1.Causes and treatment of gastrointestinal complications following anterior approach to thoracolumbar fracture and dislocation

Objective To explore the causes and treatment of gastrointestinal complications after anterior approach for thoracolumbar fracture and dislocation.Methods A retrospective analysis was carried out in 153 cases with thoracolumbar fracture and dislocation undergone anterior approach from Jan- uary 1999 to October 2003.Postoperative gastrointestinal complication was seen in 15 cases including sev- en with dynamic intestinal obstructions,three with stress ulcer,three with intestinal bacteria imbalance, one with tuberculosis peritonitis resulted from dissemination of primary tuberculosis,and one with acute relapse of chronic appendicitis.Results All patients were effectively cured by using corresponding methods in regard of different causes.Conclusions(1)Gastrointestinal complications following ante- rior approach for thoracolumbar fracture and dislocation are mainly resulted from following causes,ie,se- rious primary trauma,primary gastrointestinal vegetative nerve injury or that caused by surgical operation, intraoperative contusions of abdominal viscera,postoperative retroperitoneal hematoma,acute lesion of gastric mucous membrane as well as imbalance of intestinal flora.(2)Correct treatment of primary trau- ma,meticulous operation,reasonable utility of antibiotics and appropriate management can effectively prevent and control gastrointestinal complications.

3.Therapeutic effect of puerarin on rats with pre-eclampsia

Objective To study the effect of puerarin on the nitrogen monoxide(NO) concentration in rats with pre-eclampsia. Methods Thirty SD rats were selected and divided into three groups: normal pregnancy group, pre-eclampsia model group and puerarin group(10 rats in each group). We used endotoxin to set up the pre-eclampsia model rats on gestation 14th day and puerarin to treat these rats. The 24 hour urine protein on gestation 8th,15th,19th days were determined, as well as the heart rate, blood pressure on gestation 10th,15th,19th days. Nitrate reductase method was used to determine the plasma NO concentration of each group on gestation 21st day. Results (1) There was significant difference in urine protein concentration between the puerarin [(73 ? 20) ?g],pre-eclampsia [ (464 ? 57)?g], and normal pregnancy [(140 ? 12)?g], groups (which is equal to the value of urine protein of pregnancy 19th day subtracting that of pregnancy 8th day, P0.05).(4)There was significant difference in NO concentration (NO concentration at gestation 19th day subtracting that of 8th day) between the three groups(P

Objective To evaluate the curative effect,adverse reactions and complications of transcatheter uterine arterial embolization (TUAE) therapy for leiomyoma of uterus with three different embolization agents: silk particles, pingyangmycin-lipiodol emulsion (PLE), polyvinyl alcohol (PVA).Methods One hundred and twenty-six patients of uterus leiomyoma were included, who undergoing TUAE treatment with silk particles(54 cases, silk particles group) ,PLE(40 cases,PLE group) and PVA(32 cases, PVA group) respectively.Adverse reactions and complications were recorded until 6-60 months after TUAE treatment.Measured the volume of the leiomyoma by ultrasonic inspection or enhanced computed tomography (CT).Results All clinical symptoms of three groups were relieved, and the volumes of uterus leiomyoma after TUAE treatment of 6,12,24 months in PVA group [(50.84 ± 30.77), (39.92 ± 25.72), (31.24 ± 30.60) cm~3]were less than those in PLE group[(76.24 ± 31.16), (58.21 ± 32.05), (52.48 ± 31.41 ) cm~3]and silk particles group[(69.22 ± 36.22), (68.31 ± 32.42), (68.01 ± 30.12) cm~3].The inferior belly and perineal region pain were most slight in silk particles group, but no significant difference in complications was observed in three groups.The symptom recurrence was 3 cases, 2 cases in silk particles group and 1 case in PIE group.Conclusions Clinical outcome of TUAE treatment with all the three embolization agents is acceptable.Adverse reactions and complications are associated with volume of the leiomyoma and the area being embolized.PVA is superior in prevention of complications and relapse of uterus leiomyoma after TUAE treatment.

7.Antioxidative and antiapoptotic effects of the Attractin gene on Sertoli cells in mice.

OBJECTIVETo evaluate the effects of Attractin (Atrn) silence on the anti-oxidative and anti-apoptotic abilities of TM4 Sertoli cells and its influence on the expressions of superoxide dismutase (SOD) and caspase6 in the cells.
METHODSWe observed the apoptotic indexes of TM4 Sertoli cells with normal expression (control), partial deletion, and complete deletion of the Atrn gene (psiRNA-TM4, psiAtrn-TM4, and mu-SC). We determined the mRNA and protein expressions of SOD and caspase6 by Q-PCR and Western blot, measured the SOD activity and malondialdehyde (MDA) contentby spectrophotometry, and detected the apoptotic index of the cells by TUNEL.
RESULTSCompared with psiRNA-TM4, after inhibition of the Atrn expression, the Sertoli cells in the psiAtrn-TM4 and mu-SCgroups showed significantly decreased expressions ofSOD mRNA (70.76% and 92.58%) and protein (65.11% and 71.0%) (both P < 0.05). The levels of caspase 6 mRNA and protein were increased 5.28 and 3.40 times in the psiAtrn-TM4 and 2.97 and 2.50 times in the mu-SCgroup as compared with the normal control (both P < 0.05). Atrn deletion markedly increased the apoptotic indexes of the cells in the psiAtrn-TM4 and mu-SC groups by 16.22% and 22.03% (P < 0.05) and reduced the activity of SOD by 23.00% and 39.37% (P < 0.05); it also elevated the level of MDA by 155.22% (P < 0.05).
CONCLUSIONThe Atrn gene exerts influence on the function of Sertoli cells in multiple ways, in which antioxidative stress and apoptosis regulation may play an important role.

8.Assessment of regional myocardial function in patients with hypertrophic cardiomyopathy by tissue strain imaging.

The value of tissue strain imaging (SI) in regional myocardial systolic and diastolic function assessment was studied. In 18 patients with nonobstructive hypertrophic cardiomyopathy (HCM) and 20 age-matched healthy subjects, regional myocardial longitudinal peak systolic strain in eject time (represented by epsilon(et)) was measured at basal, mid and apical segments of septal, lateral and posterior walls of the left ventricle (LV) and compared between groups. Epsilon(et) had no significant difference between segments in control group (P > 0.05), which displayed a decreasing trend from basal segments to apical ones. Epsilon(et) in the HCM group was significantly decreased (P < 0.05) as compared with that in the healthy group. In the HCM group, epsilon(et) in the midseptum was significantly less than at the basal and apical septum, and was also less than at the rest LV walls in the same group (P < 0.01). The systolic reversed epsilon(et) was noticed in 35% of the hypertrophic segments in HCM group. Significantly negative correlation existed between the absolute value of epsilon(et) and wall thickness in the midseptum (r = -0.83). The post-systolic strain (PSS) segment number the and amplitudes in healthy group were significantly less than those in HCM group (P < 0.05). Both regional myocardial systolic and diastolic functions were impaired in hypertrophic or non-hypertrophic segments in patients with the HCM, especially in hypertrophic segments. Strain imaging technique is a sensitive and accura tool in myocardial dysfunction assessment.
9.Diagnostic value of HNF-1βin the diagnosis of ovarian clear cell carcinoma

Purpose To study the expression of HNF-1β in different type ovarian carcinoma and to explore the diagnostic value of HNF-1β in the diagnosis of ovarian clear cell carcinoma. Methods Immunohistochemical EnVision method was used to detect the ex-pression of HNF-1βin 27 clear cell carcinomas, 35 high-grade serous carcinomas, 21 endometrioid adenocarcinoma, 10 mucious carci-nomas, 13 metastatic Krukenberg tumors, and 2 transitional carcinomas. Results All of ovarian clear cell carcinomas variably ex-pressed HNF-1β and the positive rate of HNF-1β in ovarian cleat cell carcinoma was 85. 2%. Of 21 ovarian serous carcinomas, the nuclear positive of HNF-1β was 4 and HNF-1βpositive rate was 2. 9%. There was 5 HNF-1βexpression in endometrioid adenocarci-noma and the positive rate of HNF-1β was 23. 8%. The positive rate of HNF-1β in mucious carcinoma and metastatic Krukenberg tumor was 60. 0% and 53. 8%, respectively. No HNF-1βexpression was seen in transtional cell carcinoma. The sensitivity and speci-ficity of HNF-1β in the diagnosis of ovarian clear cell carcinoma was 85. 2% and 76. 5%. Conclusions HNF-1β may be a higher sensitive marker for the diagnosis of ovarian clear cell carcinoma. The diffuse and strong positive of HNF-1βmay have higher specific for diagnosis of ovarian clear cell carcinoma.
10.Quality Analysis of Three Components in Shuanghuanglian Powder for Injection Before and After Ultrasonic Atomization

Objective To establish a new RP-HPLC method for simultaneous determination of chlorogenic acid,forsythin, and baicalin in shuanghuanglian powder for injection after ultrasonic atomization. Methods Hypersil ODS2 C18(250 mmí4. 6 mm,5 μm) was used as the chromatographic column. The mobile phase was methanol-0. 2% phosphate acid solution (4060). Flow rate was 1. 0 mL·min-1 . Sample volume was 5μL. Column temperature was 30℃. Detection wavelength was 324 nm at 0-10 min and 277 nm at 10-25 min. Results Contents of chlorogenic acid,forsythin, and baicalin had good linear relationship with the respective peak area (r≥0. 999 7) within the scope of the sample volume. The RSD was <2% for precision, reproducibility, and stability. Recovery rate was 98. 50%-101. 12% (n=6). Conclusion The method is rapid, accurate and reproducible, with high resolution. It can determine the content of three kinds of components at the same time. The three components in shuanghuanglian powder for injection did not change significantly before and after ultrasonic atomization.
