1.EFFECT OF VITAMIN A AND IRON SUPPLEMENTATION ON IRON STATUS DURING PREGNANCY

Objective: To observe the effect of vitamin A and iron supplementation on iron status during pregnancy. Methods: 167 pregnant women aged 21~33 years in the second trimester were recruited and randomly allocated to four groups: placebo; vitamin A (1.1 mg/d); iron(60 mg elemental iron); vitamin A and iron, for 10 weeks. Results:The incidences of vitamin A deficiency, anemia and iron depletion were 0.6%,7.8% and 6.2% in the second trimester respectively. Serum VA level was not changed in three supplemented groups but decreased in placebo group. Hemoglobin in all groups before and after supplementation was in normal range, with no significant differences. The improvement in serum ferritin, free erythrocyte proporphyrin and transferrin saturation was better in the group supplemented with VA+Fe than VA or Fe alone. Conclusion: The simultaneous supplementation of vitamin A and iron is better in improvement of iron status than supplementation of vitamin A or iron alone during pregnancy. Improvement of iron status by simultaneous supplementation of pregnant women with vitamin A and iron was better than supplementation with iron or vitamin A alone.
2.Emergency reparation and reconstruction for complicated tissue defects in hand

Objective To explore the operative methods and clinical outcomes in emergency or sube- mergency repair for the complicated tissue defects in hand in first stage applying microsurgical technique. Methods From Jan.,2000 to Aug.,2005,49 emergency cases of complicated hand tissue defects were re- paired in the first stage with replantation,reconstruction,free flaps,combined finger reconstruction and flap transplantation,including 21 cases in mini tissue mass replantation or reconstruction,15 cases in replantation combined with free flap transplantation,8 cases in replantation and reconstruction combined with free flap transplantation,5 cases in combined multiple digits reconstruction with free flap transplantation.The free flap transplantation included the anterolateral femoral flap,the latissimus dorsi myocutaneous flap,the dorsalis pe- dis flap,the media pedis flap and the instep flap.Results All the flaps,the replanted and reconstruceted finger survived uneventfully except for one replanted finger necrosis.45 cases healed in the first stage and the other 4 cases healed in the second stage.During a follow-up from 6 months to 3 years postoperatively,a satis- factory appearance and function of the reconstructed hand was achieved.The excellent and good rate was 85.7% assessed with provisional functional assessment criterion for upper limbs issued by Chinese Society of Hand Surgery.Conclusion The emergency or subemergency repair for the complicated tissue defects in hand has the advantage of short-term treatment and desirable functional outcome.The emergency replantation and reconstruction combined with various flaps or tissue mass can be applied to repair tissue defect in hand in the first stage according to the position and area of the defect along with the technique level of the surgeon, having been proved to achieve desirable clinical outcomes.And the key points leading to a successful operation is the correct treatment for the raw surface of the defects,suitable choice for various flaps,logical design of combination pattern and prevention and timely treatment for vessel crisis.
3.Using next-generation sequencing technology to evaluate biomarkers associated with anti-EGFR efficacy in metastatic colorectal cancer

Objective:To explore genes associated with sensitivity to anti-EGFR therapy. Methods:From March 2012 to August 2013, 31 metastatic colorectal cancer patients in Peking University Cancer Hospital&Institute were treated with anti-EGFR mono-therapy. A total of 21 genes associated with oncogenesis, metastasis, and EGFR signaling pathway were profiled in these 31 patients by using tar-geted next-generation sequencing technology. Results:A total of 31 patients with Kras exon 2 wild-type received anti-EGFR therapy as third-line treatment. Among these patients, the median progression-free survival (PFS) was 89 days, overall survival was 311 days, and objective response rate was 16.1%. Five cases harbored Kras exons 3/4 or Nras exons 2/3 mutations. These five Ras mutation patients showed disease progression during the first evaluation with 31-day PFS. One PIK3CA mutation case exhibited disease progression dur-ing the first evaluation (PFS 35 days), and one case showed mTOR mutation with 91-day PFS. The PFS of two cases with SMAD4 muta-tion were 58 and 59 days, whereas that of the case containing FBXW7 mutation was 93 days. Among the 26 Ras wild-type patients, MLL3, TP53, and APC were the three genes with the highest mutation frequencies of 92.3%(24/26), 53.8%(14/26), and 42.3%(11/26), respectively. Conclusion:Extended Ras analysis (including Kras and Nras exons 2/3/4) is recommended for patients who are candi-dates for anti-EGFR therapy. Mutations in the downstream effectors of the EGFR signaling pathway, such as PI3KCA and mTOR, may al-so have a predictive role in anti-EGFR therapy. Mutations beyond the EGFR pathway such as FBXW7 and SMAD4 may be associated with anti-EGFR efficacy and deserve further attention.
4.Effect of Wumeiwan on cytokines TNF-alpha, IL-6, IL-8, IL-10 and expression of NF-kappaBp65 in rats with ulcerative colitis.

The effects of Wumeiwan (WMW) on TNF-alpha, IL-6, IL-8, IL-10 and NF-kappaBp65 in rats with ulcerative colitis (UC) were investigated, the curative effectiveness of WMW vs salicylazosulfapyridine (SASP) was compared, and the action mechanism was analyzed. Fifty-Six Sprague-Dawley (SD) rats were randomly divided into four groups (n=14 in each group, with equal ratio of male and female): normal control group, model group, SASP group, and WMW group. Except normal control group, the rat UC models in the remaining three groups were established using the method of 2.4-dinitrochlorobenzene (DNCB) immunization and acetic acid local enema. The rats in model group, SASP group, and WMW group were treated with distilled water, SASP, and WMW respectively. The changes in the symptoms and signs were observed, and levels of IL-6, IL-8, TNF-alpha, IL-10 and the expression of NF-kappaBp65 in the colonic tissues were statistically analyzed. The results showed that the levels of IL-6, IL-8, and TNF-alpha were significantly increased (P<0.01), while those of IL-10 significantly reduced (P<0.01) after establishment of rat UC models as compared with normal control group. The levels of IL-6, IL-8, and TNF-alpha were obviously lower, but the level of IL-10 was obviously higher in WMW and SASP groups than those in model group (P<0.05). The levels of IL-6, IL-8, and TNF-alpha were lower, while the level of IL-10 was higher in WMW group than in SASP group. NF-kappaBp65 was expressed negatively or weakly in normal colonic tissues. The positive expression rate of NF-kappaBp65 in WMW group and SASP group was obviously lower than in model group (P<0.01), and there was significant difference between WMW group and SASP group (P<0.05). It was concluded that rat UC model was established successfully. WMW could up-regulate the expression of IL-10, down-regulate the expression of TNF-alpha, IL-6, IL-8, and inhibit the NF-kappaBp65 activity to adjust immune function, indicating WMW had better curative effects on UC in rats.
5.PI3K/AKT inhibitor influence on the radiosensitivity of breast cancer cells

Objective To evaluate whether Ly294002,suppressing phosphatidylinositol 3 kinase (PI3K)/AKT survival signaling pathway,can change the sensitivity of breast cancer cells to radiotherapy. Methods Breast cancer cultured MCF7 cells treated with:radiation alone;Ly294002;or the combination of radiation and Ly294002.The inhibition of PI3K/AKT by Ly294002 was confirmed by Western blot.Clo- nogenic assay was used quantitatively to measure the mitotic cell death,and caspase-3 assay was used to e- valuate apoptosis.Results 1.Ly29400 could partially inhibit phosphorylated AKT but not radiation,the combination of both could enhance the inhibition of phosphorylated AKT,2.Timing of exposing cells to Ly294002 had some impact on clonogenic survival by radiation,one hour pre-radiation and 10 days post-ra- diation exposing to Ly294002 could maximally sensitize the cells to irradiation,3.Ly29400 combined with radiation could synergistically enhance mitotic death and apoptosis of MCF7 cells,with SER of SF_4 and D_0, being equal to 1.25 and 1.42.Conclusions PI3K/AKT pathway may be a potential target for enhancing the response of breast cancer cells to radiotherapy.
6.The effects of a rotating magnetic field on Li pilocarpine-induced seizures in rats

Objective To study the effects of a rotating magnetic field(RMF)on Li pilocarpine-in- duced seizure activity and the expression of mGluR1 and mGluR5 in the hippocampus.Methods Thirty rats were divided into 5 groups.Each rat in the model(M),short treatment(ST)and long treatment(LT)groups was treated with intra-peritoneal injections of lithium chloride(60 mg/kg),followed by an intra-peritoneal injec- tion of pilocarpine(35 mg/kg)24 h later.The rats in the ST group were exposed to 20 mT RMF for 20 min ev- ery day for 3 d before seizure induction,while the rats in LT group were exposed to the same RMF for 8 d.The latency,severity and duration of seizure,as well as accompanying symptoms and electroencephalogram data, were recorded,and the expression of mGluR1 and mGluR5 was calculated using an electrophoretic imaging anal- ysis system.Results The duration,times and accompanying symptoms of seizure were significantly decreased in the LT group.The mGluR1 mRNA level and mGluR1/mGluR5 ratio in the M group were markedly increased, but the mGluR5 mRNA level was obviously decreased,while the expression of mGluR1 in the ST and LT groups was decreased,and mGluR5 was increased.Conclusions Seizure activity in rats can be inhibited by 20 mT RMF,and the expression of mGluRl and mGluR5 in the hippocampus of rats suffering seizures can be markedly influenced by longer-term RMF.

9.BiocompatibiIity of a noveI caIcium-magnesium phosphate composite cement in dentistry

BACKGROUND: In our preliminary experiments, calcium phosphate cement, magnesium phosphate cement, calcium silicate and bismuth were mixed to manufacture a novel inorganic composite material, which would be used to repair furcal perforation. This novel composite cement has been confirmed to have excellent physiochemical and mechanical properties. OBJECTIVE: To study the biocompatibility of calcium phosphate-magnesium phosphate-calcium silicate-bismuth cement (CMSBC). METHODS: Cytotoxicity test: Relative growth rate of L929 cells cultured in CMSBC extract was analyzed using cell counting kit-8 assay. Acute hemolysis test: The leaching solution of test samples, saline and distilled water were added into the rabbit anti-clotting, to detect the rate of hemolysis.Acute systemic toxicity test:CMSBC lead liquor and normal saline were respectively injected via the tail vein into the mice in experimental group and control group, respectively. The general situation of mice was observed at 24, 48, 72 hours respectively and body weight change was valued. Ames test: In the flat-plate incorporation test of Salmonella typhimurium, the average number of spontaneous revertants of TA97, TA98, TA100 and TA102 of CMSBC leaching liquor was calculated after incubation at 37 ℃ for 72 hours. S-9 mixture was added to the plates as an in vitro activation system. RESULTS AND CONCLUSION:The cytotoxicity of CMSBC in vitro was grade 0.Negative results from the acute hemolytic test,acute systemic toxicity test and Ames test of CMSBC indicate that this novel inorganic composite material has good biocompatibility and biosecurity.
10.Transurethral transumbilical laparoendoscopic single-site surgery for radical prostatectomy.

OBJECTIVETo investigate the feasibility and advantages of transurethral transumbilical laparoendoscopic single-site surgery (TU-LESS) for radical prostatectomy.
METHODSFive patients with prostate cancer underwent TU-LESS for radical prostatectomy, with a four-channel single-port device inserted into a 2. 5 cm periumbilical incision and another placed through the urethra, followed by analysis of the perioperative data.
RESULTSAll the operations were successfully accomplished, with neither conversion to open surgery nor additional channel. The mean operation time, intraoperative blood loss, and postoperative hospital stay were 168 min, 120 ml, and 15 d, respectively. No severe perioperative complications were observed. TNM stage classification manifested T2cN0M0 in 2 cases and T2bN0M0 in the other 3. Postoperative pathology showed no negative surgical margins in any of the cases.
CONCLUSIONTU-LESS is safe and feasible for radical prostatectomy and can reduce the complication of low urinary tract surgery by single-site laparoendoscopy.
