1.Clinical analysis of pathogens in elderly patients with nosocomial lower respiratory tract infection in ICU

Objective To study the distribution characteristics and drug resistance of pathogens in elderly patients with nosocomial lower respiratory tract infection in ICU. Methods 67 cases of the distribution characteristics and drug resistance of pathogens were analyzed retrospectively in elderly patients with nosocomial lower respiratory tract infection in ICU. Results A total of 67 pathogens were identified liq specimens from the lower respiratory tract secretion. The rate of gram-negative bacteria,gram-positive bacteria and fungi were 56.1% ,22.3% and 21.6% respectively. The most common pathogens of gram-negative bacteria were Pseudomonas aeruginosa( 34. 6% ), that was followed by Klebsiella pneumoniae( 19.2% ). The most common pathogens of gram-positive bacteria were Staphylococcns aureus(58.1% ). The most common pathogens of fungi were Candida albicans(76.7% ). They were resistant to multiple antibiotics,the pathogens of gram-positive were sensitive to Vanco. Conclusion The most common pathogens in elderly patients with nosocomial lower respiratory tract infection in ICU are gram-positive bacteria. Pseudomonas aeruginosa, Klebsiella pneumoniae,Staphylococcus aureus and Candida albicans are important pathogen of targeted therapies.
2.Study on the overlapping applications of multiple imaging to middle-aged women with constipation in the west area of Shandong

Objective To investigate the pathogen and inspection methods of the middle-aged women with constipation and evaluate the accuracy and clinical application of imaging to constipation.Methods First colonic transit test were used to 98 cases of middle-aged women who were diagnosed constipation by doctors,then air-barium double contrast examination were applied.Defecation radiography or simulation rectum defecography were used to those who were diagnosed as outlet obstruction.According to the measure target and diagnostic criteria of professor LU,the results were studied.Results 6 cases were abnormal within 8 cases who were inspected by barium enema.14cases were abnormal within 16 cases who were inspected by defecation radiography.Colonic transit test were used to 74 cases and only 6 cases were normal.16 cases were true slow transmission and 20 cases were false slow transmission.52 cases were outlet obstruction.20 volunteers were inspected by suspension barium sulfate and simulation rectum defecography.Conclusions Air-bariun double contrast examination could discover the organic disease and abnormal of rectum.colonic transit test,defecation radiography and simulation rectum defecography were simply and perfect methods to discover the pathogen of constipation.
3.Effect of Bobath Approach Combined with Acupuncture on Spastic Cerebral Palsy

ObjectiveTo investigate the effect of Bobath approach combined with acupuncture on spastic cerebral palsy. Methods30 children with cerebral palsy treated with acupuncture combined with Bobath approach were as study group, other 30 children treated with Bobath approach only as the control group. ResultsThere were 27 cases (93%) were effective in study group, and 21 cases (80%) in control. The scores of Gross Motor Function Measure (GMFM) improved in both groups (P<0.05), but more in the study group (P<0.05) after treatment. The spasticity alleviated in both study group (76.7%) and control group (50%). ConclusionThe combination with acupuncture is more effective on spastic cerebral palsy than Bobath approach alone.
4.Comparative study on effect of radiofrequency ablation and operation for the treatment of small hepatocellular carcinoma

Objective To comparison between radiofrequency ablation introduced by tri-dimension reconstruction and ultrasound and operation for the treatment of small hepatocellular carcinoma. Methods 94 patients with small hepatocellular carcinoma from Jan 2009 to Mar 25 in Shanxi Cancer Hospital were analyzed. 45 patients were both given CT examination to rebuild tri-dimension and radiofrequency ablation treatment. 49 patients were given excision. Both groups were given CT examination to rebuild tri-dimension before the treatment. The differences in recurrence rate, overall survival and complication ratio between the two groups were compared. Results After 1, 2 and 3 years radiofrequency ablation treatment, the survival rate in radiofrequency ablation group were 95.56 % (43/45), 86.67 % (39/45), 60.00 % (27/45) respectively, comparing with 93.88%(46/49), 79.60%(39/49), 59.20%(29/49) in operation group. The difference was not statistically significant (P>0.05). There was no statistical significance in 1-year, 2-year and 3-year recurrence rates between the two groups (P> 0.05). There were statistical significances in incidence of post-treatment pains [13.33%(6/45) vs 100.00%(49/49),χ2=60.416, P<0.05] and complication (P<0.05) between the two groups. By Mar 25th, 2015, there was no needle tract implantation in the patients with radiofrequency ablation treatment. Conclusions For the small hepatocellular carcinoma with less than 3cm in diameter, introduced by CT tri-dimension image reconstruction and ultrasound, the radiofrequency ablation treatment excels in effect with fewer damages, infective complications, lower cost and can be applied many times. The overall effect is close to surgical removal, and therefore it can be used as the first line therapy for small hepatocellular carcinoma.

6.Effects of CO_2 pneumoperitoneum on levels of interleukin-1? and interleukin-6,and peritoneal macrophages in tumor-bearing rats

Objective To investigate effects of CO_2 pneumoperitoneum on cytokines and peritoneal macrophages in a rat model with implanted liver tumor.Methods A total of 32 Wistar rats with implanted liver tumor were randomly divided into 4 groups((n=8)): Control Group(anesthesia only),Laparotomy Group,Gasless Group(gasless laparoscopy),and Pneumoperitoneum Group(laparoscopy under CO_2 pneumoperitoneum).Serum samples were collected at the 2nd and 24th hours after the procedure respectively for the detection of levels of interleukin-1?(IL-1?) and interleukin-6(IL-6).Samples of peritoneal macrophages were collected and incubated for the detection of levels of tumor necrosis factor-?(TNF-?),a product of macrophages.Results At the 2nd and 24th hours after surgery,levels of serum IL-6 in the Laparotomy Group(57.92?2.06 pg/ml and 35.49?1.15 pg/ml) were significantly greater than those in the Pneumoperitoneum Group(14.64?0.34 pg/ml and 15.39?0.86 pg/ml),the Gasless Group(24.75?1.53 pg/ml and 17.10?0.97 pg/ml),and the Control Group(17.75?1.60 pg/ml and 14.55?0.25 pg/ml)(P
7.Thoughts in TCM and Western Medicine about Drug Resistance of Helicobacter Pylori

Helicobacter pylori (Hp) is a kind of Gram negative pathogen which serious threats human health. With the widespread use of antibiotics, the drug resistance of Hp is more and more serious, which has become the leading cause of failure in the treatment of Hp related diseases. Therefore, effective ways to find the intervention of drug resistance of Hp have become a practical problem to be solved in clinic. TCM and Western medicine in the treatment of drug resistant of Hp play their respective advantages. This article analyzed the research status quo and intervention effects of TCM and Western medicine for drug resistance of Hp, with a purpose to provide a new path for clinical treatment of Hp related diseases.
8.Construction of Nedd4 knockout BMDM cell line by CRISPR/Cas9 technology

Objective To construct Nedd4 knockout bone marrow-derived macrophages(BMDM) cell line by CRISPR/Cas9 technology and to provide an effective tool for studying the function and mechanism of Nedd4 in macrophage.Methods First,three high-grade sgRNAs targeting Nedd4 gene exons were screened using the online tool before synthesized sgRNAs were inserted into the PX330 plasmid respectively.Secondly,the recombinant plasmids were transferred into BMDM cells and monoclonal cells were obtained by limiting dilution method.The protein levels of NEDD4 in monoclonal cells were detected by Western blotting.Finally,the DNA sequence of the monoclonal cells was confirmed by sequence analysis.Results One Nedd4 knockout BMDM cell line was obtained.The sequencing result showed that the Nedd4 gene had 16bp deletion mutation in this cell line.Conclusion The Nedd4 knockout BMDM macrophage cell line constructed by CRISPR/Cas9 technology will be a useful tool for studying the function and mechanism of Nedd4 in BMDM cells.
9.Spinal tumors treated with total spondylectomy and spinal stability reconstruction

Objective To evaluate the possibility and clinical value of total spondylectomy for the treatment of spinal tumors and to investigate a reliable method of spinal reconstruction after total spondylectomy. Methods Twenty seven cases of spinal tumors which eroded both vertebral body and its attachments were treated with total spondylectomy and internal fixation as reconstruction techniques. There were benign, malignant and metastatic tumors, which involved different levels from upper cervical to lower lumbar spines. One to 3 spinal vertebrae were removed. Results Twenty three cases were followed up for 7 to 96 months (with an average of 25 months). Among them, 1 case of L 5 malignant neurofibroma and 1 case of C6,7 giant cells tumor recurred in 10 and 12 months after operation, but the patients refused further treatment. One case of C2-4 chordoma recurred 1 year after operation, after second surgery, the result was satisfactory. No recurrent signs were found in the rest of 20 cases. Among 25 cases with neurological lesions, obvious improvement were obtained in 23 after operation. Conclusion For patients with involvement of spinal vertebra eroded by tumors, total spondylectomy is an effective procedure. After total spondylectomy, spinal stability can be reconstructed by stable internal fixation system.
