1.Effect of L-NNA on GFAP production in the gerbil hippocampus after recirculation following ischemia

AIM To observe the effects of L NNA on glial fibrillary acidic protein (GFAP) production in the gerbil hippocampus after recirculation following ischemia. METHODS Both vessels occlusion and immunofluorescent methods in gerbil hippocampal tissue slices incubated in vitro were used. RESULTS Recirculation following ischemia led to a rise on GFAP concentration in the hippocampus. GFAP positive cells mainly distributed in rediatum layer and molecullar layer in the hippocampus, L NNA decreased GFAP synthesis. CONCLUSION L NNA is a strong inhibitor of nitric oxide synthase(NOS). It is possible that the role of endogenous NO mediated GFAP synthesis is inhibited by L NNA.
2.Effect of Formula Granule of Siwu Drugs on Sex Hormone and Its Receptor in Bone Marrow Depressed Mice

Objective: To investigate the effect of abstracted formula granule of Siwu drugs on the sex hormones and expressions of androgen receptor (AR)and estrogen receptor (ER) in bone marrow depressed mice. Methods: One hundred and eighty mice (90 male and 90 female) were randomly divided into 6 groups. The effects of the formula granule of Siwu on sex hormones and expressions of AR and ER were explored by SABC method in bone marrow of depressed mice. Results: For the male rats, the levels of androgen were increased in male of treatment groups compared with those of model groups (P<0.05), and the levels of estrogen were decreased in male of treatment groups compared with those of model groups (P<0.05).For the female rats, the levels of androgen were increased in female of treatment groups compared with those of model groups,but no significant differences (P>0.05), and the levels of estrogen were decreased in femal of treatment groups compared with those of model groups (P<0.05). There was a dose dependent manner between treatment groups. Conclusion: The results suggested that formula granule of Siwu drugs may play a role in the hematopoietic function to repair bone marrow depression by improving AR and the balance of estrogen / androgen in mice.
3.Metal on Metal Surface Replacement of Hip in Young Patients with Rheumatoid Arthritis of Aseptic Necrosis of Femoral Head

PurposePresent study investigated the role of metal on metal surface replacement of the hip in treating the young patients suffering rheumatoid arthritis or aseptic necrosis of femoral head.Methods Between Jan. 1995 and Dec. 1996.16 patients with 17 hips( rheumatoid arthritis 14, aseptic necrosis of femoral head 3) were resurfaced with hybrid metal on metal surface replacement prothesis. Patients were evaluated by self assessment form,hip function examination. Radiographs and Harris hip score. The average follow up time was 58.2 months. ResultsNo patient received revision. Radiographs showed that all implants were in position satisfyingly. No loosening were revealed. The result of self assessment showed that patients were satisfied with 16 hips. The Harris score rose from preoperative 44(31 - 52) to postoperative 91 (79- 96). ConclusionsThe metal on metal surface replacement of hip is a useful treatment to release pain and keep the hip function of young patients suffering rheumatoid arthritis or aseptic necrosis of femoral head.
4.Assessment of left atrial function by speckle tracking imaging and real-time three-dimensional echocardiography in patients with essential hypertension

Objective To evaluate left atrial(LA) function in essential hypertensive patients by speckle tracking imaging (STI) and real-time three-dimensional echocardiography (RT-3DE).Methods Thirty seven patients with essential hypertension were divided into normal geometric left ventricle(LV) group (LVN, n = 17) and remodeling geometrc LV group (LVR,n = 20).Twenty-six healthy participants were also include.The strain rate curve was auquired in each LA segment (anterior, posterior, lateral, inferior and septal) by STI. The LV mass, LA maximal, minimal and pre-systolic volume (LAVmax, LAVmin and LAVp), LA passive ejection fraction (LAPEF), LA active ejection fraction (LAAEF) were measured by RT-3DE. Results ①Compared with the control group, the mean peak systolic strain rate(mSR_S) in LVN group was lower(P <0.05), and the mSR_S, mean peak early diastolic strain rate(mSR_E) in LVR group were reduced (P <0.01, P<0.05), and the mean peak late diastolic strain rate (mSR_A) in LVR group was increased (P<0.01).②Compared with the control group,the LAVmax, LAVmin and LAV_P all increased in LVN and LVR group(P <0.05 or P <0, 01), the LAPEF was degraded (P<0.05),and the LAAEF was increased (P<0.05) in the LVR group.③A significant relation between mSR_E and LAPEF(r = 0.788, P <0.01), mSR_A and LAAEF(r = 0.804, P<0.01) could be found in the LVN and LVR group. Conclusions STI and RT-3DE may play important roles in the evaluation of left atrial function. The deformation ability of LA wall may decrease before left ventricular remodeling and it showed a significantly positive correlation with LA ejection function in patients with essential hypertension.
5.Efficacy of Total Pelvic Floor Reconstruction with Pesh Match for Presby-pelvic Floor Dysfunction

Objective To compare the efficacy of total pelvic floor reconstruction with mesh patch and vaginal hysterectomy with anterior and posterior vaginal wall repair for presby-pelvic floor dysfunction patients.Methods We randomly chose 92 pelvic floor dysfunction inpatients of our hospital from September 2007 to February 2009 and divided them into 2 groups:total pelvic floor reconstruction group(n=70) underwent total pelvic floor reconstruction with mesh patch[12 patients of this group complicated with stress incontinence,4 patients received tension free vaginal tape-obturator(TVT-O) simultaneously];control group(n=22) received vaginal hysterectomy with anterior and posterior vaginal wall repair(3 patients of this group complicated with stress incontinence).A follow-up for 3 to 18 months after the surgery were taken in all the patients.Results In total pelvic floor reconstruction group,no recidivate case was found and the FOP scores and cure rate were 0 and 100% respectively.In control group,there were 7 recidivate cases,all of who were primary vagina anterior wall bulge patients.The cure rate of control group was 68%;For the patients complicated with stress incontinence,they were all cured by TVT-0 or by ascending the front patch in total pelvic floor reconstruction group,but 1 case was as before operation and 2 cases were even worse in control group.Conclusion Total pelvic floor reconstruction with vicarious materials had a definite efficacy for presby-pelvic floor dysfunction.
6.The antiglide plate for the Danis Weber type B ankle fracture in aged patients

Objective To assess the clinical results of antiglide plate in the treatment of the Danis Weber type B aged ankle fracture. Methods Totally 36 aged cases of Danis Weber type B ankle fracture received ORIF (open reduction with internal fixation) with antiglide plate. Their functional results were evaluated according to the ankle scoring system. We also observed the complications, including failure of fixation, infection, wound dehiscence, and need for hardware removal. Results The clinical results were satisfactory, with the excellent and good ratio of ankle score being 91.6%. Conclusion Since the antiglide plate has theoretical advantages over the conventional lateral plate, it is recommendable for stabilization of Danis Weber type B ankle fractures.
7.Pathogenic Bacteria in Hospital:Distribution and Drug Resistance of 1 213 Strains

OBJECTIVE To determine the distribution of bacterial flora in hospital infection and to provide laboratory(evidence) for controlling hospital infection and selecting rationally antibiotics in clinic practice.METHODS All(isolates) were identified by routine procedure.MRSA and ESBLs-producing rate of Escherichia coli and Klebsiella pneumoniae were(examined.) RESULTS Among all these clinical infectious specimens,there were 202 strains of Gram negative bacilli,(accounting) for 40.9%(202/495);166 strains of fungi,accounting for 33.5%;621 strains of Gram positive cocci,for 20.6%(102/495).Candida albicans,E.coli,Pseudomonas aerugionosa,C.tropicalis and C.glabrata took the first five bacteria in infection.Analysis of drug resistant bacteria suggested that the isolated rate of ESBLs-producing strains in Staphylococcus aureus be 47.6%,be CNS in MRCNS 78.1% and MRSA in SA be 42.3%.CONCLUSIONS Multidrug resistance and fungus infection are the main risk factors in our hospital.We must improve means of treatment on clinical work and use antibiotic rationally to reduce the infection rate.
8.Determination of prulifloxacin active metabolite in human plasma and urine by RP-HPLC

Objective To establish a RP-HPLC method for determining the concentration of prulifloxacin active metabolite in human plasma and urine.Methods The supernatant obtained by centrifugation after the sample was precipitated with methanol- acetonitrile (1:1) was chromatographically separated on a Diamonsil C_(18)(250 mm?4.6 mm,5?m) using a mobile phase con- sisting of acetonitrile and 0.05 mol/L potassium dihydrogen phosphate (pH2.2) containing 1% tetrabutylammonium bromide. The solutions of 20:80 (V/V) and 12:88 (V/V) at a flow rate of 1.0 mL/min and 1.6 mL/min were used for plasma and u- rine, respectively.Then the samples were assayed at wavelength of Ex 280 nm and Em 425 nm.Results The linear range for prulifloxacin active metabolite in plasma and urine were 0.005-5 mg/L (r=0.9999) and 0.05-5 mg/L(r=0.9999)with a low- er limit of quantitation of 0.002 mg/L and 0.01 rag/L, respectively.In plasma, the relative recovery ranged from 100.64% to 101.00% at the concentration of 5.00, 0.50 and 0.05 mg/L and within-day and between-day precisions were less than 2.5% and 4.6% respectively.Meanwhile, the relative recovery ranged from 97.20% to 100.20% at the concentration of 2.50, 0.50 and 0.10 mg/L in urine.The within-day and between-day precisions were lower than 1.3% and 4.3%, respectively.The method had been successfully used for the pharmacokinetic studies of a prulifloxacin formulation after oral administration to healthy volunteers.Conclusions The present method is simple, rapid, accurate, reproducible and suitable for the pharmacoki- netic study of prulifloxacin in humans.

10.The Treatment of Severe Intraventricular Hematoma through Midfrontal Keyhole Approach

Objective To evaluate the clinical value of midfrontal keyhole approach for the treatment of severe intra-ventricular hematoma. Methods The clinical data of 21 cases of severe intraventricular hemorrhage through midfrontal key-hole approach were analyzed retrospectively. Results Both inside and outside intraventricular hematoma were satisfied cleared. The GCS score and intraventricular hemorrhage Graeb score were improved. There were complications after opera-tion including 1 patient with diffuse brain swelling, 3 patients with cerebral vasospasm, 1 patient with intracranial infection, and seven patients with pulmonary infection. Follow-up schedules included 1-6 months. According to ADL score, 5 patients recovered well, 9 patients were moderately disabled, 3 were severely disabled, 1 was in a vegetative state and 3 died. Conclu-sion The intraventricular hematoma can be removed through midfrontal keyhole approach. The obstructive hydrocephalus can be relieved, the secondary brain damage was reduced and the prognosis was improved in patients.
