1.Children Blood Lead Levels and Influencing Factors after Unleaded Gasoline Used in Guangzhou

BLLs(G) in Baiyuen (suburb) children(60.33 ?g/L) was the highest among three districts, Yuxiu (downtown) was 58.09 ?g/L and Zengcheng (countryside)was 56.72 ?g/L. Linear regressions (stepwise) were taken with geometric BLLs as dependent, 34 items as independents in SPSS 11.5. Fathers with lower diploma, narrower houses, better aired houses, fewer hand washings and sucking fingers were the risk factors for children high BLLs. Conclusion BLLs in suburb children become the highest after unleaded gasoline used in Guangzhou. The housing circumstance and hygienic behavior of children may effect the blood lead level.
2.Determination of the Content of Ceftazidime in Renal Dialysate Effluent in Burn Patients

OBJECTIVE:To determine the content of ceftazidime in renal dialysate effluent in burn patients.METHODS:HPLC was used.The sample was separated on Bondapak-C18 analysis column with a mobile phase of mixture of methanol-ammonium acetate(17∶83,V/V).The flow rate was 1.0ml/min.The UV detector wavelength was set at 254nm.RESULTS:A linearity was obtained from 0.5~100?g/ml with a good correlation(r=0.9 999).The RSDs of inter-day and intra-day precision were not more than 3.The average recovery was 100.30%.CONCLUSION:The method is simple,rapid and accurate.
3.RIFLE and AKIN criteria for mortality and risk factors of acute kidney injury in hospitalized patients

Objective:To evaluate the mortality and risk factors for acute kidney injury (AKI) in hospitalized patients by the risk, injury, failure, loss, end stage kidney disease (RIFLE) and acute kidney injury network (AKIN).
Methods:We constructed a retrospective study of all AKI patients in the Second Xiangya Hospital of Central South University between February 2006 and January 2011. The diagnosis and classiifcation of AKI were reconifrmed and categorized by RIFLE and AKIN criteria. To compare the clinical characteristics, mortality and associated risk factors in AKI patients by the RIFLE and AKIN stage, univariate analysis and multivariate logistic regression analysis were performed. Results:The patients were diagnosed as AKI by AKIN (n=1027) or by RIFLE criteria (n=1020). There was no signiifcant difference in the hospital mortality, hospital length stay (days), or the proportion of complete recovery in each stage of AKI patients by RIFLE and AKIN (P>0.05). In the univariate analysis, age, pre-renal causes, proportion of hospital acquired AKI, mechanical ventilation, hypotension, the number of failed organs, acute tubular necrosis-index severity score (ATN-ISS), and the peak of serum potassium ion concentration were signiifcantly higher in the non-survivors than in the survivors (P<0.05). Logistic regression analysis revealed that age older than 65, hospital acquired AKI, hypotension, number of failed organs, ATN-ISS scores, and the peak of serum potassium ion concentration were independent risk factors for hospital mortality. Conclusion:Both RIFLE and AKIN criteria have similar scientiifc value in assessing hospital mortality. AKI stage is associated with the recent prognosis of AKI patients.

5.Effect of remifentanil on calcium-activated potassium currents in human mesenteric arterial smooth muscle cells

Objective To investigate the ionic mechanism of remifentanil-induced vaso-relaxation by recording calcium-activated K~+-currents(IKCa)in human arterial smooth muscle cells(MASMCs).Methods Human MASMCs were obtained freshly by the method of enzymolysis.IKCa were separated with 4-AP 5 mmol?L~(-1) and recorded by the whole-cell voltage-clamped technique.The effects of remifentanil(1.2,4.8,19.4 mmol?L~(-1))on amplitude and threshold of activation for IKCa were assessed.Results Remifentanil significantly increased IKCa and had no effect on the threshold of activation for IKCa as compared with the control group(P<0.05 or 0.01),and the IKCa returned to the baseline level after remifentanil was washed out.Conclusion Remifentanil produces vasodilation by activating IKCa in MASMCs.
6.Expression of axon guidance cues Slit2 and Robo4 in lung tissue of rat with acute lung injury

Objective To observe the expression of axon guidance cues Slit2 and Robo4 in lung tissue of rat with acute lung injury (ALI) and explore the function of Slit2 and Robo4 in ALI.Methods Forty-eight Sprague-Dawley rats were randomly (random number) divided into control group (n =24) and ALl group (n =24).ALI model was reproduced by cecum ligation and puncture (CLP).The control group only experienced a simulated operation without CLP.Both groups were further divided into 3 subgroups with 8 rats in each subgroup:12 h,24 h,and 48 h subgroups.artery blood gas analysis,lung tissue wet/dry weight (W/D) ratio,lung histopathologic changes,pulmonary microvascular permeability were observed.The serum tumor nocrosis factor-α (TNF-α) was measured with enzyme linked immunosorbent assay (ELISA).The expression of Slit2 and Robo4 mRNA were detected by reverse transcription-polymerase chain reaction (RT-PCR).The expression of Slit2 and Robo4 protein in lung tissues was assessed by immunohistochemistry.Date were analyzed by one-way ANOVA with SPSS version 13.0 software.Statistical significance was established at a P value of less than 0.05.Results Compared with the control group,in ALI rats at different time points,partial pressure of oxygen in arterial blood (PaO2) decreased significantly,lung W/D weight ratio and pulmonary microvascular permeability,the serum TNF-α increased significantly (all P < 0.05),histopathology of lung revealed signs of injury.The expression of Slit2 mRNA in lung tissues was decreased markedly after CLP compared with control group [(0.56±0.13) vs.(0.87±0.05),F=41.39,P<0.05,(0.42±0.10) vs.(0.85±0.07),F=93.54,P<0.05,(0.26±0.08) vs.(0.89 ±0.09),F=227.05,P<0.05].but there were no significant difference in expression of Robo4 mRNA in lung tissue between ALI group and control group [(0.86±0.07) vs.(0.83±0.05),F=0.695,P>0.05,(0.82±0.05) vs.(0.89±0.08),F=2.061,P > 0.05,(0.86 ± 0.08) vs.(0.86 ± 0.05),F =0.035,P > 0.05].Immunohistochemistry study showed Slit2 protein was mainly expressed on the extracellular surface of vascular endothelial cells,while lung epithelial cell nuclei and endochylema.Robo4 protein was only expressed on the extracellular surface of vascular endothelial cells.Compared with the control group,expression of Slit2 protein in lung tissue in ALI group decreased markedly [(0.37 ± 0.05) vs.(0.45 ± 0.07),F =6.82,P < 0.05,(0.32±0.06) vs.(0.47±0.09),F=23.54,P<0.05,(0.28±0.07) vs.(0.46±0.06),F=28.01,P < 0.05].As good as RT-PCR,there were no significant difference in expression of Robo4 protein in lung tissue between two groups [(0.53±0.04) vs.(0.52±0.05),F=0.155,P>0.05,(0.53± 0.09) vs.(0.50±0.05),F=0.498,P>0.05,(0.55±0.06) vs.(0.56±0.07),F=0.073,P > 0.05].Conclusions Lung tissues of control group rats express Slit2 and Robo4.The decreased Slit2 mRNA and protein expressions in the lung tissue of rat with ALI caused by CLP may be associated with the occurrence of ALI.
7.A hospital-based case-control study on influencing factors of nonalcoholic fatty liver disease.

OBJECTIVESTo investigate the influencing factors of nonalcoholic fatty liver disease (NAFLD).
METHODSA hospital-based case-control study was conducted in patients with NAFLD and controls without NAFLD in a hospital from January to August in 2007. All data were analyzed by SPSS 13.0 software.
RESULTSOne-way analysis of variance found that the two groups were significantly different in cigarette smoking, alcohol and tea comsumption, movement index, speed of food intake, frequency of social engagement, kinds of edible oil, marine products, family history of NAFLD, hypertension, higher blood sugar, abnormality of blood fat, higher level of ALT, higher level of AST, hyperuricemia, obesity, decrease of high density lipoprotein (HDL), and increase of low density lipoprotein. By non-conditional logistic stepwise regression analysis, 12 of 18 factors were used to construct a model, ten of which were the risk factors and two were protective factors of NAFLD. Risk factors included obesity (OR=6.35), hypertension(OR=3.82), dyslipidemia (OR=2.95), decrease of HDL (OR=2.85), hyperglycemia (OR=2.82), increase of ALT (OR=2.80), hyperuricemia (OR=2.35), HBsAg positive (OR=1.99), family history of fatty liver (OR=1.79) and frequently intake of marine products (OR=1.58), and protective factors included tea drinking (OR=0.72) and exercise (OR=0.90).
CONCLUSIONSThere are many influencing factors of NAFLD, and life styles are the key factors. Genetic background may also play some roles in NAFLD.

8.A simple skin flap plasty to repair tracheocutaneous fistula after tracheotomy.

The tracheocutaneous fistula after tracheostomy is a complex clinical problem. An ideal fistula closure is still difficult at present though a variety of fistula-closing methods have been reported in the literature. We used a turnover skin flap to cover the fistula. All the procedures were completed at bedside under local anesthesia. The fistula was successfully closed and well healed without complications within 7-9 days. It has been proven that this operation is simple, effective, and safe.

9.Reproductive toxicity of triptolide and its mechanism in male rats.

The arrenotokous toxicity of triptolide was evaluated, and the rate of sperm abnormality, the changes of the lipid peroxide, the enzyme activity and the hormone in male rats were observed. With the negative and positive control group, the healthy rats were respectively given by gavage triptolide suspension at the dose of 0.025, 0.05, 0.1 mg x kg(-1) for 30 days. Then the rats were killed for the measurement of the indicators in testis and serum, as well as the study on the sperm abnormality. The results showed that the positive control group had significant difference, compared with the negative control group. The content of SOD, LDH, G-6-PD, Na+ -K+ -ATPase, Ca+ -Mg+ -ATPase decreased significantly in 0.05 mg x kg(-1) group, and reduced more obviously with exposure to the dose of 0.1 mg x kg(-1). The levels of GSH-Px and beta-G showed a significant decrease in the testis of rats only at the dose of 0.1 mg x kg(-1). Nevertheless, the MDA levels, the FSH levels and the LH levels showed no significant difference. The deformity rate of sperm increased significantly in 0.05 mg x kg(-1) group and 0.1 mg x kg(-1) group. The results indicated the triptolide had the effect of the lipid peroxidation to damage Spermatogenic cells, Sertolis cells and Leydig cells. At the same time, the triptolide interfered not only with the energy supply process of aerobic and anaerobic glycolysis,but also with the energy utilization in testis by affecting the activities of testis marker enzymes, and produced a damage chain of the male reproductive system

10.A controlled clinical trial on the therapy of injured shoulder arthrochalasis by shoulder-elbow elastic band immobilization with functional exercise.

OBJECTIVETo observe the therapeutic effects of shoulder-elbow elastic immobilization and functional exercise for injured shoulder arthrochalasis, and compare it with forearm-suspending immobilization and functional exercise therapy.
METHODSThe patients with injured shoulder arthrochalasis were randomly divided into treatment group (38 cases) and control group (37 cases). Treatment group underwent the therapy of shoulder-elbow elastic band immobilization and functional exercise; control group was managed by forearm-suspending band immobilization and functional exercise therapy.
RESULTSIn treatment group, 29 (76.3%) cases recovered, 7 (18.4%) improved and 2 (5.3%) failed; in control group, 15 (40.5%) cases recovered, 15 (40.5%) cases improved and 7 (19.0%) failed. The differences between two groups were statistically significant (P<0.01). The AHI (acromio-humeral interval) ranged from 8 to 19 mm (11.9+/-5.1) in treatment group and 8 to 27 mm (14.2+/-5.4) in control group, and the difference was also statistically distinct (t=2.7525, P<0.01).
CONCLUSIONThe treatment with shoulder-elbow elastic band immobilization and exercise therapy for injured shoulder arthrochalasis is a safe and effective method. Immobilization with shoulder-elbow elastic band is better than forearm-suspending band for injured shoulder arthrochalasis.
