1.Application of Mammotome minimally invasive biopsy in diagnosis of non-palpable small lesions in breast

Objective To evaluate the application of Mammotome minimally invasive biopsy(MMIBS) in the diagnosis of non-palpable small lesions in breast. Methods A total of 42 patients with non-palpable small breast lesions and abnormal imaging findings were performed MMIBS.After MMIBS,21 patients received open operations,and the pathological diagnosis was compared and analysed. Results All these 42 patients were successfully performed MMIBS,with no severe complications,and the pathological diagnosis of MMIBS were breast cancer(8 patients) and benign pathological changes(34 patients).Among the 21 patients with open operations,the post-operative pathological findings of 20 patients were same as the results of MMIBS,with the sensitivity of 88.9% and specificity of 100%.The other 21 patients without operations were followed up for 6 to 12 months,and no abnormal mammographic and ultrasonographic findings were observed.Conclusion MMIBS boasts the characteristics of minimal invasion,safety,high sensitivity and high specificity,which can serve as the first choice in the diagnosis of non-palpable small lesions with abnormal imaging findings.
2.Isolation and Resistance of Acinetobacter calcoaceticus-baumannii in Hospital: A 7-Year Surveillance Study

OBJECTIVE To study the isolation and resistance tendency of Acinetobacter calcoaceticus-baumannii to antimicrobial agents from 1998 to 2004 to provide valuable data for infection prevention and therapy. METHODS We reviewed the isolation rates,distribution in clinical specimens and wards,and the resistance rates of(A.calcoaceticus-baumannii)to 14 kinds of antimicrobial agents from 1998 to 2004. RESULTS There was an increasing tendency of isolation rates of A.calcoaceticus-baumannii every year,which was 0.18% in 1998 but 1.48% in 2004.In the seven years,there was the highest isolation rate of 70.58% in specimens from respiratory tract,the next was from the urine(9.42%),and blood(4.63%).Concerning the wards distribution,ICU had the highest rate of 47.28%.In 1998,A.calcoaceticus-baumannii had resistance rates more than 50% only to one kind of antimicrobial agents(aztreonam),but in 2004,it had increased to thirteen kinds(except cefoperazone/sulbactam).About the fourteen kinds of antimicrobial agents we inspected,that were increased in their resistance rate.The highest increasing of resistance rate was ceftazidime from 11.1% in 1998 to 88.9% in 2004,the imipenem was second for 0.0% to 64.8%,and the third was sulfamethoxazole/trimethoprim form 0.0% to 64.0%,while there still was an increasing resistance tendency to them. CONCLUSIONS The clinical isolation rate of A.calcoaceticus-baumannii is increasing,and it has higher resistance rates to many antimicrobial agents as well as an increasing resistance tendency to relatively susceptive antimicrobial agents every year.So physicians should prescribe on the basis of antimicrobial agents susceptibility tests in vitro.

Objective To investigate the imaging manifestations of lung neoplasms in pre- and post- treatment with CT-guided Argon-Helium cryoablation.Methods All the lung neoplasms in 96 patients have been treated with CT-guided percutaneous Argon-Helium targeted cryoablation.All patients have pre- and post-treatment CT scanning in measurement of lesion size and CT value.The CT scanning has been rerpeated afterl,3,6,12 months of treatment.Results Among total 96 cases,there are 82 cases of lung cancer and 14 cases of metastasis with 110 lesions(89 cases with single lesion,7 cases with multiple lesions).The Ar-He cryoablation has been given 103 times in total.The size of each lesion ranged from 1.2 cm to 15.0 cm in diameter with mean value of(4.0?2.5)cm,including 12 lesions less than 2 cm,51 lesions between 2— 4 cm,24 lesions between 4—6 cm,23 lesions over 6 cm.There are 25 patients whose lesions covered by iceball with 1 cm of overloaping it's margin.There are 63 lesions with diameter less than 4 cm gained 100% ablation rate,24 lesions with 4—6 cm diameter gained 95.8% ablation rate,and 23 lesions with over 6 cm diameter gained 69.6% ablation rate.The post-treatment CT show a progressively enlarged round,low density refrigerant area which clearly demarked with non- refrigerant area.The center of each refrigerant area has negative CT value,the mean decreased CT value of lesion instantly after the treatment are about 30— 50 HU with P
4.Ecology survey of Microtus fortis in natural foci of plague in Dingbian town Shaanxi province in 2010

Objective To investigate the constitution,density changes and carrier rate about Yersinia pestis of rodents in plague foci,and to provide the scientific evidence for plague prevention.Methods According to the program of national monitoring plague,two survey procedures,namely quadrat of single-ha for 24 h and 5 m mouse jam,were used to monitor the host animals; culture and identification of Yersinia pestis in liver or spleen of the experimental animals was carried out by using self-made medium in the north of Beiyuanzi village in Dingbian town Shaanxi province.Results One hundred twelve rodents were captured using the first procedures and the rodent average density was 8.62 ind./hm2 and six species of rodents were found namely Meriones unguiculatus ( 100 individuals),Microtusfortis(5 individuals),Ochotona daurica(3 individuals),Meriones meridianus (2 individuals),Mus musculus Linnaeus (1 individual) and Cricetulus barabensis (1 individual).One hundred seventy-three field mouses were captured using the second procedures including Mus musculus Linnaeus (136 individuals),Cricetulus barabensis (36 individuals),and Microtus fortis ( 1 individual ).Among them,Microtus fortis was found in the salt marshes in the southern edge of Ordos Plateau steppe in plague area of Dingbian county.Yersinia pestis was not identified in all animals.Conclusions Microtus fortis is found in natural foci of plague in Shaanxi province for the first time,and a new geographic region was found.Its epidemiological significance needs further study.
5.Microscopic study on skin and soft tissue after repeated expansion.

OBJECTIVEThe study was to evaluate the microscopic changes on skin and soft tissue after repeated expansion for clinical work.
METHODSSix little pigs were divideded as: conventional expansion group, repeated expansion group, and blank control group. Histologic, ultrastructure and bFGF of the skin were observed and measured in each group after samples had been made.
RESULTSThe skin and soft tissue after repeated expansion were healthy on the whole. Compared with the conventional expansion group, there was more microscopic change in the repeated expansion group. Collagen fibers were injured evidently. Cells were injured slightly and proliferated much more, and moreover, they were more activated. The content of bFGF was more higher.
CONCLUSIONSThe skin and soft tissue after repeated expansion are healthy on the whole by more growth and more repair though repeated expansion may result in more injuries. So repeated expansion is safe and feasible.

6.Inhibition of Paeoniflorin on TNF-α-induced TNF-α Receptor Type I /Nuclear Factor-κB Signal Transduction in Endothelial Cells.

OBJECTIVETo study the inhibitory effect of paeoniflorin (PAE) on TNF-α-induced TNF receptor type I (TNFR1)-mediated signaling pathway in mouse renal arterial endothelial cells (AECs) and to explore its underlying molecular mechanisms.
METHODSMouse AECs were cultured in vitro and then they were treated by different concentrations PAE or TNF-α for various time periods. Expression levels of intercellular cell adhesion molecule-1 (ICAM-1) were detected in the normal group (cultured by serum-free culture media), the TNF-α group (cultured by 2-h serum-free culture media plus 6-h TNF-α 30 ng/mL), the low dose PAE group (cultured by 2-h PAE 0.8 μmo/L plus 6-h TNF-α 30 ng/mL), the middle dose PAE group (cultured by 2-h PAE 8 μmol/L plus 6-h TNF-α 30 ng/mL), the high dose PAE group (cultured by 2-h PAE 80 μmol/L plus 6-h TNF-α 30 ng/mL) with Western blot analysis. Nuclear translocation of transcription factor NF-κB (NE-κB) was detected in the normal group (cultured by serum-free culture media), the TNF-α group (cultured by 2-h serum-free culture media plus 45-mm TNF-α 30 ng/mL), and the high dose PAE group (cultured by 2-h PAE 80 μmol/L plus 45-min TNF-α 30 ng/mL) by immunofluorescent staining. Expression levels of the phosphorylation of extracellular signal-regulated (protein) kinase (ph-ERK) and p38 (ph- p38) were detected in the normal group (cultured by serum-free culture media) and the high dose PAE group (2-h PAE 80 μmol/L culture) by Western blot. NF-κB inhibitor-α (IκBα) protein expressions were detected in the normal group (cultured by serum-free culture media), the TNF-α group (cultured by 2-h serum-free culture media plus 30-min TNF-α 30 ng/mL), the high dose PAE group (cultured by 2-h PAE 80 μmol/L plus 30-min TNF-α 30 ng/mL), the p38 inhibitor group (SB group, pretreatment with SB238025 25 μmol/L for 30 min, then treated by PAE 80 μmol/L for 2 h, and finally treated by TNF-α 30 ng/mL for 30 min), the ERK inhibitor group (PD group, treated by PD98059 50 μmol/L for 30 min, then treated by PAE 80 μmol/L for 2 h, and finally treated by TNF-α 30 ng/mL for 30 min) by Western blot.
RESULTSCompared with the normal group, ICAM-1 protein expression levels obviously increased (P < 0.01). Compared with the TNFα group, ICAM-1 protein expression levels were obviously inhibited in the high dose PAE group (P < 0.05). Protein expression levels of ph-p38 and ph-ERK were obviously higher in the hIgh dose PAE group (P < 0.05). Compared with the normal group, IκBα protein expression levels obviously decreased in the TNF-α group (P < 0.01). Compared with the TNFα group, TNF-α-induced IκBα degradation could be significantly inhibited in the high dose PAE group (P < 0.01); the inhibition of PAE on IκBα degradation could be significantly inhibited in the SB group (P < 0.05). NF-κB/p65 signal was mainly located in cytoplasm in the normal group. NF-κB/p65 was translocated from cytoplasm to nucleus after stimulated by 45 min TNF-α in the TNF-α group, while it could be significantly inhibited in the high dose PAE group.
CONCLUSIONSPAE inhibited TNF-α-induced expression of lCAM-1. Its action might be associated with inhibiting TNFR1/NF-κB signaling pathway. p38 participated and mediated these actions.

7.Estimation report on the outcome of defluoridation and rebuilding stove in Ankang City of Shaanxi Provincein 2006

Objective To evaluate the condition of rebuilding stove for preventing coal-buming fluorosis in Ankang City of Shaanxi Province in 2006.Methods Guided by local health administration department in Shaanxi.five counties-Zhengping,Langao,Hanyin,Ziyang,Shiquan-were chosen as rebuilding stove regions in Ankang City.Fifteen housewives were chosen randomly in each village in each county,they were quizzed for the knowledge of health.Fifty students were chosen randomly in each school in each county,they were quizzed for the knowledge of health too.Data of rebuilding stove were refferred.Results Ninty-seven point two per cent(243/250)of the ovens were appropriately sealed,96.8%(242/250)were efficiently exhausted,the rate of oven correctly used was 97.2%(243/250),the awareness rates of healthful knowledge were 85.6%(214/250)in adults and 97.8%(485/496)in children.Data of rebuilding stove were documented systemically,completely and precisely.ConelusionFive model counties in Ankang City have fulfilled the demand.
8.Effects of Wenxiao Decoction on the expression of interleukin-6, intercellular adhesion molecular-1 and monocyte chemoattractant protein-1 in experimental atherosclerotic rabbits.

OBJECTIVETo observe the effects of different doses of Wenxiao Decoction on the expression of interleukin-6 (IL-6), intercellular adhesion molecule-1 (ICAM-1), and monocyte chemoattractant protein-1 (MCP-1) in experimental atherosclerotic rabbits and to explore the mechanism by which it alleviates atherosclerosis.
METHODSSixty New Zealand rabbits were randomly divided into six groups: a blank group, a model group, a Simvastatin group, and high-, medium-, and low-dosage Wenxiao Decoction groups. Except for those in the blank group, all rabbits were fed with a high-cholesterol diet. Carotid atherosclerosis was established by balloon-induced carotid artery endothelium injury in conjunction with the high-cholesterol diet. After 8 weeks, all animals were euthanized to evaluate levels of IL-6 and ICAM-1 expressions (by enzyme linked immunosorbent assay) and of MCP-1 (by immunohistochemistry staining).
RESULTSThe expressions of IL-6, ICAM-1, and MCP-1 were significantly increased in all groups except the blank group (P<0.05). However, the rabbits in the Wenxiao Decoction groups and the Simvastatin group showed significantly lower levels of IL-6, ICAM-1, and MCP-1 expression than those in the model group (P<0.05). The expressions of IL-6, ICAM-1, and MCP-1 in the highdosage Wenxiao Decoction group and the Simvastatin group were lower than those in the low-dosage Wenxiao Decoction group (P<0.05). The expression of MCP-1 in medium-dosage Wenxiao Decoction group was lower than that in the low-dosage group (P<0.05).
CONCLUSIONSHigh, medium, and low doses of Wenxiao Decoction can inhibit the expressions of IL-6, ICAM-1, and MCP-1, which may prevent and stabilize atherosclerotic plaques. There may be a direct relationship between dosage and therapeutic efficacy of Wenxiao Decoction.

9.R1239H mutation of CACNA1S gene in a Chinese family with hypokalaemic periodic paralysis.

OBJECTIVEMutation screening was performed to a Chinese family with hypokalaemic periodic paraiysis(HOKPP) for locating the corresponding mutations of gene and for specifying the clinical features associated with mutations.
METHODSThe cilnical features of patients from HOKPP family were summurized. Techniques of target exon PCR and direct sequencing were used to screen the mutation in CACNA1S and SCN4A genes in all numbers of the family.
RESULTSTwo patients of the family showed the typical features of HOKPP: the age of disease onset is during the childhood, acetazolamide is effective to patients treated. A heterozygous point mutation 3716 (G>A) causing R1239H was found in exon 30 of CACNA1S gene of the patients, but not found in normal members of the family.
CONCLUSIONThe mutant R1239H in CACNA1S gene exists in Chinese patients with familial hypokalaemic periodic paralysis.

10.Clinical value of the model for end-stage liver disease score in predicting the prognosis of liver transplantation in patients with end-stage liver disease.

OBJECTIVETo assess the value of the model for end-stage liver disease (MELD) in predicting the early-stage outcome of liver transplantation in patients with end-stage liver disease.
METHODSThe MELD scores of 87 liver transplantation recipients with end-stage liver disease were calculated, and their early-stage complications and mortality were analyzed.
RESULTSThe incidence of severe complications was 20.7%; in these recipients, with the 28-day and 3-month survival rates of 89.7%; and 88.5%;, respectively. The mean MELD scores showed significant differences between the complication-free group and survival group (14.6 vs 12.9, P<0.05), and also between the complication group and death group (21.6 vs 29.4, P<0.05). Compared to patients with MELD no greater than 15, patients with MELD between 16 and 24 showed significantly increased complication rate but had comparable survival rate (P>0.05); but in patients with MELD no less than 25, the survival rate was significantly decreased with also increased complication rate.
CONCLUSIONSA higher MELD score before liver transplantation is associated with greater likeliness of early-stage complication rate and mortality. High MELD score (over 25) can be a useful index in predicting severe complications and death in patients undergoing liver transplantation.
