1.Clinical observation of auricular point sticking combined with western medicine for preventing and treating postoperative complications of external excision and internal ligation on mixed hemorrhoid.

OBJECTIVETo explore the effect of auricular point sticking before operation for postoperative complications of external excision and internal ligation on mixed hemorrhoid.
METHODSEighty patients with mixed hemorrhoid were randomly divided into an observation group and a control group, 40 cases in each group. In the control group, paracetamol and dihydrocodeine tartrate tablets were applied with oral administration 1 h after operation, 510 mg each time, twice a day; at the same time, diosmin tablets were treated with oral administration, 0.9 g each time, twice a day. Three days' treatments were required successively. In the observation group, auricular point sticking was used before operation based on the treatment in the control group. The auricular points of Shen (Co10), Pangguang (CO9), Shenmen (TF4), Pizhixia (AT4), Jiaogan (AH6a) and Gangmen (HX5) were selected. The patients were asked to press the points 3-6 times per day, 3-5 min each time, 3 days' treatment in total. The scores of the postoperative complications in the 1st and the 2nd days were compared between the two groups such as pain, edema, hematochezia, retention of urine, etc.
RESULTSAfter operation, the scores of pain, edema, hematochezia, retention of urine in the 2nd day were all decreased obviously than those in the 1st day in the two groups (all P<0.05); and the scores of pain, edema, hematochezia, retention of urine in the 1st and the 2nd days of the observation group were lower than those in the control group (all P<0.05).
CONCLUSIONAuricular point sticking before operation combined with conventional western medicine with oral administration for preventing and treating postoperative complications of external excision and internal ligation on mixed hemorrhoid achieves positive and reliable efficacy.

2.Effect of intercellular adhesion molecule-1 and hyaluronic acid on monocyte-renal tubular epithelial cell adhesion induced by tumor necrosis factor α

Objective To investigate the effect of tumor necrosis factor ot (TNF-α) on monocyte-renal tubular epithelial cell adhesion and to explore its associated mechanism. Methods Human renal proximal tubular epithelial cell line (HK2 cells)and monocyte cell line (U937)were used to perform cell co-culture. TNF-α (10 μg/L)was used to stimulate HK2 cells and then U937 was put into the HK2 monolayers. Three locational parts of hyaluronan around HK2 cells were extracted by different ways and the HA expression was detected by enzyme-linked binding protein assay. Monocyte adhesion was detected by florescence recording assay. Flow cytometry was applied to assess intercellular adhesion molecule-1 (ICAM-1) expression in HK2 cells. Results Stimulation of HK2 cells with TNF-α significantly increased U937 binding to HK2 cell monolayer [(2.25±0.05) folds vs control, P<0.01]. Incubation of TNF-α (10 μg/L) with HK2 cells for 24 hours did not induce the change of total HA expression surrounding the HK2 cells, whereas HA redistribution happened with conditioned medium fraction (CM-HA) increased [(1.17±0.16) fold vs control, P<0.05], the pericellular fraction (trypsin extract fraction, TE-HA) decreased (83%±11% vs control, P<0.05) and the remaining cells-associated HA (cell associated fraction, CA) did not change significantly. In addition, pre-treatment of HK2 cells with TNF-α significantly increased the ICAM-1 expression [(1.85±0.22) folds vs control, P<0.01] and ICAM-1-dependent monocytes binding. Removal of HA from the surface of confluent monolayers of HK2 cells by hyaluronidase before addition of U937 cells increased monocyte binding[ (1.35±0.06) folds vs control, P<0.05]. In contrast, the presence of either sICAM-1 (400 μg/L) or anti-CDI8 antibody (10 mg/L) led to a significant decrease in ICAM-1-dependent monocyte binding (78%±7%, P<0.05; 75%±8%, P<0.01 vs those before adding sICAM-1 or anti-CD18 antibody, respectively). Conclusion HA redistribution induced by TNF-α may facilitate ICAM-1-dependent monocyte-epithelium binding, and pericellular HA plays a protective role in monocyte-driven tissue inflammation.

Objective To investigate the feasibility and clinical value of pulmonary carcinoma in middle-advanced stage bypercutaneous intratumor carboplatin injection(PCI) under CT guided in combination with bronchial artery infusion(BAI) and intrathoracic artery infusion(IAI).Methods There were totally 58 cases with central bronchial carcinoma in stage Ⅲ and Ⅳ.Of them,30 cases(treatment group) were treated by BAI+IAI and PCI,while 28 patients(control group) were treated by BAI and PCI.Therapeutic effect was evaluated according to the improvement of the following variables:CT,life quality and survival period after treatment.Results The responsive rate in the treatment group(73.3%) was significantly higher than that in the control group(46.4%),the life quality and survival period were much improved.Conclusion Percutaneous intratumor carboplatin injection in combination with BAI and IAI is a effective method for bronchial carcinoma in Ⅲ and Ⅳ stages,it can not only improve the shorten effect,prolong survival period,but also can improve patients life quality.
4.Effects of Jieyu-Chufan capsule on the behavior and the level of cerebral monoamine ;neurotransmitter in depression mouse model

Objective To observe the effects of Jieyu-Chufan capsule(JCC) on the behavior and the monoamine neurotransmitter content of cerebral in depression mouse model and explore its mechanism. Methods The ICR male mouse were randomly divided into seven groups according to their weights(the model group, the low, middle and high doses group, Baiyoujie group, the amitriptyline group, the normal control group). The low, middle and high doses group were given JCC with 1.250, 2.500, 5.000 g/kg separately by intragastric administration. Baiyoujie group was given 0.014 g/kg Baiyoujie by gavage. The amitriptyline group was given 0.050 g/kg amitriptyline by gavage. The model group and the normal control group received the same volume of distilled water intragastrically. The drugs were administrated once a day. The brepharoptosis and movement conditions of mouse were observed after 22 days. The monoamine neurotransmitter content of each group was measured after 25 days. Results Compared to the model group, the number of mouse with brepharoptosis decreased in the high dose group after 1, 2, 6 hour with reserpine injection (P<0.05 or P<0.01);the number of mouse came out from the circle increased after 4 hours with reserpine injection (P<0.05 or P<0.01);the content of NA (880.45 ± 428.81 ng/g, 875.98 ± 449.33 ng/g vs. 299.92 ± 267.08 ng/g) and DA (2 305.99 ± 530.37 ng/g, 2 169.99 ± 278.19 ng/g vs. 1 439.34 ± 357.33 ng/g) in the middle and high dose group increased (P<0.01);the content of 5-HT (781.43 ± 135.10 ng/g vs. 492.01 ± 192.80 ng/g) in the middle group increased (P<0.01). Conclusions JCC showed an antidepression influence in depression mouse model induced by reserpine. The mechanism may be related to regulating the level of monoamine neurotransmitter in the nervous centralis .

Objective To develop a set of information system for managing procedure in rehabilitation hospital, which can coordinate the services of rehabilitation teams characterized by the modern rehabilitation model. Methods Based on the working pattern of the rehabili-tation teams, and combined with the hospital information system and electronic medical records, the closed-loop management information system was developed, with the core of treatment-task schedules and feedbacks, and treatment-record communications. Results and Conclu-sion The Rehabilitation Treatment Management System was developed, which established the intelligent treatment process, realized the closed-loop management of resource sharing and treatment task.

Objective To explore the change of sexual life quality and its influencing factors of premenopausal breast cancer patients.Methods 94 premenopausal breast cancer patients were included. Changes in Sexual Functioning Questionnaire short-form (CSFQ-14) was used and part of its items was changed. Their sexual life was evaluated before and after treatment by using depth interview and anonymous methods. Results 94 cases received face-to-face communication and study. 88 (93.6%) questionnaires were received, in which 81 (92.1%) questionnaires were effective. Their sexual life quality and sexual satisfaction significantly decreased and there was significant difference in the CSFQ-14 score (P<0.001) before and after treatment. The ratio of bad, worse, and worst sexual life were 39.5%, 29.6%, and 14.8% respectively after treatment. 65.4% of the patients had no sexual activity. The reasons included medical related factors, fear of cancer metastasis and recurrence, physical discomfort, poorer body image, partner sexual attitudes changed, life stress, common psychological problems,
traditional ideas and so on. Conclusion Sexual dysfunction was obvious and sexual life quality significantly decreased in premenopausal breast cancer patients after treatment. Patients' physical and mental damage, the change of body image and spouse's attitude are significant influencing factors. It is necessary to give sexual rehabilitation guidance to patients and their spouses.
