Objective To evaluate the clinical efficacy of cattle encephalon glycoside and ignotin with salviae miltiorrhizae and ligustrazine hydrochloride injection for acute cerebral infarction.Methods 50 patients with acute cerebral infarction from July 2012 to October 2013 admitted in our hospital were randomly divided into observation group and control group.Observation group had 25 patients,they were treated with cattle encephalon glycoside and ignotin with salviae miltiorrhizae and ligustrazine hydrochloride injection.Control group had 25 patients,they were treated with only salviae miltiorrhizae and ligustrazine hydrochloride injection.After two groups'treatment,the clinical efficacy,indicators of hemorheology (include whole blood viscosity,platelet aggregation and plasma viscosity),neurological deficiency score and activity of daily life were observed and compared.Results There was no significant difference in the clinical data between two group.After treatment,the total efficiency rate in observation group and control group were 92.00% and 60.00%,separately,the difference between two group was significant (u=2.4129,P=0.0158);the whole blood viscosity (high cut, low cut)of observation group were shorter than control group(P<0.05 );platelet aggregation of observation group were shorter than control group(t=0.5632,P=0.5759);plasma viscosity of observation group were shorter than control group(t=3.7682,P=0.0004);the neurological deficiency score and activity of daily life of observation group were shorter than control group(t =3.7682,P=0.0004).Conclusion The clinical efficacy of cattle encephalon glycoside and ignotin with salviae miltiorrhizae and ligustrazine hydrochloride injection for acute cerebral infarction is well.

From 1977 to 1990, 50 patients with renal cell carcinoma were given postoperative radiotherapy. The 3-,5- and 10-year survival rates were 60.0%,51.5% and 31.8%. The 5-year survival rate of StageⅠ～Ⅱ lesions was 69.2% and that of stageⅢ～Ⅳ lesions was 43.8%, which are better than those treated by surgery alone. Regional lymphatic metastasis, invasion of the nearby organs and vena caval cancerous thrombosis were the ominous signs, especially the former two. 83.3% of the patients died of distant metastasis. Lung was most common(58.3%)and then, bone metastasis (25.6%). The authors believe newer combination therapy against distant metastasis is needed to improve the results.

[Method]The definition of the concept of the Yin of liver and the Yang of liver and the mainstream view of Yin and Yang of before the Song Dynasty, Song Dynasty and modern is investigated in this paper. The definition of the concept of the Yin of liver and the Yang of liver of different periods, different ideological background and different mainstream view of Yin and Yang is clarified by historical investigation. [Result]Because of the influence of Confucius studies, there is not the definition of the concept of the Yin of liver and the Yang of liver based on the object of liver in the TCM theory of Han and Tang dynasties. The Yin of liver and the Yang of liver mainly refer to the yin qi and yang qi in the liver. Because of the influence of neo-confucianism, the concept of the Yin of liver and the Yang of liver based on the object of liver was established in the TCM theory of Song and Ming dynasties. The Yin of liver and the Yang of liver mainly refer to the body and function of liver. Under the background of modernization of TCM, the concept of the Yin of liver and the Yang of liver based on the object of liver was established in the basic theories of TCM. However, there is a controversy of the concept of the Yin of liver and the Yang of liver, because of the effect of modern philosophy and science, ignoring the change of Chinese philosophy. [Conclusion]The clear concept of the Yin of liver and the Yang of liver of different periods, different ideological background and different mainstream views of Yin and Yang will reduce the influence of the fictional historical. Whether in ancient literature research, the liver like zang-fu theory of TCM theory, research, or the modern liver zang-fu theory of TCM, and establishing the theory of Chinese medicine has a positive meaning.

Objective To study clinical and histopathological features of high-risk small renal carcinoma and provide theoretical basis for the individualized treatment.Methods This retrospective study analyzed the clinical and histological data of 21 patients of high-risk small renal cell carcinomas which were highly aggressive or advanced stage, admitted from Jan 2000 to July 2015.There were 15 males and 6 females,and the average age was (61.5 ±7.1) years (ranged 27 to 75 years).The mean diameter of tumors was 3.3cm (ranged 2.0 to 4cm).The study mainly focused on pathological character and stage, Fuhrman grading, existence of perirenal tissue or vascular invasion, venous embolus and distant metastasis.Results Of all these 21 cases, 17 cases were clear cell carcinomas, 1 was papillary carcinoma, 2 were hybrid cellular tumor ( malignant rhabdoid tumor and sarcomatoid carcinoma) and 1 was sarcomatoid carcinoma. Among the 17 clear cell carcinoma cases, 12 were Fuhrman grade Ⅱ, 5 were grade Ⅲ and one was gradeⅣ.Tumor infiltrated renal vein or its branches and renal vein embolus were found in 7 cases.The tumor infiltrated perirenal or renal sinus fat were found in 11 cases.Synchronous lung and local lymph nodes metastasis were found in 3 patients respectively.Moreover, there was 1 case with metastasis to the brain (FuhrmanⅣ).The final clinical stage were T3a N0M0 in 14,T1a N1M0 in 3 and T1a N0M1 in 4 cases. Conclusions Small renal tumors are heterogeneous in its biological behavior and the minorities are aggressive with infringement of perirenal fat or simultaneous local lymph node or distant metastasis.When the tumor is greater than 3.0 cm in diameter and with high grade in Fuhrman classification, sarcomatoid carcinoma are more likely to be highly aggressive and advanced stage.

Objective To explore the significance of complement split product C4d in monitoring chronic rejection of renal allografts in rats.Methods Healthy closed population rats were used as donors and SD rats as recipients. The Wistar to SD model of graft rejection was developed. All the 42 recipients were randomly divided into 2 groups: group A receiving nothing except CsA (5mg?kg~ -1 ? d~ -1 ) in the first 10 days after operation, and group B receiving MMF (10 mg/kg) and CsA after operation. On the 3rd, 5th and 10th week, all the allografts were tested by light microscopy and immunofluorescence. Pathological changes of the kidneys and the expression of C4d in allograft tissue were observed.Results From the 3rd week, the rats in group A showed light pathological changes of chronic rejection and they became more and more obvious as time increased. Pathological changes occurred in group B at the 5th week and lighter than in group A. At the same time, C4d deposition in PTC was obviously observed on the 3rd week in group A, and on the 10th week C4d widespread deposition in allografts.Conclusion The deposition of complement split product C4d in allografts appears earlier than pathological change of chronic rejection, which can be regarded as a significant indicator to predict chronic rejection of renal allografts.

Objective To investigate the effect of rocuronium of on the function of acetylcholine receptor (AchR)/channel in cone neurons of hippocampal cortex in rats.Methods SD rats of both sexes (2-5 days old) were killed by decapitation. Brain was immediately removed and hippocampal cortex was cut into slices which were incubated in decalcified and magnesium-free artificial cerebro-spinal fluid (CSF) and mechanically dissociated and then digested in artificial CFS containing 2.5% trypsin. Cell suspension was prepared. The AchR/channel function was studied in the absence and presence of various concentrations of rocuronium (0, 0.05, 0.1, 0.2, 0.4mmol? L-1) and/or atropine 0.1 mmol ?L-1 using patch clamp technique.Results (1) The conductance, open probability (P0) and short term open constant (?01) were significantly higher in R-0.05 group than those in the control group (R-0) , but there was no significant difference in long term open constant (?02) , short term close constant( (?c1) and long term close constant(?c2) between the two groups. The conductance, P0 ,?01 ,?02 , ?c1, and ?c2 were significantly longer or higher in R-0.0, 0.2 and 0.4 groups than those in control group. (2) There was no significant difference in the conductance, P0 , ?01 , ?02 , ?c1 , and ?c2, between R-0.5 and control group but those in R-0.0, 0.2 and 0.4 groups were significantly higher or longer than control group, when rocuronium was used with atropine. Conclusions AchR/channel ( muscarinic and nicotinic ) in cone neuron of hippocampal cortex were excited by different concentrations of rocuronium. The results suggest that adverse effect may be induced on CNS by rocuronium in case it permeates the injured blood-brain barrier.

Objective To investigate the value of modified loop ileostomy in laparoscopic low and superlow radical resection of the rectal cancer with reservation of anus.Methods The clinical data of 108 patients who received laparoscopic low and super-low radical resection of rectal cancer with preservation of anus at the Subei Hospital of Jiangsu Province from January 2010 to June 2012 were retrospectively analyzed.Fifty-three patients who received preventive ileostomy were in the ileostomy group,and 55 patients who did not receive preventive ileostomy were in the non-ileostomy group.The operation,incidence of complication and follow-up of the 2 groups were analyzed.All data were analyzed using the t test,chi-square text or Fisher exact test.Results The operation time and blood loss were (185 ± 14)minutes and (31 ± 26)ml in the ileostomy group,and (154 ± 12)minutes and (28 ± 19)ml in the non-ileostomy group,with no significant difference between the 2 groups (t =2.34,1.82,P ＞ 0.05).The time to first flatus,time to liquid diet intake,time to pelvic drainage tube removal,duration of hospital stay,hospital costs were (1.1 ± 0.3) days,(1.1 ± 0.2) days,(5.2 ± 1.0) days,(7.2 ± 1.4) days,(3.0 ± 0.2) ×104 yuan in the ileostomy group,and (4.2 ± 0.6) days,(4.3 ± 0.6) days,(8.4 ± 3.9) days,(12.9 ±4.4)days,(3.8 ±0.7) × 104 yuan in the non-ileostomy group,with significant differences between the 2 groups (t =7.25,28.12,15.34,34.01,7.83,P ＜ 0.05).The incidences of postoperative complications and anastomotic fistula were 5.7％ (3/53) and 0 in the ileostomy group,and 21.8％ (12/55) and 12.7％ (7/55) in the non-ileostomy group,with significant differences between the 2 groups (P ＜ 0.05).Conclusion Modified loop ileostomy in laparoscopic low and super-low radical resection of rectal cancer can effectively reduce the incidence of anastomotic fistula,and helps the recovery of patients.
