Economization of water,electricity and gas plays an important part in the construction of an economical hospital.The metering and accounting system for water,electricity and gas management in our hospital can rationally and accurately account the consumption of water,electricity and gas.Under this system,we have implemented quota management and cost control in the medical departments,efficiency assessment in the production and supply units,cost inclusion in the internal services unit,cost collection from the logistics support contractor units,and the maintenance of pipeline leakage,which has proved to be a set of effective and scientific solutions to the promotion of the efficiency of water,electricity and gas management in the hospital.
4.Construction of integrated finance management in the military hospital

5.The Clinical Significance and Prognosis of Fetal Arrhythmias

Objective To summarize the common causes of fetal arrhythmia and explore its clinical significance and correlation with fetal prognosis. Methods The clinical situation and diagnostic procedure as well as the results of long-term follow-up of 26 cases of fetal arrthythmias were analyzed retrospectively. Results The incidence of fetal arrhythmias is 0.2%. 11.5%(3/26) of them were diagnosed fetal tachycardia, 15.4% (4/26) of them were diagnosed fetal bradycardia and 73.1% (19/26)were diagnosed irregular fetal cardiac rhythm . The average gestational age they were diagnosed was 35 +2 weeks (15 weeks to 41 +1weeks). Twenty-two cases were diagnosed by antenatal auscultation, 1 case was diagnosed by ultrasonography at 15 weeks' gestation, 3 cases were diagnosed by fetal heart monitoring. We performed fetal echocardiograms on 17 fetuses, 6 cases (35.3%)showed that premature atrial contraction with normal structure of fetal heart. All of the neonates survived postnatally and 24 of them (92.3%) accepted follow-up. Echocardiograms for16 neonates were performed and 2 of them were identified as atrial septal defects with normal heart rhythms. The incidence of structure heart disease in fetal arrhythmia is 7.7%. Conclusion The prognosis is well for most of the fetuses with arrhythmias, the incidence of heart structure disease is very low among them. We should pay attention to diagnose the fetus of heart structure disease with persistant bradycardia. Temporization for the management of arrhythmiatic fetuses should be accepted and the decision for termination of pregnancy should be made according to obstetric indication.
6.Relation on the serum estradiol levels and the susceptibility to thromboangitis obliteransin in a population of Uyghur nationality male from Xinjiang

Objective To evaluate the relationship between the serum estradiol levels and susceptibility to thromboangitis obliterans(TAO) in a population of Uighurs from Xinjiang.Methods Clinical data were concluded 500 Uighur healthy male through the method of multistage and cluster sampling randomly in Xinjiang Kashi area,300 cases of Hans male came from the healthy physical examination in Xinjiang Uygurs Autonomous Region Pepole's Hospital,examining the serum estradiol levels and analysising whether had the statistical significance between Uygurs and Hans nationality.Results The estradiol levels of Uighur male was (31.49 ± 12.55) pg/ml,the hans was (26.68 ± 12.40) pg/ml,single factor analysis,the difference with plasma estradiol levels have statistical significance between ethnic groups (t =5.267,P <0.05),and serum estradiol levels of uygur people was higher than Hans,and the estradiol levels with different smokers and age groups,there was no statistically significant difference (t =0.346,F =1.820,P >0.05).Logistic regression analysis indicated that only ethnic difference have statistically significant influence on estradiol levels (P < 0.05).Conclusions The serum estradiol levels of Uighur healthy male in Xinjiang are higher than Hans.The high serum estradiol levels may be a susceptible factor of thromboangitis obliterans to Xinjiang Uygur.
7.Effect of taking L-T4 in bedtime vs morning on changes of serum thyroid hormone concentration in patients with hypothyroidism

Objective To compare the effect of L-T4 taken in the morning vs bedtime on serum thyroid hormone in patients with hypothyroidism.Methods Seventeen patients with primary hypothyroidism were prospectively included.They took L-T4 in the morning for 2 months followed by L-T4 taken during the bedtime for 2 months.Thyroid hormone levels and cholesterol,triglyceride,albumin,creatinine and heart rate were measured.Wilcoxon test and paired t test were used to compare the data.The correlation between the changes of TSH and FT3,FT4 was analyzed respectively by Pearson correlation analysis.Results TSH,FT3,FT4 were:(12.10±7.19) vs (3.90±3.47) mU/L,(4.14±0.86) vs (5.24±1.11) pmol/L,(12.72± 4.40) vs (18.48±2.87) pmoL/L for L-T4 taken in the morning and during the bedtime respectively (t value:6.371,-3.166,-5.435,all P<0.01).There was no correlation between the changes of TSH and FT3,FT4(r=O.12,0.22,both P>0.05),respectively.The averaged changes were (8.20±5.31) mU/L,(1.09± 1.42) pmol/L and (5.76±4.37) pmol/L.Triglyceride,cholesterol,albumin,creatinine levels and heart rate were not significantly different with L-T4 taken in the morning and during bedtime (t=0.240,0.017;z=0;t=-0.610,1.588,all P>0.05).Conclusions L-T4 taken during the bedtime by patients with hypothyroidism would reduce TSH and increase FT3,FT4 levels.This method can maintain the stability of TSH level with no need to increase the dose.
8.Effect of parecoxib on sufentanil dosage and its analgesic effect in patients with laparoscopic radical resection of rectal carcinoma under combined anesthesia

Objective To study effect of parecoxib on sufentanil dosage and its analgesic effect in patients with laparoscopic radical resection of rectal carcinoma under combined anesthesia.Methods 120 cases patients diagnosed with rectal cancer and prepared to laparoscopic radical resection of rectal cancer were selected in Kailuan General Hospital, and randomly divided into experimental group and control group, 60 cases in each group, experimental group induced anesthesia with 40mg parecoxib sodium intravenous injection, while the control group were treated with normal saline, the postoperative dosage of sufentanil, analgesia pump using frequency and other indicators were detected at 6,12,24 and 48 h, and analgesic effect were observed.Results Compared with control group, in experimental group, postoperative dosage of sufentanil was less ( P <0.05 ) , analgesia pump pressing times and effective pressing numbers were fewer (P<0.05), VAS pain score was lower (P<0.05) and Ramsay score was lower (P<0.05) at each time point.Conclusion Parecoxib could reduce the dosage of sufentanil after laparoscopic radical resection of rectal cancer, and enhance the analgesic effect.

Objective Study of interleukin-6 (IL-6),erythrocyte sedimen-tation rate (ESR),C-reactive protein (CRP) in diagnosis and treatment of abdominal aortic aneurysm.Methods Between June 2014 and May 2016,we detected of abdominal aortic aneurysm in 62 cases that were diagnosed and treated at Department of Vascular Surgery,People's Hospital of Xinjiang Uygur Autonomous Region including interleukin-6,erythrocyte sediinen-tation rate,and C-reactive protein.Results There was no significant difference between interleukin-6,erythrocyte sedimen-tation rate and C-reactive protein in the patients with and without complications.The correlation analysis results of interleukin-6,erythrocyte sedimen-tation and C-reactive protein were confirmed to have positive correlation respectively.Compared with non special types of rupture (48.0 ± 34.2) pg/ml,interleukin-6 was higher in special types of rupture (187.6 ± 110.4) pg/ml.Conclusions Preoperative detection of interleukin-6,erythrocyte sedimen-tation and C-reactive protein,combined with patient history and imaging examination were help for predict cause,choose better surgical approach and predict the prognosis in certain guiding significance.
10.Effects of Tuina on serum creatine kinase and skeletal muscle mitochondria in delayed onset muscle soreness model rats

Objective: To observe the effect of Tuina (Chinese therapeutic massage) on creatine kinase (CK), mitochondrial Ca2+ concentration, and ultrastructure of skeletal muscle in delayed onset muscle soreness (DOMS) model rats.Methods: A total of 130 healthy male Sprague-Dawley rats were randomly divided into a blank group, an exercise control group, a pre-exercise Tuina group, and a post-exercise Tuina group. According to the time points for sample collection, the exercise control group was divided into a 0 h exercise control group, a 24 h exercise control group, a 48 h exercise control group, and a 72 h exercise control group; the pre-exercise Tuina group was further divided into a 0 h pre-exercise Tuina group, a 24 h pre-exercise Tuina group, a 48 h pre-exercise Tuina group, and a 72 h pre-exercise Tuina group; and the post-exercise Tuina group was divided into a 0 h post-exercise Tuina group, a 24 h post-exercise Tuina group, a 48 h post-exercise Tuina group, and a 72 h post-exercise Tuina group. Rats in all groups except for the blank group received DOMS modeling. Professionals performed Nie-Pinching manipulation and finger Nian-Twisting manipulation on the lower limbs of the rats. The samples were collected at 0 h, 24 h, 48 h, or 72 h after exhaustive exercise for each pre-exercise Tuina group. The samples were collected at 0 h, 24 h, 48 h, or 72 h after Tuina for each post-exercise Tuina group. The changes in serum CK, skeletal muscle mitochondrial Ca2+ concentration, and Ca2+-adenosine triphosphatase (ATPase) were determined. The ultrastructure changes of skeletal muscles in each group were observed by a transmission electron microscope. Results: The electron microscope showed that compared with the exercise control group, the skeletal muscle structures of the pre-exercise Tuina group and the post-exercise Tuina group were significantly improved, and the overall performance of skeletal muscle in the pre-exercise Tuina group was more similar to that of the blank group. The level of serum CK in the pre-exercise Tuina group and the post-exercise Tuina group was significantly lower than that in the exercise control group (P<0.01). The Ca2+ concentration of skeletal muscle in the 24 h, 48 h, and 72 h pre-exercise Tuina groups was lower than that in the post-exercise Tuina group at the same time point (P<0.01). The Ca2+-ATPase concentration of skeletal muscle in the 24 h and 72 h pre-exercise Tuina groups was lower than that in the post-exercise Tuina group at the same time point (P<0.05).Conclusion: Tuina effectively prevents muscle damage caused by heavy exercise and long-term exercise, which may be related to the increase of skeletal muscle Ca2+-ATPase activity and mitochondrial Ca2+ transport.
