Currently,the popular notions on the mechanisms of gut-derived-infection are described as follows.The stress may cause the damage of intestinal mucosal barrier and dysfunction of intestinal immune response,which leads to the alteration of intestinal flora,abnormal proliferation of opportunistic pathogen as well as the translocation of alive bacteria and their toxin into systemic compartment.As a result,the proinflammatory cytokines are released to induce the outbreak of intestinal inflammation or systemic inflammatory response syndrome,even the MODS.This review focuses on the relationship between biological behavior of intestinal bacteria and gut-derived-infection.
2.Analysis of the effect of three-dimensional reconstruction of mandible and its relevant influencing factors

Objective To analyze and evaluate the effect of three-dimensional reconstruction of mandible and its relevant influencing factors, in order to provide references for research and application based on three-dimensional reconstruction of mandible. Methods Layered anatomic information of a mandible specimen was collected by CT scanning and then three-dimensional reconstruction of the mandible was carried out by software. Anterior-posterior dimension of the ramus and the thickness of the corpus of both the three-dimensional reconstructed model and the specimen of the mandible were measured and compared to evaluate the effect of three-dimensional reconstruction. Results High resolution of three-dimensional reconstruction was gained by CT scanning. Statistical analysis showed that there had been no difference of the measurement between the three-dimensional reconstructed model and the specimen. Conclusion Three-dimensional reconstruction of the mandible based on CT scanning could get good reconstruction effect and could meet the need of relevant research and clinical application that based on three-dimensional reconstruction of the mandible.

4.Bonding between nickel-based alloys and resin-veneers: strength and mechanism

Objective:To determine the shear bond strength and analyze the bonding mechanism of home-made and imported nickel-based alloys to resin-veneers.Methods: Home-made and imported alloy disks were cast by lost-wax technique and embedded in acrylic resin fabricating cylinders.Sixteen specimens of each alloy were divided into 2 subgroups: thermocycling group and non-thermocycling group.Specimens in thermocycling groups were submitted to thermocycling(300 cycles) and those in non-thermocycling groups were just stored in water at 37?C for 24 h(0 cycles).Then the shear bond strength values were determined by universal testing machine.The highest shear bond strength values were recorded and metallurgical analysis was performed on the interface between alloys and resin-veneers.Results: The shear bond strength values of both alloys to resin-veneers were relatively high either before or after thermocycling.The shear bond strength values of home-made alloy+thermocycling group,imported alloy+thermocycling group,home-made alloy+non-thermocycling group and imported alloy + non-thermocycling group were(10.50?2.72),(13.12?2.82),(15.51?3.04) and(18.44?3.15) MPa,respectively.The bond strength was significantly different before and after thermocycling for both alloys(P
5.Percutaneous suture techniques in patients undergoing percutaneous endovascular aortic procedures

Objective To evaluate the application of percutaneous suture-mediated closure device (Perclose ProGlide) in patients undergoing percutaneous endovascular aortic procedures.Method From Apr 2011 to Jun 2012,23 patients underwent percutaneous endovascular aortic procedures.The stent delivery system used included one 24Fr,twelve 22Fr,two 20Fr,one 18Fr,two 16Fr,four 14Fr,one 10Fr and four 6Fr.The success rate,complications and the technical feasibility was analyzed.Result The success rate was 93.3% (28/30),in which two patients were converted to open surgery because of severe stenosis and bleeding of femoral artery.There was no hemorrhage,hematoma,false aneurysm,thrombosis,and serious artery stenosis during the follow-up period ( 8 ± 4 months).Conclusions Percutaneous suture techniques is safe and effective in patients undergoing percutaneous endovascular aortic procedures.Percutaneous suture techniques can be safely used to the branch of aortic arch.
6.Effects of modified Sijunzi decoction on the expression of p53 and Bcl-2 in burn-induced esophageal lesion

Objective To establish burn-induced esophageal lesion model by adding NaOH with different concentrations in rabbits, and investigate the effect of modified sijunzi decoction on the p53 and Bcl-2.Methods After injection with different concentrations of NaOH, esophagus was dissected and observed anatomically.Rabbits were given a gavage of modified Sijunzi decoction daily for 20 days, and then were injected with NaOH.Esophageal epithelium isolated from each group was observed by hematoxylin-eosin staining.p53 and Bcl-2 protein and mRNA expression was measured with western blot and qRT-PCR, respectively.Results The degree of corrosion of esophageal epithelium was positively correlated with the concentration of NaOH.p53 protein and mRNA levels were increased after NaOH challenge; this increase was inhibited by treatment with modified Sijunzi decoction.Additionally, NaOH decreased Bcl-2 protein and mRNA, which was attenuated by modified Sijunzi decoction.Conclusion Modified sijunzi decoction can relieve the esophageal alkali burning in a dose-dependent manner, suggesting that modified Sijunzi decoction may be a useful strategy to treat chemical injuries in esophageal tissue.
7.The causes and management of recurrence of Budd-Chiari syndrome after radical correction

Objective To analyse the recurrence rate of patients with Budd-Chiari syndrome (BCS) after radical correction.Method We retrospectively analyzed the clinical characters and follow-up of patients who underwent radical correction and suffered recurrence.Patency rate of inferior vena cava (IVC) and hepatic veins (HVs) were examined.We present the causes of recurrence and clarify risk factors for recurrence by survival analysis.Result Among the 102 patients undergoing radical correction,34 patients suffered from postoperative recurrence,including 32 cases of IVC lesions,21 cases of HVs lesions (19 patients suffered concurrently from IVC and HVs lesions).One patient received reoperation,16 patients received balloon angioplasty of IVC or HVs,3 patients received stent implantation,and 14 patients received conservative treatment.Conclusions The recurrence rate is high after radical correction for BCS.Once the abnormal syndrome recurs,the patients can be managed by radical correction,balloon or stent angioplasty.The main causes of recurrence are thrombosis of IVC,compression of caudate lobe,scar contracture,and the risk factors for recurrence are concurrent hypercoagulation status,too short period of postoperative anticoagulation (less than 6 months).
8.Early diagnosis values of C-reactive protein and procalcitonin for children with bacterial infectious pneumonia

Objective To explore the early diagnosis value of C‐reactive protein (CRP) and procalcitonin (PCT ) for bacterial in‐fectious pneumonia of children .Methods A total of 101 children with bacterial infectious pneumonia ,64 children with non‐bacterial infectious pneumonia and 73 children with non‐infectious disease were selected in bacterial infectious pneumonia group ,non‐bacterial infectious pneumonia group and non‐infectious disease group respectively .Serum PCT and CRP levels were measured before treat‐ment in the three groups .With sputum culture results as gold standard for the diagnosis of bacterial infectious pneumonia ,the sensi‐tivity ,specificity ,positive predictive value ,negative predictive value ,Youden index and diagnostic accuracy of PCT ,CRP ,PCT /CRP series test and parallel test were calculated for bacterial infectious pneumonia diagnosis .Results The levels of PCT and CRP of children in bacterial infectious pneumonia group were significant higher than those of children in non‐bacterial infectious pneumonia group and non‐infectious disease group(P < 0 .05) .The sensitivity ,specificity ,positive predictive value ,negative predictive value , Youden index and diagnostic accuracy of PCT ( ≥ 0 .5 ng/mL as the best intercept point) for bacterial infectious pneumonia diagno‐sis were 0 .743 ,0 .719 ,0 .806 ,0 .639 ,0 .461 and 0 .733 .And the same values for PCT /CRP series test were 0 .604 ,0 .875 ,0 .884 , 0 .583 ,0 .479 and 0 .709 respectively .All the values were higher than those of children in non‐bacterial infectious pneumonia group and non‐infectious disease group except sensitivity .Conclusion The combination of PCT and PCT /CRP series test is ideal method for early diagnosis of bacterial infectious pneumonia with high sensitivity and specificity .
9.A retrospective questionnaire analysis on the association of obstructive sleep apnea syndrome and aortic dissection

Objective To investigate obstructive sleep apnea syndrome (OSAS) in patients with aortic dissection (AD). Methods Questionnaire analysis was applied to patients with or without AD according to Berlin questionnaire. Questionnaires were collected and common characters and related symptoms were compared between the two groups. Further comparison on related symptoms was made between the AD group and hypertensive patients in the control group. Results Totally 70 questionnaires were collected with 33 for the AD group (29 males and 4 females) and 37 for the control (29 males and 8 females). The average age (P <0.05) was 50.9 years for the AD group (range 32 to 70) and 53.4 years for the control (range 25 to 83). Snoring occurred in 29 AD patients (87. 88% ) and in 22 control patients (59. 46% ) (P < 0. 05 ). Snoring everyday occurred in 19 AD patients (57. 58% ) and in 12 controls (32.43%) (P <0.05). Loud snoring was reported from 23 AD patients (69.70%) and 10 controls (27.03%) (P <0. 05). Apnea occurred in 15 AD patients (45. 45% ) and 8 controls (21.62%) ( P <0. 05). Apnea nearly everyday occurred in 9 AD patients ( 27.27% ) and 5 controls ( 13. 51% ) ( P <0. 05). Fatigue after sleep occurred in 23 AD patients (69. 70% ) and 15 controls (40. 54% ) (P <0. 05).Fatigue nearly everyday after sleep occurred in 10 AD patients (30. 30% ) and 6 controls ( 16. 22% ) ( P <0. 05). Hypertension was found in 28 AD patients ( 84. 85% ) and 20 controls ( 54. 05% ) ( P < 0. 05 ).The average age of hypertensive control were 62 ± 16, greater than that of AD group (P <0. 05). In the 20 hypertensive control patients, loud snoring in 7 (35%), lessen than that of AD group (P <0.05).Compared with hypertensive controls, AD patients had greater body length ( P < 0. 05 ) and lesser waist-tohip ration (P < 0. 05 ). Conclusions Compared with normal control, OSAS is more common in AD patients.
10.Expression and significance of elastin and fibulin-5 in anterior vaginal tissue of women with pelvic organ prolapse

Objective To investigate the expression and significance of elastin and fibulin-5 in anterior vaginal tissue of women with pelvic organ prolapse (POP). Methods Between November 2006 and June 2008, 68 patients with POP underwent surgical treatment in Shengjing Hospital of China Medical University were enrolled in this study, who were classified into 10 patients with grade Ⅰ , 21 patients with grade Ⅱ , 25 patients with grade Ⅲ and 12 patients with grade Ⅳ in accordance with pelvic organ prolapse quantitation (POP-Q). Meanwhile, 18 cases with early cervical cancer at stage of Ⅰ b were treated by total hysterectomy and bilateral salpingo-oophorectomy, their anterior vaginal tissues were selected as controls. Immunohistochemical staining was performed to detect the expression of elastin and fibulin-5. Results (1 ) Elastin and fibulin-5 were mainly expressed at extracellular matrix(ECM). (2)The positive rate of fibulin-5 expression in anterior vaginal wall were 5% (2/37 ) in grade Ⅲ/Ⅳ and 26% (8/31 ) in grade Ⅰ/Ⅱ POP patients, which reached statistical difference (P = 0. 035). However, no statistical different expression was found between postmenopausal (13%, 8/60) and non-menopausal patients (2/8), vaginal delivery ≤ 2 (19%, 5/27 ) and > 2 patients (12%, 5/41, P > 0. 05 ). (3 ) The positive rate of elastin expression in anterior vaginal wall in POP group was 31% (21/68), which was significantly lower than 72% (13/18) of control group (P = 0. 002). Among POP group, 19% (7/37) of elastin expression in grade Ⅲ/Ⅳ POP was significantly lower than 45% (14/31 ) in grade Ⅰ/Ⅱ of POP patients. However, no statistical difference was found between postmenopausal (30%, 18/60 ) and non-menopausal patients (3/8 ), vaginal delivery ≤ 2 (26%, 7/27) and >2 patients(34%, 14/41, P>0. 05). (4)In POP group, both positive expression of fibulin-5 and elastin of anterior vaginal wall was in 6 cases, both negative expression of fibulin-5 and elastin was in 43 cases. It was illustrated that elastin and fibulin-5 had an positive relationship (P = 0. 031 ). Conclusion The decreased expression of elastin and fibolin-5 was correlated with degree of POP, which indicated that elastin and fibulin-5 may play a role in the pathogenesis of POP.
