Objective To explore the clinical characteristics and diagnosis of mycoplasma pneumoniae(MP) pneumonia showing lobar pneumonia.Methods Patients from Jan.2004 to Dec.2005 were selected as researching case,which had samptoms and signs of respiratory in clinic,the chest Xray shown lobar pneumonia,the MP of down respiratory′s secretion detected by polymerase chain reaction(PCR) and MP-IgM of serum detected by indirect hemagglutination were positive.The clinical data of them were reviewed.Results MP pneumonia showing lobar pneumonia was same to streptococcus pneumoniae pneumonia in clinic,but it had some characters:1.majority of the lobar MP pneumonia were children being student;2.the process was long,the samptoms of respiratory was tipical,the continuous fever was few,and the infectious samptoms was not obvious as streptococcus pneumoniae pneumonia;3.the injury to lung was more and tipical;4.the increase of white blood cell,neutrocyte and C-reactive protein were not clearly as to streptococcus pneumoniae pneumonia,but the absorbing of lung′s focal was relatively fast;5.the ?-lactams antibiotic was not effective,but macrolides was effective.Conclusions Clinical characters of MP pneumonia are needed to master,the variety of chest X-ray changes and differentiate from other pneumonia infected by other are needed to notice.

ObjectiveTo investigate the serum soluble urokinase-type plasminogen activator (suPAR)in patients with multiple myeloma,its relationship with the 2-year survival rate and its clinical significance in predicting the disease prognosis.MethodsEnzyme-linked immunosorbent assay was applied to determine the levels of suPAR of 40 initial diagnosed patients with MM and 30 healthy persons as healthy control group.The patients were divided into three groups:effective (13 cases),remission (19 cases) and invalid (8 cases).ResultsUsually there was suPAR expression [(233.47±83.22) pg/ml] in serum of the healthy adults.There was no statistical difference of the suPAR expression in effective group or remission group before treated [(257.60±32.47) pg/ml,(331.00±99.80) pg/ml] compated with healthy group (t =2.04,t =1.83,P ＞ 0.05),and that in the invalid group,serum levels of suPAR [(562.20±291.0) pg/ml] was statistically significant with that of the effective group and that of the healthy control group (t =3.92,t =1.93,P ＜ 0.05).Through monitoring 17 patients,the suPAR of patients before treatment [(437.65±131.43) pg/ml] was statistically significant with that of healthy control group (P ＜0.01) and effective group after treatment [(298.76±108.59) pg/ml] (P ＜0.01).Serum suPAR of more than 2-year-survival patients [(333.02±85.37) pg/nl] during initial diagnosis was not statistically difference with that of the healthy control group (t =1.81,P ＞ 0.05).Serum suPAR of less than 2-year-survival patients [(646.01±103.97) pg/ml] was statistically different with that of healthy control group (t =3.84,P ＜0.01) and that of more than 2-year-survival patient group (t =3.50,P ＜0.01).ConclusionThe suPAR value was correlated to the stability of multiple myeloma,and can be used for disease prognosis and estimation of survival time.And the high expression of suPAR was related to the adverse prognosis.

Objective To observe the efficacy and toxicity of low dose thalidomide combined with VAD regimen contained one-week dexamethasone in treatment on patients with multiple myeloma (MM). Methods 42 patients with MM were diagnosed in our hospital.The VAD regimen contained one-week dexamethasone was vincristine 0.4 mg/d, adramycin 10 mg/d and dexamethasone 40 mg/d for 4 days. At the same time, thalidomide is given 150-200 mg/d. Results The overall response rate (ORR) is 80.95%. Before and after treatment, all the detection indicators in MM patients were statistically different (P＜0.001). There is small adverse reactions. Conclusion Low dose thalidomide combined with VAD regimen contained one-week dexamethasone in treatment of MM has more merits such as less side effects, good tolerance, convenient administration and significant curative effects, so it should be recommended in clinical works.

ObjectiveTo evaluate the application value of magnetic resonance diffusion weighted imaging (DWI)in diagnosis and differential diagnosis of prostate cancer. Methods53 Cases of suspected prostate cancer patients with elevated prostate-specific antigen, abnormaldigital rectal examination or ultrasound examination were underwent DWI examination before operation. Apparent dispersion coefficient was measured and compared to pathological results. Results There were 15 cases of prostate cancer, 37 cases of benign prostate hyperplasia and one prostatic intraepithelial neoplasia. When b =1 000 s/mm2 ,the ADC values of foci area in prostate cancer group and peripheral zone in benign prostate hyperplasia group were (0.813 ±0.25) × 10-3mm/8 and (1.45±0.24) × 10-3mm/s respectively, and the difference had statistical significance(t =-26. 117,P ＜0. 01 ). When b =60D s/mm2, the ADC values of foci area in prostate cancer group and peripheral zone in benign prostate hyperplasia group were ( 1.03 ± 0. 27) × 10-3 mm/s and ( 1.62 ± 0. 33) × 10- 3 mm/s respectively, and the difference had statistical significance( t =- 24. 191, P ＜ 0. 01 ). The area under the ROC curves was 0.862, and the sensitivity and specificity of DWI were 78.1％ and 91.5％ respectively.ConclusionDWI can provide quantitative information for prostate cancer diagnosis and differential diagnosis,therefore it possesses a good clinical application value.

OBJECTIVEThis study aims to investigate the caries status of the first permanent molar and primary tooth among 7- to 9-year-old children in Tangshan city and the correlation between caries of primary tooth and the first permanent molar.

METHODSA stratified cluster sampling method was used on the basis of the Basic Approach of Oral Health Survey of WHO and the Guideline of the Third National Oral Health Survey. A total of 10 primary schools were randomly sampled in clusters that included 51 primary school sampling spots in Tangshan. The caries status of 1 794 children aged 7 years to 9 years was examined.

RESULTSThe prevalence rate of the first permanent molar caries was 47.49% (852/1 794), the mean number of DMFT was 1.30 ± 1.59, the mean number of DMFS was 1.96 ± 3.40, and the filling rate was only 2.35% (20/852). The preva- lence rate and mean DMFT of the first permanent molar caries showed significant difference between different ages and genders (P < 0.05). However, the mean DMFS of the first permanent molar caries only showed significant difference between the various ages (P < 0.05). The prevalence rate of the primary tooth caries was 83.11% (1 491/1 794), the mean number of dmft was 4.82 ± 3.55, the mean number of dmfs was 6.51 ± 5.61, and the filling rate was 12.41% (185/1 491). Caries of primary tooth was the first permanent molar caries risk factor [r = 0.277, OR = 5.736, P = 0.00, 95% CI (4.175, 7.881)]. Significant correlation was found between the caries of primary tooth and first permanent molar (r = 0.274, P < 0.001).

CONCLUSIONCaries status of the first permanent molar among 7- to 9-year-old children in Tangshan city is serious. Caries status of primary tooth may play an important role in predicting the first permanent molar caries in clinic and prevention.

Objective:To study the relationship between telomerase activity and the expression of proliferating cell nuclear antigen(PCNA) in oral squamous cell carcinoma (OSCC). Methods:Telomerase activity and expression of PCNA in 68 cases of OSCC,34 cases of epithelium immediately adjacent to carcinomas and 12 cases of normal oral mucosa were detected by TRAP-PCR-ELISA and immunohistochemical SP method. Objective To evaluate the value of ultrasound-guided nasotracheal intubation in the patients undergoing oral maxillofacial surgery by comparing with blind intubation.Methods Forty ASA physical status Ⅰ or Ⅱ patients of both sexes,aged 18-75 yr,weighing 45-90 kg,scheduled for elective oral maxillofacial surgery,were randomly divided into blind intubation group (group B,n =20) and ultrasound group (group U,n =20) according to a random number table.Nasotracheal intubation was performed after routine topical analgesia and conscious sedation.The front end of catheter was adjusted to the aditus glottidis according to the sound of respiratory air,and tracheal intubation was placed when the strongest inspiratory phase appeared in group B.A linear array probe (frequency 7-15 MHz) was used,and the images of glottis expansion and wired catheter insertion were visualized in the thyroid cartilage window in U group.Before intubation and at 0,1,3 and 5 min after successful intubation,mean arterial pressure (MAP),HR and SpO2 were recorded.The development of responses to intubation was recorded during intubation.The successful intubation at first attempt,the number of intubation,intubation time,and postoperative complications such as sore throat or hoarseness were recorded.Results Compared with group B,the number of intubation was significantly reduced,intubation time was shortened,the rate of successful intubation at first attempt was increased,the failure rate of intubation and incidence of sore throat and hoarseness were decreased,and no significant changes were found in the parameters of hemodynamics and incidence of responses to intubation in group U.No intraoperative awareness of intubation occurred in patients.Conclusion Compared with blind intubation,ultrasound-guided nasotracheal intubation can raise the probability of successful intubation at first attempt,reduce the number of intubation,and shorten intubation time,and it is safe and convenient and provides significant value clinically for the patients undergoing oral maxillofacial surgery.
