1.Application of plan-do-check-act cycle in quality management for loaner surgical instruments

Objective To investigate the management status of loaner surgical instruments,and evaluate the effect of plan-do-check-act cycle (PDCA)quality control on loaner surgical instrument management.Methods From July 2011 to June 2012,8 965 pieces of loaner surgical instruments before adopting PDCA quality control management was as control group;from July 2012 to June 2013,8 564 pieces of loaner surgical instruments adopting PDCA quality control was as ob-servation group.The defects of loaner surgical instruments during application process and effect of PDCA quality control on loaner surgical instrument management were analyzed.Results There were many problems in checking-tracking,cleaning quality and company personnel of loaner surgical instruments.The qualified rate of observation group was higher than that of control group(99.36% vs 96.27%)(χ2 =194.74,P <0.01).The main causes for unqualification of observation group were unqualified cleaning (n=21 ,38.18%)and incomplete function of instruments(n=8,14.55%);while the main causes for unqualification of control group were the loss of instruments(n=81,24.25%),lack of monitor and record (n=71, 21.26%),unqualified cleaning(n=55,16.47%)and the soaking of package(n=54,16.17%).Conclusion PDCA quality control is an effective method for loaner surgical instruments management,it is helpful for building long-term effective quali-ty control system for loaner surgical instruments,and make loaner surgical instrument management more scientific,system-atic,and standard.
2.Effect of prescription Zu Zhong 1 Hao pretreatment on the levels of nitric oxide, malondialdehyde, superoxide dismutase and tumor necrosis factor-α in brain Issue after focal cerebral ischemia-reperfusion in rats

Objective To investigate the protective effect and its mechanism of prescription Zu Zhong 1 Hao (a traditional Chinese medical prescription, including Astragalus membranaceus, Atractylodes macrocephala, Arisaema cure Bile, Rhizoma pinelliae, the seed of Prunus persiea , Angelica sinensis , Ligusticum Chuanxiong, Paeonia lactiflora , and Pueraria ,etc) pretreatment on focal cerebral ischemia-reperfusion in rats. Methods Sixty SD rats were randomly divided into sham-operation, ischemia-reperfusion, flunarizine and prescription Zu Zhong 1 Hao low-, medium-and high-dose groups (n=10 in each group). A rat model of focal cerebral ischemia-reperfusion was induced by suture method (ischemia for 3 hours followed by reperfusion for 24 hours). Nitric oxide (NO) was measured by the nitrate reductase method; superoxide dismutase (SOD) activity was assessed by the xanthine oxidase method; maiondialde-hyde (MDA) was determined by the thiobarbiturie acid method; and tumor necrosis factor-a(TNF-α) was detected by the enzyme-linked immunosorbent assay (ELISA) method. Results Prescription Zu Zhong 1 Hao significantly improved neurological deficits in focal cerebral ischemia-reperfusion in rats, reduced the content of NO and MDA in brain tissue, increased SOD activity, and down-regulated the expression of TNF-α. Among them, the role of the high-dose group was more significant (P<0. 01). There were also significant differences between the low-and medium-dose groups and the ischemia-reperfusion group (P<0. 05). Conclusions The pretreatment of prescription Zu Zhong 1 Hao has the protective effect on focal cerebral ischemia-reperfusion injury, and its mechanism may be associated with the decreased content of NO and MDA in brain tissue, increased SOD activity, and down-regulated TNF-α expression.
3.Study on the Characteristics and Influential Factors of Microbial Rapid Detection Reagent by Bioluminescence

In this assay, the reaction kinetics, optimum temperature, pH and various influential factors of ATP microbial rapid detection reagent by bioluminescence were studied. The results showed that it's enough for detection system to have 40 ~ 50?g/mL D-Luciferin. The light production decreased fastest in the first minute of reaction, then began to decay slowly. The optimal reaction temperature was 24℃~25℃and the optimal pH was pH 7.2 -7.4 in the reaction system. In addition, when stored at 4℃for 45h, the dissolved reagent solution could keep its 86% activity. When preserved at 25℃, the enzyme activity decreased less for 1h, and degraded gradually as time went by and only left 53. 5% of its activity after 6. 5h. While stored at 33℃, the enzyme activity decreased quickly with the time and only left 59. 1% after 1. 5h. The result indicated that storage temperature was a very important influential factor to the activity of reagent Meanwhile, different chemical substance such as acid, alkali, salt and surfactants inhibited the ATP bioluminescent reaction. When the concentration of NaCl reached 1. 5g/L, it could inhibit 52. 5% light production. Triton X-100, acid, and alkali also had some effects on the reaction, while CTAB, SDS and TCA would inhibit the bioluminescent reaction seriously.

5.Epidemiological survey of astigmatism among 926 preschool children in a kindergarten in Enshi City

AIM:To study the type, degree and axial distribution of low vision astigmatism in preschool children.METHODS:A group of 3-6 years old children were selected for astigmatism screening, and statistical analysis was performed on the detected 445 eyes of 308 people.RESULTS:With more than 0.50D astigmatism criteria, astigmatism examination of 308 people, accounting for 36.2%, of which 137 eyes astigmatism, astigmatism 171 monocular.The five types of astigmatism were compound hyperopia 40.7%, mixed 35.5%, compound myopia 8.5%, myopia 8.3%, simple hyperopia astigmatism degree 7.0%;69.0% were mild, 16.6% moderate, 14.4% severe.Astigmatism axial distribution was with the rule for 54.9%, against the rule 28.8%, oblique 16.6%.In binocular astigmatism eyes, axial symmetry was in 35.8%, asymmetry in 64.2%.CONCLUSION:The main type of astigmatism in preschool children are compound hyperopia and mixed astigmatism.Astigmatism degree is mainly mild.With the increase of age, the detection rate of moderate and high astigmatism increased.

This study was aimed to investigate the immunophenotypic characteristics of 109 cases of B-cell chronic lymphoid leukemia (B-CLL) so as to provide evidences for the diagnosis and therapy of B-CLL, and for the detection of the minimal residual disease and its prognosis. Immunophenotyping was performed in 109 patients of B-CLL by two/three color multiparameter flow cytometry analysis using a panel of monoclonal antibodies. The results showed that in 109 cases of B-CLL, all cases expressed CD19, the positive ratios of other B lineage antigen such as CD20, CD22 and CD23 were 95.40%, 94.50%, 86.20% respectively. None of the B-CLL cases expressed CD10. The expression ratio of FMC-7 and CD38 in 105 cases of B-CLL were 28.60% and 36.20%. Among the B-CLL cases the CD5(+) cells amounted to 86.23%, CD5(-) cells amounted to 13.76%, ZAP-70 protein was expressed in 12 out of 50 patients. It is concluded that immunophenotypic data are very useful for the diagnosis and detection of minimal residual disease of B-CLL, and the relationship between the immunophenotypic characteristics and the prognosis of B-CLL needs further study.

9.Effects of resveratrol on the proliferation and CaN of vascular smooth muscle cell induced by angiotensin II.

AIMTo investigate the effects of resveratrol (Res) on the proliferation of VSMCs induced by Ang I and the expression of calmodulin (CaM) and calcineurin (CaN) in the proliferation of VSMCs treated by Ang 1 and to discuss the mechanism.
METHODSRabbit arterial VSMCs were cultured in vitro and VSMCs were identified with the method of immunocytochemistry. A cell proliferating model of VSMCs induced by AngII was established. VSMCs were cultured for 4-8 passages. The experiments were randomly divided into control group, AngII group (0.1 micromol/L) and AngII + Res groups with different concentrations(20, 40, 80, 160) micromol/L. VSMCs proliferation was determined with MTT colorimetric method. CaM was detected with Coomassie brilliant blue method and CaN was determined by enzyme reaction phosphorus measurement.
RESULTSRabbit VSMCs were cultured successfully and could be passaged. After immunocytochemistry staining, all the cells cytoplasm were stained and positive. Cell proliferation, CaM and CaN activities were increased significantly in VSMCs proliferation induced by AngII (P <0.05, P < 0.01). The index of AngII + Res groups were obviously reduced compared with AngII group ( P < 0.01).
CONCLUSIONThe VSMCs proliferation induced by AngII can be inhibited by Res significantly, and the inhibiting mechanism of Res may be related to inhibiting CaM and CaN activities then restraining the proliferation of VSMCs in a dose dependent manner.

10.Correlation between the red blood cell distribution width and prognosis in elderly patients with cerebral infarction and severe hemiplegia

Objective Toinvestigatethecorrelationbetweentheredbloodcelldistributionwidth (RDW)and in-hospital prognosis in elderly patients with cerebral infarction and severe hemiplegia. Methods Atotalof209consecutiveelderlypatientswithcerebralinfarctionandhemiplegiadiagnosedat the Emergency Department,Central Hospital of Jiading District in Shanghai from January 2012 to December 2013 were enrolled retrospectively. All patients were divided into an L-RDW group (n=73 ),an M-RDW group (n=56),and an H-RDW group (n=80)according to the RDW trisection level. A single factor analysis of variance was used to compare the differences of clinical data,laboratory indexes,and mortality during hospitalization among the three groups. The patients were redivided into either a survival group (n=170)or a death group (n=39)according to their clinical outcomes. Its differences of RDW levels in the acute phase were compared. Logistic regression analysis was used to analyze the independence risk factors for the death of patients,and the risk ratio (OR)was calculated. Spearman correlation analysis was used to analyzeRDWandtheNationalInstitutesofHealthStrokeScale(NIHSS)scores.Results Themortality of the M-RDW group was significantly lower than that of the L-RDW and the H-RDW groups (1. 8%[n=1] vs. 12. 3%[n=9]and 36. 2%[n=29],all P<0. 01). the RDW level of the death group was significantly higher than that of the survival group (14.2 [13.3,15.1]vs.12.5 [11. 9,13. 5];P =0. 002). The H-RDW level (OR,12. 164,95%CI 2.544-58.181,P=0.002)and the NIHSS score (OR,1. 136, 95%CI 1. 056-1. 221,P=0. 001)were the independence risk factors for the death of the elderly patients with severe hemiplegia cerebral infarction;the RDW level was positively correlated with the NIHSSscore(r=0.167,P=0.016).Conclusion ThemortalityofdifferentRDWlevelsinelderly patients with cerebral infarction and severe hemiplegia showed aU shape. The mortality of the H-RDW is the highest. The RDW level has an important reference value for identifying the prognosis in elderly patients with cerebral infarction and severe hemiplegia.
