2.Detection of inflammation in early rheumatoid arthritis patients: a comparison study of single photon emission computed tomography combined with computed tomography and magnetic resonance imaging

Objective To evaluate the interactive relationship between inflammation detected by single photon emission computed tomography combined with computed tomography (SPECT/CT) and magnetic resonance imaging (MRI) detection of synovitis,bone marrow edema (BME) in the early rheumatoid arthritis (RA) patients.Methods Twenty patients with early RA were included into this study.All patients were diagnosed based on the 2010 ACR/EULAR classification criterion.Patients' unilateral hands (MCP 2-5 and PIP 2-5 joints) were inspected by SPECT/CT (label:99Tcm-MDP) and MRI (3.0 T).SPECT/CT and MRI images were analyzed by two reviewers blinded to other clinical information.Increased bone metabolism of the regional area of interest (ROI) was analyzed and the uptake ratio was calculated.MRI quantitative evaluation was completed by RAMRIS score system.Statistical analysis was performed using SPSS 17.0 software.Student's t-test,analysis of variance (ANOVA),x2 test and post-hoc analysis (Scheffe) were used for comparison of datas,correlation analysis was carried out with Spearman rank correlation test.Results Twent ypatients with a total of 160 joints were examined.MRI examination revealed synovitis (39,24.4%),or BME (24,15.0%) in 63(39.4%) joints.99Tcm-MDP uptake increased in 79(49.4%) joints,and the uptake ratio was higher than normal joints (1.59±0.26 vs 1.15±0.09,t=6.408,P<0.01).The uptake ratio between the graded synovitis level and RAMRIS synovitis score had a moderate correlation (r=0.535,P=0.001).The uptake ratio of BME level and BME score was well correlated (r=0.765,P=0.001).Conclusion In early RA patients,MRI is sensitive for synovitis and BME.SPECT/CT can detect early abnormal bone metabolism associated with inflammation,which is more sensitive than MRI.SPECT/CT can be used to detect changes of RA at the molecular level.Compared with synovitis,BME is more closely related to bone metabolism changes,which may suggest that BME may play a major role in the bone destruction in RA.However,further studies are needed to verify this hypothesis.

4.Expression levels of Tim and TLR gene family and its clinical significance in children with different types of mycoplasma pneumonia

Objective:To explore expression levels of Tim and TLR gene family in children with different types of mycoplasma pneumonia and their clinical significance.Methods: 56 patients with mycoplasma pneumonia were randomly selected in Ji′nan Maternity and Child Care Hospital from March ,2013 to March,2014.All the patients were divided into bronchopneumonia group ( n=40) and segmental pneumonia group (n=16) according to their X-ray manifestations.Bronchopneumonia group was characterized as dot,nodular increased density shadow with or without reticular ,stripe-like interstitial lesion.Segmental pneumonia was described as segmental or lobar consolidation of lung in X-ray.Meanwhile,30 healthy children were involved in control group with matched age and gender.Peripheral venous blood of all the children in every group was collected and total RNA was extracted .The mRNA of Toll-recepter 2,4 and Tim1,3 was quantified by quantitative real-time PCR (qRT-PCR),and compared between different groups.Results:The expression of mRNA of TLR-2,4 and Tim1 significantly increases in both bronchopneumonia group and segmental pneumonia group compared to healthy control.Meanwhile,the mRNA of TLR-2,4 and Tim-1,3 was significantly elevated among segmental pneumonia group compared to healthy control.In addition,the gene expression of Tim1,3 was higher in segmental pneumonia group than broncho-pneumonia group.Conclusion:The TLR-2,4 and Tim1 gene are over expressed in both types of mycoplasma pneumonia and Tim-1,3 are highly expressed in children with severe infection in lung.The expression alteration of TLR-2,4 and Tim1,3 gene is involved in different types of mycoplasma pneumonia.

6.Study on Transformation of Complete Atrioventricular Block in Children

Objective To observe the transformation features of children with complete atrioventricular block(CAVB), and to improve the diagnosis and treatment of this disease.Methods Seventeen children with primary diagnosis of CAVB were reviewed by retrospective and follow-up study and the clinical characteristics, treatment schemes and prognosis were evaluated.Results Four cases of congenital CAVB lived normally without obvious symptoms in the tracing period from 6 months to 9 years;1 case had the onset of Adams-Stokes syndrome induced by diarrhea;3 cases of the CAVB caused by virus myocarditis turned into sinus rhythm after comprehensive therapy;2 cases persistently presented with the CAVB Complicated with enlarged heart;1 case gave up treatment after deterioration, and one case died.Three cases of temporary CAVB after the open heart operation turned to sinus rhythm in transitory time, while other two cases presented with permanent CAVB and treated with epicardium pacemaker,among whom one had the pacemaker replaced for one time in the 8-year follow-up,and the follow-up of other cases were intermitted.Conclusions The congenital CAVB in the study group with normal QRS interphase and no obvious symptom might not require treatment but follow-up is needed. While infants with heart malformation and wide QRS wave could not endure the low ventricular rhythm are in high risk. Virus myocarditis induced CAVB children tend to present the Adams-Stokes syndrome, and require effective treatments. Partial cases of the CAVB caused by open heart operation may turn to normal sinus rhythm in 2-4 weeks after surgery. cases has persistent CAVB for over 4 weeks, or Adams-Stokes syndrome onset after the surgery demand epicardium pacemaker treatment.
7.Investigation and analysis of critical thinking ability in medical students

Objective To investigate the critical thinking ability of medical students and its related factors in order to provide evidence for higher medical education reform. Methods A total of 325 medical students were selected by cluster sampling method, and Critical Thinking Disposition Inventory-Chinese Version (CTDI-CV) was adopted for survery. Results The average total score of critical thinking ability of medical students was above 280, and 51.08% of students showed positive critical thinking ability. Among all the seven critical thinking dispositions, average scores of each item were all positive (>40) except that the significance of truth seeking (37.23) and systematicity (39.05) were indefinite. There were significant differences in critical thinking ability between medical students with different gender and academic years (P<0.05). Conclusion The critical thinking ability of medical students is positive, however, there still exist some shortages. An equal relationship between teachers and students should be established, the traditional course system be continuously reformed, a good environment for clinical training be created, and a variety of teaching methods be implemented to further strengthen the critical thinking ability of medical students.

9.Study on Inhibitory Effect of Aqueous Extract of Taxus Chinensis var. mairei Combined Erlotnib on A549 Xenograft in Nude Mice and Its Mechanism.

OBJECTIVETo study the effect of aqueous extract of Taxus chinensis var. mairei (AETC) combined Erlotnib on the growth of A549 xenograft in nude mice and its mechanism.
METHODSThe xenograft model in nude mice was established by inoculating A549 cells subcutaneously. BALB/c nude mice bearing A549 xenograft were randomly divided into six groups, i.e., the low dose Erlotinib group (A) , the standard dose Erlotnib group (B) , the low dose Erlotinib combined AETC group (C), the standard dose Erlotnib combined AETC group (D), the AETC group (E), the control group (F), 12 in each group. Different medication was performed for 7 successive weeks after 24 h. One mL blood was withdrawn and tumor tissues taken. The tumor inhibition rate was calculated. The combined effect was analyzed by Jin's Formula [Q = Ea + b/(Ea + Eb-Ea x Eb) ]. mRNA and protein expression levels of epidermal growth factor receptor (EGFR), cyclooxygenase-2 (COX-2), and B cell lymphoma-2 (Bcl-2) in xenografts were detected using real-time RT-PCR and ELISA.
RESULTSCompared with Group F, the xenograft weight was obviously lowered in Group B-E (P < 0.05, P < 0.01). The q value was 0.92 in Group C and 0.96 in Group D, which was obtained by simple adding of the two drugs. Compared with Group F, EG- FR mRNA expression in Group D and E, COX-2 mRNA expression in Group A-E; Bcl-2 mRNA expression in Group B-D; COX-2 protein expression in Group B-E; Bcl-2 protein expression in Group C and D were obviously lowered with statistical difference (P < 0.05, P < 0.01).
CONCLUSIONSAETC combined low dose and standard dose Erlotinib had synergistic effect on tumor inhibition. Its mechanism might be associated with down-regulating mRNA and protein expression levels of COX-2 and Bcl-2.

10.Effect of oxymatrine treatment on the invasion of human cervical cancer cells

Objective To investigate the anticancer effect of oxymatrine on cervical cancer cell line (HeLa). Methods MTT assay was used to detect the anti-proliferative effect of oxymatrine.Transwell chamber was used to detect the anti-metastatic effect of oxymatrine.Real-time PCR was used to detect the mRNA levels of MMP-2 and MMP-9.Western blot was used to detect the protein levels of MMP-2,MMP-9,AKT,p-AKT and GADPH. Results We found that application of oxymatrine significantly inhibited the growth of HeLa cells at the concentration above 0.8 mg/mL.We also found that oxymatrine (0.1,0.2 and 0.4 mg/mL)inhibited the invasion of HeLa cells under cytotoxic dose,which was (77.07±20.43)%,(53.95±18.17)% and (20.35±11.20)% of cells that migrated through the matrigel when compared with those of non-oxymatrine treatment group (P<0 .0 5 ). Further research found that oxymatrine (0.1,0.2 and 0.4 mg/mL)could reduce the expression of MMP-2 at the mRNA level,i.e.(82.76±8.71)%,(39.51±12.79)% and (21.53±5.38)% of the expression level when compared with that of non-oxymatrine treatment group (P<0 .0 5 ).The protein expression level of MMP-2 in 0 .4 mg/mL group was (64.69 ±16.52)% of non-oxymatrine treatment group (P<0.05).The phosphorylation level of AKT in 0.4 mg/mL group was (41.27±7.13)% of non-oxymatrine treatment group (P<0.05).Conclusion Oxymatrine can inhibit the invasion of HeLa cells by reducing the expression of MMP-2 via inhibiting the activity of AKT signal pathway.All together,our findings bring new insights into the mechanism of the anticancer effects induced by oxymatrine treatment.
