis in Jiangxi province,which was the same as other regions,suggesting that HLA-B27 gene is significance for diagnosis of ankylosing spondylitis in an eedy phase.
6.Distribution and risk factors of cerebral artery stenosis in patients with acute ischemic stroke

Objective To investigate the distribution characteristics of cerebral artery stenosis and its risk factors in patients with acute ischemic stroke.Methods The patients with acute ischemic stroke examined with MRI and magnetic resonance angiography (MRA) were divided into either a stenosis group or a non-stenosis group according to whether they had cerebral artery stenosis or not.The patients in the stenosis group were redivided into a simple intracranial stenosis,simple extracranial stenosis,and intracranial + extracranial stenosis subgroups according to their stenotic sites; they were redivided into either a young and middle-aged subgroup (<60) or an elderly subgroup (≥60) according to their age; they were redivided into either a single-branch lesion subgroup or multibranch lesion subgroup according to the number of vascular stenosis.The distribution characteristics and influencing factors of cerebral artery stenosis were analyzed.Results A total of 232 patients with acute ischemic stroke were enrolled,and 114 of them (62.0%) were simple intracranial stenosis,30 (16.3%) were simple extracranial stenosis,and 40 (21.7%) were intracranial+ extracranial stenosis.The patients with anterior circulation stenosis (76.6%) were more common than those with posterior circulation stenosis (33.7%).They were mainly in the middle cerebral artery (64.4%) and posterior cerebral artery (53.8%) respectively.Multivariate logistic regression analysis showed that age (odds ratio [OR] 1.049,95% confidence interval [CI] 1.015-1.084; P =0.005),hypertension (OR 10.063,95% CI 4.402-23.004; P < 0.001),diabetes (OR 3.873,95% CI 1.141-13.147; P =0.030),smoking (OR 3.311,95 % CI 1.112-9.855; P =0.031),and fibrinogen (OR 6.085,95% CI 1.396-26.533; P=0.016) were the independent risk factors for cerebral artery stenosis in patients with acute ischemic stroke; hypertension (OR 10.779,95% CI 4.468-26.007; P< 0.001),diabetes (OR 3.593,95% CII.018-12.685; P =0.047),and smoking (OR 4.408,95% CI 1.403-13.826; P =0.011) were the independent risk factors for simple intracranial artery stenosis; hypertension (OR 6.143,95% CI 1.838-20.537; P=0.003),diabetes (OR 8.179,95% CI 1.844-36.287; P=0.006),and fibrinogen (OR 2.410,95% CI 1.046-5.551; P =0.039) were the independent risk factors for simple extracranlal artery stenosis.C reactive protein (CRP) level of the intracranial + extracranial stenosis group was significantly higher than that of the simple intracranial stenosis (P=0.001) and simple extracranial stenosis (P =0.018) groups.There was no significant difference between the two groups,but the mean level of the 3 groups was higher than that of the normal value.The simple intracranial stenosis and the simple extracranial stenosis were most common in the young and middle-aged group,and the simple intracranial stenosis and the intracranial + extracranial stenosis were more common in the elderly group.The age (P=0.036) and uric acid level (P=0.006) in the subgroup of multiple branches stenosis were significantly higher than those in the subgroup of single branch stenosis,but only age (OR 1.030,95% CI 1.003-1.057; P =0.028) was significantly independent correlated with the multiple branches stenosis.Conclusions Intracranial artery stenosis is common in cerebral artery stenosis of patients with acute ischemic stroke.The proportion of intracranial + extracranlal stenosis increases sfightly with age.Age,hypertension,diabetes,smoking,and fibrinogen are the independent risk factors for cerebral artery stenosis in patients with acute ischemic stroke.Hypertension and diabetes are the common independent risk factors for simple intracranial and extracranial artery stenosis in patients with acute ischemic stroke.Smoking is an independent risk factor for simple intracranial artery stenosis in patients with acute ischemic stroke.Fibrinogen is a simple independent risk factor for extracranial artery stenosis in patients with acute ischemic stroke.CRP and uric acid may be the inflammatory predictive factors associated with the patients with acute ischemic stroke and cerebral artery stenosis.
7.Position nursing of patients with ankylosing spondylitis and severe kyphosis during operation

Objective To sum up the experience of position nursing of patients with ankylosing spondylitis and severe kyphosis. Methods Eleven patients with ankylosing spondylitis and severe kyphosis underwent surgery. The patients′spine and head were kept in a line during anesthesia to avoid spinal cord injury. The eye mask was used to make sure the eyelid close, so the bulbar conjunctiva and corneal did become dry. The angle of operation bed in time during operation was adjusted. The crushed area was observed and the position was changed to avoid pressure sores and edema. After surgery, they were keep inactive to make sure the spine stable and relieve the pressure. Results The 11 patients′surgery were successful. There were no pressure ulcer, fracture, spinal cord injury or eye injury occurs. The operation time was 9.3 ± 2.9 h. Conclusion Position nursing is very important to improve the success rate of surgery and reduce complications.
8.The inhibitory effect of tamoxifen on the proliferation of the anterior pituitary cell of rats

AIM To investigate the effect of tamoxifen on the proliferation of the anterior pituitary cell of rats and its mechanism. METHODS Primary culture of the anterior pituitary cell of rats and 3H TdR incorporation method were applied. The changes of cell morphology were observed directly by electric microscope. RESULTS Tamoxifen could inhibit the proliferation of the anterior pituitary cell of rats. The inhibitory effect of tamoxifen (0 1 ?mol?L -1 ) could be reversed by estrogen.The classical apoptotic changes appeared in the cells after tamoxifen incubation for 48 h. CONCLUSION Tamoxifen can inhibit the proliferation of the anterior pituitary cell of rats and resultin the cell apoptosis.
9.Polymorphisms of TNF-αgene and HSP70-2 gene in patients with acute pancreatitis

Objective To investigate the association of tumor necrosis factor (TNF)-α, heat shock protein (HSP)70-2 gene polymorphisms and susceptibility of acute pancreatitis(AP). Methods Using case-control method,The gene polymor?phism of TNF-α and HSP70-2 was detected by PCR-RLFP in 72 patients with AP and 71 healthy controls. Results There were no significant differences in proportion of TNF-αgenotype and alleles between AP and control groups (P>0.05). There were no significant differences in TNF-αgenotype and alleles between severe acute pancreatitis (SAP) and light acute pancreatitis (MAP) of AP group (P>0.05). There were no significant differences in white blood cell count, C-reactive pro?tein (CRP), amylase, three acyl glycerin and glucose between TNF-a and HSP70-2 gene of AA type and GA+GG type pa?tients (P>0.05). The HSP70-2 genotype GA+GG proportion was significantly higher in AP group than that of control group (69.4%vs 49.3%). The ratio of patients with G allele was significantly higher in AP group than that of control group(46.5%vs 31.7%). The ratio of patients with GA+GG type AP was significantly higher in SAP patients than that of MAP patients of AP group(81.0% vs 53.3%). There was no significant difference in G allele between SAP and MAP patients (P>0.05). Conclusion TNF-α polymorphisms is not associated with acute pancreatitis. There is an association between HSP70-2 polymorphisms and acute pancreatitis. Carrying the G allele increases the possibility of a severe acute pancreatitis ,which is one of the genetic susceptibility factors of severe acute pancreatitis.
10.CHOP expression and its correlation with proliferative/apoptotic ratio in colorectal adenoma-carcinoma sequence under same genetic background

AIM:To investigate the expression of C/EBP homologous protein (CHOP) and its correlation with proliferative/apoptotic ratio (PAR) in colorectal adenoma-carcinoma sequence under the same genetic background .ME-THODS:Four kinds of tissue samples under the same genetic background from 23 patients, including normal colorectal tissue, adenoma with low-grade intraepithelial neoplasia , adenoma with high-grade intraepithelial neoplasia and colorectal adenocarcinoma samples , were collected .TUNEL method and Ki-67 immunohistochemistry were applied to determine the PAR.The expression of CHOP was detected by immunohistochemistry SABC method .RESULTS: ( 1 ) Under the same genetic background , the level of CHOP expression is significantly higher in colorectal adenocarcinoma than that in the ade -noma with high-grade intraepithelial neoplasia , the adenoma with low-grade intraepithelial neoplasia and the normal muco-sa.The level of CHOP expression was significantly higher in the adenoma with high -grade intraepithelial neoplasia than that in the adenoma with low-grade intraepithelial neoplasia and the normal mucosa .The level of CHOP expression was signifi-cantly higher in the adenoma with low-grade intraepithelial neoplasia than that in normal mucosa .(2) Under the same ge-netic background , PAR was significantly higher in the colorectal adenocarcinoma than that in the adenoma with high -grade intraepithelial neoplasia , the adenoma with low-grade intraepithelial neoplasia and the normal mucosa .PAR was significant-ly higher in the adenoma with high-grade intraepithelial neoplasia than that in the adenoma with low-grade intraepithelial neoplasia and the normal mucosa .PAR was significantly higher in the adenoma with low-grade intraepithelial neoplasia than that in the normal mucosa.(3) CHOP levels were positively correlated with PAR in the adenoma with low-grade intraepi-thelial neoplasia , adenoma with high-grade intraepithelial neoplasia and colorectal adenocarcinoma .CONCLUSION:CHOP expression and PAR continuously increased and positively correlated along the adenoma -carcinoma sequence , indica-ting that endoplasmic reticulum stress mediates the carcinogenesis of colorectal adenomas .
