Decoy receptor 3(DcR3) gene expresses highly in some malignant tumors,it involves cellular proliferation regulation, inhibition of apoptosis, immune escape and tumor angiogenesis by competitively blocking the Fas/FasL, LIGHT and TL1 A/DR3 signal pathway, therefore it deeply relates to the malignant tumors's generation and progression.DcR3 is supposed to be the new tumor-specific marker that may cast the light to the tumor generation,development,prevention,diagnosis,treatment,prognosis and effect observation.DcR3 is expected to be open a new chapter in clinical application.

OBJECTIVE To investigate whether PDCA(plan-Do-check-action) cycle could lower the incidence of the nosocomial infection in the male psychiatric closed wards.METHODS Data were collected both prospectively and retrospectively.PDCA cycle was used to manage infection control in the male psychiatric closed wards in 2006;whereas the usual method used in the infection control in 2005 was treated as a control group.RESULTS The incidence of the nosocomial infection lowered from 9.94% in 2005 to 5.57% in 2006(P

Objective To study the relationship between the coping style and self-concept of college engi-neering students.Methods 559 engineering students were assessed with the Coping Style Questionnaire and the Tennessee Serf-Concept Scale.Results (1) The positive factors of self-concept had significantly negative correla-tion to the coping styles of serf-blame,illusion,avoidance,and rationalization(-0.445～-0.136,P<0.01),but had significantly positive correlation to the coping style of solving problem and seeking help(0.119～0.432,P<0.01).Serf-criticism had significantly negative correlation to the coping styles of serf-blame,illusion,avoidance,and rationalization(0.134～0.215,P<0.01);(2)There were significant differences between high self-concept students and low self-concept students for coping styles(P<0.01) ;(3) The factors of serf-concept had significant predictability on coping styles.Conclusion There was a higher correlation between the coping style and serf-con-cept of college engineering students.

Objective To investigate the efficacy of budesonide combined with terbutaline atomization inhalation in the treatment of acute asthma attacks, and to provide reference for clinical treatment.Methods 118 patients with acute asthma attack were selected and randomly divided into observation group and control group, 59 cases in each group.The control group was treated with budesonide inhalation, the observation group was treated with budesonide combined with terbutaline aerosol inhalation The clinical efficacy of the two groups, clinical symptoms and signs of the disappearance time were compared, the pulmonary function test results and the occurrence of adverse reactions were compared.Results In the observation group,the total effective rate was 72.88%, which was significantly higher than 52.54% of the control group (x2=5.218,P<0.05).The disappear time of shortness breath, cough, pulmonary rales in the observation group were (4.01±1.42)d, (6.58±1.51)d, (7.02±227)d, which were significantly shorter than (5.71±1.59)d,(8.52±1.97)d,(8.69±2.61)d in the control group (t=4.557,4.421,4.659,all P<0.05).After treatment, the FEV1,FVC and peak expiratory flow (PEF) of the observation group were (2.35±0.67)L, (2.76±0.62)L, (2.91±0.49)L, which were significantly higher than(2.09±0.57)L,(2.31±0.51)L,(2.48±0.59)L of the control group (t=3.625,3.641,3.887,all P<0.05).The two groups had no serious adverse reactions.Conclusion Budesonide combined with terbutaline atomization inhalation can relieve the clinical symptoms of patients with acute asthma,improve lung function,improve the clinical treatment effect and has good security.

OBJECTIVE: The aim of this study is to explore the applications of digital department in otolaryngology concern with the emergent and dangerous of otolaryngology diseases. METHOD: We have constructed digital otolaryngology head and neck surgery department on the basis of hospital information system, which is constructed under the guidance of the theories and methods of modern information integration technology, computer software technology, health informatics and network information technology. RESULT: The foundation of digital otolaryngology department can optimize digital workflow, improve work efficiency and eliminate the manual operation. CONCLUSION Digital department in otolaryngology will promote the quality and upgrade the level of medical staff, it plays an important role in the hospital informationization.

Objective To explore the influence of integrated Chinese and western medicine therapy on anxiety,sugar metabolism and subjective well-being for type 2 diabetes Patients associated with anxiety.Methods Use glycated hemoglobin (HbAlc),trait anxiety inventory(TAI),and Memorial University of Newfoundland Scale of Happiness (MUNSH) to evaluate the condition of 60 patients with type 2 diabetes associated with anxiety.The patients participated in the study were randomly divided into Western medicine therapy group(control group)and integrated traditional Chinese and Western medicine therapy group (experimental group),with 30 cases in each group.The control group was treated with paroxetine while the experimental group was treated with paroxetine plus Jiawei-Xiaoyaowan.6 weeks after treatment,we tested the HbAlc level.TAI score and MUNSH score of those patients.Results After 6 weeks,the TAI score of patients in both experimental group and control group have obviously decreased (before treatment:54.07±6.41 and 56.33 ±6.01,respectively; after treatment:38.89±6.70 and 45.29±6.55,respectively) and the difference was statistically significant (P＜0.05).And the TAI score of the two groups after treatment was significantly different (P＜0.05).For both experimental and control groups,the HbA1e level were not obviously changed before and after the study with P＞0.05 (before treatment:8.14±2.69 and 8.07±2.11,respectively; after treatment:8.21±2.07 and 7.92± 1.90,respectively).Comparison between groups also showed no significant difference.MUNSH score have obviously increased in experimental group (before treatment:40.49±5.22; after treatment:44.53 ± 6.28,P＜0.05).The result of comparison between the two groups after treatment shows that MUNSH score of patients in the experimental group were obviously improved compared with patients in the control group (57.56 ±6.39),and the difference was statistically significant (P＜0.05).Conclusion The integrated traditional Chinese and western medicine therapy can obviously release anxiety and enhance subjective well-being in patients with cerebral infarction associated with anxiety.

Objective To discuss the operative methods,clinical effect and safety of endovascular stenting in treatment of upra-arch extra intracranial artery stenosis.Methods Symptomatic carotid artery stenosis ＞ 50％,or no symptomatic stenosis ＞ 75％ with upra-arch extra intracranial artery stenosis patients were treated by endovascular stenting,of all patients,85 cases with carotid artery stenosis,37 cases with vertebral artery stenosis,6 cases with subclavian artery stenosis.Results One hundred and twenty-eight patients were treated with 130 vessel,used 131 stents,59 patients used vascular protection,the achievement rate of stent implantation was 100.00％.The degree of stenosis were significantly improved,the degree of stenosis from (87.65 ± 8.70)％ to (14.28 ± 4.87)％.Twenty-six patients appeared transient decline in blood pressure and heart rate,31 patients appeared persistent hypotension and decrease in heart rate.One patient (no using of vascular protection) appeared obhteration in ipsilateral middle cerebral artery,the artery was recanalization after thrombolysis treatment.The rest of 127 patients did not appear intraoperative vascular embolization complications.Eighteen patients appeared perioperative perfusion in transition,1 case happened cerebral hemorrhage (＜ 10 ml).Postoperative symptoms were improved significantly.Followed up for 6-36 months,84 patients had not restenosis in stent,1 patient had symptomless restenosis (＜ 50％).In 37 patients with vertebral artery stenosis,3 patients again appeared ischemia symptoms in postoperative 1 year,digital subtraction angiography (DSA) showed restenosis in stent,the patient was again given endovascular stenting,the symptoms disappeared.Conclusion As long as the correct choice of indications and perioperative treatment properly,the endovascular stenting in treatment of upra-arch extra intracranial artery stenosis has good clinical effect and high security.

Objective To detect the chromosomal aberration in the cells of renal carcinoma and to evaluate the rela-tionship between the chromosomal aberration and development of renal carcinoma.Methods CGH technology was used to analyze the global genomic aberration in cancer cells from 12 patients with renal carcinoma.Results All of 12 cases deeeted by CGH showed chromosomal aberrations.The common extension regions of renal carcinoma were 1p,4p,5q,7p,9p and 16p.The common deletion regions of renal carcinoma were 3q,4q,6q,9q,14q and 18q.Conclusion The hereditary material of renal carcinoma is unbalanced.The extension and/or deletion of chro-mosome may facilitate the occurrence of renal carcinoma.

Objective To investigate the long-term effect of angioplasty of patients with symptomatic vertebrobasilar artery stent.Methods Thirty-three patients with symptomatic vertebral artery stenosis who underwent endovascular treatment stent.The clinical symptoms and stent restenosis were recorded.Results The clinical symptoms of 33 cases with angioplasty were improved significantly.The preoperative stenosis rate was (92.36 ±3.23)％ before operation and the average postoperative residual stenosis rate was (9.13 ±2.57)％ after operation.Clinical symptoms of 23 patients disappeared completely after operation,and 10 cases improved significantly.As to complications,1 case with severe basilar artery stenosis had a side numbness of extremities,1 case showed postoperative cerebral embolism,and return to normal after 3 days.When follow up periods lasted for 6-24 months,9 cases were with dizziness or other symptoms.Of which,29 cases were performed DSA imaging,the residual stenosis rate was (19.27 ± 5.62)％,3 cases of vertebral artery in-stent restenosis were reached over 50％.Those 3 cases were give stent angioplasty again and the uncomfortable symptoms disappeared.Conclusion Stenosis opeation can significantly alleviate the symptoms of cerebral ischemia and symptomatic vertebrobasilar artery,and improve the quality of life of patients.However,restenosis should paid more attention.

Objective: To study the possible mechanism of malnutrition in peritoneal dialysis patients. Methods: Multicenter prospective cohort study was designed . Forty-four peritoneal dialysis patients who survived two years after a cross-sectional study were included in the present study. We divided our patients into two groups according to the dietary protein intake(DPI) during follow-up period, DPI lower than 0.78 g/( kg?d) or DPI higher than 0.78 g/( kg?d). Patients' nutritional status was evaluated during the cross-sectional study and at the end of follow-up. 22 patients were divided into two groups according to the nutritional status improved or worsened in subjective global assessment (SGA) during the follow-up period. Besides nutritional status, patient's residual renal function, volume status and the incidence of cardiovascular event during the follow-up period were also evaluated. Results: Based on SGA , the incidence of malnutrition in low-DPI group was 60.9% in the cross-sectioned study and 45.5% after the follow-up. In high-DPI group, it decreased from 57.1% to 28.6%. The residual renal function declined significantly in worse nutritional status group, the incidences of volume overload and cardiovascular events during follow-up study were significantly higher in worse nutritional status group than that in improved nutritional status group. Conclusion: Malnutrition in peritoneal dialysis patients may not be simply due to lower protein intake. The changes in residual renal function, fluid overload and cardiovascular disease, accompanied with low dietary protein intake may be the possible cause of malnutrition in peritoneal dialysis patients.
