1.Clinical analysis of stenting administration on angioplasty of patients with vertebrobasilar artery stenos

Objective To investigate the long-term effect of angioplasty of patients with symptomatic vertebrobasilar artery stent.Methods Thirty-three patients with symptomatic vertebral artery stenosis who underwent endovascular treatment stent.The clinical symptoms and stent restenosis were recorded.Results The clinical symptoms of 33 cases with angioplasty were improved significantly.The preoperative stenosis rate was (92.36 ±3.23)% before operation and the average postoperative residual stenosis rate was (9.13 ±2.57)% after operation.Clinical symptoms of 23 patients disappeared completely after operation,and 10 cases improved significantly.As to complications,1 case with severe basilar artery stenosis had a side numbness of extremities,1 case showed postoperative cerebral embolism,and return to normal after 3 days.When follow up periods lasted for 6-24 months,9 cases were with dizziness or other symptoms.Of which,29 cases were performed DSA imaging,the residual stenosis rate was (19.27 ± 5.62)%,3 cases of vertebral artery in-stent restenosis were reached over 50%.Those 3 cases were give stent angioplasty again and the uncomfortable symptoms disappeared.Conclusion Stenosis opeation can significantly alleviate the symptoms of cerebral ischemia and symptomatic vertebrobasilar artery,and improve the quality of life of patients.However,restenosis should paid more attention.
2.Cross-sectional study on the community medical care service of chronic diseases of the elderly in an urban area

Objective To study the health state and service of the elderly in the Tongbai community. Methods The educational degree, disease histories, daily living capability, health care demand and the controlled state of hypertension and diabetes were recorded by questionnaires and physical examination. Results Overweight or obesity (66.1%) was the common disease, hypertension (42.2%) next, then coronary disease (10.0%) and diabetes (9.1%). Nearly 43.3% hypertension patients never or seldom measured their blood pressure. The rate of ideal blood pressure was lower than 25%. 30.4% diabetes patients never or seldom measured their fasting glucose and 69.3% patients never or seldom measured their post-meal glucose. 46.1% of the elderly required one or more kinds of community health services, in which annual health examination was 32.6%, and the next was health guide (23.0%), 53.9% of the elderly had no requirement of community services except medical care. Conclusion The elderly in community had high prevalence and poor control of chronic diseases. They did not fully understand the community health services, which brought the challenges to the medical care, health prevention, health education and reasonable utility of hygiene resources in community health services.
3.Stress analysis of distal-extension removable partial dentures supported by mini implants

Objective To analyse the stress distribution of distal-extension removable partial dentures supported by mini implants. Methods The finite element analysis models of conventional removable partial dentures ( CRPD) and mini implants supported removable partial dentures (ISRPD) were established by modular denture model. The stress distribution of abutments and supporting tissues was compared by finite element analysis software Abaqus 6.5.Results Under vertical loading, the stress of abutment teeth, mucosa of edentulous region and alveolar bone of ISRPD model was lower than that of CRPD model. Under oblique loading, the stress of each part of CRPD model and alveolar bone of ISRPD model significantly increased, while that of abutment teeth and parodontium of ISPRD model decreased. Conclusion The distal-extension removable partial dentures supported by mini implants work well in protecting the abutments and supporting tissues.
4.Stress analysis of precision attachment dentures for mandibular distal-extension edentulous cases

Objective To analyse the stress distribution of precision attachment dentures used in distal-extension edentulous cases. Methods The finite element models of conventional removable partial dentures and precision attachment dentures were established by modular denture model, and the models of precision attachment dentures were established with 4┬4 as one abutment or 43┬34 as two abutments. The stress distribution of abutments and supporting tissues was compared by finite element analysis software Abaqus 6.5.Results Under vertical loading or oblique loading, the stress of abutments and supporting tissues from attachment dentures with two abutments was significantly lower than that from attachment dentures with one abutment. The stress of conventional removable partial dentures significantly increased under oblique loading. Conclusion It is reasonable for the design of attachment dentures with two abutments for mandibular distal-extension edentulous cases.
6.Management of chylothorax with nutrition support

Objective: To review the efficacy of nutrition support and operation in management of chylothorax.Methods: 6 patients underwent conservative way(fat-free enteral nutrition+PN+ somatostatin) and 3 patients then received the operation.Results: 3 patients were cured with only conservative way,but 3 others were cured under operation.Conclusion:The conservative way(fat-free enteral nutrition+PN+ somatostatin) is a effective method in patients with chylothoral,and the operation is needed in some patients.
7.Nasal mucosa recovery after endoscopic surgery using the plasma radiofrequency ablation at low temperature for treatment of nasal inverted papilloma.

OBJECTIVE:
To assess the therapeutic effect and study the role of nasal mucosa epithelization after endoscopic surgery using the plasma radiofrequency ablation at low temperature in patients with nasal inverted papilloma.
METHOD:
The clinical data of 104 patients with nasal inverted papilloma underwent endoscopic surgery u sing the plasma radiofrequency ablation at low temperature from July, 2008 to July, 2012 were analyzed,and the recovery of mucosa was observed under nasal endoscope.
RESULT:
The mucosa recovery extent showed a decreasing trend from mucosa pattern degree I to III, where the difference was statistically significant by chi-square test between groups (P < 0.05). The average epithelialization time was 2.7 months. It showed a significantly decreasing trend among average epithelialization time of different degrees of mucosa (P < 0.05). The nasal mucosa of most patients completed epithelialization 2.9 months after surgery.
CONCLUSION
It is safe and effective to treat nasal inverted papilloma with plasma radiofrequency ablation at low temperature. The patients should be followed up with regular reexamination for at least three months after surgery.

8.Length of warm ischemic tolerance for epithelial regeneration in heterotopic rat tracheal isografts

Objective To determine the length of warm ischemic (WI) tolerance in bronchial graft from non-heart-beating donors. Methods Forty-eight rats were randomly divided into 4 groups (each group having 12 rats) according to different WI durations including WI-0 min (group A), WI-30 min (group B), WI-45 min (group C) and WI-60 min (group D). In each group, the tracheae from 6 rats were respectively imbedded in greater omentum of other 6 rats, and 14 days later, the transplanted tracheae were taken from recipients to evaluate epithelial thickness and regeneration. Results Epithelial thickness and the degree of epithelial regeneration had no significant difference (P >0. 05) between the syngeneic control group and the WI-30 minutes group. All of the grafts with WI duration of 45 min were viable, but the epithelium was significantly thinner than that in the syngeneic control group (P<0. 05). However all of the grafts with WI duration of 60 min showed lower viability rate. Conclusion The time limits of tolerance to WI of tracheal grafts from NHBDs may be 45 min.
9.Stress analysis of distal-extension prosthetics with mesial and distal occlusal rest

Objective To investigate the stress distribution in the abutment and supporting tissues of distal-extension removable partial dentures with mesial and distal occlusal rest under loading. Methods A modular denture model was used to build a model of mandibular dentition defect ( 765┬567 loss) with HyperMesh 7.0 software. Prosthetics with mesial (M model) and distal occlusal rest (D model) were designed with UG 5.0 software, and the finite element models were completed with HyperMesh 7.0 software. The stress distribution was analyzed in the abutment and supporting tissues of distal-extension removable partial dentures with mesial and distal occlusal rest when bilateral vertical forces were applied. Results Compared with M model, D model provided much larger maximum stress in abutments and periodontal membrane. Stress of D model mainly concentrated on roots of 4┬4 , while that of M model uniformly distributed on roots of 43┬34 . The maximum stress of M model was significantly larger than that of D model on the mucosa of edentulous region. The maximum stress on alveolar bone of two models' edentulous region was equal, while the stress of M model distributed more widely. Conclusion It is prior to select mesial occlusal rest in distal-extension removable partial dentures.
10.Clinical study on palpation-negative breast tumor resection with ultrasound-guided methylene blue labeling and percutaneous suture traction

Objective To explore the significance of palpation-negative breast tumor resection with ultrasound-guided methylene blue labeling and percutaneous suture traction.Methods 46 cases of small breast tumors with the diameter from 0.5 cm to 1.0 cm were double labeled with methylene blue under the guide of ultrasound l hour preoperatively.The tumors were fixed,drew outward with percutaneous suture and resected accurately.All the 46 cases were rechecked by ultrasound to verify whether residual or false resection occurred 1 month later.Results All the 58 tumors of the 46 patients were accurately resected.No residue or false resection occurred.The average operation duration was 10 min to 20 min.Conclusion Palpable-negative breast tumor resection with ultrasound-guided methylene blue labeling and percutaneous suture traction has the advantages of small invasion,accurate positioning and short operation duration.
