1.Clinical study on severe acute pancreatitis complicated by hepatic insufficiency

OBJECTIVE: To investigate the pathogenic factors of severe acute pancreatitis (SAP) complicated by hepatic insufficiency, the prognosis and the effective preventive and therapeutic interventions. METHODS: One hundred and fifty-two patients with SAP (from January 2003 to June 2004) were divided into 2 groups, SAP with hepatic insufficiency group and SAP without hepatic insufficiency group. The related factors such as causes of disease, serum biochemical criteria, complications, mortality and course of disease were observed. RESULTS: The concentrations of serum amylase, creatinine and lactate dehydrogenase were all much higher in the SAP with hepatic insufficiency group than those in the SAP without hepatic insufficiency group (P<0.05). The incidences of complications such as acute renal failure, heart failure, gastrointestinal hemorrhage and infection were also higher in the SAP with hepatic insufficiency group than those in the SAP without hepatic insufficiency group (P<0.05). There was no significant difference in mortality between these two groups, but the course of disease in SAP with hepatic insufficiency group was longer than that in the other group (P<0.05). CONCLUSION: The causes of SAP complicated by hepatic insufficiency are related to the imbalance of internal environment and the characteristics of the liver function. To control the biliary tract diseases, intervene with traditional Chinese medicine, recover the intestinal function in time, reduce the damage of pancreatic enzyme, maintain the stability of internal environment and avoid using drugs that can induce liver injury are all important aspects of reducing the incidence of hepatic insufficiency.

3.Expression of CD1a+ DC in fair follicle of human normal skin and psoriasis

Aim To explore the expression of CD1a+ DC in fair follicle of normal human skin and psoriasis patients. Methods Expressions of CD1a+ DC in 10 fair follicles of psoriasis patients and skins of 8 normal human were detected by SABC immunohistochemical staining. Results The numbers of CD1a+ DC in upper limb skin and scalps's fair follicle were 13 and 15 respectively. The number of CD1a+ DC in fair follicle of psoriasis patients was 16,slightly more than that in skins. There was obviously higher density of CD1a+ DC in psoriasis fair follicle and its number was 46± 15/mm2. Conclusion The area of fair follicle is connected with the start and redistribution of CD1a+ DC,and the fair follicle may have important effect on development of vulgaris psoriasis.
4.Interventional mechanical thrombectomy treatment for chronic vascular thrombus

Objective To investigte the possibility of interventional mechanical thrombectomy dealing with chronic vascular thrombus, and assessing its curative effects. Methods Twenty three cases included 17 males 6 females, and aged from 46 to 80 year with a mean of 64.8. All patients were coinsided with MRA, vascular ultrasound and angiography, in which 12 patients with chronic heart and vascular disease, and 11 patients with serious diabetes. Thrombi were located in iliac artery ( n=9,L=4,R=3 , bilaterals=2), femoral artery( n =2), populiteus artery ( n =2), popliteus artery( n =3), iliac vein ( n =7, L=5,R=2), portal vein ( n =1). The length of thrombi varied from 3 to 8 cm, and the diameter from 5 to 10mm.Filter should be placed in inferior cava vein before venous thrombus treatment ( n =7). Mechanical thrombectomy was undertaken as following: ATD ( n=9, Vein=7), PTD( n =2), and Oasis( n =1). After guide wire passing through thrombus segment, Oasis was inserted for remove thrombus. Urokinase (500 000U) was usually administered for catheter directed thombolysis before/during mechanical thrombectomy. Stent should be placed in the segment if stenosis was confirmed after thrombectomy angiography. Drugs were used after thrombectomy including thromboyltic drug, heparinzed anticoagulation and changing microcirculation. Vascular ultrasound, reconstruction CTA and clinical follow up had all been carried out. Results 12 cases (85.7) were undergone successfully thrombectomy. One case failured with portal vein thrombus formation, the other with multiple segments involvement in femoral artery was treated by combined ATD and Oasis. Sy mptoms of 15 cases were either released or disappared, including ischemia, swelling and motion limitations. The patency shown by vascular ultrasound follow up were 100% in three months, 85.4% in six months, 73.2% in twelve months, with simultaneously blood flow improvement to normal and obviously corrected 76.5%,65.4% 60.1% in 3,6,12 month respectively.Conclusions Interventional mechanical thrombectomy is a new choice of treating chronic vascular thrombus with its direct curative effects, but still needs long term follow up.
5.Effect of L-NNA on GFAP production in the gerbil hippocampus after recirculation following ischemia

AIM To observe the effects of L NNA on glial fibrillary acidic protein (GFAP) production in the gerbil hippocampus after recirculation following ischemia. METHODS Both vessels occlusion and immunofluorescent methods in gerbil hippocampal tissue slices incubated in vitro were used. RESULTS Recirculation following ischemia led to a rise on GFAP concentration in the hippocampus. GFAP positive cells mainly distributed in rediatum layer and molecullar layer in the hippocampus, L NNA decreased GFAP synthesis. CONCLUSION L NNA is a strong inhibitor of nitric oxide synthase(NOS). It is possible that the role of endogenous NO mediated GFAP synthesis is inhibited by L NNA.

The study is designed to investigate if human interleukin 18 (hIL-18) cDNA expressed in nasopharyngeal carcinoma (NPC) cells could enhance the cytotoxicity of CD8+ T cells from the PBMC of the MFC-bearing patient and how to enhance it.Methods: A constructed eukaryotic expression vector for human IL-18 ,pcDNA3.1(-)/hIL-18, was used to transfect the NPC cell strain-SUNE cells. Then, the transfected SUNE cells were mix-cultured with CD8+ T cells which were separated from the PBMC of the patient with NPC, the cytotoxicity was measured by lactate dehydrogenase(LDH) releasing method, the expression for Perforin, Fas-L and Granzyme B were detected with the mix-cultures. Results: The eukaryotic expression vector could highly express hIL-18 in transfected SUNE cells and only traces of the protein exist in non-transfected SUNE cells. The concentration of hIL-18 in supernatant of the transfected SUNE cells was (85 ? 10) pg/ml, but less than 5 pg/ml of hIL-18 contained in supernatant of the non-transfected SUNE cells.The expression of hIL-18 in SUNE cells could enhance the cytotoxicity of CD8+ T cells from the NPC patient significantly( P
7.Anesthetic efficacy of epidural ropivacaine mixed with different doses of snfentanil for hysterectomy

Objective To assess the efficacy of ropivacaine in combination with different doses of sufentanil for epidural anesthesia in patients undergoing hysterectomy. Methods Eighty ASA Ⅰ or Ⅱ patients aged 30-55 yrs weighing 40-70 kg undergoing elective hysterectomy were randomly divided into 4 groups ( n = 20 each) : ropivacaine group (R) and 3 ropivacaine-sufentanil groups (R-S1-3). The patients were unpremedicated. ECG, BP, HR and SpO2 were monitored during ansthesia. Each patient received an epidural catheter placed at L2,3 interspace. After correct placement of epidural catheter was confirmed 0.75% ropivacaine 13 ml and normal saline (NS) 2 ml were given through epidural catheter in group R whereas in the 3 R-S groups 0.75% ropivacaine 13 ml and sufentanil 10 (R-S1), 20 (R-S2) or 30 (R-S3) ?g in NS 2 ml were injected into epidural space. BP, HR and SpO2 were recorded every 3 min. The onset time, upper spread and duration of sensory block; onset and duration of motor block (Bromage scale); degree of abdominal muscle relaxation; level of sedation (OAA/S scale); anesthetic efficacy and side-effects were recorded. The dose-response curve constructed by probit regression analysis was established to calculate ED50 and ED95. Objective To explore the determination of elements selenium content in Se Semen Cassiae. Method Selenium content in Se Semen Cassiae was measured by graphite atomic absorption spectrophotometer. Result and Conclusion An appropriate extraction method and digesting condition were screened out. The determined results were accurate when the digested sample was dissolved and metered volume with 1% HNO3 which containing 0.1% Tween 80.
9.Comprehensive Assessment of Septic Encephalopathy in Children

Objective To investigate the clinical features and characteristics of imaging,cerebral hemodynamics and electrophysiology of septic encephalopathy in children.Methods The clinical data and examination result of computerized tomography(CT),ultrasound and electroencephalogram(EEG) on encephalon of the 44 consecutive children with septic encephalopathy from Jan.2003 to Jun.2006 were reviewed retrospectively.According to the prognosis and the etiology,the totally 44 patients were divided into survival group,death group and pneumonia group,enteritis group,septicemia group,appendicular perforation group,skin gangrene group as well as EBV infection group,respectively.Fi-sher′s exact test was used to compare the differences of ratio.Results 1.Clinical manifestations:loss of consciousness was observed in all 44 cases,convulsions in 29 cases,upper-right-limb paralysis in 1 case,cerebral hernia in 1 case,and 14 cases resulted in death.2.Forteen cases were detected by CT scanning.Multiple local and diffuse lower density in cerebral parenchyma was observed in 3 cases and 2 cases respectively.Intracranial hemorrhage was found in the latter 2 cases.3.Color doppler ultrasound was detected in 11 cases.Diffuse brain edema was found in 5 cases,and among them intracranial hemorrhage was found in 1 case.4.EEG was detected in 10 cases.Flat-to-isoelectric EEG was found in 3 cases.Diffuse slow wave activities and sharp wave with sharp-slow wave complex were found in 5 cases and 2 cases,respectively.5.Between the sharp wave group and the flat-to-isoelectric EEG group,the number of cases who was less than 7 with Glasgow coma scale was significant(P
