1.Clinical,imaging and pathologic features of hypertrophic cranial pachymeningitis

Objective To explore the clinical,imaging and pathologic features of hypertrophic cranial pachymeningitis (HCP).Methods The clinical data of one HCP patient who treated in our hospital and other 77 HCP patients from literatures were analyzed retrospectively.Results The clinical manifestation of all the 78 HCP patients had chronic headache,multiple cranial nerves impairment.The secondary was psychiatric disorder(10.3%),ataxia(9.0%) and seizure disorder(6.4%).Hemiplegia,menorrhea and galactosis were found few.Headache was the first onset symptom in the 74 cases (94.9%).HCP was often misdiagnosed subarachnoid hemorrhage,hypotensive cranial pressure headache and cerebral venous sinus thrombosis in early stage.MRI demonstrated local or diffused thickened dura,especially in cerebral falx and/or tentorium of cerebellum,which could be enhanced through reinforced scanning.Pathological evidence indicated an obvious proliferation of dura fiber tissue accompanied with inflammatory cells infiltration.Corticosteroid was effective to all the cases.Conclusions The clinical manifestation of HCP was multiplicity,but it mainly was chronic headache and multiple cranial nerves impairment.MRI demonstrates local or diffused thickened dura especially in cerebral falx and/or tentorium of cerebellum.MRI has an important significance for diagnosis.
2.Researching Progress of Tumor Lymphangiogenesis and Lymphatic Metastasis

Objective To understand the features of lymphatic vessel,and to summarize the foundation and mechanism of the promotion and inhibition of tumor lymphangiogenesis recorded on the current studies of animal ex- periments and clinical researches.Methods The related literatures of the structural features of lymphatic vessel, lymphatic endothelial molecular markers,the origin of lymphatic tumors,the molecular mechanisms and regulatory factors were reviewed,and the relationship between tumor lymphangiogenesis and lymphatic metastasis,the treat- ment targeting at the formation of the anti-tumor lymphatic vessel and its existing problems were also analyzed.Re- suits Hyperplasia of lymphatic vessels occurred during the process of tumor formation and progression.The struc- tural features of the lymphatic vessels in the tumor were conducive to tumor lymphatic metastasis.In recent years, methods of anti-lymphangiogenesis and inhibition of tumor lymphatic metastasis had achieved considerable success in animal experiments.However,there were still a lot of problems need to be solved.Conclusion Tumor lymphangio- genesis has a significantly positive correlation between tumor lymphatic metastasis and patients'prognosis,which may indicate that treatment against the formation of tumor lymphatic vessel maybe effective.
3. Research on dose-effect relationship of Wuwei Xiaodu Drink on antibacterial metabolism by MSPQC

Objective: The effect of Wuwei Xiaodu Drink (WXD) on the metabolism of bacteria and the dose-effect relationship were investigated by MSPQC. Methods: The frequency shift-time curves of the growth and metabolism of Escherichia coli, Staphylococcus aureus, and Pseudomonas aeruginosa affected by WXD were obtained by using MSPQC. The minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) were gained according to FDT and the frequency shift. The dose-effect relationship was analyzed according to the curve which represented the relationship between the concentration of WXD and FDT. The parallel test was carried out with the traditional tube dilution method. Results: The results indicated that WXD inhibited the growth of E. coli, S. aureus, and P. aeruginosa, which was dose-dependent. The higher the concentration was, the stronger the antibacterial effect became. The MBC of WXD for three kinds of pathogenic bacteria was 0.9 g/mL. The MIC of WXD for S. aureus and P. aeruginosa was 0.8 g/mL, however for E. coli was 0.7 g/mL within 24 h. Conclusion: MSPQC is a sensitive, quantitative, quick method, which could provide the process information in real time for determining the dose-effect relationship of antimicrobial Chinese materia medica.
4.Effects of Self-Management Education on Quality of Life in Children with Asthma

Objective To study the effects of self-management education on quality of life in children with asthma.Methods Seventy-five out-patients and in-patients with asthma were randomlydivided into 2 groups.The educational group(38 cases)received the basic knowledge education of asthma and self-management,while the control group received only the basic knowledge education of asthma.A inquiry was conducted in all patients using a Chinese PAQLQ(pediatric asthma quality of life questionnaire)before and 6 months after the treatment.The correlation and difference between 2 groups were analyzed.Results There were no significant difference before the therapy.After 6 months's treatment,there were uptrends on scorecard in some fields in such as symptom,activity,emotion and the total score in all patients(P
5.Expression of Fas/FasL in Children with Aplastic Anemia and Its Clinical Significance

Objective To investigate the expression of Fas/FasL in children with aplastic anemia(AA) and its clinical significance.To explore the pathogenesis of AA.Methods CD34+ cell counts and the expression of Fas/FasL in 12 children with severe AA(SAA),18 with chronic AA(CAA)and 10 normal children were detected by flow cytometry combining monocolon antibody.Results The expression of Fas in children with AA were significantly higher than that in control group(P0.05).Conclusions Fas/FasL systeam take part in inducing CD34+ cell apoptosis.This may contribute to understanding the decrease number of stem cells and bone marrow failure.
6.Detection of CFTR gene mutations in azoospermia patients with congenital unilateral absence of the vas deferens.

OBJECTIVETo discuss the results and significance of the detection of the CFTR gene mutation in azoospermia patients with congenital unilateral absence of the vas deferens (CUAVD).
METHODSWe collected peripheral blood samples from 6 azoospermia patients with CUAVD for detection of the CFTR gene mutations and single nucleotide polymorphisms. We analyzed the genome sequences of the CFTR gene in comparison with the website of the UCSC Genome Browser on Human Dec. 2013 Assembly.
RESULTSMissense mutation of c. 592G > C in exon 6 was found in 1 of the 6 azoospermia patients with CUAVD and splicing mutation of c. 1210-12T[5] was observed in the noncoding region before exon 10 in 2 of the patients, both with the V470 haplotype in exon 11.
CONCLUSIONMutations of the CFTR gene can be detected in azoospermia patients with CUAVD and the detection of the CFTR gene mutation is necessary for these patients.

8.Common gram-negative bacilli lower respiratory tract infection in PICU: analysis of pathogens and drug resistant pattern from 2004 to 2009

Objective To study common Gram negative bacterial pathogens and their drug resistant pattern in sputum isolates from patients in pediatric intensive care unit (PICU) of Chengdu children' s hospital. Methods Comparing to the period of 2004-2006 with 2007-2009, we retrospectively reviewed eligible sputum isolates of common Gram negative bacilli and their drug resistance pattern. All specimens were processed according to standard microbiological methods for both quantitative and qualitative cultures. Pathogen was the single predominant growth after two consecutive cultures. Strict quality control was used in all test agents and paper disk for drug resistance analysis. Results Total of 598 Gram negative bacterial isolates were identified in patients over 6 years. The first 5 bacteria respectively were Escherichia coli 167 samples(27.93%), Klebsiella pneumoniae 109 samples ( 18.23% ), Enterobacter cloacae 93 samples ( 15.55% ),Acinetobacter baumannii 64 samples ( 10. 7% ), Pseudomonas aeroginosa 43 samples (7. 19% ). They were mostly sensitive to Imipenam, Amikacin, Piperacillin/Tazobactam. Most drug resistance rate was found in Amoxicillin, Ampicillin, Cephalosporin. Apart from Pseudomonas aeroginosa and Acinetobacter Baumannii,no Gram negative bacteria was found to be resistant to Imipenam. Klebsiella pneumoniae and ESBL secreting Escherichia coli isolates were obviously increasing when compared to 3 years ago. Conclusion Comparing the period of 2004-2006, the Gram negative bacterial isolates were increasing in our PICU patients during 2007 to 2009. Close monitoring of drug resistance pattern changes in PICU would guide rational use of antibiotics in these critically ill children.
9.Effect of checklist tool implemented in the safety management of clinical blood transfusion in intensive care unit

Objective To explore the method and effect of checklist tool implemented in the safety management of clinical blood transfusion in intensive care unit. Methods A checklist was specially designed according to the technical standard and check system of clinical blood transfusion. 379 patients prescribed a blood transfusion enrolled in ICU from January to May 2013 were conducted as the control group before the checklist was designed. After the application of checklist during the blood transfusion process, 846 patients prescribed a blood transfusion enrolled in ICU from June to December 2013 were conducted as the experimental group. The incidence of blood transfusion safety related adverse events between the two groups was compared. Results The incidence of adverse events was 1.32% (5 of 379 patients) in the control group, and there was no sign of adverse events occurred in the experimental group. The difference had statistical significance (χ2=11.21, P<0.05). Conclusion The application of checklist reduced the incidence of blood transfusion safety related adverse events,and effectively improved the quality of nursing safety and patient satisfaction.
10.Prenatal ultrasound diagnosis of otocephaly and review of the literatures

Objective To investigate the ultrasound characteristics of prenatal fetus otocephaly . Methods Three prenatal fetus with otocephaly were examined with two -and three-dimensional ultrasoundand examined results were compared with the those of induced labor or autopsy ,and ultrasound characteristicsof prenatal fetus were analyzed and summarized .Results The ultrasound performance of three prenatal fetuswith otocephaly and the examination of the appearance after induced labor showed :(1) The most intuitiveinitial sonographic performance of otocephaly was manifested by the absence of stomach bubble andoverabundance of amniotic fluid.Among three fetus,one fetus had overabundance of amniotic fluid at the midstageof pregnancy,one fetus had normal amniotic fluid at the mid-stage of pregnancy and one fetus hadextremely high amount of amniotic fluid and absence of stomach bubble at late stage of pregnancy .(2) Allthree fetus showed agnathy and synotia (shifts of both ears to the midline) and microstomia deformity.(3) All three fetus had associated complications with deformity in other systems including two cases of patients withcleft lip and palat,both were the fracture unilateral cleft lip derived from small mouth .One fetus withdysmelia and one fetus with complicated cardiovascular deformity and situs inversus and .(4) The results ofexamination after induced labor or autopsy were consistent with those of the prenatal ultrasound examination . Conclusions Prenatal ultrasound examination is an effective and feasible means for the diagnoses ofotocephaly.When the symptoms of “absence of stomach bubble and extremely high amount of amniotic fluid ”occurred,the fetal ear and submaxilla should be examined to confirm stand -alone otocephaly prenatally.
